Q. What exactly is “long-term care at home” and how is it different from traditional retail or LTC pharmacy models?
Long-term care pharmacy at home refers to the services and supports that a phar macy provides specific patients residing in their own homes that is equal to or greater than the level of care that they would receive if they were residing in a long-term care facility. Unlike traditional retail models that focus on point-of-sale transactions, LTC at home pharmacies provide comprehensive medication management including adherence packaging, 24/7/365 availability, cyclical medication reviews, delivery coordination, and care coordination with other community providers and caregivers. In traditional LTC pharmacy, pharmacies are providing medications to facilities; LTC at home is rooted in the community, serving patients who may never enter a facility but still require high-touch, ongoing clinical support.
Q. Who qualifies for long-term care at home pharmacy services? Is it only for patients receiving home health?
Not at all. While patients receiving home health services may qualify, the LTC at home model is designed for any individual who requires a skilled level of care, regardless of whether they are enrolled in a formal home care program. This includes patients with chronic conditions, cognitive impairment, mobility limitations, polypharmacy, or frequent hospitalizations. Eligible populations may range from medically complex children to adults living in private residences. In fact, many patients receiving Medicaid home and communitybased services or those who are dually eligible for Medicare and Medicaid often meet the criteria, even if they are not actively receiving in-home nursing support. It’s important to note that there is a formal qualification process. Phar macies are responsible for identifying, qualifying, and continually re-qualifying patients to ensure they meet the criteria for LTC-level pharmacy services. This must be clearly documented and maintained at the pharmacy to support compliance, payer expectations, and patient safety.
Q. What ser vices must a pharmacy provide to be considered compliant with LTC at home?
First and foremost, any phar macy operating under their LTC NPI must meet the CMS requirements for LTC-level of pharmacy care. The CMS requirements include comprehensive inventory, adherence packaging requirements, pharmacist on call service, IV and compounding access, medication delivery, and emergency medication supplies.
In addition to meeting CMS requirements, pharmacies must recognize that LTC at home patients are often living independently or supported by non-clinical caregivers. This reality demands that the pharmacy elevate its role, delivering a level of care equivalent to what the patient would receive in a facility setting. That means stepping beyond dispensing and into true medication management. Pharmacies must provide comprehensive care coordination, including medication synchronization, up-to-date clinical documentation and qualification forms, and most importantly, perform a complete Medication Dispensing Review (a combination of medication reconciliation and drug utilization review) every time medications are dispensed. By doing so, the pharmacy ensures patients are not only receiving the right medications but are also being actively monitored and clinically managed in the home. This model moves beyond simple delivery and becomes a robust, patient-centered system of care, supporting both safety and better health outcomes.
Q. Why is LTC at home becoming more important in today’s healthcare landscape?
A shift towards aging in place, combined with rising facility costs and a sever shortage of LTC beds, has created a growing demand for LTClevel services delivered at home By 2030 five short years nearly one in five Americans will be 65 or older. Meanwhile, more than 70% of individuals who need long-term care reside in the community, not in institutions. The LTC phar macy at home model fills a critical gap by offering continuity or care, improving medication adherence, and reducing hospitalizations … all while also supporting caregivers, community providers, and reducing health care costs.
Q. How are payers and PBMs responding to LTC at home?
Are they recognizing it as a distinct service model?
Yes, but it’s a work in progress. Forward-thinking industry leaders are beginning to recognize that LTC pharmacy at home is not just retail or LTC pharmacy, it’s a hybrid model that requires its own compliance structure, reimbursement model, and credentialing process. This year we are seeing some payers and plans have already begun carving out service sets and are requiring proof of service standards. Although this can be frustrating for pharmacy owners and operators, it’s exciting! This evolution opens the door for contracting opportunities for pharmacies that can help them better support their at home patient populations, and even provides space for value-based opportunities for pharmacies that can demonstrate outcomes.
Q. What does the future of LTC at home look like and how can independent pharmacies position themselves to lead?
The future of LTC pharmacy at home is both urgent and promising. As health plans, ACOs, and regulatory bodies continue to prioritize aging-inplace initiatives, the demand for qualified LTC phar macy at home partners will grow. Independent pharmacies are uniquely positioned to lead because of their community relationships, agility, and ability to provide personalized care. To lead in this space, pharmacies must ensure compliance with CMS standards for long-term care, implement dedicated LTC at home workflows, and actively engage with local providers, MCOs, and community partners to build trust and demonstrate that independent pharmacies are capable of delivering high-touch, clinical care, not just filling prescriptions.
Q. What are some of the biggest misconceptions about LTC at home pharmacy services?
The biggest misconception is that LTC pharmacy at home is just “retail with delivery.” Many assume it’s simply a matter of putting prescriptions in a bag and dropping them off at a patient’s home In reality, the LTC at home model is a clinical, outcomes-driven model that mirrors the level of pharmacy services traditionally provided in a skilled nursing facility. In a retail model, the interaction often ends at the point of sale. In contrast, LTC at home pharmacy care involves ongoing clinical oversight, robust
documentation, proactive medication management, and close coordination with healthcare providers, caregivers, and home-based services. Pharmacies are responsible not only for dispensing but also for ensuring that the medications are administered correctly, safely, and consistently, often to medically complex or cognitively impaired patients.
Other common misconceptions include:
• “It’s only for hospice or home health patients.” LTC at home services are not limited to patients in those programs. Patients do need to qualify for this level of service, but any individual who requires a skilled level of medication management, typically due to chronic illness, polypharmacy, or functional impair ment, may qualify, even if they’re not receiving formal in home care.
• “Only big LTC pharmacies can do this.” In fact, independent, community-based pharmacies are leading the charge in LTC at home. With the right infrastructure and training, smaller pharmacies can provide these services with greater flexibility and personalization than many larger operations.
• “T here’s no formal framework or regulation.” This model must meet CMS’s long-ter m care phar macy requirements, including medication regimen reviews, 24/7 pharmacist access, adherence packaging, and timely delivery. It also requires pharmacies to meet minimum care standards, including qualifying patients, providing care coordination, clinical documentation, and cycle medication reviews. It is not a gray area; it’s a regulated level of care that is being audited for service support and even now has accreditation pathways.
Ultimately, LTC pharmacy at home is about bringing the institutional pharmacy model into the community … with all the clinical rigor, patient safety measures, and outcome tracking required to support highrisk populations aging in place. My favorite aspect of the model is that pharmacy is no longer just a dispenser … it becomes a central player in the care team, exactly what it should be.

