thought I had seen it all but leave it up to the internet to prove me wrong. I have seen that there are 2 companies on the internet that offer “do it yourself” aligner treatments. Yes, you heard me right. First you take a “selfie”

of your smile and send it to them. (No x-rays, no exam for pathology, no nothing). Once they get it, they will determine if you could use their aligner system. Do you really think they would say NO! If so, they will send you a kit so you can take your own impressions of your teeth for a “modest fee”. Taking impressions with accuracy is difficult enough for a trained professional and according to Florida law has to be taken by a licensed practitioner. Once and if it’s accepted, a trial tray is sent to you for fit. You decide if the fit is correct (not an Orthodontist with training). If you feel the fit is acceptable, they will send you your aligners. Now the fun part begins. You have to determine if they are working. I’m not sure if there is a problem and

who determines what a problem is or is not, where does the responsibility lie? This is really scary to say the least. If interproximal reduction is necessary (removing a small amount of tooth structure between teeth to help resolve crowding) you are sent to a participating dentist for a onetime $200 fee for that to be done. Yikes, what people will sell to make a buck! I cannot believe we have come to this. If you are really concerned about your smile and your bite seek professional care from a reputable licensed Orthodontist who has been trained to take care of you. We will have to see where this all goes. I am waiting for a “do it yourself appendectomy kit with a money back guarantee”. Sounds funny but that where we are headed.

IT'S TIME TO FOCUS ON HEALTH CARE... Not Sick Care

T

here has become an inexhaustible amount of medical technology and natural cures claiming to either get you well or enhance your health in some way. But although billions of dollars are being spent and more and more doctors and specialists are going into practice every year, the amount of illness, depression, disease and the number of people on prescriptions dramatically increase every year. America has become one of the sickest countries in the world. When it comes to health, medical tech-

L

osing weight, getting in shape, dropping a pants size: these are phrases we associate with improving our general health. A desire for weight loss has many motivations. We may not like the way we look, or the way that older clothing feels tight or uncomfortable. Perhaps we have noticed a drop in energy, that we feel lackluster or less vibrant. The impact weight loss can have on an individual’s confidence and energy levels is definitely an important factor in making changes toward a healthier lifestyle. However, what the scientific community has continued to discover in recent years and decades is that weight loss also has a greater impact on our internal health, and lack thereof, than any other single factor. Excess weight is, in fact, considered to be a disease. Obesity is a compounding of many hormonal imbalances, which lead to chronic inflammation in the body and are ultimately linked to almost every disease or condition that exists. Insulin resistance is a major hormonal imbalance involved in obesity. When you carry excess weight, your body is no longer effectively using its own insulin. Less effective usage means the body requires you

to produce more insulin to accomplish the same job as before. Insulin, in high levels, is the number one cause of fat and of pro-inflammatory hormones in the body. Too much insulin makes you gain more weight, which makes you produce more insulin, creating a vicious cycle. Excess weight leads to a propensity toward liver and kidney dysfunction, cardiovascular disease, stroke, arthritis, joint inflammation, and even cancer. The thyroid is also greatly affected by excess weight. This endocrine gland regulates metabolism, which in turn regulates our energy levels. Acting as a traffic controller, the thyroid monitors bodily functions to make sure things run smoothly. In order to function properly, the thyroid hormone T4 must convert effectively to the hormone T3. This requires an enzyme that comes from tissue in the liver and kidneys. Obese people do not make adequate levels of this enzyme, which prevents T4 from being properly converted, which slows your metabolism and creates another cycle in which you continue to gain weight. Testosterone plays a hormonal role in weight gain as well. Testosterone is linked to our ability to build and maintain muscle, cardiovascular health, optimism, level of satisfaction within relationships (personal, professional, etc.), and of course sexual energy. We function more optimally with appropriate testosterone levels, but as we gain weight, testosterone drops, libido drops, our level of happiness drops, and we become more susceptible to disease. Fortunately, with proper lifestyle changes, many of these conditions can be reversed. Weight loss can significantly improve testosterone levels, thyroid levels, and insulin resistance over time.

