CHRONIC ILLNESS Diabetes In addition to the benefits provided under the care plan, the diabetic passport and the diabetic convention, we provide additional reimbursements, e.g. 100% reimbursement of the patient’s co-payment for the ophthalmologist (with the exception of customers who are covered by the care plan or the diabetic convention).

As a patient with diabetes you are also entitled to the reimbursement of extra sessions of psychotherapy and dietetics. You will be reimbursed 30 Euros for the first consultation with a qualified diabetes educator.

The Partena Health Boutique is a recognised supplier of equipment for diabetes.

You are entitled to a reimbursement of 40 Euros per year for a glucose meter and test strips and 10% discount on every purchase valid on the whole range of products (this may not be combined with the 40 Euros reimbursement).

Obesity Reimbursement of 200 Euros for counselling in a multi-disciplinary centre.

For children and adults.

Psychotherapy Reimbursement of 20 Euros per session with a maximum of 12 sessions per year if you have a chronic illness.

Incontinence 30% discount on the purchase of incontinence products from the Partena Health Boutique.

Special clothing Reimbursement of 75 Euros per year for the purchase of special clothing for persons suffering from cancer, eczema, Crohn’s disease or with a stoma.

Wigs Persons suffering from baldness caused by an illness, who are not entitled to any reimbursement via the legal health insurance, are, thanks to Partena, entitled to a reimbursement of up to 270 Euros.

Breast prostheses and reconstruction The legal health insurance grants reimbursements.

Alzheimer’s disease Reimbursement of up to 420 Euros: 30 Euros per day and 14 days per year for the specialised home care service of Baluchon Alzheimer vzw.

Lending Department Wheelchairs, special beds, walking frames,‌.You can rent medical aids here. Reimbursement of 50% of the rental costs if they exceed 200 Euros per year.

Support stockings Under certain circumstances your health insurance fund grants a reimbursement for support stockings. Have a look at the website for more information.

Gluten-free food The health insurance reimburses certain food products in the case of an allergy to gluten.

The amount reimbursed is 38 Euros per month.

HOSPITALISATION AND REHABILITATION Hospitalia Hospitalisation Insurance Discover our cast-iron Hospitalia hospitalisation insurance on page 12 and protect yourself against high hospital costs.

Patient Transport In collaboration with Solidariteit voor het Gezin (Solidarity for the Family) we handle the non-urgent transport of patients to a hospital or rehabilitation centre. In addition, we reimburse 60% of this cost. If you call on another organisation, we reimburse 30%.

No reimbursement is granted for private transport.

For ambulatory (out-patient) care, we reimburse up to 250 Euros per year for non-urgent transport. In the case of a hospitalisation, there is no limit. For chemotherapy, the maximum is 500 Euros per year, and for kidney dialysis a maximum of 200 Euros per month.

For special transport (helicopter) there is a reimbursement of 70% of the invoice, with a maximum of 2.500 Euros. The legal health insurance reimburses 50% of the cost of urgent transport (via the 100 emergency number).

Lending Department You can rent equipment from our Lending Department. We will reimburse per year half of the amount by which the rental costs exceed 200 Euros.

Rehabilitation in a Multi-disciplinary centre Rehabilitation for persons suffering from a chronic or severe condition: a reimbursement of up to 75% of the costs which the patient has to pay for rehabilitation in a recognised centre, with a maximum of 200 Euros.

Care stay in a convalescent home We work together with recognised convalescent homes and will be happy to arrange for your temporary stay. We also grant reimbursements: 35 Euros per day for the patient and 15 Euros per day for the carer, with a maximum of 28 days per year.

Legal advice Free legal advice in the case of disputes with healthcare providers or hospitals.

INDEPENDENT LIVING Caddy Home With Caddy Home you will no longer have to drag your shopping home. This is ideal for older people or people with a disability. You are entitled to 10 Euros discount on every home delivery for orders as of 100 Euros. Consult the regions on www.caddyhome.be.