Each pharmacy is empowered to make local leadership decisions, and purchasers continue to manage the onsite, daily operations. “Our job is to set pharmacies up for success with behind-the-scenes support that simplifies medication management and lowers overall costs,” says Posten.
Posten explains that researching drug availability and costs is one of the biggest challenges pharmacies face, regardless of their size. “When I was a pharmacy owner myself, I could lose three or four hours a day just looking up National Drug Code (NDC) numbers on various providers’ websites to access catalogs and compare prices.” This tedious process required jumping between multiple websites simply to gather information. That was before even comparing products and then deciding what to purchase. “It was just incredibly time-consuming.”
Finding the best drug prices every day is only part of the story. To maintain discounts on popular drugs, Guardian must also meet contractual agreements with vendors. “That’s a very tough ‘ask’ to manage across all of our locations,” says Posten. “But it’s a key factor for purchasers to consider when placing orders.”
Maintaining accurate inventory with manual processes is another long, slow process that can consume several hours a day. “Having a handle on inventory is more important than ever as the industry potentially faces a new round of drug shortages,” Posten says.

The system also provides visibility into rebate structures. “As our purchasers research cost and availability, they can also see where we stand in terms of compliance on rebate thresholds before an order is submitted,” explains Posten. That means Guardian can proactively shift volumes as needed to fulfill commitments. “It’s not just about finding the cheapest drug,” says Posten. “It’s about maximizing savings by making the smartest decision — something that would be nearly impossible for me to manage across all of our locations.”
This automated process, on its own, can cut the time spent on research and submitting purchases by as much as 50% for most pharmacies. Inventory Manager also improves operational efficiency by significantly cutting the time spent on manual cycle counts.

