Flu Vaccination Service gets the green light Community pharmacy contractors can now access the necessary paperwork to begin providing the national Flu Vaccination Service. Here we highlight the process to follow to make sure you are ready to offer the service.

Following the publication of the service specification, Patient Group Direction (PGD) and Secretary of State Directions, the Community Pharmacy Flu Vaccination Service launched on Friday 1st September.

Community pharmacy teams across England have been working hard to ensure they fully meet the requirements for provision of the service and, subject to vaccination delivery arrangements with their supplier, they can now offer flu jabs to those in eligible groups.

Alastair Buxton, Director of NHS Services at PSNC, commented: “I hope that pharmacy teams will really get behind this important service, like they did last year, to show patients and the NHS just what community pharmacy is capable of.”

Getting ready to provide the service For any contractors who are still getting ready to begin providing NHS flu vaccinations, here’s what you need to know.

You MUST:

Have a Standard Operating Procedure and a needle stick injury procedure in place.

Ensure your consultation room meets the service specification requirements.

Notify NHS England of your intention to begin providing the service (complete form on NHS BSA website).

Ensure all pharmacists involved have completed the Declaration of Competence and a signed copy of the PGD.

Advise those pharmacists that they should consider being vaccinated against hepatitis B.

Print copies of the Patient Questionnaire.

Have made arrangements for the removal and safe disposal of any clinical waste generated by the service.

You SHOULD:

Ensure all members of the team have reviewed the paperwork for the service.

Ensure they have reviewed the IT platform for recording patient questionnaire details.

Print copies of the Record & Consent Form and GP Practice Notification Form (unless you are using a web-based platform for these).

Promoting flu vaccinations PSNC has updated its patient communication materials and launched a toolkit for LPCs to help with the promotion of this year’s national Flu Vaccination Service.

To support community pharmacy teams in matching the incredible success of last year’s Flu Vaccination Service, PSNC has once again made available a wide range of resources to help with communications and marketing. There are posters, flyers and letters to encourage those who are eligible for the NHS flu vaccination to speak to their local pharmacy team for more information.

Producing your own materials Community pharmacy contractors are reminded that if they decide to develop their own marketing materials to promote the service they must ensure they comply with the requirements of the Terms of Service relating to promotion of services funded by the NHS.

Whilst many of the resources were available in previous years, they have all been updated to relate specifically to this year’s service specification. All the resources can be found at: psnc.org.uk/ flupatientcomms

In addition, if contractors choose to use the NHS identity on their marketing materials, they must ensure that the revised guidance (published in January 2017) is adhered to when producing, updating or replacing designs. Full details are available from: psnc.org.uk/nhsidentity

PSNC has also worked with those who are fluent in other languages to create translations of our posters. Currently we have posters in Arabic, Gujarati, Hindi, Polish, Portuguese, Punjabi and Urdu. PSNC and pharmacyTALK have published a video providing guidance for community pharmacy teams on promoting the Flu Vaccination Service. It’s the first in a new monthly series called #PSNCtalk.

The series aims to help those who work in community pharmacy by sharing guidance and offering advice, as well as making pharmacy staff aware of the vast range of PSNC resources available and where to find them.

Community pharmacy contractors must notify NHS England of the intention to provide the Flu Vaccination Service by completing the online notification form available. If the form is not completed, claims for payment will not be accepted by the NHS BSA. Complete the form via: psnc.org.uk/flunotification

Payment claims for flu vaccinations administered must be made by completing the Flu vaccination claim form 2017/18 [available from psnc.org.uk/ flu] at the end of each month. Contractors must submit this to the Pricing Authority with their script bundle submission.

PSNC recommends that contractors keep a copy of their monthly claim forms so these can be used to compare with the amount shown in the FP34 Schedule of Payments. Payments relating to the Flu Vaccination Service will appear under the ‘Details of other amounts authorised’ section, labelled as ‘Flu Reimbursement Payment’ and ‘Flu Remuneration Payment’.

