Welcome Looking back over 2008, we think we can all agree that it was a year we won’t soon forget. Change is certainly one of the themes of the past year. Yet so is consistency. The demand for health care at the end of life continues, but the need for those with limited financial wherewithal has never been greater. The area we serve is vast. More than 1,800 patients and families from Los Angeles and Orange counties – from the northern tip by Ventura County, to the southern end of Orange County – were cared for under hospice in 2008, a year in which The Gathering Place counseled more than 850 people from the South Bay. One of the most noticeable changes in 2008 was our new name: Providence TrinityCare Hospice Foundation. Over the 22 years that our dedicated Foundation has existed, we have had three names: The Hospice Foundation, TrinityCare Hospice Foundation, and now, Providence TrinityCare Hospice Foundation.

But what is consistent is our mission to support the work and programs of our hospice, Providence TrinityCare Hospice.

Our new name recognizes our place in the continuum of care provided through the many fine medical centers and physician groups in Southern California, including our esteemed Providence Health & Services “sister” medical centers.

But we also serve patients from other outstanding medical facilities: Cedars-Sinai, UCLA, Memorial Medical Center and St. Mary’s in Long Beach, the VA Hospital, and the terrific pediatric hospitals in our area, among them. And it is through our Foundation that you, our dedicated donors, fund the much-needed end-of-life care and programs.

With tough economic times come tough decisions, and in 2008 the Foundation reviewed our many programs and ways we conduct our work to support hospice and raise necessary funds. As a result of this process, we made the difficult decision to close our thrift store, Hospice Thrift Avenue. This was a hard choice, as the store was staffed by a team of dedicated volunteers and a full-time staff person, all of whom put their hearts into making it a warm place for friends to gather and find treasures. We thank Tricia King and the volunteers for their service and efforts over these years.

Consistency and change also describe our cherished supporters, who stepped up once again, even – and most particularly – in these challenging times. When the patients’ and families’ needs shift from cure to comfort, it is still your hospice that is there to fill the need in the community. And it is your Foundation that helps fill in the gaps financially for these special services that only philanthropy will support. It is you who are consistent in your caring, and it is you who make it possible for us to be consistent in our service to those at the end of life. Thanks for all that you do to allow us to maintain, as well as enhance, one of our greatest constants: a level of compassionate and expert care for which we are known throughout Southern California.

Providence TrinityCare Hospice’s After-Hours Team

It’s the wee hours of the morning, maybe 2 a.m.

After an exhausting evening preparing your wife, who has advanced Alzheimer’s disease and a chronic lung condition, for bed, she has suddenly awakened. She is agitated and inconsolable, crying of pain but unable to identify it. You are inconsolable yourself – and nearing your wit’s end.

And you feel all alone. Except you remember that you are not alone.

You call the number for after-hours care at Providence TrinityCare Hospice and leave a message explaining your and your wife’s situation. In 15 minutes, you receive a return call from an experienced nurse, who begins talking you through the crisis. After listening for a few minutes, she determines that a home visit is in order.

And within a half hour, another nurse is at your wife’s bedside helping you get her back to sleep.

This is no hypothetical scenario for Cyndee Duby, the clinical manager for Providence TrinityCare Hospice’s after-hours triage team, who each year responds to approximately 10,000 calls from concerned patients and their family members and caregivers.

“It’s an extraordinary thing,” remarked Cyndee. “We’re with patients and families in the moment, in the middle of the night. We see the picture as it is. They’re scared, they have questions. We can make a difference in minutes – whether it’s over the phone or in the home.”

The after-hours team may talk with callers about the possible side effects of a medication, explain the best ways to operate a hospital bed or listen for details of symptoms to determine whether a home visit is needed.

“A lot of patient and family education takes place over the phone,” she said.

But education and patient care are only part of the after-hours team’s responsibilities. Emotional support is just as critical.

“One mother who was losing her daughter to brain cancer called me every night,” Cyndee recalled. “We talked about how unfair it was for a parent to lose a child. We never met, but I felt like I knew her.”