Founded in 2008, UnitedRx LLC is a long-term care pharmacy serving nearly 30,000 patients across more than 300 skilled nursing facilities, primarily located throughout the Midwest. Their mission from the start has been to provide state-of-the-art pharmaceutical services combined with the personal touch of a local pharmacy.
According to Chuck Benain, RPh, UnitedRx’s CEO, the company has remained true to that mission over the years. “We focus on putting our customers first — and everything we do is with the intention of supporting them as best we can. That’s really how we’ve built our business.”
That’s why UnitedRx has consistently invested in the latest technologies to add automation and efficiency in its processes. “We were one of the first to put automated dispensing cabinets in facilities so nurses had instant access to medications,” says Benain. They were also an early adopter in offering unit-dose strip pouch packaging over punch cards in a skilled setting.
“One of the challenges in skilled nursing facilities is that medications can change frequently,” he explains. Strip packaging is ideal in this situation because it allows pharmacies to run shorter cycles — bi-weekly or even weekly. Additionally, since packages are unit dose, when there is a change, only one pouch needs to be swapped out, not the entire strip. The result is less medication waste overall.
“When we began offering strip packaging, probably only about 10% of our skilled nursing facilities were using the service,” says Benain. Today, Jeffrey Medrano, director of operations for UnitedRx, says that number is closer to 60%.
As their original strip packaging machines were nearing the end of life, Benain says they began exploring alternatives. “Machines were breaking down all the time, and I was constantly on the phone working on a resolution or waiting for someone to come onsite to do repairs.” His pharmacy technicians were frustrated and many times brought to tears, he says. “What we do every day is almost like an orchestra of how we get medications to our customers. When a machine is down, there’s always a risk that

we won’t meet our commitments.” This often meant added stress for pharmacy techs and extra overtime expenses to ensure medications are delivered on time.
Accessing dispensing data was also a challenge, says Medrano. “With our legacy system, if I wanted a count of a particular medication across the four machines we were running, I’d have to go to each machine and pull the data individually.” That’s because each machine ran on its own server. The process of compiling data could take up to 30 minutes — or longer if a machine was in use and he had to return. After running reports, he’d have to export them into Excel and manually reformat the data. “There was no way for the system to put all that data in one place for us. Getting the information I needed for inventory was just cumbersome.”
Aside from the four legacy systems, one of UnitedRx’s pharmacy locations was using a Noritsu strip packager. “When we started exploring other options, it occurred to us that the Noritsu machine rarely needed a service call,” Medrano says. Benain, familiar with Noritsu from other companies, decided to learn more about their solutions. “I went to St. Louis and visited a pharmacy that was running one of their machines, and they really liked the product,” he says. “I also met with the engineers who designed the system, and it was just a great experience, so I decided to try them out in one of our pharmacies.”
UnitedRx implemented Noritsu’s NX4000 strip packaging system, the NexusRx software, and the automated visual verification solution. “We noticed a difference right away,” says Benain. “These machines are built like tanks. We weren’t having daily breakdowns or any weekly problems.”
As a result of that initial experience, the company has since installed a total of six NX4000 machines. “What we found from the outset is that Noritsu was very responsive to our needs,” Medrano says. “I didn’t want to move too quickly. It was important that all our employees felt comfortable with the new system before we moved facilities over.” They began with weekly phone calls for about six months leading up to implementation to go over each step and identify potential challenges. Then they transitioned one or two facilities to the new machines.
“An implementation like this is a fine balance,” explains Benain. “You can’t just stop everything and switch over to a new system. On the other hand, I was eager to get producing because I knew what a difference it would make for us.” Medrano agrees, saying that once they realized how much better the Noritsu system was, they ramped up quickly. “I’d say we went from zero facilities onboarded to 100 in about a month.”
Medrano says the Noritsu system operates similarly to their previous technology, but its speed and efficiency are unmatched.

Now that same 1,000 takes less than 10 minutes.” He also notes that it’s easy to deploy a facility onto the system when switching from bingo cards to strip packaging. All they need to do is set the facility up in the system,

