Overview On July 19, 2018, the Federal Transit Administration (FTA) published the Public Transportation Agency Safety Plan (PTASP) Final Rule, which requires certain operators of public transportation systems that receive federal funds under FTA's Urbanized Area Formula Grants to develop safety plans that include the processes and procedures to implement Safety Management Systems (SMS).

The PTASP rule became effective on July 19, 2019.

FTA published a Notice of Enforcement Discretion on April 22, 2020 effectively extending the PTASP compliance deadline from July 20, 2020 to December 31, 2020.

The plan developed by transit providers must include safety performance targets. Additional guidance on planning and target setting on FTA's Performance-Based Planning is included on the following pages. Transit providers also must certify a safety plan will be in place meeting the requirements of the Final Rule by December 31, 2020. The plan must be updated and certified by the transit provider annually.

The Final Rule applies to all operators of public transportation systems that are recipients and subrecipients of federal financial assistance under the Urbanized Area Formula Program (49 U.S.C. § 5307).

However, FTA is deferring applicability of this requirement for operators that only receive funds through FTA’s Enhanced Mobility of Seniors and Individuals with Disabilities Formula Program (Section 5310) and/or Rural Area Formula Program (Section 5311).

The Final Rule does not apply to agencies that are subject to the safety jurisdiction of another federal agency, including passenger ferry operators regulated by the U.S. Coast Guard and rail operators regulated by the Federal Railroad Administration.

Public Transportation Agency Safety Plan The Federal Transit Administration (FTA) is providing the Public Transportation Agency Safety Plan Template for Bus Transit and accompanying Reference Guide to assist with the development of a Public Transportation Agency Safety Plan (Safety Plan) for bus transit modes. Use of this template is voluntary.

The template and reference guide are intended for use by States and operators of public transportation systems that are required to draft a Safety Plan in accordance with 49 C.F.R. Part 673 (Part 673). The full text of Part 673 is available at http://www.transit.dot.gov/PTASP. Certain requirements in Part 673 do not apply to small public transportation providers1. The relevant sections in this template are noted in red to indicate where requirements differ. Transit operators that are subject to Part 673 may choose to include additional sections beyond what is required in Part 673.

Under Part 673, a transit agency is required to maintain documents that describe its Safety Plan, including those related to implementation and results from processes and activities. Also, a transit operator may have existing documentation that describes processes, procedures, and other information required in Part 673. You may reference these documents in your Safety Plan by specifying the document names and locations within the appropriate sections of the plan.

Annual Review and Update of the Public Transportation Agency Safety Plan Describe the process and timeline for conducting an annual review and update of the Public Transportation Agency Safety Plan.

The PTASP annual review will occur in October of each year or the first month of the new fiscal year (FY).

Each Key Staff member will review his or her portion of the plan and will make revisions as needed based upon the data obtained through record keeping for the previous FY. A draft plan of the revised PTASP will be submitted to the Accountable Executive for final review and approval for the current fiscal year keeping our current successful mitigations or approving revised reduction percentages from the safety performance Indicator and targets described in section 3. Once approved, CATA will have new Safety Performance Targets established. The Agency Safety Plan (ASP) will address all applicable requirements needed to be compliant with the four Safety Management System Components (or pillars) and ten required subcomponents described on page 6. Subcomponents, #9 Management of Change and #10 Continuous Improvements, are not required for Small Bus Agencies.

All documentation and records will then be kept for a period of three years. Upon request by the FTA or other Federal entity, or a State Oversight Agency having jurisdiction, this documentation will be made available.

(Supporting Document: Shares I: > Safety and Security > PTASP > SPI - SPT) Safety Performance Targets are based on four National Public Transportation Safety Plan (NSP) Safety Performance Measures: 1. Fatalities 2. Injuries 3. Safety Events 4. System Reliability Safety Performance Indicators (SPI) Performance measure or indicator means an expression based on a quantifiable indicator of performance or condition that is used to establish targets and to assess progress toward meeting the established targets.

