Some Good News to Share

T

his issue of CORE Ink is being published during a period in our history where much seems different. While many of us may have been stressed around the news related to the COVID-19 pandemic, we thought we might take this opportunity to share some stories of positivity, healing, and hope to help lift your spirits. Right now, I suspect that each and every one of us can benefit from stories that motivate and inspire and maybe even make us smile. This issue of CORE Ink is all about those ideals.

Our cover story is one of triumph. It tells the story of Ioan, a 42-yearold triathlete in peak physical condition. During a high jump, Ioan tore his anterior cruciate ligament (ACL) and meniscus, both of which needed to be surgically repaired by Dr. William Kesto. Instead of considering this the end of his career, he willed his way through rehabilitation with The CORE Institute team and had his sights set on his next Ironman competition less than a year later.

We will also share the story of John, an active man in his 70’s who refused to let the arthritis in his knees stop him from being active. With the assistance of innovative new surgical technology, he was able to get back into the gym and live a healthy lifestyle just weeks after knee replacement surgery. His motivation is a shining example of positivity.

On the topic of knee replacements, we will tell you about new MORE Foundation research that is using an incredible new smartphone app to accurately predict how much rehabilitation a patient might need after a knee replacement. This research is truly game changing and could set the new standard for how surgeons manage the post-operative care of patients.

WHAT’S NEW NEW PROVIDERS AMANDA WRIGHT, DO, MS is a board-certified Orthopedic Surgeon who is Fellowship Trained in Orthopedic Traumatology.

Before joining The CORE Institute, Dr. Wright cared for patients as a Trauma and Fracture Care Specialist in Florida. Dr. Wright earned her fellowship education from Wright State University at Miami Valley Hospital in Dayton, Ohio. Prior to earning her fellowship training, Dr. Wright received her residency training from Northwell HealthPlainview Hospital and Peninsula Hospital Consortium. She earned her Doctoral Degree from Touro University – College of Osteopathic Medicine. Before pursuing her Doctor of Medicine, Dr. Wright earned her Master’s Degree in Biomedical Science from Barry University. hile many Eastern medical techniques or philosophies have mainstream appeal, there are some seemingly related techniques available for patients who prefer a more Western approach.

Dryneedling is one such procedure.

VARIETY OF THERAPIES

While the number of sessions necessary varies by the patient and the injury, dryneedling appointments generally last an hour or less and the needles remain applied for only 20 to 30 minutes. The procedure is one of several therapies that includes myofascial cupping, taping, joint manipulation and joint mobilization.

MOTIVATING GADGETS

Dr. Anderson points out that the gadgets, gear and fancy watches many people use to pursue such trends as counting steps and motivating weight loss may be gimmicks but they have the value of putting money where goals are. Once the latest technology, pricey running shoes, or an elite bike is bought, people tend to use the products to justify the purchase.

Whether you gear up or not, follow the World Health Organization recommendations of at

least 150 minutes of moderate-intensity physical activity throughout the week, or 75 minutes of vigorous intensity. To achieve exertion in the correct moderateintensity range, look up heart rate targets by age on the American Heart Association website. To accurately track intensity, there is an endless array of technology and many treadmills and other equipment have built-in monitors. “You want to break a sweat,� Dr. Anderson said. “Set a realistic goal and pick an activity you like at a time best for you or you’ll just get frustrated and quit.�

MAKE A PLAN

He stresses that one day of intense exercise is not equal to three days of moderate. Weekend warriors who do little during the week and then go for a grueling eight-hour hike, run a marathon or play aggressive contact sports usually wind up in his office with an ruptured Achilles or some other injury that is caused when the body isn’t prepared for exertion. “Make a plan for regular exercise,� he recommends. “People often think if they don’t get 10,000 steps per day or some other more intense goal, they might as well not exercise and that thinking is counterproductive. Any activity is good.

F

or the first 42 years of Ioan Duca’s life, it would’ve been completely reasonable to believe his joints were made of a magical combination of springs and rubber.

When Duca needed to get from point A to point B, he didn’t walk.

