It is with great pleasure and enthusiasm that I extend a warm welcome to each one of you to the Fiji Pharmaceutical Society Annual Conference, themed “Respiratory Health: Just Breathe!” - a gathering that promises to be an exhilarating voyage for the mind.

This year's conference holds particular significance as we delve into the critical domain of respiratory health. The intricate interplay between pharmaceutical advancements, clinical practice, and patient care in respiratory health underscores the importance of our collective efforts to stay abreast of the latest developments.

Respiratory Health Hello and welcome to the Fiji Pharmaceutical Society Annual Conference for 2023 with the theme of Respiratory Health, Just Breathe! Respiratory Health has really come to the forefront of discussion owed to the COVID-19 pandemic. Our breath is a fundamental element of life, and maintaining the well-being of our respiratory system is crucial for overall health and longevity.

In a world where air quality is increasingly challenged, understanding and prioritising respiratory health becomes paramount. Whether it's the impact of environmental pollutants, the rise in respiratory infections, or the challenges posed by lifestyle choices, our respiratory system faces numerous threats.

It's incumbent upon us to be proactive in safeguarding our lungs. This involves adopting healthy habits, such as regular exercise, a balanced diet, and avoiding exposure to harmful substances.

Additionally, being mindful of indoor air quality and workplace environments contributes significantly to respiratory well-being.

Technology and innovation also play a pivotal role in advancing respiratory health. From medical breakthroughs to wearable devices that monitor lung function, we have the tools to enhance early detection and management of respiratory issues. Let us embrace these advancements and ensure they reach those in need.

Equity in respiratory health is a goal we must tirelessly pursue. Access to quality medicines, irrespective of socioeconomic status, should be a fundamental right. By addressing health disparities in providing affordable healthcare to all patients, we can create a society where everyone has the opportunity to breathe freely. Pharmacists have the power to change lives and this is something we should strive towards. One of the key roles we play is in health education and promotion. Let us take a stance to be more mindful of how we interact with patients and pay more attention to those who take complicated dose delivery systems such as inhalers. Patient compliance is always a major factor in recovery and disease management and we should all take time out to review these patients thoroughly at every visit.

How often do we check if our patients are using their inhalers and medications correctly? You may have heard of cases where improper usage of these devices have led to worse outcomes. I would like to encourage you all to affect the quality of life of patients towards better outcomes.

New respiratory syndromes like Long-COVID also pose particular challenges and these need to be addressed. The health impacts of decreased respiratory capacity are vast, not only affecting the quality of life but it places the patient at great risk of other complications.

As we move forward, let us unite in our commitment to respiratory health. Through education, advocacy, and collective action, we can create an environment where we clear the air of doubt, and respiratory diseases are minimised or managed effectively. Our breath is a gift—let's cherish it and work together to build a healthier future for all.

Programme Guide MESSAGE FROM COMMONWEALTH PHARMACY ASSOCIATION PRESIDENT MARY ANNE CIAPPARA President Colleagues It is a pleasure and a privilege for me to address the Annual conference of the Fiji Pharmaceutical Society. On behalf of the Commonwealth Pharmacy Association, I thank you.

The theme of your conference 'Respiratory Health- Just Breathe' is appropriately chosen as your Ministry of Health and Medical Services has recently published guidelines which aim to provide healthcare professionals with a holistic approach to the management of Respiratory Diseases and promote quality care for patients.

Central to the provision of pharmaceutical care is the patient.

The good of the patient cannot be defined by you, pharmacists alone, using your knowledge supported by the Respiratory Guidelines and experience, and thinking what is best for the patient; but must take into consideration the perspective and experience of the patient or carer.

How is the patient experiencing his or her illness and his or her suffering, how is it effecting his or her life? How does he or she perceive pharmacological treatments, what are his or her concerns, fears, misconceptions, and beliefs? What are his or her expectations and wishes. Gaining an insight into and understanding of the patient's experience, perspective and expectations will enable you, pharmacists, to acknowledge a patient as a person, treat him or her as a person, understand his or her needs and ensure the most effective outcome.

The evolving relationship between the pharmacist and the patient is one based on partnership whereby the patient is an active participant in his or her care, involved in decisions and the management of his or her condition, thereby respecting the patient and his or her right to self determination.

Patients should be a key partners in their care. Empowerment is seen as the capacity and the freedom to make informed choices based on information and knowledge. Through empowerment, patients will become more knowledgeable about their condition, comprehend their treatment and its effects. Thus, they will be able to take on responsibilities for their care, and cooperate on mutually accepted courses of action.

While today the emphasis is on patients' rights, it is the character, values and attitudes of you, pharmacists which will ultimately promote their wellbeing. Character traits, values and attitudes define the character and quality of the interaction between the pharmacist and the patient. I would like to focus on the following character traits and values:-trust, dialogue beneficence and compassion.

Trust, an intellectual and moral virtue, is gained by being competent and committed.