In our weight management program, our goal is to help people change their lifestyle. We focus on three major areas: Exercise: Start slowly, perhaps walking 2-3 times a week. Keeping an exercise routine fresh not only makes it mentally interesting, but is great for your body. A diversified regime might include weightlifting, walking, and yoga. WHOLE GRAIN AND GLUTEN FREE BY CHARLYN FARGO

F

rom amaranth to quinoa, millet, teff, buckwheat and sorghum, ancient grains are increasingly popular. "I'm not entirely gluten-free, but I'm moving in that direction," says Camilla Saulsbury, author of Bob's Red Mill, "Everyday Gluten-Free Cookbook." She became interested in ancient grains to add variety to her diet. She ended up with 281 whole-grain recipes. The book is published by Robert Rose Inc. "Working with all these ancient grains was a little daunting in the beginning, but I had so much fun creating new recipes," she said. "The ancient grains are so versatile." Millet can be used in a creamy polenta, the seeds can be toasted like nuts or millet flour is a one-to-one substitute for all-purpose flour. "It has incredible diversification and a unique flavor," she said. "With ancient grains, you're not taking anything away, you're adding to your diet. It's about more, not less." Saulsbury is a food writer, cooking instructor and writes a cooking blog, called Power Hungry. She is also a fitness trainer and endurance athlete and holds a doctorate in medical sociology. Her love for cooking was sparked by working for Alice Waters' restaurant in Berkeley, California.

"I didn't realize at the time who Alice Waters was -- this was 25 years ago," said Saulsbury. "Cooking has been a big part of our family life. My mom was a dedicated cooperative member, and after college, I began catering and doing food writing on the side. Being a fitness and long distance runner fed into the way I cook, and I began developing recipes. I wanted to eat things that power my activities. Ancient grains fit right in."

Q: Will keeping a food record help me improve my eating habits? A: Studies show that people who keep track of eating behaviors tend to be more successful at changing them. If you're considering using some kind of paper or online food diary, think about what you're trying to achieve. If you already know which habits you want to change, you may be successful keeping a simple record focused on one or two specific behaviors you want to change, such as evening snacks, soft drinks or second portions. On the other hand, if you overeat or lack balance in your food choices but aren't sure when, why, and how much you eat, one way to find out is to record what you eat all day. Include notes of time, portion sizes, where you are eating (restaurant or home, kitchen table or sofa with TV), how hungry you are (1 to 10 scale) and whatever thoughts or emotions you can pinpoint. All this yields crucial information to identify specific problem areas and provide insights about what needs to change. Don't track only meals; for many people, it's snacks and unplanned eating here and there that adds up to create the most trouble.

If you're not sure what to do with the information you have, use a website that compares your eating to calorie- and nutrient-based standards such as the SuperTracker at choosemyplate.gov. Or send your records to a Registered Dietitian Nutritionist who agrees to provide you with feedback. If you have a history of going on and off diets or are vulnerable to give up at setbacks, reconsider how you use food records. You can foster a positive attitude with a log that keeps track not of failures, but of successes (like how many times you workout). Look at a completed food record like a detective, looking for clues on what changes might be most helpful. Attitude is crucial: You need to expect gradual improvements, not immediate perfection. Recipe

