Various things contribute to psoriasis flare-ups

P

soriasis is a chronic skin condition blamed on an improperly functioning immune system. The body may detect a perceived anomaly and attack itself. When a person is suffering from psoriasis, his or her skin cells are produced at an accelerated rate, resulting in red, raised and scaly patches on the skin known as plaques. Plaques are well-defined and typically appear on the scalp, lower back, face, palms, and soles of feet.

About one-third of people with psoriasis have a family member with the disease. Studies show the emotional and social effects of psoriasis are more significant for women than men, but both can be affected negatively by symptoms.

There is no cure for psoriasis, and some people are affected more severely than others. In addition, certain factors can aggravate psoriasis and lead to flareups. Understanding these triggers may help those affected by the disease better manage their symptoms. Although some of these triggers are not backed up by firm scientific evidence, many people with psoriasis attest to their accuracy.

• Overexposure to sunlight:

Exposure to sunlight and other forms of ultraviolet rays can be beneficial in reducing symptoms of psoriasis, and some dermatologists may even prescribe UV treatment. However, overexposure to the sun can worsen symptoms, particularly if men and women get a sunburn.

• Stress:

Doctors don’t know exactly why, but psoriasis and stress seem to be linked. As stress escalates, flare-ups of psoriasis may become more common. In turn, flare-ups can lead to additional stress.

Finding ways to reduce stress may result in clearer skin. Deep breathing, yoga and meditation may help alleviate stress.

• Hot water: Bathing or washing in very hot water can worsen symptoms of psoriasis. Use lukewarm water and a moisturizing soap. Some dermatologists recommend colloidal oatmeal or Epsom salts in a bath to soften skin, relieve itching and help slough off scaly patches. Additionally, avoid situations that will lead to dry skin, such as exposure to hot or cold weather.

• Infections:

After a bout of strep throat or an ear infection, people with psoriasis may find their condition takes a turn for the worse.

• Hormones:

Women may find that hormonal changes trigger their psoriasis. Research isn’t clear as the connection between psoriasis and hormones, but doctors do know that pregnancy and menopause

can cause psoriasis to worsen or even appear for the first time. Others find that an increase or decrease in estrogen levels improves their psoriasis symptoms. But symptoms are unique, and psoriasis outbreaks need to be evaluated on a personto-person basis.

How Midwives Make a Difference “Midwife” is a term translated from Old English to mean “with woman”. From Biblical times to the era of free love, midwives have been the ones helping generations of women give birth.

With the rise of modern obstetrics at the turn of the 20th century, midwives went from attending half of all births to fifteen percent. Over the past several decades, midwifery care has had a resurgence, and the number of midwife attended births has steadily risen.

Modern midwives are healthcare professionals who care for women throughout their lifespan. They are educated as nurses first, then dedicate themselves to a two year intensive training in obstetrics and gynecology in order to earn their Master’s degree in midwifery. Midwives honor birth as a normal process while also vigilantly monitoring for complications that may arise.

Midwives are trained to manage obstetrical emergencies and work in collaboration with our physician colleagues to provide evidence based care. Choosing to have a midwife as part of your care team can significantly improve outcomes for you and your baby.

Recently, a large review looked at thirteen trials of over 16,000 women, who were both low and high risk. Women who had midwives leading their care had lower rates of preterm delivery, pregnancy loss before 24 weeks of pregnancy, episiotomy, and use of forceps or vacuum during birth.

Furthermore, women who sought natural childbirth were more likely to achieve this goal with a midwife caring for them. Women who were high risk had similar outcomes when

being cared for by mostly midwives, suggesting that even women with more complex conditions may benefit from midwifery care while still being safely monitored by her physician.

A study this year found that states which had “midwife-friendly” laws and regulations, meaning midwives are fully integrated into the obstetric healthcare system, are significantly out performing other states in pregnancy and birth outcomes. Although it’s hard to

draw direct correlation from this study, it does reaffirm that everyone benefits when midwives and physicians coordinate care.