Pharmacy Minister pledges to make best use of sector In his first speech as Minister with responsibility for pharmacy, Steve Brine MP outlines plans for the future which seem to be more positive than recent approaches to the sector.

Best use has not so far been made of the “tremendous skills and knowledge that pharmacists and pharmacy teams have”, pharmacy minister Steve Brine MP has said.

During his first speech to the sector this month, Mr Brine told the Royal Pharmaceutical Society’s Annual Conference that he and his colleagues were “determined to correct this”.

The Minister went on to praise the work of Healthy Living Pharmacies (HLPs) and the success of last year’s Community Pharmacy Flu Vaccination Service, noting the role that pharmacies are playing in keeping people healthy and reducing health inequalities.

Mr Brine also confirmed his commitment to seeing legislation on dispensing

errors in community pharmacy laid, and he said that the Murray Review’s recommendations would help shape the Government’s efforts to optimise the skills of pharmacists and pharmacy teams.

The Minister has also this week set out his intention to work with PSNC to achieve fair, realistic and sustainable funding arrangements (in response to a letter from the editor of Chemist+Druggist).

PSNC Chief Executive Sue Sharpe said: “We are very encouraged by this indication from the Minister of a more positive approach to community pharmacy than we have seen in recent years. The network of community pharmacies is highly valued by all those who use it; but there is a great deal more that can be done, particularly to build team working with our colleagues in

Are you meeting NHS Identity guidlines?

Community pharmacy contractors are reminded that if they choose to use the NHS Identity they must ensure that the revised guidance is adhered to when promoting NHS services, for example, if developing marketing materials to promote the NHS Flu Vaccination Service. The NHS Identity should not be used when promoting non-NHS (private) services.

When the revised guidelines were published, NHS England advised that implementation of the updated NHS Identity guidelines could be gradual; existing offline materials could continue to be used until they needed to be replenished or updated. Contractors, however, are advised that they should aim to update existing digital channels, such as websites and social media accounts by January 2018.

Community pharmacy needs certainty for the future and sustainable funding levels, and we are pleased to note that the minister has acknowledged this.

Funding models must value the services that pharmacies provide in their local communities and protect provision in areas of greatest need. I look forward to working with the Minister and Department of Health officials to achieve this and to repair the damage of the past two years so that we can ensure that the skills of community pharmacists and their teams are properly utilised to support the NHS and patients. Whilst it is of course disappointing that two years progress has been lost, we must all now focus on the future.”

Reminder on whistleblowing – action required Community pharmacy contractors are reminded that they should review and update existing whistleblowing policies and procedures by the end of September 2017.

As part of the review, contractors have to name an individual as the Freedom to Speak Up Guardian who can ensure that policies are in place and that staff know who to contact if they have a concern; this person must be independent of management within the pharmacy and not the direct employer.

PSNC has updated its briefing on whistleblowing (PSNC Briefing: 020/17: PSNC’s guidance to contractors on whistleblowing) which now includes a template whistleblowing policy, if required, and guidance on the Freedom to Speak Up Guardian.

Are you ready for Quality Payments?

The November review point for the Quality Payments Scheme is fast approaching; make sure you meet the gateway criteria Editing or validating your NHS Choices profile For the November Quality Payments review point community pharmacy contractors are required, as part of the gateway process, to edit or validate three parts of the pharmacy’s NHS Choices profile by 11:59pm on 24th November 2017.

Contractors are reminded that they must edit or validate the three sections of their NHS Choices profile to meet the gateway criterion: • their opening hours; • the facilities the pharmacy provides e.g. consulting room, parking etc; and • the services the pharmacy provides e.g. Medicines Use Reviews (MURs), New Medicine Service (NMS), etc.

Even if all three sections are correct or contractors have recently edited or validated their NHS Choices profiles, contractors will still need to log into their profile and validate this information during the above time period.

Any contractors that do not currently have NHS Choices editing rights for their pharmacy profile can apply on the NHS Choices website (www.nhs.uk/web-editor). If a contractor has simply forgotten their password they can request a new one from the login page of the NHS Choices website.

