At PruittHealth, we have spent more than 50 years caring for our neighbors in hundreds of communities throughout the Southeast. We value the trust they put in us each day, and we remain committed to providing highquality services to all our patients and residents.

We continue to build back the business after the devastating, industry-wide effects of a global pandemic. While our sector lost almost 250,000 jobs, we invested in our talent acquisition structure, building creative methods to attract and retain skilled caregivers. We leaned into technology, offering the My PruittHealth app that allows family members to follow their loved ones’ health journey even when they cannot be together. We continued to grow, taking advantage of the financial market’s impact on some providers. Also, we prepared to break ground on our $385 million active senior living and continuing care community, The Oaks at Whitaker Glen.

While we have made great progress, rising health care costs and Medicaid underfunding continually impact our industry. Additionally, a proposed federal staffing mandate further threatens care, particularly in rural locations where access to health care is already limited.

THE FUTURE OF SENIOR LIVING

The Oaks at Whitaker Glen is nestled in the heart of Raleigh, North Carolina. This active senior lifestyle community is a distinctive collection of homes and amenities being built with special attention to detail and surrounded by lush, communal spaces

Once complete, this $385 million, multi-phased campus will feature nearly 250 luxury, one and two bedroom, independent living units, 82 assisted living apartments, and a state-of-the-art, 106-bed skilled nursing center.

Amenities will include:

SKILLED NURSING & REHABILITATION

At PruittHealth, our skilled nursing services help patients through every challenge. Our experienced staff delivers advanced care in specially designed centers that feature the latest technologies.

Our expert staff delivers high-quality care at our skilled nursing centers on an inpatient and outpatient basis, depending on patient needs. Services are personalized to our patients, from dayto-day recovery and wellness monitoring to specialized rehabilitation regimens.

Our patients include those recovering from planned surgeries and serious emergencies, such as stroke or heart attack, as well as those with chronic disease and/or pain, such as Alzheimer’s, diabetes, heart, and respiratory illnesses.

HOME HEALTH

PruittHealth @ Home allows patients to return home and continue healing with nurses, therapists, and support teams who facilitate their progress and monitor their well-being. Our expert caregivers support recovery from illness, injury, or surgery in the comfort of patients’ homes.

Our home health teams plan regular in-home visits and telehealth virtual visits to ensure comprehensive monitoring and consistent treatments to get patients back to the activities they love most.

Our Services Include:

• Development of individualized care plans

• Diagnosis and discipline-specific clinical pathways

• Point-of-care technology and therapies, including ultrasound, electronic stimulation, balance training, fall prevention, skilled nursing services, and certified nursing assistance, as well as physical, occupational, and speech therapy

Home Health patients must be age 18 or older and can be referred to PruittHealth by a variety of provider sources, including a hospital, skilled nursing center, primary care physician, same day surgical center, oncology treatment center, or others, for as long as the care is medically necessary for the patient’s recovery.

Care falls into several categories: recovery care, rehabilitative care, chronic issue management, and specialized care.

HOSPICE & PALLIATIVE CARE

PruittHealth Hospice & Palliative Care provides tailored services to support patients and their families, delivered in-home or at a skilled nursing, assisted, or independent living location – wherever the patient calls home.

HOSPICE CARE

Our family-focused approach to end-of-life care helps patients live their final days well cared for, comfortable, and surrounded by their loved ones. Typically, hospice care serves individuals who have decided to stop pursuing treatments that could cure or control their condition and who have a life expectancy of six months or less.

With a team that includes nurses, social workers, certified nursing assistants, chaplains, and volunteers, PruittHealth delivers individualized hospice care 24 hours a day, 7 days a week in the setting of the patient’s choice.

Hospice care does not hasten or postpone death. Instead, it prioritizes caring for the person, rather than the disease, with a focus on quality of life. Hospice is not a place; it is a specialized service approach to end-of-life care. Our hospice care is always family-centered, involving the patient and family in the decision-making process.

PALLIATIVE CARE

Palliative care is specialized medical care for people living with a serious illness. Focused on providing relief from the symptoms and stress of the illness, the goal is to improve quality of life for both the patient and the family.