In the United States, formal midwifery training began with Frontier Nursing University, which had a strong focus on rural outreach. To this day, eighty percent of it’s graduates practice in rural and underserved communities providing care to some of the most vulnerable populations. Globally, midwives fill important gaps in care.

Organizations such as Midwives for Haiti train skilled midwives and provide prenatal care through mobile clinics for women who otherwise would have no access to care. The United Nations Population Fund works vigilantly to support midwives and improve quality education so every woman has a skilled birth attendant. They report that, according to their

estimates, quality midwives could prevent two-thirds of infant and maternal deaths around the world.

Midwifery care is the merging of science and the art of simply being with women.

Choosing a midwife is one of the earliest choices women can make in pregnancy to improve outcomes for themselves and their babies.

By assimilating the wisdom of the midwives that have come before us and the ever-evolving knowledge of modern medicine, midwives are able to care for women in mind, body, and spirit while improving their healthcare experience. Only when midwives are integral parts of the healthcare team, working collaboratively with physicians, are women able to obtain optimal care and experience their best birth possible.

Reduce your lung cancer risk

By Dr. John E. Moore Atlanta Cardiac and Thoracic Surgical Associates There’s no sugarcoating this. Lung cancer is the leading cause of cancer death in both men and women. The American Cancer Society estimates that more than 154,000 people will die from lung cancer in 2018. That’s more than the next three most common cancers combined (colon, breast and prostate).

Despite formidable research proving that smoking is responsible for more than 80 percent of all lung cancers, approximately 16 percent of adults (age 18 and up) continue to smoke. Former smokers make up 60 percent of diagnosed lung cancers, while 20 percent are current smokers and 20 percent have never smoked (secondhand smoke).

However, while smoking is the primary cause of lung cancer, additional offenders include genetic factors, air pollution, radon and occupational

exposure through asbestos and certain chemicals and variables.

If you experience chest pain, unexplained weight loss, difficulty breathing or new-onset wheezing, or are coughing up blood, see a physician right away. While these symptoms often are caused by something much simpler than cancer, if it is lung cancer, getting treatment right away may mean a better outcome.

Discuss symptoms or health changes with your doctor and follow up on your doctor’s recommendations for screening and smoking cessation to ensure you stay cancer free.

Why are there such high death rates from lung cancer? The disease is hard to detect because it does not cause symptoms in its early stages.

Approximately 85 percent of patients are diagnosed with late-stage disease.

However, when found early, lung cancer is actually highly curable.

Screening of high-risk patients with low-dose-radiation CT can be lifesaving.

Studies have shown that if lung cancer is diagnosed and removed when it is less than 1 centimeter in size survival is greater than 90 percent. Medicare and some insurance companies will cover low-dose CT scans as a screening tool.

There also are many new advances in the field of lung cancer treatment. Most lung cancer resections are performed either robotically or thoracoscopically (VATS). Immunotherapy and targeted chemotherapy agents tailored for specific tumors offer greater hope in the treatment of advanced lung cancer.

Stereotactic-targeted radiotherapy provides much better results than standard radiation.

Lung cancer screening will lead to early detection of lung cancer and therefore the application of some of these newer treatments.

The average age of someone diagnosed with lung cancer is 70. If you are a smoker or former smoker, you are generally at the highest risk for developing lung cancer. It is vital to stop

smoking to reduce your cancer risk.

Even after you have been smoking for many years, stopping smoking will improve your overall health. Within 20 minutes after quitting, your heart rate and blood pressure drop. Within two weeks to three months, your circulation improves and your lung function increases.

Studies have shown that even people with an inoperable lung cancer live longer if they quit smoking when they are diagnosed. Participating in a group smoking cessation program improves your chances of quitting and remaining a nonsmoker. Melanoma and Breast Cancer: A New Warning If you’ve had breast cancer or melanoma (the deadliest form of skin cancer), be aware that having either one raises the odds of developing the other, according to a recent study published in the Irish Journal of Medical Science. While earlier studies have noted an association between the two malignancies, this is the first time researchers have explicitly advised doctors to monitor breast cancer patients for signs of melanoma, and vice versa. They also recommend that melanoma patients with a family history of breast cancer have regular mammography’s or breast MRIs (magnetic resonance imaging).

