Independent Living
PPH offers 11 different floor plans in 3 buildings, all inter-connected to the main community center. From cozy studios to large two-bedroom with den floor plans, there is a perfect fit for everyone’s taste. All one and two-bedroom apartments feature a patio or balcony, and each floor in every building offers a community lounge for game nights or simply another place to gather with friends and family.

Personal Care
PPH’s Personal Care neighborhood is located in the center of the community and is close to activities, dining, the library and other community spaces. Personal Care residents enjoy their own private residences and amenities, while also being welcomed to take advantage of the entire campus’s spaces.
Residents in personal care are able to get additional assistance with the activities of daily living, such as bathing, medication management, and meals. Our staff is committed to providing assistance while helping residents maintain and achieve their highest level of independence.

SKILLED NURSING REHABILITATION
Memory Care
Our Chapters program is designed to support residents that suffer from memory impairment. This safe neighborhood offers a soothing environment for our residents while providing specialized care so that they can thrive. In addition to care, residents enjoy a life enrichment program that creates a stimulating environment.
Skilled Nursing -Long-Term Care
For those residents in need of the highest level of care, Pathways skilled nursing provides 24-hour care while focusing on dignity and respect for the individual. Residents’ rooms are spacious and bright, with many lounges and dining venues to enjoy. There is also a beautiful outdoor garden with seating for residents and their loved ones to visit.
Rehabilitation
PPH offers a comprehensive therapy program to assist residents in their recuperation from an illness, injury, or surgery. The ultimate goal is to improve their quality of life and get them back to their lives as soon as possible. On-site physical and occupational therapy offers a great convenience to our short-term rehabilitation patients.

Leadership Development
Marge Felix, MSW, LSW, NHA, Associate Director of Social Services

Marge recently completed the 120 hour course work for her Nursing Home Adminstrators license. When asked about how it felt to graduate, Marge stated “it made me feel accomplished,” while also giving credit to her co-workers. “I had a lot of of support. I didn’t think I could do it. It’s a challenging courseload, and challenging tests.”

Shannon Grieb, VP of Marketing & Communications
Shannon graduated from the LeadingAge PA Fellows in Leadership program in May 2023. The program is constructed around the needs of today’s providers. Fellows in Leadership equips leaders with actionable strategies and practical knowledge to help solve real-world challenges in member communities. The program assists emerging and experienced leaders to define, clarify, and practice the skills needed to lead aging services organizations into the future. Shannon stated, “I am honored to be selected to be a part of this program. I have already learned so much and have been able to immediately apply the teachings to my role at PPH.”