One of Camilla Saulsbury's favorite recipes in her book, Bob's Red Mill, "Everyday Gluten-Free" cookbook is a breakfast "cupcake" made with millet, apple and raisins. Here is the recipe. BAKED MILLET, APPLE AND RAISINS BREAKFAST CUPCAKES • 1 1/2 cups unsweetened apple juice • 2 large tart-sweet apples, (such as Gala), peeled and shredded • 1 cup millet • 3/4 cup raisins or other chopped dried fruit • 1/4 teaspoon fine sea salt • 2 tablespoons warmed virgin coconut oil • 1/2 cup chopped toasted nuts or seeds In a medium saucepan, bring apple juice to a boil over medium-high heat. Reduce heat to medium-low and stir in apples, millet, raisins, salt and coconut oil. Cover and simmer for 5 minutes. Remove from heat and let stand for 15 minutes. Uncover and stir in nuts. Meanwhile, preheat oven to 350 degrees. Divide millet mixture equally among prepared muffin cups. Bake for 25 to 30 minutes or until tops are pale golden and slightly puffed. Let cool in pan on a wire rack for 15 minutes, then transfer to rack to cool completely. Makes 12 muffins. Note: An equal amount of olive oil or melted unsalted butter can be used in place of the coconut oil. An equal amount of amaranth or quinoa, rinsed, can be used in place of the millet.

Dear Ashley, First off, when it comes to Kim K, this is simply my opinion because I am not her stylist, but she likely used a product called Olaplex. Olaplex is a relatively newer product available. When hair is chemically treated (ie. hair color, straighteners), the internal bonds of the hair are broken down. Sometime the bonds are broken down enough that the hair is damaged or even breaking since there are no bonds to hold the hair together. Olaplex goes in and repairs the broken bonds so the hair is stronger. Olaplex is added to hair color or it can be used to as a standalone hair strengthening product. As for keeping your blonde bright, here are a few things to remember: - Blonde hair is usually very porous so it will absorb anything put on the hair. Make sure that you are using clear or white-colored hair products. This could

be anything from shampoo, conditioner or styling products. Even the slightest tone of yellow in your products can discolor your blonde over time. - Use a clarifying shampoo once every 5-7 shampoos to pull out unwanted mineral deposits or anything else your blonde hair has absorbed. - Its very important to do a Deep Conditioning Treatment after clarifying as this type of shampoo will pull out all your natural oils. PULLING A FAST ONE BY SCOTT LAFEE

T

he secret to a long and healthy life may not be skipping the occasional meal. Rather, it may be skipping a day of them and then gorging the next. Researchers at the University of Florida College of Medicine recently published a paper that showed that participants who cut their calorie intake by 75 percent one day and then ate 175 percent the next scored better on a variety of biochemical health markers. The participants alternated days of undereating and overeating for three weeks. For the average male, that meant consuming 650 calories on fasting days and 4,550 calories on feasting days. Some participants also took antioxidant supplements, such as vitamins C and E. At the end of the three weeks, the researchers found that beneficial blood markers tended to be higher in the feast/ famine folks. Interestingly, those who took the antioxidant supplements showed

lesser benefit. The scientists speculate that the fasting days kicked up the body's immune system, applying a little oxidative stress, which prompted enhanced regeneration and repair of cells. Intermittent fasting also decreased insulin levels, indicating it may have an antidiabetic effect, as well. More research is required, of course. The study was small, with just 24 participants. But it's something to chew on, assuming it's not a fasting day.

the chance to live in Europe and learn the history of the arts from a different perspective. In 1996, she moved to Tampa, Florida to complete her studies at USF. She resides in Ybor City where she works as a marketing consultant and fine artist. While taking her dogs for walks through Ybor City, Joanne finds rusty objects which she incorporates into some of her art. She also experiments with food dyes and recycled materials. "My work is an abstraction of real life combined with a riot of images, which appear as a result of improvised brushstrokes. As a traveler, I capture the small details each culture has to offer and transform a blank canvas into a dance, a performance, a scene, a documentary or simply a memory from a different country."