Safety Performance Targets (SPT) Performance target means a specific level of performance for a given performance measure over a specified timeframe. Our targets will be a reduction percentage that is realistic and attainable within that FY. Within the four required safety performance measures our targets will include the following: •

Totals- for all performance measures

Vehicle Revenue Miles (VRM)- for all performance measures (CATA will use 100k miles)

•

Chargeable- for safety events

•

Non-Chargeable- for safety events

The previous FY Safety Performance Indicators will be the baseline for current FY target reductions.

Reduction targets will be based on a percentage that will be determined by the PTASP Committee during October annual review reflecting our performance by both CATA employees and equipment.

MEMORANDUM OF UNDERSTANDING METROPOLITAN TRANSPORTATION PLANNING RESPONSIBILITIES FOR THE TRI-COUNTY REGIONAL PLANNING COMMISSION

This agreement is made and entered into as of April 25, 2018 by and between the Michigan Department of Transportation (MDOT) hereafter referred to as the DEPARTMENT; the Tri-County Regional Planning Commission hereafter referred to as the AGENCY; and the Capital Area Transportation Authority (CATA), the Eaton County Transportation Authority (EATRAN) and Clinton Area Transit System (Clinton Transit) hereafter referred to as the TRANSIT OPERATORS. WHEREAS, joint responsibilities must be met for establishing and maintaining a cooperative, comprehensive and continuing metropolitan transportation planning and programming process as required by the United States Department of Transportation (USDOT) in regulations at 23 CFR 450 Subpart A -Transportation Planning and Programming Definitions and 23 CFR 450 Subpart C - Metropolitan Transportation Planning and Programming, and WHEREAS, the regulations at 23 CFR 450.314 Metropolitan Planning Agreements require the DEPARTMENT, the AGENCY and TRANSIT OPERATOR(S) to cooperatively determine their mutual responsibilities in carrying out the metropolitan transportation planning and programming process. The responsibilities shall be clearly identified in written agreements among the Metropolitan Planning Organization (MPO) the State and the public transportation operator(s) serving the Metropolitan Planning Area (MPA). The written agreement(s) shall include specific provisions for cooperatively developing and sharing information related to the development of financial plans that support the metropolitan transportation plan and the metropolitan Transportation Improvement Program (TIP) and the development of the annual listing of obligated projects.

WHEREAS, the regulations at 23 CFR 450.104 define public transportation operator to mean the public entity which participates in the continuing, cooperative comprehensive transportation planning process in accordance with 23 U.S.C. 134 and 135 and 49 U.S.C. 5303 and 5304, and is the designated recipient of Federal funds under Title 49 U.S.C. Chapter 53 for transportation.by a conveyance that provides regular and continuing general or special transportation to the public, but does not include school bus or intercity bus transportation or intercity passenger rail transportation provided by Amtrak.

WHEREAS, nothing in this Memorandum of Understanding (MOU) shall limit the legal authorities of the parties.

NOW, THEREFORE, the DEPARTMENT, the AGENCY and the TRANSIT OPERATOR(S) recognize and agree that they will conduct a cooperative, comprehensive and continuing transportation planning and programming process for the Lansing metropolitan urbanized area and that their mutual responsibilities for carrying out this process are described in the following articles: Article 1 Scope of the Metropolitan Planning Process The AGENCY, the DEPARTMENT and the TRANSIT OPERATOR(S) will conduct a performance-based metropolitan transportation planning process that is continuous, cooperative and comprehensive and provide for the consideration of projects, strategies and services that will address the current planning factors as specified in 23 CFR 450.306: Scope of the Metropolitan Transportation Planning Process. The metropolitan transportation planning process will provide for the establishment and use of a performancebased approach to transportation decision-making to support the national goals described in 23 U.S.C. 150(b) and the general purpose described in 49 U.S.C. 5301 (c).