Lending Department Wheelchairs, special beds, walking frames,….You can rent medical aids here.

Reimbursement of 50% of the rental costs if they exceed 200 Euros per year.

Home modifications and ergo-therapeutic advice the independence reflex

Free professional advice to enable you to continue to manage in your own home as independently as possible. In our Health Boutique you can purchase appliances, both large and small, to assist you, with a 10% discount. Our ergo-therapists can give you free advice, even for very minor modifications.

Tailor-made advice and support from our Social Services Department Everyone can call on the free advice and support of the Partena Social Services Department.

In particular we support persons who are in difficulty due to illness, handicap and old age.

We can help you with your social benefits and reimbursements, home care, patient transport, the Flemish Care Insurance, the VAPH (the Flemish Agency for Handicapped Persons), how to draw up a file for invalidity,….

Home Nursing Care You can ask us for home nursing care; we will be happy to arrange this for you.

The payment of the nursing care is usually handled directly by the health insurance fund.

PAS Personal alarm system Thanks to PAS, persons needing additional care are in contact with an alarm centre 24 hours a day. Should they have a fall or some other emergency, they can quickly notify the help line with this system. We can also handle this for your parents. Customers with an increased level of reimbursement receive 2 Euros discount per month for the rental of a PAS-appliance from our partner Telehulp.

Home help for you or your care-dependent partner We organise care based on your needs: help for families and old people, home nursing care, a cleaner with or without service checks, babysitting, a handyman, hot meals and palliative care. You can count on having a care package which will correspond to your needs.

Attendance at day care centres We reimburse up to 250 Euros per year (6 Euros per day or night or 3 Euros per half day).

Short stay in a rest- and care home 15 Euros per day with a maximum of 14 days per year, if the carer is temporarily unable to take care of him/her.

Care holidays Holidays with 24h/24h medical and care services by Belgian health care providers on the Costa Blanca, in Tenerife, Mallorca, Cyprus and a ‘dialysis holiday’ in Benidorm. 5% discount.

Feel good about yourself PREVENTION Dietetics Reimbursement of 10 Euros per consultation, up to 40 Euros every 2 years.

Also valid for Weight Watchers.

Weight Watchers You are entitled to a discount on the Combi Pas or the online programme.

Mindfulness Discount of 50 Euros on various mindfulness trainings with our specialised partner ITAM.

Stopping smoking The legal health insurance provides a reimbursement for help in overcoming addiction to tobacco. There is a higher reimbursement for pregnant women.

Vaccines We reimburse 80% of the purchase price up to 25 Euros per year.

Valid for all recognised vaccines.

Preventive examinations For consultations and/or examinations related to the prevention or detection of cancer, cardio-vascular diseases, allergies, osteoporosis, respiratory disorders and diabetes (e.g. cervical cancer (smear test), prostate cancer, bowel cancer), you are entitled to a reimbursement of 10 Euros/consultation with a maximum of 3 per year.

HEARING & SIGHT Glasses and contact lenses Reimbursement of up to 50 Euros every 2 calendar years. In addition, with our network of opticians, you are entitled to a discount of at least 15% on lenses and frames and up to 15% discount on the purchase of contact lenses.

Corrective eye surgery A one-time payment of 100 Euros per eye for the surgical correction of an eye disorder (laser surgery) or the implanting of intra-ocular lenses (e.g. in the case of a cataract).

Custom-made earplugs NEW

Reimbursement of 30 Euros per year for custom-made earplugs, purchased from a hearing care professional recognised by the RIZIV (the National Institute for Health and Disability Insurance). In addition you are entitled to 15% discount if you purchase your earplugs from our partner, Lapperre.

Hearing aids The legal health insurance provides various reimbursements.

Ambulatory (out-patient) Care Insurance Learn more about the additional reimbursements of our Ambulatory Care insurance on page 13.

