Community Rehabilitation Services Community Rehabilitation Services (CCRP)

A branch of Providence Care’s Adult Treatment & Rehabilitation Program

Community Rehabilitation Services is a community-based, individualized rehabilitation program that offers a supportive environment where individuals can learn skills and coping strategies necessary for successful community living. The program helps individuals to regain valued roles and to continue along the path to recovery.

Voices, Opportunities & Choices Employment Club (VOCEC) VOCEC is a not-for-profit corporation run by a community volunteer Board of Directors responsible for overseeing numerous social enterprise businesses in the Kingston area. Since 1993, VOCEC has employed over 400 consumers of mental health services.

A range of supports for persons living in the community who have been touched by mental illness.

Assertive Community Treatment Teams

Intensive Case Management

There are two Assertive Community Treatment (ACT) Teams that work out of Community Support Services on Montreal Street. Both teams provide intensive treatment, rehabilitation and support to adults living in the Kingston area:

Community Outreach Team (COT)

Community Integration Program ACT Team (CIP) Psychosocial Rehabilitation ACT Team (PSR) The objective of both teams is to provide recovery oriented services that promote hope, empowerment, self determinationand wellness.

Both teams operate 12 hours per day during weekdays and 10 hours on weekends and holidays, with on-call support during off hours. Multiple contacts may be as frequent as two to three times per day, seven days a week, and are based on the person’s need and a mutually agreed upon plan between the person and ACT staff.

These are client centered, recovery oriented programs, which support clients in achieving their individual goals, as they become members of their community.

The Community Outreach Team provides intensive case management services to persons with severe mental illness living in the community. It also assists clients who are ready to step down from the more extensive support provided by the Assertive Community Treatment Teams.

This team operates eight hours per day and provides services Monday to Friday.

This is a client centered, recovery oriented program, which supports persons in achieving their individual goals, as they become members of their community.

Housing and Community Integration Homes for Special Care Homes for Special Care provides a residential living environment for persons who experience symptoms of a serious illness.

The program promotes and enhances the quality of life for residents and complies

with all relevant health and safety requirements. It reflects the recovery model, whereby residents can expect a warm, stable and home-like atmosphere, in which they can feel secure and comfortable, be treated in a respectful and dignified manner, be encouraged to make personal decisions, and grow and develop to the extent of their capacity.

Within Housing and Community Integration there is also the provision of residential oversight.

Residential oversight promotes relationships with home owner/operators who provide accommodation and/or services within the residential environment (i.e. food, laundry, etc.) for Providence Care outpatient clients associated with the Intensive Case Management (ICM) and Assertive Community Treatment (ACT) Teams located at 525/533 Montreal Street. This role will be in place for home operators whose operations are not part of the Homes for Special Care program.

It will provide a pathway for two-way dialogue between the clinical teams and the home operators through a liaison arrangement led by a Social Worker currently assisgned to the Housing and Community Integration program. This individual’s top priority will be to address issues that relate to client safety. In addition, it will provide the homeoperators with opportunities to convey matters that may have clinical or non-clinical concerns.
