A MESSAGE FROM OUR CEO There are many lessons each of us has learned through our individual and collective experiences in the fight against COVID-19, but the one takeaway that stands out in my mind is that Family Makes Us Stronger. I saw this lesson in action when our family of providers came together on the issue of COVID-19 vaccination to ensure we’re providing the safest possible care environment for employee partners and patients alike. And, I continue to see examples of this daily in the interactions we have with patients who entrust PruittHealth with their care and well-being. We’ve made significant strides in the fight against COVID-19, and our organization continues to evolve to build upon our legacy that already spans more than a half century. However, like so many businesses across the country, the global pandemic left our family-run organization faced with financial hardships resulting in a $260M loss over the last 18 months. Decreased admissions to skilled nursing centers and chronic Medicaid underfunding also are contributing factors, and in the following pages, we hope to shed some light on what we can do to preserve access to care for one of the most vulnerable patient populations while emerging from the pandemic. 2021 AT A GLANCE Since the start of the pandemic, there have been more than 2,550 skilled nursing and assisted living employee partner COVID-19 cases at PruittHealth. Still, our caregivers remain unwavering in their commitment to protecting one of the most vulnerable patient populations in the United States. Not only did they rise to the occasion to meet the varied needs of patients, families, and colleagues alike, they often served as the bedrock of their own families, and we set out in 2021 to reward them for their heroic efforts in the fight against COVID-19. Similarly, a total of 5,717 patients in skilled nursing and assisted living centers at PruittHealth have battled COVID-19 since March 2020. Thousands recovered, but too many lost their lives. This is why the PruittHealth family of providers – nearly 100% of whom are vaccinated for COVID-19 – works daily to educate patients and families on the importance of vaccination and to fight misinformation about COVID-19. More than 80% of our skilled nursing and assisted living center patients and residents are fully vaccinated. The global pandemic isn’t the only challenge facing the PruittHealth family of providers currently. Since March 2020, the skilled nursing center occupancy rate at PruittHealth decreased by 19%, putting thousands of heroes’ livelihoods at risk. In the same time period, we saw a 19% increase in utilization of home health services at PruittHealth. This signals that patients and their families are opting for home health care whenever possible, presenting a unique opportunity for PruittHealth to meet the ever-changing demands of today’s health care consumers.

THE FINANCIAL TOLL OF COVID-19 For the second year in a row, the global pandemic left our family-run business of 50+ years faced with financial hardships resulting in a $260M loss in revenue from March 2020 through December 2021. Decreased admissions to skilled nursing centers and chronic Medicaid underfunding continue to plague the PruittHealth family of providers today. An independent audit conducted by the highly recognized advisory firm Mauldin & Jenkins found Medicaid underfunded $4.8M in Florida, $58M in Georgia, $15.6M ($6.7M profit with add-ons) in North Carolina, and $25M in South Carolina.

Despite these losses, we take every opportunity to realize efficiencies by being good stewards of available resources. PruittHealth designed and implemented a new operating system to streamline distribution of medical supplies and unveiled a new distribution center. The new center, which cost $6.1M to build, provides much-needed storage and affords our company the ability to triple available inventory. Today, we stock up to nine months of personal protective equipment at any given time, thanks to this investment. Another major issue the PruittHealth team set out to address in the last 18 months centered on the challenges associated with COVID-19 testing. Because there were limited Point-of-Care (POC) tests available and it was very difficult to get polymerase chain reaction (PCR) testing completed across our organization from the start, our team initiated plans to begin testing in-house. We invested $250,000 in Cepheid GeneXpert PCR systems and implemented testing in February 2021. Since the start of the pandemic, 12,800 PCR tests have been completed, and this capability has proven critical in the fight against COVID-19 with improved response times on completed tests and access to real-time data on positivity rates to inform decision making at all levels of the organization. Currently, construction is underway for a high-complexity lab as we move to take our in-house diagnostic services to the next level to optimize the 1M+ labs performed at PruittHealth skilled nursing centers across the Southeast each year.

GIVING IT OUR ALL: VACCINATING AGAINST COVID-19 At PruittHealth, we take our commitment to protecting those we serve seriously. We believe that one life lost to COVID-19 is too many, and as caregivers, we have a responsibility to look out for the health and well-being of our patients, their families, and the communities we serve. We recognized early on in 2021 that one of our first challenges would be our ability to take a leading role in getting our communities vaccinated. While we knew this wouldn’t be a task we’d undertake alone, we also knew many would look to us to set an example for others, and therefore, we set out to incentivize our family of providers to get vaccinated with: •T he Protect Bonus, which granted all full-time employee partners $100 when their health care center achieved a 90% vaccination rate •T he Hero Bag, a goodie bag inclusive of a Waffle House gift card and a PruittPride bracelet, which each partner received at the time of vaccination •C ash Raffles, a six-week cash raffle series in which one vaccinated partner was chosen per week; each winner received an additional $400 per pay period for six months, totaling $5,200 •V accinated Caregiver Bonus, which granted a one-time bonus ranging from $250-$1,500 for LPNs, RNs, and CNAs who received their COVID-19 vaccine •S tronger Together Grand Prizes, a drawing for vaccinated partners that awarded 106 winners prizes ranging from $1,000-$25,000

In all, our efforts to incentivize COVID-19 vaccination and retain our talented and mission-driven workforce totaled $23.4M since March 2020. This figure includes the amount PruittHealth paid out in bonuses and extra paid time off (PTO) awarded to employee partners for getting vaccinated. Today, nearly 100% of PruittHealth’s family of providers is fully vaccinated for COVID-19, the only exceptions being a limited number of approved medical or religious exemptions. As an added measure signaling our implicit trust in the science developed to protect the global population, PruittHealth became one of the first in the post-acute care profession to implement a policy mandating COVID-19 vaccination as a condition of employment.

INSTILLING FAITH THROUGH STORYTELLING Last year, thanks in large part to the positive media coverage surrounding our caregivers’ efforts to lift the spirits of the patients and communities we serve, PruittHealth’s family of providers was introduced to the world as a group of compassionate people who live with faith and love in our hearts. Time and again, we set out to lead with light and positivity in a world full of despair and are committed to continue doing so in the future. This year, we upheld that commitment by partnering with local restaurants to surprise staff and giving back to area shelter dogs that once again surprised our patients – this time, for a special trick-or-treat event this fall. Recognizing the growing lack of trust in health care and traditional media, we expanded our own storytelling efforts in 2021 to focus on important quality initiatives and bring consumers along on their health education journey. Additionally, we celebrated top talent whose highquality caregiving earned prestigious, third-party awards, such as LPN of the Year, and reinforced the organization’s commitment to elevating the level of care provided in communities we serve by recognizing centers that attained quality awards and accreditations. As of Dec. 2021, 92% of PruittHealth centers have earned The Joint Commission’s Gold Seal of Approval® for Nursing Care Center Accreditation, and more than 70 centers have been awarded a Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL).

WHERE WE GO FROM HERE Our family-run business has survived for more than 50 years because of our partners’ collective commitment to putting patient care front and center of all they do, while growing to meet patients’ ever-changing needs. In order for our partners to continue doing so, we first must address ongoing staffing challenges that plague the post-acute care profession and barriers standing in the way of bringing to life a nationwide effort to develop the workforce and uplevel our profession. Next, we must resolve once and for all the issue of chronic Medicaid underfunding, which cost the PruittHealth family of providers more than $103.4M.