Article 13 Performance-Based Transportation Planning & Programming The Agency will establish performance targets that address the performance measures or standards

established in 23 CFR part 490, 23 CFR part 450, and 49 CFR Part 625. The AGENCY, the DEPARTMENT, and the TRANSIT OPERATOR(S) will coordinate in the establishment of state, AGENCY, and TRANSIT OPERA TOR (S) performance targets. Coordination efforts will include, but are not limited to, sharing available data related to the federally-required performance measures (subject to each agencies data sharing policies and procedures), discussing target setting methodology, establishing performance targets, and reporting on performance targets and progress in attaining targets. The AGENCY will plan and program projects that contribute to the achievement of state, AGENCY, and TRANSIT OPERATOR(S) targets as appropriate. The AGENCY will develop a Congestion Mitigation and Air Quality Performance Plan if required by 23 U.S.C. 149 (1).

(Supporting Document: Shares I: > Safety and Security > PTASP > System Safety Program Plan (SSPP)) SAFETY MANAGEMENT POLICY STATEMENT (673.23) EXECUTIVE SYSTEM SAFETY POLICY STATEMENT AND AUTHORITY CATA strives to build a safe, comfortable, reliable, and innovative transit system that delivers transportation options to our growing region. Safety is a value that affects all levels of CATA activities, including planning, route design and maintenance, equipment repair, facilities construction, and overall operations. All CATA personnel and contractors are charged with the responsibility of insuring the safety of CATA customers, employees, and property.

Every procedure must be safe and follow all CATA guidelines. Shortcuts in safe procedures will not be tolerated. Anyone who observes any unprotected job or situation that may pose a potential threat to a person’s health or safety must inform management adequate precautions can be taken.

IF A JOB CANNOT BE DONE SAFELY, IT WILL NOT BE DONE. It is the guiding principle of CATA to provide protection to all employees and the public from unsafe work conditions and practices. Our people are the most fundamental asset of our operations and their safety must always be considered first when planning any activity. All employees of CATA have the

responsibility to work safely and ensure the same for their fellow workers.

Safety is a constant process, requiring continuous effort for continuous improvement. The key to a safety-first culture is hazard identification by all employees at all levels of the organization.

Communication on safety must be top-down and bottom-up so that its importance is felt at every level of the organization.

The number of hazards identified and the number of hazards mitigated best measures safety. While we cannot quantify accidents/incidents avoided, we can measure the strength of our safety program and culture through audits, inspections, spot observations, and defect reports.

To excel at safety we will have a safety culture that is woven into the fabric of our organization, where everything we do is viewed through a safety lens.

The CATA Safety Department is empowered and authorized to develop, implement and administer a comprehensive, integrated and coordinated System Safety Program Plan (SSPP) that identifies and controls hazardous conditions, provides oversight, evaluates and audits safety systems and provide guidance to managers and directors on matters of safety that affect CATA resources, employees, and customers.

This SSPP is the document that addresses the safety philosophy of CATA and lays out its strategies for an effective safety program designed to reduce the potential for accidents/incidents and increase the efficiency of our operations. This plan describes the methods for identifying and resolving hazards pertaining to the CATA system, its employees and customers, and the public.

This plan establishes accountability for safety and outlines safety policies and goals throughout the organization for operating and maintaining our buses.

The major differences between the SSPP and the PTASP are that the SSPP is a compliance based plan that has 21 fixed safety program elements with the safety supervisor as the responsible party; while the PTASP is a performance based plan with four required safety performance measures with the Accountable Executive as the responsible party.

Authorities, Accountabilities, and Responsibilities Describe the authorities, accountabilities, and responsibilities of the following individuals for the development and management of the transit agency’s Safety Management System (SMS).

The authorized Accountable Executive is the CEO of CATA. The Accountable Executive has ultimate responsibility for carrying out the PTASP as well as the authorities for the Transit Asset Management (TAM) Plan.