Since distance selling pharmacies (DSPs) do not, currently, have full NHS Choices entries, DSP contractors should refer to the PSNC Briefing (ow.ly/pzNn30ePW6i) which details the process contractors

PHE identifies public health opportunities in pharmacy Public Health England (PHE) has published a report, Pharmacy – A Way Forward for Public Health, which sets out opportunities for commissioner and provider-led action at a local level to realise community pharmacy’s key role in improving health and reducing health inequalities.

The publication looks at the commissioning landscape and the emergence of Healthy Living Pharmacies, and examines public health interventions that are, or could be, delivered through community pharmacies. What to do if you haven’t got a pharmacy NHSmail account Having an NHSmail account for your community pharmacy is not only a gateway criterion for the Quality Payments Scheme, but it is also necessary for any contractor that wishes to provide the NHS Urgent Medicine Supply Advanced Service (NUMSAS).

If you have applied for a shared NHSmail account, but have not yet had one allocated to your pharmacy, it is important that you act now.

NHS Digital emailed those affected recently to explain how to get their account set up. The email asks contractors to complete the process for setting up a shared NHSmail account by providing some information on the NHSmail registration portal. The portal can also be used by pharmacy contractors who have not previously applied for an account. (https://portal.nhs.net/pharmacyregistration) If your pharmacy is part of a multiple group, you may want to check how your head office is managing the process before you use the portal to setup an NHSmail account.

Pharmacists’ experiences of providing MURs sought Community pharmacists who work in England are being invited to participate in the Pharmacists’ and Patients’ Experiences of medication Reviews (PaPER) study conducted by the University of Bath.

The current phase of the PaPER study aims to find out more about community pharmacists’ views on Medicines Use Reviews (MURs), including those that may occur after a person has been in hospital.

A questionnaire has been created to help discover what it is like to provide the MUR service and the factors that affect the provision of the service post-discharge. The short anonymous questionnaire will be available until Monday 2nd October 2017 at: ow.ly/Xz9N30eVbYc

Medicines optimisation service could save over £15k per year A locally commissioned medicines optimisation service has identified more than £15,000 of savings for the NHS within eight weeks of starting.

A medicines optimisation service in Bath and North-East Somerset (BANES) has identified £15,421 of annualised savings in just eight weeks.

Community pharmacists working at five pilot pharmacies identified patients from prescriptions received by the pharmacy and, following a consultation with the patient, recommendations for changes to their medicines were made.

These recommendations were recorded on PharmOutcomes and sent to the patient’s GP practice to be actioned by a Clinical Commissioning Group (CCG) pharmacist.

Currently 120 medicines optimisation opportunities have been identified by community pharmacies with 69 (£9,396 of savings) so far having been actioned by the CCG pharmacists. The average annual saving per actioned suggestion is £136 (£9,396 / 69).

Areas identified for potential medicines optimisation have included: • removal of unwanted medication; • reduction in prescription quantities to synchronise with other medicines; • changing formulation; and • reduction in prescribing frequencies to synchronise with other medicines.

New ‘acid’ attack guidance published by NHS England Community pharmacy teams should be aware of new ‘acid’ attack advice and stakeholder briefing developed

by NHS England.

NHS England has launched new guidance and information for the public on acid attacks, supported by the British Burns Association and the British Association of Plastic, Reconstructive and Aesthetic Surgeons.

A new page has been created on the NHS Choices website – www. nhs.uk/acidburns – containing information on what to do as a victim or a bystander immediately following an attack with a corrosive substance, as well as how the NHS treats injuries caused as a result.

A complementary stakeholder briefing has been published, which details information available for the public, how you can help and

how NHS urgent care services help victims.

NHS England has also endorsed an infographic produced by burns specialists giving three clear and memorable actions to take – Report, Remove, Rinse – which can be shared via social media.

Improving health outcomes for young people: The Local Government Association has published a report on how health inequalities facing young people can be reduced. Evaluating a new model of primary care: The Nuffield Trust has published an evaluation report of the National Association of Primary Care’s Primary Care Home (PCH) model, which looks at how sites can make early progress with implementing and evaluating their local PCH models and potential challenges.

