Well in this issue we go one further than that, and tell just a handful of the many examples of our staff actually becoming patients at Practice Plus Group themselves, and opting to have the surgery they needed performed by the people they work with every day (p8). This to me is the biggest vote of confidence in the quality of what we do that we could ask for.

Also in this issue, Dr Catrin Williams, Consultant Anaesthetist, gives her top tips for getting fighting fit ahead of surgery, Steve Palmer, one of our fantastic Operating Department Practitioners (ODP), describes a day in his working life, and we’re excited to share with you our new advertising campaign for self-pay surgery.

Get well sooner Practice Plus Group’s main priority is and always will be supporting the NHS. The majority of the work we do in our hospitals and surgical centres is aimed at significantly reducing the pressure on NHS services, and helping to clear the backlog.

But there are still over 400,000 people waiting for hips and knees, and the NHS can’t use all the space we have.

This provides us with an opportunity to provide the self-funded option that is increasingly in demand, but at a much lower price point so that more people can afford to get the help they need.

Until now, there have only been two options for patients- wait for the NHS, or navigate the complicated world of private healthcare.

Karen is blooming Karen Hardy used to play lots of sport when she was younger and had always been fit and active, so when her knee was starting to cause more and more issues and had to reduce her driving and gardening, she thought it was time she did something.

Initially her experience with getting an NHS appointment went smoothly but then appointments were cancelled.

“I wanted my life back.”

Karen said: “I was in limbo. My daughter said that the money was there and that paying for the knee to be sorted would be the best option. After agreeing that this was the best choice, I have never looked back.

“Unfortunately, my blood pressure was found to be

through the roof, so this had to be sorted before surgery, but it was good that this was found and I am now on the treatment I needed.”

Karen had her surgery in June 2023 and says she experienced no pain, just a little stiffness.

“For me it was the right thing.

I was getting nowhere with the NHS and I was missing my garden! I had given strict instructions to my husband what needed watering and looking after whilst I was on crutches, but I am now already feeling much better and looking after everything myself.

“The staff at Practice Plus Group Barlborough have been brilliant, right from the beginning to the end. I would thoroughly recommend them and I am just glad to have my life back.”

African adventure In May, Rachel Atkinson, a nurse at our Shepton Mallet hospital, had the opportunity of taking her job overseas by volunteering in Kenya. She spent two weeks taking her experience to an entirely new and different level and being a little more inventive with the care she provided.

Rachel said: “I spent most of my time in the community on the back of a motorbike! It was a cross between running a one-person Accident and Emergency department and an Outpatients department by myself, in the middle of nowhere, from a motorbike! From rehabilitation centres to meeting locals in their day-to-day lives, it was a privilege to be able to experience how humble the community were - I was able to learn more than I expected throughout my time there.”

Practice Plus Group Hospital, Plymouth, are celebrating after recently reaching their 10,000th knee replacement. Since 2003, Practice Plus Group Hospital, Plymouth has carried out almost half of all the knee replacements in the three hospitals in Plymouth.

Here are a few highlights from this year’s survey of our secondary care division, which is all our hospital and hospital support staff:

The hospital and staff recently received a glowing report from one of their knee patients Su of Kingsbridge is a keen walker and had always remained active but after a sports injury years ago and a fall on holiday, her knee pain prompted an x-ray which showed she needed a knee replacement.

Su said: “IMy neighbour recommended Practice Plus Group Hospital Plymouth so I decided to get in touch and within two weeks I was having a knee replacement.”

She added: “I was impressed how professional and efficient the service is. Every member of staff, from the consultant, anaesthetist, doctors and nurses, to the parking attendant and caterer are so friendly and keen to help.

“I found the environment clean and relaxing and I was reassured all the way.”

She said: “The physio team were amazing and so supportive.

The exercises were explained fully and tailored to me and I felt they really looked after me throughout.”

Equality, diversity and inclusion winners Practice Plus Group has been crowned ‘Equality, Diversity and Inclusion Champion’ at the 2023 Skills for Health Awards.

At the heart of our strategy is our EDI steering group, a longestablished committee which was recently re-launched with a new executive chair and members.

The group is responsible for the introduction of our reverse mentoring scheme, incident reporting tool, and recruitment

monitoring, amongst other initiatives.

Speaking at a glittering award ceremony Faisal Zaman, National Learning and Organisational Development Manager in our Health in Justice division, said: “We’re thrilled. The whole team has put in a lot of work over the past few years, which started in our primary care division, moved into secondary care, and has now been rolled out across our entire business. So, it’s great, it feels wonderful.”

Our lives in our colleagues’ hands

Tony Lenaghan Our staff have so much trust in their colleagues, they have even gone under the knife themselves.

Tony Lenaghan, a physiotherapist at Practice Plus Group, Ilford, has certainly got some experience under his belt, both professionally and personally.

Having already had two knee replacements at his workplace, this year he went on to be one of

the hospital’s hip replacements undertaken as a day case, where he was able to go home on the same day as surgery.