Cyndee felt just as close to another caller, a patient who would phone her every night at 8 p.m. sharp.

She welcomed the predictability of his calls. When they stopped, she knew what this signaled – and she mourned his loss.

Cyndee is supported in her work by a team of 25 to 30 seasoned and highly skilled health-care professionals, including registered nurses, social workers, licensed vocational nurses, chaplains and home health aides, who, along with physicians, are available in the middle of the night as well as on weekends and holidays.

Fifteen to 25 percent of the calls –1,500 to 2,500 – each year require an after-hours team member’s visit to a patient’s home.

To send, or not to send, an after-hours team member to the home is determined by a nurse manager, who triages all calls. “When in doubt,” Cyndee observed, “we err on the side of sending someone to the home.”

Whether or not a home visit is needed, patient and family calls are responded to immediately. The average turn-around time between a call to the after-hours team and a return call is under 20 minutes.

“Our goal is to meet patients’ and families’ needs,” Cyndee said, “and to let them know they’re not alone.

We’re only a phone call away.”

Physician Spotlight

A Focus on DR. SUE S. YIE, Providence TrinityCare Hospice Regional Medical Director, Torrance

While patient and family education is a large part of her job, Dr. Sue S. Yie believes that she learns at least as much from those she cares for as they from her. It is a grounding experience that everyday provides her with meaningful perspective.

“Patients and families teach you a lot,” said Dr. Yie, Torrance Regional Medical Director of Providence TrinityCare Hospice. “They remind you how precious life is and help you sort out the important from the unimportant.”

Assigning priority to what is important in life is, of course, one of Providence TrinityCare Hospice’s goals. “We want to empower patients and their families to make decisions that are right for them,” she noted.

Dr. Yie is assisted in her tasks by an able team, including nurses, social workers, licensed vocational nurses and other health-care professionals. “They completely understand what patient care is all about,” she said, “and they are genuinely caring in their approach.”

Dr. Yie came to the Providence TrinityCare Hospice team in April 2006 from the UCLA Center for East-West Medicine, where she was Geriatrics Program Director and Assistant Clinical Professor for four years. Previously, she was an attending physician in the geriatrics department at the Scripps Clinic in La Jolla, Calif., and a primary care physician and medical officer at the Admiral Joel T. Boone Branch Medical Clinic in Virginia Beach, Va. She also served as the medical officer in charge at the U.S. Naval Hospital Yokosuka in Chinhae, South Korea.

A native of South Korea, Dr. Yie was raised in Hawaii and San Francisco. She graduated from the University of California, Davis, and the George Washington University School of Medicine. She completed her medical internship at the National Naval Medical Center, her residency at Yale and a fellowship in geriatrics at Johns Hopkins.

When not attending to patients, Dr. Yie relishes her time with her husband, Jay Lee, who is an attorney, and their seven-year-old twins, Justin and Stephen.

A Final Goal Attained Most people wouldn’t expect hospice clinicians to focus on goals for a patient at the end of life, but that’s exactly what the patient’s interdisciplinary care team does when someone enters hospice care. Admissions nurse Tina Eisenman has frequently considered this over her 30year career. “When patients enter hospice,” she says, “we try to identify what their goals are.” Often, these goals involve an occasion or the creation of everlasting memories for loved ones to look back on in gratitude.

That happened recently when Tina met 43-yearold patient Kathy Fazendin, who had been battling metastatic ovarian cancer for more than two years and had been discharged to the home of her parents, Bob and Marlene Doyle, for hospice care. From close relatives and the patient herself, Tina learned that Kathy, also a nurse, was hanging on until the love of her life – her only child, 17-year-old Christopher – graduated from high school. “Her goal was to make sure that he was okay, that he wasn’t being left behind,” recalled Kathy’s younger sister, Paula Paine.

The Care Team didn’t believe that Kathy would be alive at the time of Christopher’s official graduation in June. But they didn’t give up the hope that they could make a meaningful difference in the lives of Kathy, Christopher, and their large and loving extended family.