Palliative care is provided by an interdisciplinary, specially trained team of doctors, nurse practitioners, nurses, social workers, and chaplains who work together with the patient’s other doctors, therapists, and specialists to provide an extra layer of support.

Palliative care is based on the needs of the patient, not on the patient’s prognosis. It is appropriate at any age and at any stage in a serious illness, and it can be provided along with curative treatment.

Common referrals to palliative care include those diagnosed with:

• Cancer

• Congestive Heart Failure

• Dementia

• COPD/Lung Disease

• Brain Disease, such as ALS or Parkinson’s

• Stroke

• Kidney Failure

• Liver Failure

• HIV/AIDS

• Multiple Sclerosis

Both palliative care and hospice care provide comfort. However, palliative care can begin at diagnosis, and continues at the same time as treatment. Hospice care begins after the treatment of the disease is stopped and helps ensure the patient’s comfort at the end of life.

SENIOR LIVING

At PruittHealth, our approach to senior living uniquely aligns with our senior care expertise. CARE MANAGEMENT

PruittHealth Home First

Supplemental Security Income (SSI) and active full Medicaid or pending Medicaid coverage.

Our program is designed to ensure members’ safety and improve their overall health and well-being. We offer adult day health, emergency response, home meal delivery, support services, and much more to meet and exceed individual health care needs and expectations.

Program Requirements:

• 65 years old or older and/or disabled

• Receiving Medicaid and SSI

• Medical conditions, such as diabetes, high blood pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke, which require monitoring and regular doctor’s care

PERSONAL CARE

PruittHealth Family First offers a concierge care program for personalized, in-home services to help support families, while providing both patients and their primary caregivers with added flexibility and peace of mind. Clients benefit from expert care and compassionate guidance and are treated as extended members of the PruittHealth family.

Our Services Include:

• Personal care, such as bathing, dressing, feeding, grooming, and medication management

• Exercise and walking assistance

• Respite care for primary caregivers in need of some relief or planning a well-deserved break

• Custom care, including appointment scheduling, home management, companionship, nutritional assistance, light housekeeping, and more

PHARMACY

PruittHealth Pharmacy Services ensures our customers receive their medications in the most convenient and secure way possible, providing individualized care with the highest standards of excellence.

With more than 50 years of expertise and a family-owned approach, our Pharmacy Services team is committed to supporting better patient outcomes, reduced costs, and increased staff efficiency for delivering patient medications.

Rx@Home

Our home health patients who choose PruittHealth Pharmacy Services can take advantage of our continuum of care through PruittHealth.

With our Rx@Home program, patients transition back to home with the prescription medications they need to support them in their recovery.

Infusion Therapy

PruittHealth provides patients with everything they need to receive quality infusion services in the comfort of their home or our in-office infusion suites. Our team of experts verifies benefits, determines out-of-pocket costs, and presents any available options to minimize costs for our patients.

We are committed to upholding the highest standards of care, offering advanced technologies, and delivering timely and accurate supplies to patients’ homes. Patients are assigned their own clinical expert who communicates at least weekly to answer any questions and ensure the patient is responding well to the IV therapy.

MEDICAL SUPPLY

PruittHealth Medical Supply offers medical and nutritional products at competitive prices, shipped directly to our customers. We provide convenient and reliable distribution processes, product quality, and cost containment, with the support of PruittHealth experts.

With decades of experience, PruittHealth concentrates on patient care needs to deliver a wide range of medical products and services, as well as Medicare Part B billing. In addition to all major brands, we also offer private-label products as cost effective alternatives.

The goal for our therapy patients is to reach the highest level of function possible. We provide techniques to get patients back to their fullest potential and make any needed changes to their environment to allow for optimal function.

Physicians Services

PruittHealth Physicians Services is a medical practice division within our family of providers, consisting of physicians and nurse practitioners who provide services to enhance patient care.

We provide better coordination and communication with hospitals, referral sources, and payers, which are critical to care transitions and reduce rehospitalizations. By ensuring physician access and engagement, we offer a level of communication, trust, and personal connection with patients and their families.