The CEO has ultimate accountability and responsibility for: Ø Ensuring transparency in safety management priorities for both the Board of Directors and the authorities employees; Ø Ensuring that the safety management policy statement is appropriate and communicated throughout the agency; Ø Directing the implementation and maintenance of SMS at CATA; Ø Ensuring action is taken, as necessary, to address substandard performance in the SMS; Ø Directing the implementation and maintenance of the TAM plan; Ø Direction over the human and capital resources needed to develop and maintain PTASP, SMS, and TAM plans; Ø Establishing guidance on the acceptable level of safety risk for CATA; Ø Creating and maintaining the Transit Asset Management (TAM) plan for CATA; and Ø Creating and maintaining documents and records related to asset management at CATA. The Accountable Executive may delegate specific responsibilities, but the ultimate accountability for CATA’s safety performance cannot be delegated and always rests with the Accountable Executive.

Chief Safety Officer: The Chief Safety Officer (CSO) reports to and has a direct line of reporting to the Accountable Executive. Has responsibility for day-to-day implementation and operation of CATA’s SMS. The Chief Safety Officer is also responsible for: Ø Monitoring safety performance; Ø Setting and practicing exemplary safety etiquette; Ø Conducting safety audits, and inspections; Ø Performing safety event investigations; Ø Advising senior management on safety matters; Ø Maintaining safety documents and records; Ø Monitoring mitigation activities; Ø Managing the programs under SMS; Ø Collecting and analyzing safety data; Ø Providing safety information and intelligence to line managers and frontline employees; Ø Acting as a conduit for communicating safety from and to departmental/operational managers, frontline employees and executive management as necessary; Ø Reviewing, revising, maintaining and communicating CATA’s safety plans and programs;

Ø Directing hazard identification and safety risk evaluation and/or analysis; and Ø Providing Safety Performance Target reports both indicators and reduction goals for safety performance at CATA. The Chief Safety Officer, along with Directors, Managers, Supervisors and frontline employees, has the authority to stop any activity or process that puts CATA’s employees, guests, and/or patrons at risk of immediate death or injury.

The Training Supervisor coordinates new employee and required annual safety training. Employees exposed to chemicals and/or overexposed to physical agents shall receive training in hazard communication, use and care of personal protective equipment, and hazards and safe handling methods of chemicals.

Annual blood-borne pathogen training is provided for employees who are required to clean up body fluids. Additionally, safety information is distributed through bulletins, newsletters, emails, and postings to provide important information to employees on current topics.

The Training Supervisor is responsible for: Ø Encouraging safe work behaviors; Ø Identifying and halting scenarios that are immediately dangerous to life and health; Ø Developing and performing/conducting driving safety programs including but not limited to, defensive driving training, refresher training, return-towork evaluations, on road operator essential job function evaluations, industrial truck training, and retraining programs for CATA; Ø Development and delivery of new bus operator and maintenance/facilities technician training; Ø Revise, Develop, and deliver Training policies, plans, procedures and best practices for affected employees; Ø Train and test personnel in the proper performance of safety-related operating policies, plans, rules, and procedures for both normal and emergency conditions; and Ø Maintenance of training records and employee training tracking.

Safety Supervisor: The Safety Supervisor has a duty to support and communicate SMS principals, policies and procedures to supervisors, frontline CATA employees and staff. The Safety Supervisor is responsible for: Ø Encouraging safe work behaviors; Ø Identifying and halting scenarios that are immediately dangerous to life and health; Ø NTD and OAR data collection and reporting; Ø Monitoring and reporting concerns from the Employee Reporting Program; Ø Scheduling and participating in safety meetings and training; Ø Monitor unsafe work practices and correcting accordingly; Ø Revising plans and keeping personnel changes current for emergency procedures; Ø Keeping Safety meeting minutes and assigning departmental responsibilities for new concerns; Ø Performing audits and assessments; Ø Participating in the investigation of accidents, incidents and occurrences using SMS principles along with collecting and analyzing data; Ø Monitoring, evaluating and providing feedback concerning safety behaviors to supervisor and front line CATA employees; Ø Directing the collection, retention, and analysis of accident and incident reports;

Ø Forwarding reports of hazards from supervisors and frontline employees, both Physical and potential, to the appropriate department; and Ø Communicating safety initiatives, processes and practices to supervisors, bus operators, department staff and ridership.