Dali Hernandez is the Creative Director and head designer for House of Dali. She is also a Real Estate Agent operating in the Tampa Bay area. Dali’s arrival to the fashion industry has been making numerous headlines; however designing is nothing new for this NY native. AN OASIS OF YOUR OWN BY JOSEPH PUBILLONES

A

n oasis for peace and quiet is essential to the well-being of each and every one of us. We all need a private area for private moments. The bedroom, the bathroom and sitting areas are the most logical places for this type of space. These spaces, above all, should be personal and meaningful. Designing a haven where good sleep can be achieved, books can be read, TV can be watched and love can be made is no easy task. Sharing these spaces with your loved

ones can be difficult, especially if your tastes and needs are vastly different, but don't fret. There is no need to run to the divorce lawyer. Designing for the sexes can happen amicably. Just like when you first start dating someone, finding common ground is part of the task and can be equally as fun. Decorating to satisfy two persons can be done. Sometimes you'll find that you agree on certain decorative things, and that's both good and easy. Other times, when you disagree, that can also be a good thing in that it can help you to achieve a balanced interior design. "Plain" and "fancy" have different meanings to everyone. Plain doesn't have to mean dull, tasteless and/or sallow, but might mean simple and uncomplicated,

with a preference for clean lines and basic colors. By the same token, fancy doesn't have to mean garish or overdone, but might make reference to luxurious textures and custom finishes. Deciding the overall feel for the bedroom is the perfect place to start. Keep in mind that creating a peaceful place is the main goal. The predominant style of the bedroom is really a matter of preference. Traditional, contemporary and eclectic styles all offer comfort, as long as you like it. Selecting a color palette that is conducive to rest and relaxation is uber-important. Generally, soft and dusty colors in the pastel range are the best for bedrooms. On the other side of the color spectrum, dark colors such as navy, green and brown are also viable colors for bedrooms and sitting areas because they recede and offer a warm "nighttime" feel all day long. Bathrooms often do well in lighter color schemes, but lately trends and materials are showing darker colors in bathrooms to be equally beautiful. Sitting areas can be located almost anywhere there is space: a corner of your bedroom, a living room nook and even tucked away underneath a staircase. These areas are meant for reading and quiet contemplation of all that happens around you. Adequate light, whether from a window or from a lighting fixture, will make a better sitting area. Some sitting areas have the added benefit of a view, and when they don't, a great piece of art can act as a virtual window. HAVE YOU EVER THOUGHT ABOUT recycled sea shells for countertops? BY GENE MCDONALD

L

ike many of the countertops I make, it’s always something different. I love the recycled products and designs out there, but this request was something that really got me excited. The customer asked if I can remove some sea shell mulch from her driveway and make countertops with the shells. Of course I said yes, and then figured out if it is possible or not. We actually went to her home with some empty buckets and a shovel and removed the shell mulch. We cleaned the shells in our shop and removed twigs. We then dried out the shells and were ready. We templated her existing countertops and came back to our shop to build the new

molds. It was exciting watching another way of using recycled materials. Many people think recycled materials for countertops are boring, bland and just out right ugly. But looking at these countertops, those three words never cross your mind. I also think they are not over the top like some other creations I make where they may be too personalized. These shells look great and can be used for beach homes, condos, and even seafood restaurants. But the real concept of diverting all these shells from the landfill has an inner happiness as well. Now you can talk about a story behind the surface, it’s a wonderful conversation piece. These

shells were poured into a premium super durable epoxy which is stain and scratch resistant. Much like quartz type maintenance, you cannot cut on it but the non-porous surface gives super resistance to everyday stains. What other materials would do you guys think would be a great idea for countertops, table tops, and similar applications?

VITAMIN D UPDATE BY CHARLYN FARGO

A

fter a recent blood test, I was surprised to find out I was low in vitamin D. I shouldn't have been surprised -- most Americans are. Despite being the "sunshine" vitamin, meaning our bodies can make vitamin D from the sun, most of us simply don't get enough of it. There are two types of vitamin D -vitamin D-2 and vitamin D-3. Our bodies prefer vitamin D-3, so that's the vitamin I chose. The current "adequate intake" for vitamin D is 400 to 800 IU daily, depending