which is much more straightforward with Noritsu’s software. “Then we put facilities on a 30-day countdown to ensure all medications are billable on the same day. When the 30 days are up, we’re ready to go. There’s no extra transition.” He also says that the process is so easy, he could start filling strip package prescriptions for a brand-new facility almost immediately.
UnitedRx has found that the Noritsu machines and software are very intuitive. “I can train a new employee on running the machines and filling trays in less than a day and have techs working independently within the first three or four days,” says Medrano. That’s partly because the company set up all six of its machines in the same way, as Noritsu had recommended for enhanced efficiency and consistency. “All the medications are in the same canisters across all six machines, so the learning curve is pretty short.” Even simple tasks, like changing the roll of paper in a machine, are easier, he explains. “So even though the technology is similar, it’s these kinds of things where I can see we are saving time.”
The automated visual verification is also another time saver, says Benain. The system takes a picture of every package and compares the image to the pharmacy’s custom medication library. The result is a highly detailed view of each medication package. “If there’s a broken tablet or it’s not the right shape or color of the medication that’s supposed to be in the package — the system tells us to check the pouch.” Pharmacists can review images and confirm or replace medications as needed. The system is typically flagging less than 3% of the pouches, leaving 97% with no need for additional review. Images are also stored in the system, allowing pharmacists to review any pouch if nurses have questions.
“For me, what’s made my job easier is Noritsu’s openness to giving us access to data,” says Medrano. Where data is concerned, the biggest difference from the legacy system is that all machines now run on the same server. “That means I can pull reports that reflect data across all six of our machines.” Any new machines they add will also be run from the same single server.
Over a typical week, UnitedRx is now running about 280 batches of different cycles. This is more than double the volume we were running on our legacy systems. Even with this volume, Medrano says they only see Noritsu about once a month for regularly scheduled preventative maintenance. “If we were running the same volume through our legacy machines, I’d estimate we’d have a service call about every four days.” When they did have an issue with one of Noritsu’s machines during implementation, Benain says the company’s service was exemplary. “They tried to fix it a couple of times, but understood what we’d been through before — so just decided to replace it.” Medrano agrees, saying the company is very responsive to their needs. “If a pharmacy is having an issue, they’ll send out a technician to identify the problem and they’ve resolved our issues quickly.
The bottom line, Benain says, is that Noritsu understands UnitedRx’s business and understands their customers. “They are empathetic to our cause, and just made the whole process so easy for us. They listened to what we needed and gave us valuable input on how to make our processes more efficient. Noritsu was really there for us — and I don’t believe I would buy from another provider at this point.”

Would you believe that Rhode Island, a state nestled among world-renowned hospital systems and a high density of biotech and medical research, has among the highest rate of adult mental illness in the United States at 24%1 of the state’s population?
That’s nearly one in four of the state’s adults living with a mental health challenge. Additionally, Rhode Island surpasses both the national average as well as all other New England states for adult inpatient admissions due to mental illness. And there’s more. Adults in Rhode Island report unmet needs for behavioral health services more frequently than adults in other New England states. Furthermore, 34%2 of Rhode Island children who required mental health treatment or counseling in the past year did not receive it. This issue is particularly pronounced for young adults experiencing first psychotic episodes who require intensive services.
In 2015, the Substance Abuse and Mental Health Services Administration (SAMHSA) published a set of criteria to improve community mental health centers and to establish CCBHC. In October 2024, Rhode Island joined the group of 183 states in the national demonstration program of this initiative.
CCBHCs are specialized clinics that provide a wide range of behavioral health services, including mental health and substance use support. These clinics provide services to individuals of all ages regardless of their insurance status or ability to pay. CCBHCs are certified by the state and offer a comprehensive range of services, including 24/7 crisis support. These specialized clinics are designed to serve as a critical part of the behavioral health safety net. They increase access to care and provide care for very challenging populations individuals with serious mental illnesses and substance use disorders. As part of their mandate, CCBHCs are required to collaborate with other healthcare providers, community organizations, and social service agencies to provide more comprehensive support to clients trying to navigate complexities of their
healthcare. This includes streamlining the referral process, improving care coordination, and ensuring holistic care. One important collaboration is with the pharmacy partners. Pharmacy collaboration is important because CCBHC’s specifically support patients using programs such as Medication Assisted Treatment (MAT) also known as Medications for Opioid Use Disorder (MOUD), Intensive Outpatient Programs (IOP), Opioid Treatment Program (OTP), and alcohol use disorders.
An important aspect to the success of these evidence-based programs is good medication adherence another area where patients need strong support. Pharmacies work to provide this support. One such example is providing clients with multi-dose packaging.
In behavioral health, individuals require clear instructions to adhere to complicated medication regimens. Multi-dose packaging is an important tool to help. It utilizes coldsealed or heat-sealed blister packs, organizing medications by specific times and dates into a 28-blister card. Each blister contains medication for a particular time of day and is color-coded to correspond with the time (e.g., yellow for
morning, white for noon, orange for evening, blue for bedtime). This approach ensures that individuals receive the correct dose at the appropriate time. It is also designed to increase medication adherence.
Multi-dose packaging is one way to support medication adherence. Another way that long-term care (LTC) pharmacies can help CCBHC’s and the patients they serve is through a combination of prompt drug reviews and efficient medication adjustments (such as repacks), which increases adherence and reduces hospitalizations. CCBHC’s also benefit from collaborations with LTC pharmacies that incorporate Quality Improvement (QI) programs, hold quarterly coordination meetings with medical and nursing teams, and provide consultations for specific challenging cases.
Who is Horizon Pharmacy and how are we unique?