Facilities Manager: As one of the keys to SMS success, the Facilities Manager works closely with the Safety Supervisor and Director of Marketing & Customer Experience to remove or reduce hazards in the workplace, especially when it involves facilities, grounds or infrastructure. Management Safety Committee

The CATA Management Safety Committee will review and approve all changes and modifications to the SSPP. Any additions, modifications, or deletions to Standard Operating Procedures (SOP) and to the existing safety configuration of buses, fixed facilities, rolling stock, and equipment are reviewed and authorized by the Management Safety Committee.

Specifically, committee members are responsible for the development and review

of proposed changes to: • Operating rules • Standard operating procedures • Configuration of equipment and facilities • Bus operations training programs • Vehicles The Management Safety Committee includes the Director of Operations, the Director of Maintenance, Safety Manager, and General Manager of Facilities. All members will participate in the evaluation of changes and must reach consensus.

Technical experts may attend committee meetings to clarify proposed changes.

Additionally, the Management Safety Committee evaluates the effectiveness of the SSPP and activities and its implementation through the review of safety audits, major incident or accident investigations, and safety-related statistics to reduce the potential for recurrence of similar incidents and negative trends.

Major safety or security incidents include any incident that has the potential for significant liability for CATA; results in significant, sustained, or unusual disruption of transit service; results in death or serious injuries to persons on CATA premises or vehicles; and/or has potential for high public/media interest or controversy.

M.2

Employee Safety Committee

The Employee Safety Committee is under the direction of the Safety Supervisor.

The committee consists of the Operations Manager, Maintenance Superintendent, Facilities Superintendent, and employee representatives from Maintenance and Operations. The committee discusses security, safety issues, hazards, hazard resolution, equipment, training, communication, and regulations. The committee will also consider proposed changes to CATA’s Safety Program and submit any suggested updates to the Management Safety Committee for approval.

The Safety Committee is required to: Ø Hold monthly meetings Ø Receive reports of injuries, accidents and incidents Ø Hear and make recommendations on unresolved issues of safety or security Ø Receive comments or concerns from employees about safety or security Ø Perform quarterly safety and security facility audits Ø Present proposals for safety initiatives or hazard resolutions to the Management Safety Committee Ø Reporting accidents, incidents, near miss events, injuries from employee groups Ø Directing employee safety reports to the appropriate committee or department for review and mitigation Ø Providing feedback to employee work groups and Ø Recommending safety mitigations as needed.

Accident Review Committee

Accident Review Committee (ARC): The Accident Review Committee consists of three voting members from the operations and/or maintenance departments. Any employee may elect to attend his or her Accident Review Committee hearing and give additional pertinent information to the accident report. The request must be in writing and at the time the accident report is written.

The primary goal of the Accident Review Committee is to promote safe driving, reduce the number of accidents involving CATA vehicles and to assess employee responsibility in an accident. Emphasis will be on training drivers and developing safe procedures. The Accident Review Committee will determine if an accident involving a CATA vehicle is Chargeable or Non-Chargeable for purposes of discipline. An accident is deemed chargeable when two or more voting members of the Committee have determined that the Operator of the CATA vehicle is at fault. In cases where the accident is determined to be Chargeable the board will recommend that appropriate disciplinary action be taken by the department head or supervisor.

