Information About Your Discharge & Transition of Care

Riverside University Health System strives to improve the health and well-being of our patients and communities through our dedication to exceptional and compassionate care, education and research. Throughout your hospital stay, we hope that you have been provided with the best possible care. After your discharge from our hospital, you, your healthcare providers and your family will support your recovery. Your aftercare plan may include follow-up visits, medications, exercise and diet changes.

This folder includes information on any follow-up tests you may need and when to schedule those appointments. It will also include phone numbers to call when you or your caregiver have questions about your after-hospital care.

Follow-Up Instructions

We hope that this guide helps you keep track of all of the things that you need to know. Riverside University Health System is here to support you in maintaining your health and wellness. Please keep this guide as a resource to reference after discharge.

Do you need a note to return to school or work? Please let us know as soon as possible.

• Make sure that you have necessary paperwork for prescriptions, referrals, etc.

• Check your room, bedside table, closet and surrounding areas for personal items

• Have someone available to pick you up

• Verify your discharge date and time with your nurse or doctor

• Please be sure that you understand the services you may need after leaving the hospital

IMPORTANT - Prior to Leaving:

When your healthcare team authorizes your discharge from the hospital, several team members may be involved, including doctors, nurses, case managers and social workers. A nurse or case manager will go over the information that is in your discharge summary, which will be included in this pocket.

This Folder Contains

Discharge Summary - This information is an overview of why you were in the hospital, which healthcare providers saw you, which procedures were done and any medications that were prescribed.

Medications - This is a listing of medications that you were given during your hospital stay, medications that you will continue after discharge and new medications that were prescribed.

Resources - This is information on who to call and where to go for follow-up care now or in the future.

Follow-Up Instructions - We will provide you with information on any tests and follow-up visits that you may need and when to schedule them. This portion will also contain phone numbers in case you have questions about your after-hospital care.

Medications

This packet includes information about medications that you received in the hospital and new medications that were prescribed. Use the chart below to list prescription and over-the-counter medications, including supplements, that you take.

Gauze and wound care items may be purchased at retail pharmacies or at medical supply stores.

Medications To Stop

GOT PRESCRIPTIONS? Do you want to receive your medications at your bedside upon discharge? Do you want to have a personalized consultation with a pharmacist on your medication regimen?

If the answer is YES, talk to your healthcare provider about sending your prescriptions to the RUHS-MC-Retail Pharmacy as part of your discharge plan!

If you have Medicare and limited income and resources, you may qualify for extra help to pay for your prescription drug coverage.

Follow-Up Instructions

Recovery and Support

• Ask if you'll need any medical equipment (like a walker). Who will arrange for this?

• Ask if you're ready to do the activities below. If you need help with some or all of the activities, tell the staff:

◦ Bathing, dressing, using the bathroom and climbing stairs;

◦ Cooking, food shopping, house cleaning and paying bills;

◦ Getting to medical appointments and picking up prescription drugs.

• Make sure you have the support of family or friends who can help you.

• Ask the staff to show you and your caregiver any other tasks that require special skills (like changing a bandage or giving a shot)

• Ask to speak to a social worker if you're concerned about how you and your family are coping with your illness