Owned by Community Care Alliance, Thrive Behavioral Health, and Newport Mental Health, Horizon Pharmacy aims to enhance medication services and treatments, and generate revenue for its non-profit owners so they can continue to support their combined mission of quality behavioral healthcare in Rhode Island.
Horizon offers customized training, including mental health first aid to all staff, board certification for pharmacists, and advanced pharmacy technician training,
at no cost. This ensures best-inclass medication management for patients while enabling access to behavioral and substance use healthcare at free or reduced costs for over 1,000 individuals annually (via our nonprofit partners).
Does the program really make a difference?
Since its founding in 2014, Horizon Pharmacy has enhanced therapeutic outcomes in longterm care through the delivery of customized pharmaceutical services. Our initiatives have also advanced the quality of behavioral health services throughout Rhode Island. This is evidenced through our proven results.
Recently, Horizon Pharmacy commissioned a comprehensive review of claims data with Integrated Healthcare Partners, an Accountable Care Organization. Our goal was two-fold: assess our work in supporting the CCBHC programs’ goal of increasing behavioral health utilization across the state; and establish specific pharmacology benchmarks for CCBHCs. The findings were compelling.
For the calendar years 20222024, Horizon Pharmacy and the patients served demonstrated the results below. Of patients engaged with Horizon Pharmacy vs. other retail and LTC pharmacies:
• 14.3-28.5% were more likely to achieve compliance with the CCBHC depression medication measure.
• 9.1-10.7% would achieve compliance with the CCBHC Schizophrenia medication measure.
• 22-36% less likely to use the emergency department (ED).
• 38-45% less likely to use inpatient services for diabetes or hypertension.
• 5-7% better compliance with blood pressure medication.
These impressive benchmarks quantitatively show that close collaboration between Horizon
Pharmacy and CCBHC’s results in significant reductions in the Total Cost of Care (TCC) for both individuals as well as insurance providers.
Among the required services of CCBHC are evidence-based practices such as Medication Assisted Treatment (MAT) and care coordination for Opioid Treatment Program (OTP) that are intended to engage individuals outside of an inpatient or emergency service setting. These treatment modalities often include short term medications provided to patients to stabilize and provide comfort. For example, a patient experiencing withdrawal who is stable and comfortable will more likely agree to enter a 30-day detox treatment program.
At Horizon Pharmacy, we believe we can help patients get access to these short-term medications in a safe and efficient way by implementing Automated Storage and Distribution Devices (ASDD). These devices can be in licensed facilities operated by licensed healthcare professionals.
The benefits of using automated dispensing onsite at these facilities, where the ASDD machine is controlled by the pharmacy servicing that facility, is that it provides security, increased accountability, and overall cost savings on medications. In addition, the ASDD machine facilitates immediate access to life-saving addiction treatment, truly meeting patients where they are now. This model has facilitated the growth of new medications for opioid use disorder treatment that often starts in addiction treatment clinical settings.4
However, despite these benefits, there are obstacles in implementing automated dispensing broadly. Automated dispensing is regulated by each state’s board of pharmacy and that means 51 different policies nationwide. Some states are very permissive in the use of technology
while others are not. These states cite patient safety, compliance, and general lack of oversight as the main reasons for not allowing the machine to be used.
We hope this will change.
Train pharmacy staff in empathy and compassion to better engage with individuals in the mental health community. At Horizon Pharmacy, all employees undergo mental health first aid training for adults and children, enhancing their sensitivity and confidence in these interactions. Our customer service training is designed for technicians to engage nursing, residential, and prescribing staff in long-ter m care settings.
Engage with your state behavioral health and substance use authority, as well as the community mental health providers in your area. Ask them if they are aware of CCBHC and if so, ask if they have a roadmap for its adoption.
Integrating CCBHC with LTC pharmacy assets can reduce costs for patients and community health systems. This will be critical as federal government continues to review Medicaid, SAMSHA, and other programs for continuation and funding.
Regardless of the CCBHC status in your community, LTC pharmacies have tools available for these communities of care. It is advisable to establish these connections and relationships.