on age. This is far less than the 10,000 IU per day that human skin can synthesize. Indeed, 10,000 IU of vitamin D is well in excess of the Food and Nutrition Board's guidelines, which specify that healthy adult should not consume more than 2,000 IU daily or they may be risking vitamin D toxicity. However, there is no evidence that people ever develop vitamin D toxicity and/or hypercalcemia as a result of exposure to large amounts of tropical sunlight for prolonged periods of time. Therefore, it seems likely that amounts of vitamin D-3 well in excess of what is currently considered the upper limit of safety are perfectly natural for the human body. Indeed, there is growing evidence that the recommended "adequate intake" for vitamin D is likely too low for many people. What's happened is that more and more Americans are avoiding even tiny amounts of sunshine on their skin for

fear of getting wrinkles, age spots and skin cancer. Sunscreens are spread over exposed areas of the body to block the dreaded UV rays. However, it is these same UV rays that enable skin cells to synthesize D-3 from cholesterol. Without regular sun exposure, the human body becomes dependent on dietary sources of vitamin D. Vitamin D is found naturally in significant amounts in few foods. Fatty fish are the best source and modest amounts are also present in liver and eggs. Cow's milk is fortified with vitamin D-2, and, for most Americans, this represents the major dietary source of vitamin D. A study reported in the New England Journal of Medicine found over half of 290 consecutive hospitalized were deficient in vitamin D. However, 37 percent of those patients with intakes above the recommended amount were also shown


muscle pain and weakness. For those unable to get sufficient sunshine, a supplement of 1000 IU daily of vitamin D-3 should supply a safe, adequate amount for nearly all adults. Information courtesy of the American Journal of Clinical Nutrition.

In the absence of sunlight, it appears that at least 1,000 IU of vitamin D is needed to maintain adequate levels in the blood. A lack of vitamin D leads to increased bone loss, osteomalacia (weak bones) and osteoporosis, which can increase the risk of hip fractures. Supplements of vitamin D in the 700-800 IU/day range have been shown to result in fewer fractures. Inadequate levels of vitamin D can cause hormonal and other changes in the body, which can lead to muscle weakness, pain, and fatigue. The bottom line: Natural vitamin D-3 produced by human skin exposed to natural sunlight is the best source of vitamin D. Consuming more fatty fish and spending more time outdoors in the sun should help reduce the risk of at least several common cancers, as well as osteomalacia and osteoporosis. It may also help relieve psoriasis,

Q: I'm trying to lose weight, but each afternoon around four o'clock my energy slumps and I end up eating junk food. How can I stop this habit? A: It sounds like you're running out of fuel. Perhaps your lunch is too small to keep you satisfied through the afternoon. If you prefer to eat a lighter lunch, get proactive and plan a small but nutrient-rich snack for a half-hour or so before your slump usually comes. Take this snack with you so you don't have only the junk food available. Keep the snack to 100 or 200 calories of foods that slowly release energy such as foods with protein, fat or fiber. For example, pair some fruit with yogurt, nuts or whole grains. Make sure you're drinking enough water, since if you get dehydrated that can also leave you feeling zapped. The types of foods you choose for lunch may also affect your energy. If your lunch is nothing but refined carbohydrates, such as sweets or a low-fiber grain like a large bagel, or even plain vegetables or salad with no protein, your blood sugar may go up and down again within a few hours, leaving you feeling pretty run-down. To avoid that slump, focus your lunch around modest portions of whole grains plus vegetables and/or fruit, and make sure to include some healthy protein (poultry, seafood or lean meat, low-fat dairy or a full serving of beans or nuts). If you're simply skipping lunch or thinking you shouldn't eat more at lunch, you may not be getting enough energy to get you through the afternoon. Try doing what many people find works well: Aim to get about a quarter to a third of your total daily calorie needs at lunch. People vary in what calorie level is right for them, but as an example, someone keeping calories to 1600 a day for weight loss might aim for 400 to 500 calories at lunch (depending on how much snacking they prefer to do and how they spread out meal times).