ARC Activities include: Ø Reviewing accident reports to determine causation; Ø Analyzing reports to determine patterns or recurring circumstances for each accident on an individual basis; and Ø Recommend methods to mitigate future like events at the authority.

Employee Safety Reporting Program Describe the process and protections for employees to report safety conditions to senior management.

Describe employee behaviors that may result in disciplinary action (and therefore, are excluded from protection).

The employee safety reporting program: is an important source of safety information and is intended to help the Accountable Executive and other senior managers receive information across the transportation authority from frontline employees as well as administrative employees that have a safety concern they would like to be heard. This is a way that gives all CATA employees a way to communicate openly or privately about safety hazards either physical or potential. It is also a way to communicate policies and procedures not working as intended, events that dispatch or management is not aware of, near misses, or possibly why a safety event took place. There will be a follow up to each meaningful report that will be investigated and evaluated by the Chief Safety Officer and then reviewed by the PTASP committee with recommended mitigation. Immediate mitigation will be executed in cases that cause immediate danger to any employee, passenger, or to the public.

Safety Risk Management means a process within a transit agency’s Public Transportation Agency Safety Plan for identifying hazards and analyzing, assessing, and mitigating safety risk.

Hazard Identification, Analysis and Resolution: Hazards are identified through supervisor and frontline employee spot observations and reporting or by means of periodic safety inspections and audits. Current records of inspection items and results are available from the Safety/Training Office.

Hazard analysis may occur within an individual department or during one of the quarterly safety committee meetings. Analysis should include a description of the hazard, supporting test results, documents and/or photos and suggestions for mitigation or resolution.

Once a hazard has been identified and analyzed, it is resolved by first, determining its risk value by using the frequency/hazard table, and then by determining the best method for remediation. Many hazards can be resolved through more than one means, but the general process for determining the best method should be by considering engineering controls, administrative work practices or employee actions, in that order.

Accident Reporting, Investigation and Review: The ultimate purpose for providing in-house management and investigation of work-related accidents, injuries and incidents is to limit injury and damage, identify facts, establish root causes, suggest methods for preventing recurrence, and eliminate or reduce safety risks for CATA’s employees, customers, and the public.

Employee Injury: Injuries are reported by the injured employee, or a witness, to dispatch or his or her immediate supervisor as soon as possible. If the injured employee needs medical attention, the appropriate response by coworkers is to: Ø Assess the injury Ø Call 911 if necessary Ø Notify Dispatch or the Operations Center Ø Begin emergency medical treatment, if willing and able Ø Continue treatment until emergency responders arrive Ø Complete a written report as soon as practical As soon as possible, the injured employee must complete an Employee Injury Report for the human resources department. Please see Appendix D for an example of the Injury Report.

The Safety Supervisor will conduct an investigation to determine the root cause of the incident surrounding the injury. Remedial recommendations may be issued by the Safety/Training office or Safety Committee and will follow normal channels of communication. Investigative resources will include the Employee Injury Report, eyewitness accounts, employee interviews, equipment testing and any other reasonable means to determine root causes. Injury reports will be kept on file for future analysis.

Vehicle Collisions: All vehicle collisions are reported to the operations dispatcher by the operator while still at the scene. Bus operators are instructed to contact dispatch at the time of the accident. In most cases, the vehicle operator and a Street Supervisor complete separate reports. When the collision is minor and is not deemed chargeable by the street supervisor, the vehicle operator may be instructed to continue in service. A written report is completed before the operator’s next show time and may be accompanied by a supervisor’s report. Reports are reviewed by the Operations Manager, who determines preventability.

Reports may also be reviewed by Safety/Training Office, and the ARC. Copies of the accident/incident reports and a summary are kept for review and reporting as necessary.