BODY & MIND Membership fee for sports clubs 15 Euros per year for everyone who is a member of a sports club.

Fitness Network You are entitled to a discount of up to 20% on your annual subscription fee to a number of fitness centres, plus 1 week’s free entry with the services of a coach. This is only valid when you register as a new customer.

Alternative therapies Osteopathy, acupuncture, chiropractic, homeopathic consultations, mesotherapy, shockwave therapy, manual therapy and reflexology: 10 Euros per session, up to 60 Euros per calendar year.

Homeopathic products A reimbursement of 50% of the purchase price of prescribed homeopathic products, with a maximum of 75 Euros per person per year.

Treatment of allergies Also called desensitisation, this makes the immune system less sensitive to allergic disorders.

We reimburse 80% of the cost price up to 50 Euros per year, for 3 years.

Pedicure We grant a reimbursement of up to 30 Euros with a medical prescription.

People over the age of 65 do not need a prescription.

Biobello bio vegetables and fruit When you purchase 10 baskets from Biobello, you get the 11th basket free of charge.

Sleep therapy NEW

We grant a reimbursement of up to 100 Euros if the treatment is carried out in a general or university hospital or in a sleep centre.

Equicoaching Equicoaching is a form of experience-based therapy which takes place using the movements of a horse. This can help people to improve their mental balance, to become more assertive and to deal with stress. For children and adults. 10% discount for each session with Equicoaching International.

Baby and children PREGNANCY & BIRTH Info session the checklist reflex

What is supposed to happen during pregnancy and after the birth? What are your rights and obligations? What reimbursements and benefits will you receive? We will answer all your questions in a session which takes about one hour.

Pregnancy gift A carry bag with high-quality items for baby or a handy hook-on highchair, an info brochure and a book of discount coupons.

Non-invasive prenatal test (NIPT) This revolutionary blood test to detect Down’s Syndrome is expensive and not yet reimbursed by the legal health insurance. Partena reimburses 150 Euros per pregnancy.

Birth- and adoption premium You are entitled to a fixed premium of 400 Euros per baby.

Maternity care We organise and coordinate maternity care and reimburse up to 150 Euros (max. 30 hours at 5 Euros per hour).

Maternity-, birth- and adoption leave During this period, you will receive a replacement income via Partena Health Insurance Fund.

Pre- and postnatal care (midwife) We coordinate pre- and postnatal care. We cover the complete cost of a consultation or a home visit by a duly registered, self-employed midwife.

Pre- and postnatal physiotherapy The health insurance provides a reimbursement for 9 sessions.

Alternative therapies for your baby For e.g., consultations for osteopathy and homeopathy: 10 Euros per session, up to 60 Euros per calendar year. We also reimburse a part of the purchase price of prescribed homeopathic products. Read more on page 7.

Fertility treatments The legal health insurance provides a reimbursement for in-vitro fertilisation.

Baby and children

Home deliveries 500 Euros reimbursement if you are insured with Hospitalia Plus and 200 Euros if you are with Hospitalia.

Hospitalisation insurance With Hospitalia Plus there is no need to worry about the invoice for your hospitalisation.

You are also entitled to reimbursements for e.g. maternity care, physiotherapy and preand post-hospitalisation costs. ➔ Learn more about Hospitalia on page 12.

FROM TODDLER TO TEENAGER Bicycle helmet Your child will receive a bicycle helmet in the year he or she turns 4.

Membership fee for sports clubs 15 Euros per year for everyone who is a member of a sports club.

Speech therapy A reimbursement of 6,50 Euros per session, with a maximum of 1.000 Euros for 5 years.

Child care Partena Child Care (child-minders and day-care centres) are happy to provide a second home for your little one. ➔ To find one near you: www.partena-kinderopvang.be

Babysitter for your children You can call on the professional and handy babysitting services of our partner, Sloebersit.

With a discount: the administrative costs (Sloeberkits) are free of charge!