That's why a small frozen entree, plain cup of soup, or energy bar usually won't suffice. If these strategies don't work perhaps the slump you feel is not about hunger. You may need to get re-energized by getting up and moving around, switching tasks or taking a few minutes for deep breathing breaks. Information courtesy of American Institute for Cancer Research.

This recipe, Grilled Chicken Salad with Orange Vinaigrette, is a great celebration of spring and lighter fare. It's from Cooking Light magazine. Grilled Chicken Salad with Orange Vinaigrette • 4 (6-ounce) skinless, boneless chicken breast halves • Cooking spray • 3/8 teaspoon salt, divided • 1/2 teaspoon freshly ground black pepper, divided • 1/2 teaspoon grated orange rind • 2 tablespoons orange juice • 2 tablespoons light sesame oil • 1 tablespoon rice vinegar • 1/2 teaspoon Dijon mustard • 1 head Bibb lettuce, torn • 1 1/2 cups cherry tomatoes, halved • 1 cup diagonally cut celery • 1/3 cup unsalted, dry-roasted cashew halves Heat a grill pan over medium-high heat. Lightly coat chicken with cooking spray. Sprinkle chicken with 1/4 teaspoon salt and 1/4 teaspoon black pepper. Add chicken to pan; cook 8 minutes, turning once. Remove from pan; let stand 5 minutes. Thinly slice chicken across the grain. While chicken cooks, combine remaining 1/8 teaspoon salt, remaining 1/4 teaspoon pepper, orange rind, and next 4 ingredients in a bowl; stir. Reserve 1 tablespoon vinaigrette. Place lettuce and remaining ingredients in a bowl. Add vinaigrette; toss to coat. Divide salad among 4 plates; top with chicken. Drizzle with reserved vinaigrette. Serves 4 (serving size: 1 chicken breast, 2 tablespoons vinaigrette, 1 1/4 cups salad). Per serving: 345 calories, 39 g protein, 9 g carbohydrate, 16.8 g fat, 109 mg cholesterol, 2 g fiber, 457 mg sodium.

PRIORITIES IN LOWERING THE RISK OF HEART DISEASE BY DAVID LIPSCHITZ

H

eart disease is the leading cause of death in the developed world. Although occurring at a later age, heart disease is just as common in women as in men. Despite this fact, fewer than 1 in 5 women believe that heart disease is a significant health threat even though 1 in 3 deaths in women each year is from heart disease, which is more than from all causes of cancer combined. While every adult is at high risk for heart disease, there are a series of factors that increase the chances of heart disease substantially. The disease is much more common in men than in women until menopause, after which time the risk is essentially the same. And with each advancing year heart disease becomes more

common. A strong family history of heart disease, particularly if symptoms occur under the age of 50, cigarette smoking, elevated cholesterol, high blood pressure, a sedentary lifestyle and obesity, all substantially increase heart disease risk. Heart attacks and other forms of heart disease are much more common in women with diabetes, but for reasons that are ill understood, diabetes does not have a similar impact in men. And anxiety and high levels of stress do not bode well for preventing heart attacks. For most adults, an assessment of cardiac risk should be done at age 30. Occasionally, markedly elevated cholesterol is identified, necessitating the need for treatment. Blood pressure should be measured at least annually, and if elevated, approaches to bringing the levels to normal is a must. Hypertension may or may not require treatment with medications. Screening for diabetes should be done every five years until age 60 and more frequently thereafter. Whenever a routine physical is done, time should be spent counseling on how to live long, age well and prevent heart disease. We are all aware of the critical importance of not smoking, diet, exercise,

