Throughout your hospital stay, we hope that you have been provided with the best possible care. After your discharge from our hospital, you, your healthcare providers and your family will support your recovery. Your aftercare plan may include follow-up visits, medications, exercise and diet changes.

This folder includes information on any follow-up tests you may need and when to schedule those appointments. It will also include phone numbers to call when you or your caregiver have questions about your after-hospital care.

We hope that this guide helps you keep track of all of the things that you need to know. Riverside University Health System is here to support you in maintaining your health and wellness. Please keep this guide as a resource to reference after discharge.
Do you need a note to return to school or work? Please let us know as soon as possible.
• Make sure that you have necessary paperwork for prescriptions, referrals, etc.

• Check your room, bedside table, closet and surrounding areas for personal items
• Have someone available to pick you up
• Verify your discharge date and time with your nurse or doctor
• Please be sure that you understand the services you may need after leaving the hospital
IMPORTANT - Prior to Leaving:
When your healthcare team authorizes your discharge from the hospital, several team members may be involved, including doctors, nurses, case managers and social workers. A nurse or case manager will go over the information that is in your discharge summary, which will be included in this pocket.