He’d hop. Flip. Jump. Leap. Bound.

He was so active and full of energy, triathlons were just extended jogs to him. He’d completed four Ironman triathlons. That sound you heard when Lee Majors did something cool in The Six Million

Dollar Man? It must be the sound Duca heard in his head all his life.

But then it happened. May 7, 2019. Duca was doing what he did — hurling his body in the air — when something went wrong.

“I had a moment of hesitation doing a high jump, and basically when I was up in the air, I knew I wasn’t fully committed to the movement,” Duca said. “I landed it, but I landed faulty. I just heard a crack, and I collapsed to the ground.” >>

DAMAGE DONE

Duca tore his ACL — an important ligament in the knee — which he needs to perform any athletic maneuver. He also sustained tears in his meniscous in his same knee.

performed surgery to repair the damage on June 10, 2019.

“I’m someone who was always jumping, flipping, front-flip, back flip,” Duca said. “Not being able to walk was humbling and terrifying.”

PLAN FOR RECOVERY

Duca worked with Dr. Kesto and Jim Watson, his physical therapist, on the best way to gradually regain strength,

movement and, eventually, the type of athletic activity Duca performed daily.

“It wasn’t easy to accept the reality and to accept direction,” Duca said.

“I have a Type A personality. I’m a triathlete. But I wouldn’t be able to walk for six weeks. Those six weeks were very tough, very hard.”

Jim Watson, PT stressed the importance of taking rehabilitation slowly and deliberately.

“It doesn’t matter how strong you are or how flexible you are, you can’t speed up healing,” Jim Watson, PT said.

“When you’re dealing with an athlete who’s used to pushing their body — they’re used to feeling a healthy body, but now they’re healing. It’s especially tough with someone who’s never been hurt before. It’s all new.”

LEARNING TO WALK AGAIN

At six weeks post-surgery, Duca was walking with crutches and a brace.

By week 10, he was walking with just crutches.

“You forget how to walk,” Duca said.

“I struggled. I didn’t think I could get

my flexibility back, but I did. I had to push so hard.”

Gradually, Duca was swimming and biking. By week 13, he was running.

“I pushed through it, pushed through it, pushed through it,” Duca said.

Six months post-surgery, Duca did light tumbling in a gym. After he passed month seven, he was performing front and back flips in a foam pit. By the nine-month mark, Duca was starting to eye Ironman schedules to see when he could make his return to competition.

COMING BACK STRONGER

All throughout the process, he sent Dr. Kesto videos of the running and tumbling.

“I had this unique connection with him,” Dr. Kesto said. “He’d send me updates of what he’d do in therapy, different kind of jumps and all that.

Some of it was scary for me to look at, but I could see he had good control.”

While Duca’s official return to competition was delayed along with the rest of the country in 2020, he continued to heal and train. He expects to be ready, by 2021.

“This has humbled me greatly,” he said. “I don’t particularly regret going through this. I’ve learned a lot about my body and myself. I’m coming out of this stronger.”

ohn Lopez doesn’t like to sit around much. The Arizona native was always active as a child and in his adult years.

Even after retiring from a career in telecommunications, he enjoyed competing in large walking and running events to benefit charities and dabbling in different types of exercise classes; among his favorites is kickboxing.

In his 60s, however, Lopez began to develop arthritis in his knees. He turned to pain medication injections at first, but after several years of that approach his physician finally admitted that there wasn’t more that could be done.

Stubbornly, he tried to persevere for a couple more years, but his knees further deteriorated, and he finally heeded his doctor’s suggestion to visit The CORE Institute.

“It got so bad I really couldn’t do anything anymore,” the now 77-year-old recalled.

As had been expected, imaging revealed cartilage in both knees was completely deteriorated, a condition commonly referred to as knees being “bone on bone,” where the femur (upper part of the leg) and tibia (lower leg bone) rub together at the knee without the natural cushion of cartilage between them.

SENSING GAIT FROM YOUR PHONE

Developed by ASU Biological and Health Systems Professor, Thurmon Lockhart, the phone app uses sensors to gather tremendous amounts of data about a person’s gait, pain tolerance, comorbidities and other musculoskeletal information within only a few minutes.