Tony said: “I felt fine. I had a nerve block and didn’t need to have a general anaesthetic so after I got home, I went to the pub for my usual. I just told my wife that it was all part of my exercise programme!

Joking aside, the team were great and everything went really smoothly.

I was really pleased to be going home.”

“My wife is glad to get rid of me I reckon and I’m just very happy to be treating patients.”

Tony found that stopping work during the pandemic and being

less active made the pain in his groin much worse. Tony worked as a physiotherapist and trainer in football clubs around the world for 35 years before finishing his football career at Spurs and then joining Practice Plus Group 10 years ago.

He is an active gardener and walker but when his activities were curtailed due to hip pain, he once again put his care in the hands of his colleagues.

He added: “At 77, I am older than a lot of my patients and I can tell them I have had multiple joint surgeries in our hospital. This is encouraging for them to hear about my own experience and this can help with anxiety.”

Tony works 20 hours a week at Practice Plus Group Hospital Ilford and has no plans to retire.

He said: “My wife is glad to get rid of me I reckon, and I’m just very happy to be treating patients.”

Nazia Kauser works as a nurse at Practice Plus Group Ophthalmology, Rochdale, and has helped care for many cataract patients on their treatment journey.

She explains: “I work in the operating theatre of the Croft Shifa Centre, in Rochdale. I know I have excellent colleagues and our patients are always delighted with the care they receive. So, when my optician told me I needed cataract removal surgery, there was not a moment’s doubt for me that I would want my own colleagues to carry out the operation.”

Having already had an operation on her left shoulder in the past, she knew the pain she was experiencing was going to need a similar procedure.

Linda said: “I wasn’t able to lift at work, and sleeping was becoming a problem. Drying my hair was an issue and my general mood was low.

I decided to use the rapid referral form for staff and was referred for a right shoulder decompression and bicep tenotomy.”

“It was very reassuring to have surgery here and with everyone I knew. It’s also useful to be able to ask the consultant questions about my rehabilitation when I am on shift!”

Linda is 12 weeks post op now and although she is still sore, she is well on the road to recovery.

“I would definitely have surgery here again and there are a number of us on my ward that already have!”

Fighting fit for surgery Improving your fitness will help prepare your body for surgery, will reduce your chances of complications and help you make a better recovery.

Dr Catrin Williams, Lead Consultant Anaesthetist at Practice Plus Group Hospital, Shepton Mallet, has a particular interest in what is called ‘prehabilitation’.

She explains: “The interval between a patient being told that they need major surgery and actually having the surgical procedure presents a unique opportunity where we can intervene and help to improve their health. This not only improves the patient’s recovery after surgery but also improves their overall health in the long term.”

Patients with chronic conditions such as hypertension, diabetes, cardiac issues, respiratory disorders, thyroid dysfunction and anaemia will already receive specialist and targeted help whereby their healthcare team work together to manage any risks associated with surgery and get the patient in the best possible place ahead of surgery.

Any patient’s functional capacity- in other words the amount of physical activity they are capable of- can be enhanced by improving general health and wellbeing before surgery so that their body is better able to withstand the stress that the body is put under when having an operation.

Follow Dr Williams’ five pieces of advice to put yourself in pole position for your operation.

Activity and fitness levels Aim to undertake moderate intensity exercise over for 150 minutes over four or five days.

Additional muscle-strengthening is also advised, and reduce sitting down time. Do something every day, whether walking, gardening, or even exercising at home using the furniture or stairs.

Improve your diet Poor nutritional status is associated with poorer postsurgery results, such as length of stay in hospital and an increased risk of complications. Improving your diet in the run up to surgery

Practice Plus Group is in the process of piloting and rolling out a number of special clinics to help patients overcome obstacles in order to be fit for surgery, including a Body Mass Index (BMI) clinic to help patients whose BMI needs to come down before they can be eligible for surgery.

James Potter, Physician’s Associate, is leading the BMI clinic in Practice Plus Group, Plymouth.

He says “A patient with an increased BMI demonstrates a clear, yet preventable risk during their entire peri-operative journey.

We guide patients with evidencebased methods to help reduce their BMI at the clinic, using an approach which should not only reduce their risk of adverse outcome during and after surgery, but educate them on making smarter choices with food for life.”

and beyond is an important and achievable aim.

Prepare psychologically Your psychological state has long been known to affect how well you do before, during and after surgery, with evidence suggesting it impacts in both the short and longterm. Prehabilitation covers information on the surgery, what to expect afterwards, information on pain and pain medications and the importance of engaging in physiotherapy programmes.

Reducing anxiety and helping patients to appreciate the importance of diet and being physically active is essential.

Stop smoking It goes without saying that smoking is detrimental to health,

and addressing this before surgery can have an impact on recovery and outcome.

Dr Williams says: “Smoking increases pulmonary, cardiovascular and neurological infections, anaesthesia risk and complications. A smoking cessation programme can be a positive, life-long change.”