Monitoring the accident log, and hazard /near miss log, will be the best measurement tools to identify the safety risk mitigations that are ineffective, inappropriate, or not working as intended. These logs are monitored and updated as needed by the safety supervisor which gives immediate feedback to identify safety risk mitigations and their effectiveness or ineffectiveness. Spot observations on the routes are a real time feedback source for these mitigation measures as well. These findings will be evaluated and revised through the quarterly safety meetings, ARC meetings typically conducted weekly on Wednesdays, or the PTASP meetings held quarterly.

Describe activities to conduct investigations of safety events to identify causal factors.

Initially the event will be investigated by the responding supervisor who then issues an accident report describing the event and their findings. The event will then be entered into the accident log which is maintained and updated on a consistent basis by the safety supervisor.

Other ways to identify causal factors is to conduct real time spot observations, monitor buses in real time or in playback with Novus, Accident Review Committee (ARC), and we can also use the employee reporting system as a tool to identify and mitigate causal factors to be discussed during PTASP committee meetings in order to determine percentage reductions for CATA’s SPT goals.

Describe activities to monitor information reported through internal safety reporting programs.

The information will be received and entered into a Hazards/Near Miss log along with the date, time, location, and description including one or more potential mitigation processes for that particular hazard or near miss. The information collected will then be reviewed at all safety meetings and PTASP meetings

to determine mitigation or measure the effectiveness of the current mitigation process. The findings of these activities will then be communicated with the authority through the appropriate conduit, where all employees will be notified and appropriate changes will be made.

Driver Training and Testing Operations implements new and on-going operator training. Driver candidates must complete six weeks of safe coach operation, practices, and procedural training. Five of these weeks are in the classroom and one week is conducted on the road overseen by an experienced operator. Written tests are administered and candidates are required to pass with a minimum score of 80%. Among the items students learn in their six weeks of training include: •

CATA operational policies and procedures;

Bus and equipment familiarization and inspections;

•

Basic operations and maneuvering;

•

Boarding and alighting passengers;

•

Operation of wheelchair lift and other special equipment;

All operators are instructed on operational and safety rules and procedures described in CATA’s SOPs, New Operators Training Manual, and CATA work rules.

Operations also runs a re-qualification program to refresh an operator’s knowledge and understanding of basic skills and agency work rules through its Annual All-Operator Training Program. Currently there is no requirement to conduct milestone training at specific intervals (e.g. 5 or 10 years). Re-qualification may be considered for an operator after a chargeable accident as a measure to strengthen skills.

Recommendations by the ARC may result in an operator either being recommended or required to participate in the re-qualification program.

Operations retain all operator training documentation and evaluations as per Board Policy 207.

The Safety Manager, as part of the Internal Safety Review process, makes independent assessments of Operations activities. Issues identified will be discussed and reviewed with the Director of Operations.

J. RULES AND PROCEDURESJ.1 Rules and Procedures Policies, rules, and procedures are reviewed and analyzed to ensure they provide for the safe operation of the bus system in normal and emergency conditions and to ensure compliance with appropriate governing bodies. Additions to or deletions from these documents, which impact the safe operation of the system, are coordinated through the Management Safety Committee and other affected departments for approval prior to implementation.

Operational Training and Qualification

J.2.1

Initial Training

All safety-sensitive Operations and maintenance personnel are initially instructed on the operational and safety rules and procedures and are provided with Standard Operating Procedures, Section 6 of the New Operators Training Manual, and CATA work rules.

J.2.2 Supplemental Training Qualified Trainers teach all new Bus Operators. The training process includes performance testing. Bus Operators may be given supplemental training, based on accidents or reported behaviors. Complaints (such as notification of speed violations) may trigger additional investigation and operator requalification.

In addition to this requalification process, any operator, supervisor, or controller may be required to take supplemental training based on their performance record.

Human Resources will retain all training documentation and evaluations. Records of accidents and rule violations are entered into the Incident Database system as they occur. Managers will address any issues with the operators and work to correct any problems. They may be discussed with the operator during re-qualification or the manager may take immediate action, based on the situation.