Under the study, that data, taken a couple weeks before surgery, is then leveraged by MORE Foundation’s research team to predict recovery care needs for each individual patient.

Again, today’s post-operative care decisions rely almost entirely on outdated averages. In most cases, patients are prescribed a set number of physical therapy visits, perhaps six to 10, regardless of their condition and situation. Often, some require more, others less.

The app has been available for download for more than a year, but it has predominantly been used by researchers in labs, not patient care.

“This study is a critical step in advancing the technology to make it available for both patients and doctors,” Marc Jacofsky, Ph.D noted.

“We’re taking a complicated data set and making it easy for the medical professional to understand so they can make the best patient care decisions.”

The study is a joint partnership between the Partnership for Economic Innovation (PEI), ASU’s Fulton School of Engineering, The CORE Institute, and MORE Foundation. The technology comes out of the WearTech Applied Research Center, which was established in central Phoenix to develop and test wearable sensor technologies to monitor health and improve lives.

Wearable sensors are a growth industry estimated to top $56.8 billion by 2025.

“We’re really excited about this project,” said Marc Jacofsky, Ph.D. “It helps cement our place in the community from a technology transfer perspective in terms of working with the University and moving ideas from the lab to the bedside.”

New Table, Fewer Complications ELIMINATING COMPLICATIONS

This traction placed on the groin area can lead to complications after surgery, including pain, numbness in the groin, and even sexual dysfunction.

“It would also occasionally make surgery more difficult because we would have to work around the post,” Dr. Rose said.

The new post-free table utilizes a special leg holder, a high friction pad, and gravity to open up space in the hip for surgery.

Dr. Rose has been using this new procedure style for six months now, and patients are reporting less pain after surgery.

“Another benefit of the new table is that it provides a slightly less surgical time, so we see less inflammation and swelling in post-op patients,” Dr. Rose said.

Post-free hip arthroscopy comes to The CORE Institute

surgeon at The CORE Institute is the first in Arizona to incorporate new technology during hip arthroscopy that can significantly reduce the risk of complications for patients.

Dr. Michael Rose, an orthopedic sports medicine surgeon, practiced in Denver, Colorado while the post-free hip distraction table was first introduced, and brought his experience to The CORE Institute.

Hip arthroscopy allows surgeons to view and treat treat hip problems with

tiny incisions. Traditionally, surgery on the hip joint would require a large incision and would require substantial force and effort to make the joint accessible. According to Dr. Rose, with traditional tables, between 60 and 100 pounds of traction force is required to visualize the hip joint.”

“It puts a lot of unwanted force on the groin region that could otherwise be avoidable,” Dr. Rose said of the typical procedure, which uses a large post between the legs to keep the body in position.

He noted there are some other advantages of this table that have surgeons very excited.

“The foot has to be held tight during surgery. The boots on the traditional tables were outdated, and people would occasionally experience some discomfort in their feet and ankles after surgery,” Dr. Rose said. “The new boots are better designed to evenly spread out the pressure — an unintended benefit of this table.”

He believes that patients who might have been on the fence about getting a hip arthroscopy before might be more willing now that potential complications are eliminated.

“Most surgeons believe this will be the standard of care going forward because this is what’s best for the patient,” Dr. Rose said.

Get Some Air Time Air fryers too good to be true? Think again!

ove over, Instant Pot. Perhaps the next best thing since sliced bread, the air fryer is next in line to the healthy cooking gadget throne. Imagine crispy fried chicken and crunchy fries without being deep-fried in oil — it sounds too good to be true.

When it comes to fried foods, we get weak in the knees for that golden-brown goodness and satisfying crispy crunch.

Research, however, has linked oils used to cook food to such health problems as heart disease, type 2 diabetes and cancer, making deep-frying foods not exactly the healthiest of options.

LOWERING FAT, CALORIES

Enter the air fryer, a popular kitchen appliance that is touted as a healthy

alternative that helps lower content of fat and calories. Contrary to the name, air fryers don’t fry food. Rather, hot air circulates around the food to cook it and give it a crispy taste and golden, brown color.