What the Numbers Say Investigators led by photo biologist Gillian M. Murphy, MD, of the Beaumont Hospital in Dublin, and a member of The Skin Cancer Foundation’s International Advisory Council, examined the 6,788 cases of melanoma and 27,597 cases of breast carcinoma between 1994 and 2007 documented in Ireland’s National Cancer Registry. They mathematically determined that if there was no statistically significant relationship between the two,

30-35 patients could be expected to have both malignancies. Instead, there were 127. “In general, patients with melanoma or other skin cancers are always at higher risk of developing other malignancies,” Dr. Murphy commented.

“But this is about a fourfold increase, which raises the possibility of a genetic predisposition linking the two cancers.”

The study corroborates the findings of journals such as Annals of Oncology and Breast Cancer Research and Treatment, which reported that breast cancer patients have between 1.4 and 2.7 times the risk of developing melanoma. And the danger is reciprocal: The International Journal of Cancer noted that female melanoma patients have a

7 symptoms of common brain tumors you should know

A

s a patient, you play a key role in your health care. Knowing your symptoms as they develop and talking with your physician early helps determine the best course of action. This is certainly the case with tumors in the brain.

There is a greater variety of types of brain tumors compared to tumors found anywhere else in the body – more than 100 in fact. Understand that all tumors are not cancerous, but if left untreated they can still be life threatening.

Signs and symptoms of brain tumors

Awareness of your symptoms will help identify the type and location of the tumor. Below are seven common brain tumor symptoms you need to know.

1. Headaches that occur almost daily, are worse at night and increase in activity; headaches with nausea and vomiting can be a sign of dangerously high pressure in the brain.

Types of brain tumors

A brain tumor diagnosis does not mean that you have cancer.

Benign brain tumors are not cancerous; however, they still can cause damage to normal brain tissue and often require treatment. The most common benign tumors include meningiomas, pituitary tumors, as well as acoustic neuromas (sometimes referred to as vestibular schwannomas).

Colon Cancer Screening Saves Lives Having a colonoscopy isn’t something people get excited about, but preventing colon cancer before it starts should be.

In the United States, colon cancer is the third most commonly diagnosed cancer in both men and woman, and it’s the second leading cause of cancer-related deaths. If that wasn’t enough, colon cancer also shows no signs or symptoms until it has advanced.

The good news – colon cancer is one of the most highly treatable and preventable cancers when detected in its early stages during a screening colonoscopy.

While other colon cancer screening methods are available, colonoscopy remains the only test that can prevent the disease through early detection

The various types of arthritis umbrella term and an informal way of referring to joint pain or joint disease. While these conditions may produce some common symptoms, such as swelling, pain and stiffness, learning to distinguish between some common types of arthritis can help men and women manage their conditions more effectively.

A

rthritis affects hundreds of millions of people across the globe. The Arthritis Foundation® notes that more than 50 million adults in the United States have some type of arthritis, while the European League Against Rheumatism estimates that rheumatic diseases such as rheumatoid arthritis affect more than 120 million people in the European Union. In Canada, the Canadian Community Health Survey found that 16 percent of Canadians age 15 and older were affected by arthritis.

The Arthritis Foundation notes that arthritis is not a single disease. In fact, the word “arthritis” is something of an

Osteoarthritis

Osteoarthritis, which is sometimes referred to as “degenerative joint disease” or “OA,” is the most common chronic condition of the joints. The symptoms of OA vary depending on the joints that are affected, but pain and stiffness, especially first thing in the morning or after resting, are common. OA can affect the hips, knees, fingers, or feet, and those with OA may feel limited range of motion in their affected areas. Some with OA may hear clicking or cracking sounds when the affected joints bend, and pain associated with OA may be more intense after activity or toward the end of the day.

