Indications:

Treatmentofacutepain,primarydysmenorrhea.Nimesilshouldonlybeusedasasecond­line drug.Thedecisiontoprescribenimesulideshouldbemadeonthebasisofanassessmentofall risksforaparticularpatient.

Contraindications:

HypersensitivitytoNimesil,anyotherNSAIDortoanyoftheexcipientsofthedrug;

Ahistoryofhyperergicreactions(forexample,bronchospasm,rhinitis,urticaria)in connectionwiththeuseofacetylsalicylicacidorothernon­steroidalanti­inflammatorydrugs;

HistoryofhepatotoxicreactionstoNimesil;Concomitantuseofothersubstanceswith potentialhepatotoxicity;

Alcoholismanddrugaddiction;Ahistoryofgastrointestinalbleedingorperforationassociated withprevioususeofNSAIDs;

Pepticulcerdiseaseintheacutephaseorahistoryofbleedinginthedigestivetract,ulcersor perforation;Cerebrovascularbleedingorotherhemorrhages,aswellasdiseasesaccompanied bybleeding;

Severebleedingdisorders;

Severeheartfailure;

Severerenalimpairment;

Liverdysfunction;

Impairedrenalfunction

Giventhepharmacokinetics,doseadjustmentisnotrequiredinpatients withmildormoderaterenalimpairment(creatinineclearance30­80ml/min),whileinsevere renalimpairment(creatinineclearance<30ml/min),Nimesiliscontraindicated(see section “Contraindications”and“Pharmacokinetics”)

Liverdysfunction.TheuseofthedrugNimesiliscontraindicatedinpatientswithimpairedliver function(seesection“Pharmacokinetics”).Adversereactionscanbeminimizedbyusingthedrug forashortperiodoftimenecessarytocontrolsymptoms(seesection“Peculiaritiesofuse”).

Mode of application

Pourthecontentsofthepackageintoaglasswithstillwater Stirwitha spoonuntilanorange­scentedsuspensionisobtained Drinkthesuspensionimmediatelyafter stirring.

Theabilitytoinfluencethereactionratewhendrivingoroperatingmachinery:

Thestudyoftheeffectofdrugscontainingnimesulideontheabilitytodrivevehiclesorother mechanismshasnotbeencarriedout,however,patientswhoexperiencedizziness,vertigoor drowsinessaftertakingnimesulideshouldrefrainfromdrivingvehiclesorothermechanisms

Overdose:

SymptomsofacuteNSAIDoverdoseareusuallylimitedtothefollowing:apathy,drowsiness, nausea,vomitingandepigastricpain,whichareusuallyreversiblewithmaintenancetherapy Gastrointestinalbleedingispossible Rarelypossiblearterialhypertension,acuterenalfailure, respiratorydepressionandcoma Therehavebeenreportsofanaphylactoidreactionswhenusing therapeuticdosesofNSAIDs,whichcanoccurincaseofoverdose.Incaseofoverdoseof NSAIDs,patientsshouldbeprovidedwithsymptomaticandsupportivetreatment.Thereareno specificantidotes.Thereisnoinformationonthewithdrawalofnimesulidebyhemodialysis,but ifwetakeintoaccountitshighdegreeofbindingtoplasmaproteins(upto97.5%),itisunlikely thatdialysiswillbeeffectiveinoverdose.Inthepresenceofsymptomsorafterasevereoverdose within4hoursaftertakingthedrug,patientscanbeprescribedvomitingand/oractivated charcoal(60­100gforadults),and/oranosmoticlaxative.Forceddiuresis,urinaryobstruction, hemodialysis,orhemoperfusionmaybeineffectiveduetothehighdegreeofproteinbinding. Kidneyandliverfunctionshouldbemonitored.