Home care for sick children If your child is sick, you can have a professional babysitter come to your home for only 3 Euros per hour, for 12 care days per year and per child (with a maximum of 3 consecutive days).

Preventive dental care The legal health insurance reimburses the patient’s co-payment for the annual preventive oral check-up for children under the age of 18.

Orthodontics Up to 800 Euros

A reimbursement of 375 or 800 Euros, depending on the seriousness and necessity for the treatment. The request must be made before 15 years of age. ➔ Learn more about the extra reimbursements of our dental care insurance Dentalia Plus on page 13.

Psychotherapy 20 Euros per session for max. 6 sessions per year, up to maximum 25 years of age (persons entitled to family allowance).

Tutoring- and homework assistance with Educaplus This tutoring service offers individual and professional guidance personalised for all pupils in primary and secondary education. You pay 30 Euros per hour of tutoring, regardless of the number of hours. This preferential tariff is normally only applied as of a block of 30 hours.

Bedwetting alarm A one-time amount of 50% of the rental- and purchase invoice is reimbursed, with a maximum of 65 Euros per invoice.

Orthopaedic soles For made to measure orthopaedic soles, you are entitled to a reimbursement of up to 24,27 Euros per sole.

Holiday Camps and activities lasting several days Per calendar year you receive per child: 40 Euros for activities lasting several days such as camps (residential and non-residential), playgrounds and school activities. 75 Euros for health camps which the children attend for medical reasons. 250 Euros for a holiday camp for disabled children.

Hospitalia, cast-iron hospitalisation insurances The Hospitalia insurances Hospitalia and Hospitalia Plus are our optional insurances with a competitive price-quality ratio.

Hospitalia Plus covers, among other things, the following costs, with no deductible or limit: • stay in a single room • all room supplements • supplementary fees up to 300% • reimbursement of the patient’s co-payment for medical care 30 days before and 90 days after the hospitalisation

Protection if you are covered by a group insurance policy: Hospitalia Continuity What happens to your group insurance if you change jobs or go on retirement? If you have a Hospitalisation Insurance via your employer, you can enroll with Hospitalia Continuity.

This transitional policy will ensure that you remain insured if you change jobs. Moreover, at a very low price.

As soon as your group insurance comes to an end, you can transfer to Hospitalia Plus with no medical questionnaire or waiting period.

Benefits of Hospitalia Continuity: • no age-related increased premium when you transfer to Hospitalia • no waiting period, therefore no interruption in your protection • no additional medical questionnaire, therefore no risk of medical restrictions • while you are still covered by your group insurance, we will pay the hospitalisation costs which are insufficiently covered (50 Euros per day)

Optimally Insured

The optional dental care insurance Dentalia Plus guarantees you a reimbursement of dental care for which the compulsory insurance does not intervene or only partially.

Reimbursement for: • orthodontics • prosthetics • implants • preventive and curative dental treatments • periodontology ➔ With Dentalia Plus you get back up to 1.025 Euros extra, per year, per person.

Ambulatory (out-patient) insurance Are you planning to go to the doctor? Do you need expensive medicines, speech therapy, glasses, crowns or a hearing aid? You prefer not to cut corners on good medical care for yourself and your family. That is why we offer you the financial projection of Ambulatory.

Thanks to this extra insurance, you will receive a reimbursement for medicines and medical assistance not linked to a hospitalisation.

Optimally insured The contribution fee for the basic insurance package is 7,50 Euros per month per policy holder.

For this price we guarantee you a smooth reimbursement of medical costs such as a visit to the doctor. We also pay you a replacement income if you are unable to work. For example, if you are pregnant or if you have an accident and cannot work for some time.

In addition, you are entitled to all the Partena benefits and you can call on us for e.g. free (legal) advice and the help and support of our Social Services Department.

This contribution fee is only payable for the policy holder, dependents are free of charge.

A small example to better clarify the above: A family with a working mother and father and three children pays 15 Euros per month.