This mindfulness to Kathy’s goals gave way to an event that no one in her family will soon forget. Hearing a colleague recollect a graduation ceremony that had been moved up months to accommodate another dying parent, Tina contacted Christopher’s high school and convinced officials there to create a similar graduation ceremony for Christopher and Kathy.

Not even two weeks after Kathy had been admitted to hospice, a full-fledged commencement ceremony took place in the large backyard of the Doyle home, where Kathy was receiving her care. It was attended by approximately 50 of Kathy’s friends and relatives and presided over by school administrators, who had furnished Christopher a cap and The Care Team... gown for the special occasion.

“Kathy sat in the center of the front row,” recounted Bob. “We all felt very privileged and blessed to realize that she was getting her dream. It was a milestone – everything else would be a bonus.”

Christopher added, “The person who loved and enjoyed the ceremony the most was my mom. For the longest time, my mom had one goal in mind. She told just about everyone, including doctors, friends and family, that she wanted to see me graduate!

After this wonderful ceremony, I can gladly say that my mom saw her only son graduate. I extend my appreciation to TrinityCare Hospice for putting this ceremony together because my mom wouldn’t have made it to my actual graduation.”

Kathy passed away soon afterward. “She had peace of mind,” said Paula.

This fall, Christopher began his freshman year at the University of Colorado in Boulder. And the care teams at Providence TrinityCare Hospice continue to work their hardest to help patients attain their goals.

A Look Within and to the Future Ask Providence TrinityCare Hospice Chief Executive Terri Warren a seemingly straightforward question about hospice care, and don’t be surprised when she serves up, with hard facts and data, a detailed response that reveals the depth of her knowledge about the subject. Of equal – perhaps greater – importance, she speaks with a passion and a clarity that invite further inquiry about the hospice movement in the United States.

One of California’s preeminent authorities on hospice care, as well as one of its most articulate advocates, Warren has gained expertise through her affiliation with Providence TrinityCare Hospice, which she first joined in 1994 as the AIDS Case Management Family Program Coordinator. An alumna of the College of the Holy Cross, from which she graduated with honors, as well as Boston College, where she earned her master’s degree in social work magna cum laude, Warren, a New Jersey native, has served in a number of positions at Providence TrinityCare Hospice. She has been in her current post for approximately four years. In addition, she sits on the Executive Leadership Committee of the National Hospice and Palliative Care Organization and is Vice President of the Board of Directors of the Children’s Hospice and Palliative Care Coalition.

We recently chatted with Warren about hospice care in general and Providence TrinityCare Hospice in particular.

Excerpts from our conversation follow.

Q: Terri, how did you become interested in hospice care?

“

What makes Providence TrinityCare Hospice exceptional... are the depth and breadth of programs and services we provide.

”

Warren: I began working on the East Coast as a medical social worker, and I had a glimpse of what working in home care would be like. I saw it as an amazing clinical opportunity to care not only for the patient, but also for the whole family. It’s real-life dynamics at work, and it’s a chance to reach people where they are and make a tremendous difference for them.

Q: Thinking back to your early days with Providence TrinityCare Hospice, can you recall an instance when you were able to make a tangible difference?

Warren: One of my first patients was a member of a family who didn’t own a refrigerator. Yet one of the medications she needed to take required refrigeration.

We were able to intervene on her behalf and get a refrigerator for the family. This was a basic need we were able to meet, and it made a huge difference for her and her family.

That’s very important to us. Throughout the year, our physicians make hundreds of home visits to patients and their families.

Q: Terri, you’ve been involved in hospice care for many years, and through your work you have observed many hospice organizations throughout the state. What makes Providence TrinityCare Hospice stand out from other agencies?

At the same time, we are placing a tremendous focus on how to best integrate technology into care so that patients and families can connect to physicians and other health-care professionals. We are working toward a future where Web cams and other high-tech means are utilized so that continuity of care is not only maintained, but also enhanced.