• On average, patients see pharmacists in their pharmacy 33 times a year, but visit their primary care physicians only three times a year.1

Duane Jones has devoted his career to improving patient access to care. A pharmacist since 1977, Duane has owned his own independent pharmacy and understands the challenges faced by small community businesses. He now serves as the Regional Pharmacy Supervisor, Community Pharmacy Residency Director and Clinical Programs Director for Harps Food Stores, a chain of 151 supermarkets across Arkansas, Kansas, Louisiana, Mississippi, Missouri and Oklahoma. To fill the healthcare gaps of Harps’ customers, Duane implemented a test-and-treat program, providing selected evidence-based testing and treatment protocols to patients, and partnering with local physicians when patients need referrals. Dr. Jennifer Griffin works closely with Duane as a clinical pharmacist for Harps food stores where she specializes in medical billing, treatment workflow and marketing clinical services. Together they have found that the Harps test-andtreat program not only improves multiple patient outcomes but can increase the survival and sustainability of community and independent pharmacies. Now Duane and his Harps team work with other organizations to teach pharmacists, technicians and administrators how the Harps test-and-treat pharmacy workflow can also work at other locations where state laws allow pharmacists to be considered providers.
The challenge: improving patient care
Duane describes gaps in patient care as twofold. “First of all, [we must collaborate] across professional lines to provide continuity of care. One of the challenges we have in this country is that over 270,000 people die every year from non-optimized drug therapy.2 That’s not a disease state. That is incorrect medication, drug-drug interactions, adverse effects, and non-adherence.”
The second challenge is access to care. “There are many, many healthcare deserts in this country— not just rural, but also in inner cities, simply because there are not enough practitioners for our aging population.”
Duane is also program director for the MTM The Future Today, an organization which provides team-based training programs for pharmacists and pharmacy technicians. Duane believes that pharmacies can help close the gaps to improve patient care. He cited several different surveys that indicated that once pharmacists are engaged with patients, their outcomes improve. “Statistics show that we [pharmacists] see patients 33 times a year on average and the primary care physician only sees them three.”1 Patient obligation does not end at the point of diagnosis. Duane believes that pharmacists should be available to teach patients about their medications and therapies, and test-and-treat in specific sets of circumstances.

The solution: test-and-treat outside of primary care
New technologies are providing user-friendly options which facilitate testing outside of the laboratory or clinic. Many patients have limited access to care due to provider shortages, transportation issues, lack of health insurance, or expense. If they can’t find primary care, they may wind up in the emergency department—which burdens our already strained healthcare system. Conversely, patients can walk into their local pharmacy and test for COVID-19, strep or flu and receive treatment. According to a 2022 study published in the Journal of American Pharmacists Association, approximately 89% of Americans live within five miles of a community pharmacy.3 This situates pharmacies as local resources to help eliminate health disparities and accelerate public health initiatives.
Not everyone agrees. The American Medical Association released a statement in June of last year coming out against expanding test-and-treat policies beyond medical doctors.
“While pharmacists and physicians each play important roles in healthcare delivery, the length, breadth, and focus of their education and training are vastly different and prepare them for separate and distinct roles in patient care. Pharmacists are medication experts, but their clinical training does not prepare them to perform physical or mental examinations, diagnose patients, interpret test results, or provide primary care services; the independent practice of medicine by pharmacists puts patient health and safety at risk.” 4
Rather than getting involved in polarizing conversations, Duane prefers to focus on how pharmacists can collaborate with healthcare professionals nationwide. “We have to get beyond thinking about self-preservation and have to think about putting the patient back at the center of care.” While not all states permit pharmacists to test and treat patients, in states where that is permitted, Duane recommends that these programs be protocol driven and used in conjunction with a referral system for local physicians. At Harps, a dichotomous decision tree is used: If they test negative, they don’t treat. If they test positive, the patient receives the proper care that they need. “We practice at the top of our license on behalf of the patient,” insists Duane. “We actually become a great referral source for physicians because if the patient tests negative, but they still have some severe symptoms, we’ll refer them on to their physician for care.”
Because the test-and-treat protocols are evidence-based, they have documented both improved outcomes and antibiotic stewardship. Duane elaborates that when seen at a physician’s office... “Statistics show that about 52% of the patients that are treated for flu actually weren’t tested. They were empirically treated with antiviral medication. In fact, data indicate that only 25% of those patients tested positive.5 So we’re overprescribing antibiotics and antimicrobials. And what that leads to is resistant strains that can potentially become life-threatening.”