maintaining an ideal weight and staying as stress free as possible. But as a nation we are not doing as well as we should in meeting these goals. If will power is involved, most of us fall short. It is for this reason that we try and do as much as we can to maintain a healthy lifestyle, without giving up because our lofty goals are not achieved. We focus far too much on weight loss and our body image. We all want to "get into shape," hopefully to become a slim beautiful woman or a male Adonis. Hating the way we look is disastrous. We need to be proud and pleased with ourselves and, most importantly, have high self-esteem; perhaps the single most important factor in living long, aging well and reducing heart attack risks. I recommend to all my patients that no matter their shape, they look in the mirror and say, "I am gorgeous." And this applies to men, too. You must feel good about yourself, confident in your accomplishments and in the way you look and dress. Believe it or not, your weight is not an important risk factor in preventing heart disease. In fact, those who are overweight but not obese, live longer than those who are thin or obese. It is also far more important to be fit and fat than thin and sedentary. Nothing is more important than exercise. And while more is better than less, walking at a reasonable pace for 30 minutes, four or five times weekly, has almost as much benefit as jogging 10 miles daily. And forget all the "too" excuses: I am too tired, too embarrassed, too sick, in too much pain; it's too expensive, too far, too cold or too hot to exercise. No matter what, there is always an exercise that is right for you. Try as much as possible to eat a balanced diet containing the right fats, proteins, vegetables and starches. Learn how to relax; have love in your life; laugh and have faith in yourself and in others. These are the keys to reduce your chances of heart disease. It's not rocket science, and no matter your age, the presence or absence of chronic disease and the stresses of life, you can do much to delay heart disease for decades or even forever.

WHEN IS AN OLDER DRIVER A DANGER ON THE ROAD? BY DR. DAVID LIPSCHITZ

T

elling an older person he should no longer drive is difficult. This can be a devastating blow to the patient, who may resist the suggestion vigorously. No longer being able to drive takes away one's independence and increases isolation, depression, risk of illness and a likelihood of placement in a nursing home. It is for those reasons that decisions about driving must be taken seriously, must be based on sound medical facts and must not rely on frantic family members (usually children) who are overly concerned about their parent's safety. Recently, an angry patient, whose family had forbidden him to drive, came to see me brandishing a report published by the RAND Corp. that showed that "seniors" are safer drivers than young adults.

It reported that drivers older than 65 have fewer accidents than those younger than 25. The older group causes only 7 percent of accidents, compared with 43 percent for the younger group. "Of course I should drive," this 85-year-old man insisted angrily. Sadly, the RAND report was misleading. Lumping everyone older than 65 together fails to tell the whole story. Many older people elect to stop driving, drive shorter distances and do not drive at night. According to a report published by the National Cooperative Highway Research Program, the number of crashes per 1,000 licensed drivers averages 68 per year, whereas there are only 37 per 1,000 involving drivers older than 65. However, as there are fewer older drivers, a better way to assess risk is to evaluate the number of accidents in relation to the number of miles driven. If we use this approach, the safest drivers range in age from 55 to 64, averaging 3.2 crashes per 1 million vehicle miles driven, compared with 8.2 crashes for those ages 20 to 24, which happens to be the same rate as the one for drivers older than 85. Of greater concern is the number of fatalities. In the United States, for every 1,000 accidents, there are two deaths. For those between the ages of 60 and 74, the number of deaths is 3.2 per 1,000 crashes,

increasing to 8.56 per thousand crashes for those older than 85. Furthermore, the percentage of times the driver is at fault increases with age. Only 42 percent of 50to 64-year-olds are at fault in fatal accidents. The percentage is 72 for those older than 85. We all know of the sad story of the man with Alzheimer's disease who crashed into a market in Santa Monica, California, killing 10 people. And recently, a patient in my office who has a physical disability slammed his foot on the accelerator instead of the brake, smashing into one of our nurse's offices, creating havoc. Had she been at her desk, she would have been crushed. There is no question there are many safe drivers who are older than 80, but just like the case with teenagers, the risk increases. Here it is because of declines in visual acuity, physical disabilities that can lead to longer reaction times and alterations in cognition that can lead to failure in judgment. Medications such as sleeping pills (including Benadryl), as well as other drugs, including alcohol, of course, also contribute. Most people with physical and visual disabilities have the insight to stop driving if they feel unsafe. This often does not apply to patients with memory problems. Anyone who has gotten lost on more than one occasion, has been in a number of fender benders or is believed by family members to be a danger on the road should be closely evaluated. Studies have shown that patients with early Alzheimer's disease who drive in familiar surroundings are not at increased risk of accidents. However, the ability to drive deteriorates as the disease progresses. If any question about driving ability exists, a driving test, either on the road with a tester or on a computer, should be done. In addition, every older person should consider taking courses offered by AARP to help improve driving skills. Just as we relent and let our teenagers drive, we must ensure that we undertake due diligence before telling an older person not to drive. Only when this person is clearly a danger on the road should this serious step be taken.