Warren: There are many fine hospices in California doing good work, but I think that what makes Providence TrinityCare Hospice exceptional, qualitatively as well as quantitatively, are the depth and breadth of programs and services we provide to our patients and to the whole family. Whether we’re speaking about medical, social or bereavement services, we see the patient and family as a whole unit that can benefit from the full complement of programs we offer.

Q: Would you elaborate on your medical services?

Warren: Providence TrinityCare Hospice is on the forefront of palliative care medicine – not just in California, but in the country. Our physicians are experts in hospice and palliative care medicine. They received Board certification in this specialty and all are fellowship trained. They are called upon quite frequently to consult on cases.

As the country continues to age, and as more Americans view home care as the most compassionate and costeffective method for treating patients at the end of their lives, there will be a greater demand for the model of care that we at Providence TrinityCare Hospice have developed. Already, we receive many calls from other hospices seeking to learn from our experiences and frequent requests from medical schools whose students would like to train with us. Given the demand, one of our future goals is to develop a physicians’ institute to provide expertise in palliative care and hospice medicine in a more formalized fashion.

Q: What does Providence TrinityCare Hospice’s physician model look like?

Warren: It’s a model that recognizes the best of traditional medicine with an eye toward the future.

Q: It sounds as if Providence TrinityCare Hospice strives to remain ahead of the curve.

Warren: Absolutely. Best practices in patient care continue to change rapidly, so it is critical that we remain knowledgeable and proactive about what is on the horizon. I will give you a very concrete example. By 2012, regulations mandate that all hospice programs will need to have in place electronic medical records.

We saw this change coming and took appropriate action. Hospices that do not have systems in place to make this shift, as well as those that do not have the resources to do so, will be facing a great challenge. This is a massive, costly undertaking. Providence TrinityCare Hospice, among other better-prepared hospices, will need to be ready to work with more patients and families as smaller hospices seek partnerships with us to provide compassionate care. Such collaboration is already occurring.

Q: Tell us about other recent developments.

Warren: A few months ago, we opened three hospice suites in the transitional care unit of Providence Little Company of Mary Medical Center – Torrance. These suites, designated specifically for hospice patients, provide round-the-clock care to those patients who can most benefit from such a service.

Q: Any additional changes?

Warren: We’re always looking toward tomorrow, but the mission of Providence TrinityCare Hospice will remain unchanged. Our friends know that our primary focus has always been caring for people at the end stages of life, as well as for their families. At the end of the day, we answer to them. That’s what keeps us going.

A Year at a Glance We’re at the Head of the Class Because we want to excel in everything that we do, Providence TrinityCare Hospice underwent this past year a voluntary accreditation process by the Community Health Accreditation Program (CHAP), the nonprofit organization that surveys, assesses and accredits agencies and facilities that provide home health care services, including hospices.

We not only received the maximum three-year accreditation, but we also were awarded two commendations for quality of services exceeding expectations – in the areas of staff leadership development and physician certification.

An Afternoon with Mandy Moore, our TKC Ambassador Singer/actress Mandy Moore joined Hospice friends at the home of Foundation Board Chair Jacqueline Glass for a poolside concert to benefit TrinityKids Care. More than $80,000 was raised, as guests responded to an appeal to support the only dedicated pediatric hospice service in Los Angeles and Orange counties.

“We were very gratified by the results of the CHAP accreditation survey,” said Terri Warren, Chief Executive of Providence TrinityCare Hospice. “Most hospices opt out of the accreditation process because it is a time- and labor-intensive process.

However, we believe the process allows us to uphold, even enhance, our high standards. The fact that we received two commendations when only ten percent of CHAP-accredited agencies receive a single commendation speaks volumes about our efforts to provide a superior level of compassionate care.”

The tournament, chaired by Trustee Scott Sharpe, shown here with wife, Jan, raised approximately $205,000.

In 2008, TrinityCare Hospice served more than 1,800 patients and their families, including more than 100 children on hospice.

We cover more than 4,000 square miles across Los Angeles and Orange counties to provide comfort and care where they are needed – in the homes and communities in which our patients and their families live.