There are several barriers that exist for pharmacists to be able to implement test-and-treat programs to improve patient care, namely test availability, time and reimbursement. Harps systematically addressed all three during the creation of their pilot program. First, the task was to select the right assay for their pharmacies. There are lots of point-of-care platforms to use for test-and-treat. Duane chose QuidelOrtho assays. He explained why: “We started our test-and-treat program back years ago when state laws were silent on whether or not pharmacists could do point-of-care testing. We wanted to make sure that we had the best system and did some research to find out what the clinics were using. We went with Quidel.” QuidelOrtho is one of the largest manufacturers of clinical testing equipment in the United States and their specialists trained Duane’s pharmacy staff, using the same protocols with the same hardware as found in point-of-care assays in primary care, urgent care or a hospital settings. They also created marketing materials to promote test-and-treat at pharmacy sites.
Next, Harps pharmacists had to tackle time constraints. Pharmacists are always busy, and many don’t have the time to provide enhanced clinical services to patients. The Harps solution was to institute a workflow that put pharmacy technicians in charge of all non-clinical operations, including

data entry, product dispensing, and managing the adjudication queue. Another key part of the workflow is medication synchronization that allowed Harps to transition a majority of their prescription workload from acute to scheduled, so pharmacists have more flexibility to step away from the verification queue when they need to see a patient.

For many pharmacies, billing and obtaining insurance reimbursement for patient testing, treatment and counseling can be very time consuming and frankly onerous. To address the reimbursement barrier at Harps, Jennifer has streamlined reimbursement and explained their process: “To be honest, it can be time consuming on the front end when you’re developing your process. But once your system is in place and your team is trained, it becomes a smooth and sustainable workflow.” At Harps pharmacies, they discovered a way to bill medical claims through their pharmacy management system to help streamline the claim creation and submission process. In a nutshell: When a patient enters the pharmacy for a test-and-treat service, a technician takes their insurance card and verifies eligibility and benefits. They make sure the patient’s insurance is active on the date of service and they verify what kind of copay deductible or co-insurance the patient may have. Then they collect that amount, the patient receives the service, and the patient is on their way to feeling better. The technician completes the interaction when they bill the appropriate CPT code in the pharmacy management system.
Duane and Jennifer want to share this workflow with other pharmacies to help them build their process, empower their staff, and make reimbursements totally manageable. But will this make community and independent pharmacies more sustainable? “Yes!” according to Jennifer. “It really comes down to getting more payers on board. The more payers that we have that recognize and reimburse us for the value of these services offered in the pharmacy setting, the more successful and sustainable we will be.” This workflow is described as win-win. The patients receive fast, convenient access to care, and pharmacy viability and sustainability are supported. Duane recommends the National Community Pharmacists Association (NCPA)6 and the Community Pharmacy Enhanced Services Network (CPESN)7 for resources on implementing test-and-treat programs and to find out about individual state laws governing pharmacists and reimbursement. Pharmacies will have to contact their Department of Health to inquire about reporting requirements and CLIA waiver, and then will have to ensure both the pharmacist and pharmacy are credentialed by the various payers.

The best way to encourage patients to use pharmacy test-and-treat is to market directly to the patient. Jennifer agrees that this is a new concept for many people. Marketing is a process that they are still learning and evolving. She explains, “Since starting our test-and-treat program, we’ve created bag stuffers and added eye-catching signage inside each pharmacy. We actively promote our services on Facebook and Instagram where our audience continues to grow. But honestly, our most effective marketing is word of mouth. Once patients experience how convenient and simple the process is, they tend to tell other people.”
The final result
Duane has, over many years, collected test-and-treat data from the Harps working pharmacies. He has partnered with Elevance Health Inc., an American for-profit health insurance provider, and CPESN, who together have collaborated to publish studies reporting that pharmacy intervention in medication therapy saved 35% in hospital visits and 15% in emergency visits.8 He is presently working with other organizations to provide more health economic data. He and his team feel certain that the data will show more cost savings. He explains it this way, “The [insurance] plans will get on board with compensating for these [pharmacy test-and-treat] services because they see how much money they’re going to save. For us, our win is a healthy patient. That’s why I get out of bed every morning and that’s why I’m so passionate about what we’re doing.”