Inflammatory arthritis

Inflammatory arthritis occurs when the immune system, which can employ inflammation to fight infection and prevent disease, mistakenly attacks the joints with uncontrolled inflammation. Such a mistake can contribute to joint erosion and even organ damage. Psoriatic arthritis, which the Arthritis Foundation notes affects roughly 30 percent of people with psoriasis, and rheumatoid arthritis are two examples of inflammatory arthritis.

Genetics and environmental factors, such as smoking, may trigger instances of inflammatory arthritis.

Infectious arthritis

Bacterium, a virus or a fungus that enters the joint may trigger inflammation and lead to infection arthritis. The Arthritis Foundation notes that the most common bacteria to cause infection arthritis is staphylococcous aureus, or staph. The majority of infectious arthritis cases occur after an infection somewhere else in the body travels through the bloodstream to the joint, though some infections may

enter the joint directly through a puncture wound near the joint or during surgery near the joint. Intense swelling and pain, typically in a single joint, are the most common symptoms of infectious arthritis, which is most likely to affect the knee, though it can affect the hips, ankles and wrists. Some people with infection arthritis may also experience fever and chills.

Metabolic arthritis

The body produces uric acid to break down purines, a substance found in many foods and in human cells. But some people produce more uric acid than they need, which they then struggle to get rid of quickly. As a result, uric acid can build up. The Arthritis Foundation notes that this buildup can lead to the formation of needle-like crystals in the joints that cause sudden spikes of extreme pain.

Memory care a component of assisted living

A

ging brings about many changes, including some that may compromise seniors’ ability to remain independent.

In such instances, many families consider assisted living facilities for their aging relatives.

More than 835,000 Americans currently reside in assisted living communities, according to the National Center for Assisted Living.

While communities and homes provide various services, one service in particular may be needed for seniors with Alzheimer’s disease, other dementias or memory problems. Memory care units are separate facilities that cater to people with dementia or other memory issues.

According to the senior residence referral and resource guide A Place for Mom, memory care is a

distinct form of long-term skilled nursing. Also called special care units, memory care facilities provide 24-hour supervised care for individuals. This care is often carried out in a separate wing or floor of a residential facility or community.

Memory care facilities aim to provide a safe and secure environment for residents. This means limiting entry or egress to other areas of the facility for those who may have trouble navigating and remembering where they started out.

The Alzheimer’s Association notes that six in 10 people with dementia will wander and become disoriented.

This is dangerous for the individual and others. Memory care units often have alarms and “wander guard” precautions in place to make sure residents cannot roam unsupervised and risk injury.

Another goal of memory care is to provide a stress-free, structured lifestyle that has set schedules and routines, states Brookdale Senior Living. This can help keep distress minimal and make for more comfortable and happy residents.

In addition, memory care units offer programs that help slow the progression of dementias and

cultivate cognitive skills. Fitness exercises, games, specialty food programs, activities, and much more may be offered to make residents more comfortable. Memory care facilities include common spaces for socialization, meals and activities.

“Alzheimer’s patients must maintain their activity, walking, interacting with music, and art,” says Lory Bright-Long, MD, CMD, of the American Medical Directors Association. “We have to awaken the senses so that the memory can have a hook. This is accomplished in the memory care setting.”

Medical oversight, including skilled nursing visits, hygiene care, emotional and spiritual support, medications, incontinent supplies, gloves, oxygen, hospital bed are some of the services offered by the hospice benefit. All of this sounds wonderful and appears to meet your needs but how do you tap into the hospice benefit? There are a couple of ways to explore the hospice benefit. One way to pursue hospice services is to directly contact the patient’s physician and discuss the needs of your loved one. A second option is to directly contact a hospice of your choice and inquire about hospice services. A hospice representative can help answer questions and review your needs and how they can be met by the hospice team. They can also offer to contact the physi-

Medications will also be delivered to the home. As patient needs change so does the hospice services offered. The hospice journey does not end when your loved one dies.

Bereavement services are offered and continue for 13 months. Individual and group support opportunities are offered.