Reduce alcohol intake We know that drinking too much alcohol is bad for our health. But it has been shown that patients who drink excess alcohol are three times more likely to suffer complications at the time of surgery. Importantly, abstinence from alcohol for one month before surgery will reduce the risk of complications.

Steve Palmer, ODP at Practice Plus Group Hospital, Ilford, describes his typical day and why he loves his job.

I see an Operating Department Practitioner (ODP) as the engine that drives the elective surgical lists after the surgeon and anaesthetist have seen and consented patients.

We’re in the background, checking everything in theatre is ready, completing detailed diagnostic checks of the anaesthetic machines, ensuring they meet and adhere to all safety requirements, and that they are fully operational.

We have to perform a pre-op safety checklist prior to the patient being taken to the anaesthetic room where we go through details of the consent form and what’s going to happen.

We need to be professional and work in a timely manner at all times.

However, even with these checks and procedures, we also need to be human. We make sure the patient is reassured and gets to ask any questions they have.

As an ODP, we work within a multidisciplinary team and we are responsible for the care of patients in a range of specialities based in the operating theatres. We deliver that care to patients throughout the whole of their journey - whether in anaesthetics, surgery, recovery, or emergency settings.

During the operation, we maintain safety and when a patient is anaesthetised, especially if they are unconscious, we are their advocate and part of their safety net. We check their fluids and make sure their vital statistics remain optimised.

The risk of complication after surgery can be mitigated by getting all these things right in theatre. The surgeon and the anaesthetists are focussed on their roles and the ODP works harmoniously within this team.

“We’re a team and everyone’s roles are valued.”

You can never be complacent and you must be alert at all times, as even when it feels as if you cruising along and everything is under control, you might need to spring into action quickly at any given moment.

The patient is paramount and we all have a responsibility to ourselves and our professional practice to challenge something and speak up if we spot anything that needs attention.

Mr Gyorgy Lovasz, Lead Orthopaedic Consultant Surgeon, on carpal tunnel syndrome What is carpal tunnel syndrome?

Carpal Tunnel Syndrome (CTS) occurs when the median nerve becomes compressed at the wrist. The median nerve controls sensation in the thumb, index finger, middle finger, and half of the ring finger, along with some movement in the thumb. When the nerve is compressed and the electrical signals which travel in the nerve are altered, symptoms may appear.

It is more commonly seen in adults between the ages of 30 and 60.

What are the signs and symptoms of carpal tunnel syndrome?

Pain and discomfort in the hand and wrist. This pain may radiate up the arm or towards the fingers. It’s often described as a dull ache or a burning sensation. Another hallmark symptom of CTS is numbness and tingling in the affected hand and fingers.

Patients may experience this in the form of “pins and needles”, particularly in the thumb, index, and middle fingers. These sensations may worsen at night or upon waking People with CTS often develop increased sensitivity to touch.

The affected hand may become more sensitive to cold or hot temperatures. Light touch or pressure on the palm and fingers may cause discomfort or even pain.

Many people with the syndrome report worsening symptoms at night.

You may wake up with numbness or tingling in the affected hand.

What are the causes of carpel tunnel syndrome?

Several factors can contribute to the development of CTS symptoms: a narrow existing carpal tunnel; conditions that affect the bones and joints, such as arthritis,

fractures, or dislocations; frequent, repetitive stress or movements of the hand; hormonal changes; certain medical conditions are associated with an increased risk of carpal tunnel syndrome, including diabetes, thyroid disorders, obesity, rheumatoid arthritis and kidney failure. There can also be a preexisting genetic predisposition to developing the syndrome.

How is carpal tunnel syndrome diagnosed?

Diagnosing carpal tunnel syndrome involves a combination of clinical assessments and diagnostic tests. During the physical exam, a healthcare professional may assess the sensation, strength, and appearance of your hand. They may also perform a Tinel’s test, tapping the median nerve to elicit tingling or electric shock-like sensations.

To reproduce your symptoms, a doctor may bend your wrist down. When a diagnosis is not forthcoming, your doctor may request a set of nerve conduction studies. This involves the use of small electrical currents that are passed across the nerves. The idea is to find out whether the electrical signal slows down or reduces in strength as it travels across the carpal tunnel. This is usually performed in a neurophysiology department.

What is the treatment for carpal tunnel syndrome?

Treatment for carpal tunnel syndrome depends on the severity of symptoms and your specific situation. Initially it is usually non-surgical, and might include some or all of the following: physiotherapy; rest; modification or restriction of activities; use of wrist splints or medication such as anti-inflammatory drugs or steroid injections into the carpal tunnel.

You may also need to adjust your working setup. For example, an ergonomic keyboard and mouse can help to keep your wrist straight and reduce strain.

Will I need surgery?

If the above non-surgical treatments don’t work, or if there is severe nerve compression, surgery may be recommended. Carpal tunnel release surgery involves enlarging the carpal tunnel by cutting the ligament that forms its roof. This relieves pressure on the nerve.

Carpal tunnel syndrome can significantly impact your quality of life. However, if the symptoms are spotted early enough, it is possible to find relief and regain full use of your hands.