Competence incorporates updated scientific and professional knowledge, skills and ability to exercise professional judgment. Participation at this Conference and supported by the Respiratory guidelines enables you to update your knowledge and skills on respiratory diseases. Commitment is a form of altruism- an unselfish concern for the wellbeing of patients, whereby every decision reflects the good of the patient and not what is profitable. This commitment is reflected in the following attitudes and values. Being honest by imparting truthful information; being accessible and having a friendly disposition; being respectful, respecting privacy and confidentiality; showing an interest; listening to the lived experience of illness, health, wellbeing and medicines, and by showing empathy and compassion.

Dialogue is a two way process. It involves listening to and understanding what patients have to say.

Dialogue involves talking in a simple and clear language that can be understood, being honest and enabling them to believe in that information.

Beneficence entails you, pharmacists, to advance the interests of patients. It seeks to help them in their needs, ensuring that treatments are appropriate and that there are no interactions, giving them information, enabling adherence to treatment and discussing quality of life issues.

Compassion- co-suffering, means that you, pharmacists, are moved by the suffering and distress of patients and act to alleviate that suffering. Compassion enables you, pharmacists, to choose well their words when communicating information and to instil hope. Compassion is directly related to justice. Being just in your relationships by allocating enough time to them and giving the same care and attention to all patients depending on their needs.

In a reciprocal relationship based on trust, dialogue, beneficence and compassion, patients will be able to make informed decisions based on information and unhindered by misconceptions, fears and other limitations which makes patients vulnerable.

The profession and university at undergraduate level should address ways and means to promote an active pharmacist-patient relationship. The approach needs to take into consideration the patients' perspective and their experience of illness. Patient empowerment is a challenge for the profession, patient associations and other health care professionals, and can only be attained through the collaboration of everyone concerned.

Programme Guide SPEAKER PROFILES

Prof. Bandana Saini A Holistic Perspective of Respiratory Health Professor Saini trained as a pharmacist at the University Institute of Pharmaceutical Sciences, Chandigarh, India (1990). She continued on to a path in research, starting with a Masters in the Discipline of Pharmaceutics (1992) at the same institute. Her master’s work involved exploring the mutagenicty of halogenated nitrobenzenes. After completing a masters degree in Business Administration in 1996 from the University of Western Sydney, she changed her research direction to exploring pharmacy health services, an area which strategically combines her pharmacy background and an understanding of management principles.

Prof. Saini’s PhD project at the University of Sydney in 2004 focused on asthma. Since then Professor Saini has led and collaborated in implementing and evaluating several successful pharmacy-based models that involve screening (sleep disorders and chronic obstructive pulmonary disease), patient self-management (allergic rhinitis, sleep disorders, asthma) and clinical audits (sleep medications), as well as pharmacovigilance (sleep medications).

Ms. Joshila Lal Fiji National University Ms. Joshila Lal is a registered pharmacist and a senior lecturer at Fiji National University’s College of Medicine Nursing and Health Sciences with over a decade of experience as an educator, promoting flexible, dynamic and supportive learning environment for pharmacy students. She holds a Bachelor of Pharmacy from Monash University and a Masters in International Relations & Diplomacy from University of Fiji and is currently completing her Master’s in Education.

A strong believer in continuing education, Joshila has organized many seminars, workshops and conferences for Fiji Pharmaceutical Society, which brings together pharmacy professionals from across Fiji.

She is an executive board member of Fiji Pharmaceutical Society and has also served as the Vice President in the past. She is also an elected Commonwealth Pharmacy Association Councillor. Her enthusiasm and passion for the pharmacy profession has earned her opportunities to actively contribute to the Fiji Pharmacy Profession Board and the Fiji Pharmacy Standards Committee, by leading the development and implementation of the Fiji Continuing Professional Development Guidelines and the Code of Ethics.

Ms. Lal is a firm believer of serving the community and has been an active member of Rotary Fiji for many years. She has been awarded the Paul Harris Fellow twice in recognition of her commitment to ‘Service Above Self’ to benefit local and international communities.

Ms. Lal is also the Director of PharmAce Healthcare Ltd, a pharmaceutical distribution firm dedicated to bringing quality affordable medicines to the people of Fiji.

Ms. Rashika Goundar Fiji National University Ms. Rashika Vishal Gounder is a distinguished healthcare professional, pharmacist and an academic leader currently serving as a Lecturer and Head of Department at FNU. With a career spanning over a decade, Rashika has consistently demonstrated a commitment to advancing pharmaceutical practices and education. Her expertise extends across various domains, from implementing national antimicrobial resistance initiatives during her tenure at the Fiji Pharmaceutical and Biomedical Services Centre to shaping the next generation of pharmacists through her role as a Lecturer at CMNHS. Holding a Bachelor of Pharmacy from Fiji School of Medicine, a Masters of Clinical Pharmacy from Curtin University, Australia and a Postgraduate Certificate in Public Health from FNU, Ms. Gounder is a dedicated and qualified pharmacist committed to advancing pharmaceutical practices and education.