Consulting Services

PruittHealth Consulting Services provides contracted health care centers and medical providers with clinical guidance and support in nursing, nutrition, social services, rehabilitation, medication therapy management, spiritual care, and more. We provide teams with tools to improve compliance, develop best practices, and create policies and procedures.

Aquatic Therapy

Aquatic therapy helps patients through state-of-the-art programs featuring HydroWorx® therapy pools with adjustable depth, resistance jets with massage features, underwater cameras and viewing monitors, and computer documentation.

Balance Program

Our balance program uses validated assessment tools and appropriate intervention to help patients improve balance, reduce fall risks, and treat associated problems, including incontinence, pain, muscle weakness, and hypotension. Our program creates personalized care plans and home exercise programs that work to improve balance, strength, range of motion, endurance, walking, daily activities, and overall quality of life.

Cardiac Care

Cardiac care works with a patient’s team of health care providers, including primary care physicians, cardiologists, clinical nurse specialists, dietitians, mental health professionals, case managers, and pharmacists to help improve overall health and quality of life.

Fitness Therapy

Fitness therapy utilizes physical and occupational therapy, nursing, and exercise-monitoring tools to help patients restore lost function and regain independence.

Life Enrichment

Life enrichment programs help patients stay physically and mentally fit by providing extensive resources to expand knowledge and fulfill spiritual needs.

Memory Support

Memory support is designed to assist Alzheimer’s and dementia patients by attempting to slow memory loss and other cognitive changes. Memory support provides life enrichment activities that stimulate the mind and promote overall health. The program provides family support and education to help caregivers keep loved ones engaged.

Pain Management

Pain management helps patients gain control over chronic pain to enhance enjoyment of life and limit lost productivity and wages. Our pain management team addresses pain and its underlying causes with therapy and treatment plans that include physical or occupational therapy, medication assessment and management, education, injections, or surgery.

Respiratory Therapy

Respiratory therapy offers high-quality care and independence to patients who have had a tracheostomy, experience an exacerbation of respiratory symptoms, or require BiPAP/CPAP or ventilator support. Our team of clinical experts develops comprehensive care plans designed to increase long-term health and overall wellbeing.

Stroke Rehabilitation

Stroke rehabilitation works with patients and loved ones to improve physical, cognitive, and lifestyle functioning after minor and major strokes. Each plan is personalized to ensure the patient’s unique needs, priorities, and preferences are met.

Wound Care

Wound care offers comprehensive, advanced treatment options to alleviate suffering and mitigate infection, hospitalization, and limb loss. Our policies and procedures are grounded in research, consultation, and the expertise of certified Wound, Ostomy, and Continence Nurses (WOCNs).

EDUCATION AND CAREER DEVELOPMENT

The CNA Career Ladder Program offers skilled nursing and hospice caregivers an opportunity to gain experience and become leaders amongst their peers.

ADMINISTRATOR-IN-TRAINING PROGRAM

PruittHealth is dedicated to creating opportunities for individuals who are interested in pursuing a career as a health care administrator. Through our multi-year, Administrator-in-Training program, an individual receives classroom instruction, hands-on training, and mentorship from experienced administrators. The goal of this program is to develop strong leaders throughout our health care centers, resulting in increased patient care and partner retention.

NURSE ADVANCEMENT PROGRAM

The PruittHealth Nurse Advancement Program pays the tuition (up to $15,000) of 20 caregivers, including CNAs, Senior CNAs, and LPNs, who wish to become registered nurses.

PRUITTHEALTH UNIVERSITY

We recognize that quality people thrive in an environment that promotes lifelong learning. That is why we established PruittHealth University, an online learning resource that is continually updated with new and innovative training programs.

RETAIN

Retain is an external employee engagement software that allows our employee partners to provide feedback about their experience. Program benefits include:

The PruittCares Foundation provides relief to those in need. Through crisis grants to families or caregivers, educational scholarships to future health care heroes, and support for bereaved children, the PruittCares Foundation shines as a beacon of hope to those experiencing tough times.

Since its founding in 1998, PruittCares, along with its staff and the many volunteers who support its efforts, has worked continually to enrich the lives of our employee partners and the communities served by PruittHealth.