J.3 Operational Safety Compliance The Operational Safety Check Program serves as the foundation for observing, correcting, and documenting safety-related behaviors and activities. The program is also used to reinforce positive safety behaviors.

Street Supervisors and Trainers are responsible for conducting periodic bus operations safety checks.

Supervisors will ride along to observe and evaluate adherence to rules, policies and procedures, as well as verbal or written instructions and special instructions. An operational safety spot observation check is made of each operator at least annually. Operator safety checks are recorded on “Check Ride Forms” and entered into database with a hardcopy to the managers for follow up as needed.

Operators are observed for fitness-for-duty at this time as well. Additionally, Street Supervisors observe Operators to ensure compliance with rules, procedures, work permits, operational orders, and special instruction requirements.

A trend analysis of all operational safety check activity is performed and submitted to the Management Safety Committee for quarterly review. The results of the analysis are then used to revise training and safety programs as warranted.

J.4 Motor Vehicle Records Operator Motor Vehicles license checks are made by Operations during initial hire, bi-annual checks and when notice o[f a violation is received from the Secretary of State.

K. TRAINING AND CERTIFICATION PROGRAM The Safety/Training Department coordinates new employee and required annual safety training.

Employees exposed to chemicals and/or overexposed to physical agents shall receive training in hazard communication, use and care of personal protective equipment, and hazards and safe handling methods of chemicals. Annual blood-borne pathogen training is provided for employees who are required to clean up body fluids.

Additional, safety information is distributed through bulletins, newsletters, emails, and postings to provide important information to employees on current topics.

K.1 Training New Operators must complete six weeks of training concerning safe coach operation, practices, and procedures. Five of these weeks are classroom and road experience and the sixth week are on the job training with oversight by an experienced Operator. Written tests are administered requiring the operator to pass with a minimum score of 80%. Generally, three of the six weeks are spent in actual road operation with a trainer.

The Operations Training Department maintains a complete curriculum.

Re-Qualifying Operators are afforded time to replenish and refresh their skills at the as per Work Rules and during the Annual All Operator Training program. Currently there is no requirement to conduct milestone training at specific years such as 5 and 10 years. Re-Qualifications may be considered after the first or second chargeable accident and is considered skill strengthening. Mandatory re-qualification is required on the third offense. In addition, any operator may be recommended for re-qualification by Operations or Safety. Re-qualification is generally the result of an accident occurrence. This requalification is aimed at strengthening identifiable skill weaknesses and may be in the classroom or in actual road operation of the vehicle.

Operators that have been off work for greater than 30 days must receive a re-qualification by a Street Supervisor.

Safety training is conducted on CATA equipment using established operating rules and procedures.

Copies of SOPs and rules are given to all CATA personnel. Training programs, SOPs, and rules are developed by the appropriate department and submitted to the Management Safety Committee for review and approval.

Operations and Maintenance personnel are not currently required to pass an annual examination of bus operation safety rules and procedures. However, if an annual examination program is adopted, each person who fails the annual examination is given re-qualification. The preparation, administration, and

maintenance of these qualification examinations and related records are the responsibility of the respective department.

Performance Measure or Indicator

Performance Target

A specific level of performance for a given performance measure over a specified timeframe.

Root Cause Safety Hazard

The basic condition that leads to an accident or incident. The root cause does not always produce accidents and injuries, but does produce an environment where accidents and injuries become more likely to occur.

Safety Risk Assessment The methods or processes to assess the safety risks associated with identified safety hazards.

Work Injury

The methods or processes to identify mitigations or strategies necessary as a result of safety risk assessment.

The strategic and systematic practice of procuring, operating, inspecting, maintaining, rehabilitating, and replacing transit capital assets to manage their performance, risks, and costs over their life cycles, for the purpose of providing safe, cost-effective, and reliable public transportation, as required by 49 U.S.C. 5326 and 49 CFR part 625.