While air fryers do require using oil, the amounts are considerably less, often just a tablespoon to achieve a similar taste and texture to deep-fried foods. And by most measures, air frying is healthier than frying in oil — it cuts calories by 70 to 80 percent and has a lot less fat.

Air frying may also cut down on other harmful effects of oil frying. Recent studies have shown air frying lowers the amount of acrylamide, a chemical produced when you fry starchy foods, in fried potatoes by 90 percent. According to the International Agency for Research on Cancer, acrylamide may be linked

to the development of some cancers, including endometrial, ovarian, pancreatic, breast and esophageal cancer.

COOKING OPTIONS

While air fryer cooking tends to be healthier — primarily in the way food is prepared — it doesn’t necessarily mean you should use it for every meal. Rather, put your air fryer to good use by creating a more wholesome version of a comfort food recipe you love. You can also use it to give an already-nutritious ingredients — like vitamin-packed sweet potatoes — a little extra crunch.

When cooking with an air fryer, Dana Angelo White, registered dietician and author of “Healthy Air Fryer Cookbook: 100 Great Recipes with Fewer Calories and Less Fat,” says it can be used for healthier versions of fried foods, but also for baked goods, egg dishes and desserts. “As long as you know the proper cooking temperature and how to place the food in the fryer basket, you can cook just about anything in an air fryer,” she says.

Air Fryer Cinnamon Rolls 2 tbsp. melted butter, plus more for brushing 1/3 cup brown sugar, packed 1/2 tsp. ground cinnamon Kosher salt All-purpose flour, for surface 1 8-oz. tube refrigerated Crescent rolls Glaze 2 oz. cream cheese, softened 1/2 cup powdered sugar 1 tbsp. whole milk, plus more if needed Line bottom of air fryer with parchment paper and brush with butter. In a medium bowl, combine butter, brown sugar, cinnamon and a large pinch of salt until smooth and fluffy.

On a lightly floured surface, roll out crescent rolls in one piece.

Pinch seams together and fold in half. Roll into a 9-by-7-inch rectangle. Spread butter mixture over dough, leaving 1/4-inch border. Starting at a long edge, roll up dough like a jelly roll, then cut crosswise into 6 pieces. Arrange pieces in prepared air fryer, cut-side up, spaced evenly. Set air fryer to 350 degrees, and cook until golden and cooked through, about 10 minutes.

While rolls are cooking, prepare the glaze by whisking cream cheese, powdered sugar and milk together in a medium bowl. Add more milk by the teaspoonful, if necessary, to thin glaze. Spread glaze over warm cinnamon rolls and serve.

Air Fryer Garlic Herb Turkey Breast 2 lb. turkey breast Kosher salt Freshly ground black pepper 4 tbsp. melted butter 3 cloves garlic, minced 1 tsp. thyme, freshly chopped 1 tsp. rosemary, freshly chopped Pat turkey breast dry and season on both sides with salt and pepper. In a small bowl, combine melted butter, garlic, thyme and rosemary. Brush butter all over turkey breast.

Place in basket of air fryer, skin side up, and cook at 375 degrees for 40 minutes or until internal temperature reaches 160 degrees, flipping halfway through. Let rest for 5 minutes before slicing.

Air Fryer Crispy Potatoes 1 lb. baby potatoes, halved 1 tbsp. extra-virgin olive oil 1 tsp. garlic powder 1 tsp. Italian seasoning 1 tsp. Cajun seasoning (optional)

Kosher salt Black pepper, freshly ground Lemon wedge, for serving Parsley, freshly chopped for garnish

In a large bowl, toss potatoes with oil, garlic powder, Italian seasoning and Cajun seasoning, if using.

Season with salt and pepper. Place potatoes in basket of air fryer and cook at 400 degrees for 10 minutes.

Shake basket and stir potatoes and cook until potatoes are golden and tender, 8 to 10 minutes more. Squeeze lemon juice over cooked potatoes and garnish with parsley before serving.