Apart from her dedicated professional pursuits, she holds a keen interest in spiritual and religious facts, often delving into new age books to broaden her understanding of holistic well-being. This passion for exploring diverse realms aligns with her commitment to continuous personal and intellectual growth. She is hoping to incorporate this knowledge with her professional knowledge in developing a research proposal in the future.

Her presentation today is on the new Fiji Respiratory Guideline where she will highlight some features of the new guidelines.

Mr. Ben Gilbert Therapeutics Goods Administration, Australia Mr. Gilbert graduated from the University of Tasmania in 1999 and has worked as a pharmacist, pharmacy owner, consultant and academic. He attained postgraduate qualifications in toxicology and tertiary education.

Currently he works as a technical advisor in the Australian TGA Regulatory Strengthening Program. The goal of the RSP is to improve access to quality, safe, and effective medical products. They do so by supporting countries in the Indo-Pacific region to strengthen regulation of medical products, including in pharmacovigilance, good manufacturing practices and market authorisation.

His presentation during the conference, titled “Substandard and Falsified Medical Products – How can pharmacists help to stop their spread in Fiji?” looks at pharmacists’ role in minimising distribution of substandard and falsified medical products in Fiji. After participating in this activity, pharmacists will be able to: Discuss the problems caused by SFMPs in Fiji and internationally.

Understand the features that can help identify SFMPs. Use Visual Inspection methods to conduct a first-level analysis to identify SFMPs. Develop a Visual Inspection checklist to use in their practice.

Ms. Rupali Lal Holistic Practitioner/Pharmacist After graduating as a pharmacist in New Zealand, Ms. Lal worked in the community setting for many years in Auckland and then in Fiji after moving back a few years ago. In the last couple of years, she found herself at a crossroads, realising that she lacked the knowledge and tools to lead a life of holistic wellness; how to nourish the body with the right foods or to effectively manage stress. This awakening set her on a transformative health journey. Ms. Lal delved deep into the world of well-being, acquiring new skills and knowledge. She emerged as a certified Pause Breathwork Facilitator and a Functional Nutritional Therapy Practitioner. These qualifications paved the way for her to offer a holistic approach to health, recognizing that wellness encompasses not just physical health but also the mind and spirit.

Her business, Pause.Breathe.Live, embodies her passion for health and her commitment to providing a judgment-free, empathetic space for individuals seeking a more comprehensive approach to their well-being. Her perspective bridges conventional healthcare with holistic practices while she continues to maintain her pharmacy practicing licenses in Fiji and New Zealand.

Ms. Lal’s presentation will be sharing a holistic perspective of respiratory health. The lungs stand as the sole organ over which we exert direct control. The question arises: does the manner in which we breathe truly matter? As pharmacists, our expertise lies in traditional pharmaceutical interventions within the pharmacy setting. However, through her presentation she aims to broaden our perspective by exploring holistic considerations surrounding the respiratory system. It delves into the interconnected factors that influence cellular health and the functioning of an organ we can actively influence – the lungs.

She will also offer insights into the transformative practice of breathwork and its profound implications for health. By examining breath as a dynamic force that extends beyond mere oxygen exchange, we uncover its potential to influence not only the respiratory system but also broader aspects of our physiological and mental well-being.

Ms. Priyanka Prasad Fiji Pharmaceutical Society Ms. Priyanka Prasad graduated with Bachelor of Pharmacy from Fiji National University in 2015. Following this, she pursued her Pharmacy Internship Program at the Colonial War Memorial Hospital and gained registration in 2017. Having also worked at retail pharmacies part-time as a student and having had a keen interest in community pharmacy, she joined Chovhan Pharmacy Ltd as Manager Pharmacist.

Ms. Prasad has been a member of the Fiji Pharmaceutical Society since 2016 and is currently the Vice-President after having been elected to serve as an executive member previously.

Programme Guide SPEAKER PROFILES

Mr. Reenal Chand Ministry of Health and Medical Services Mr. Chand is a pharmacist having graduated from the (then) Fiji School of Medicine in 2008 and registered in 2010. He completed his internship at CWM Hospital and worked in the public sector from 2009 to 2011 before moving to retail practice. Having worked in retail pharmacies around Suva for 10 years, Mr. Chand then took up the role of Senior Pharmacy Officer for Antimicrobial Resistance at the Ministry of Health and Medical Services, Fiji Pharmaceutical and Biomedical Services Centre from October 2021. He serves as the secretary to the National Antimicrobial Resistance Committee as well as being Fiji’s Human Health AMR Focal Point.

Mr. Chand is also the current President of the Fiji Pharmaceutical Society (FPS) and has previously served as the Vice-President and Secretary. He has been with FPS since 2013.
