The ABCS of a Healthy Heart
Cardiac Toolkit Cardiovascular disease (CVD) accounts for one in three deaths in the United States each year. Strategies that address major CVD risk factors, such as high blood pressure, high cholesterol, and smoking, can greatly reduce the burden of CVD. However, only 46% of American adults with high blood pressure and 33% of adults with high cholesterol have these conditions under control, and only 23% of smokers who are trying to quit receive help.1 VHQC is a partner in the Million Hearts® Initiative, a joint effort of the Centers for Medicare and Medicaid Services, (CMS), the Centers for Disease Control and Prevention (CDC), the American Heart Association (AHA) and other national care stakeholders. The initiative aims to reduce cardiac risk factors that include hypertension, smoking and high cholesterol and increase heart-healthy behaviors such as aspirin for those who need it. The toolkit is arranged with a section for patients and providers based on the ABCS: • Aspirin: appropriate low-dose aspirin therapy use • Blood Pressure: measurement and control in patients with hypertension • Cholesterol: management among adults with ischemic vascular disease • Smoking Cessation: intervention among patients who smoke