Pregabalin price concessions PSNC raises significant concerns about pregabalin prices with the Department of Health following its move into Category M of the Drug Tariff.

Following the expiry of the final pregabalin patent and NHS England guidance published in July 2017, pregabalin was switched to Category M from 1st August 2017.

PSNC routinely monitors contractor margins and the availability of medicines at Drug Tariff prices, and we raised concerns about pregabalin and the need for a managed transition to Category M with the Department of Health (DH). The entry prices for pregabalin into Category M were lower than current market prices, and PSNC received hundreds of reports from contractors about the availability and pricing of pregabalin, warning that it has not been available at the Drug Tariff prices.

Price concessions were granted for all strengths in August, but

community pharmacy teams should be aware that concessions only apply for the month in which they are granted. PSNC has once again applied for price concessions for September and, as CPN went to press, discussions were still ongoing. We will share more information regarding pregabalin prices for September via our website and email newsletters as soon as it is available.

Contractors are reminded that concessions granted at any point during the month are retrospective for the whole month.

Ask PSNC The PSNC Dispensing and Supply Team provide pharmacy teams with support and advice on a range of topics related to the Drug Tariff and reimbursement. Questions asked in recent weeks have included: Q. If a prescriber has not endorsed ‘ACBS’ on an item listed in Part XV of the Drug Tariff, can I dispense it?

A. Yes. If an item found in the Borderline Substances list (Part XV of the Drug Tariff) has not been endorsed by the prescriber, this product can still be dispensed and it will be passed for payment as it isn’t a compulsory requirement.

However, the prescriber may be asked by their Clinical Commissioning Group (CCG) to justify why the product has been dispensed at NHS expense. Pharmacy staff should not add the ACBS endorsement.

Q. I have received a lilac FP10P prescription asking for 150 Paracetamol tablets, is this allowed?

A. It depends on the type of nurse prescriber. The Drug Tariff only permits a community practitioner nurse prescriber to prescribe Paracetamol in quantities up to 100 (see Part XVIIB(i) of the Drug Tariff) so they cannot prescribe this amount.

However, nurse independent prescribers are not limited in the quantity of Paracetamol that they can prescribe. This type of prescriber would have the words “nurse independent/ supplementary prescriber” printed onto the prescription forms they use.

Q. I have received a lilac FP10P prescription for Phenobarbitone annotated “Nurse Independent/ Supplementary Prescriber”, is this allowed?

A. Yes. Phenobarbitone is a Schedule 3 Controlled Drug. Nurse independent prescribers are able to prescribe any Schedule 2, 3, 4 or 5 Controlled Drug (except diamorphine, dipipanone or cocaine for the treatment of addiction), so whether the nurse is an independent or supplementary prescriber does not matter as both are able to prescribe this drug.

Scam calls targeting pharmacies

PSNC has been notified that some community pharmacy contractors have recently been called by individuals falsely claiming to be from the NHS BSA investigations team.

The Pricing Authority produces a quarterly newsletter called “Hints & Tips for dispensing contractors”.

The unsolicited callers are quoting the ODS code (F code – which can be found in various reports published online by the NHS), company directors’ names (which can be obtained from Companies House online information) and other pharmacy details when calling and they are stating that there has been a problem with payments to the pharmacy. They have then gone on to request personal/business details from the contractor, such as their mobile telephone number, date of birth, address and business bank account details.

We would like to draw your attention to the latest edition (Issue 28) which contains some really useful information and advice regarding: • Amending endorsements; • Referred backs (returned items); • P roducts now available as licensed medicines; • Endorsing FP10MDA forms correctly; and • Ensuring that EPS messages are submitted on time.

An initial phone conversation may be followed up by a text message purporting to be from the contractor’s bank, asking to call a specific phone number. If this number is called, it provides another opportunity for the individuals to obtain details which they can use to access the contractor’s business bank account.