SALT EFFECTS

We may need to rethink salty snacks. BY CHARLYN FARGO

ew research suggests that even if your blood pressure isn't affected by excess consumption of sodium, it may affect your blood vessels, heart, kidneys and the brain. That's the findings of a paper, "Dietary Sodium and Health: More Than Just Blood Pressure," published in the Journal of the American College of Cardiology. of the University of Delaware College of Health Science, William Weintraub, chief of cardiology at Christiana Care Health System, and Claudine Jurkovitz, a nephrologist epidemiologist and senior scientist in the Value Institute Center for Outcomes Research at Christiana Care. "Blood pressure responses to alterations in dietary sodium vary widely, which has led to the concept of 'saltsensitive' blood pressure," says Farquhar. "There are no standardized guidelines for classifying individuals as having saltsensitive blood pressure, but if blood pressure increases during a period of high dietary sodium or decreases during a low-sodium period, the person is considered salt sensitive. If there's no change

in blood pressure with sodium restriction, an individual is considered salt resistant." Potential effects on the arteries include reduced function of the endothelium, which is the inner lining of blood vessels. Endothelial cells mediate a number of processes, including coagulation, platelet adhesion and immune function. Elevated dietary sodium can also increase arterial stiffness. "High dietary sodium can also lead to left ventricular hypertrophy, or enlargement of the muscle tissue that makes up the wall of the heart's main pumping chamber," Edwards says. "As the walls of the chamber grow thicker, they become less compliant and eventually are unable to pump as forcefully as a healthy heart."


Regarding the kidneys, evidence suggests that high sodium is associated with reduced renal function, a decline observed with only a minimal increase in blood pressure. Finally, sodium may also affect the sympathetic nervous system, which activates what is often termed the fight-orflight response. "Chronically elevated dietary sodium may 'sensitize' sympathetic neurons in the brain, causing a greater response to a variety of stimuli, including skeletal muscle contraction," Farquhar says. "Again, even if blood pressure isn't increased, chronically increased sympathetic outflow may have harmful effects on target organs." This evidence has resulted in the American Heart Association's recommen-

dation that we consume less than 1,500 mg of sodium a day. Taking the salt shaker off the table is a good way to start, but it's probably not enough, says Weintraub, whose work focuses on cardiology outcomes. "Approximately 70 percent of the sodium in our diets comes from processed foods, including items that we don't typically think of as salty such as breads and cereals," he says. "Also, restaurant food typically contains more salt than dishes prepared at home, so eating out less can help reduce salt intake, especially if herbs and spices -- instead of salt -- are used to add flavor to home-cooked meals."

Q: I'm confused about all the different types of fiber. What is functional fiber? A: It can be confusing. Growing research shows that fiber is not all the same. Functional fiber is isolated fiber added to a food or supplement. Dietary fiber refers to fiber that occurs naturally in foods, including vegetables, fruits, grain products, legumes, nuts and seeds. Regardless of whether fiber is added to a food or occurs naturally, different forms seem to differ in potential health benefits. Some types of fiber may help reduce problems with constipation. Other fiber tends to form a gel within the digestive tract, binding up cholesterol and helping to reduce its level in the blood. Still other fiber is a type that bacteria in the gut can ferment. This creates compounds that emerging research suggests may help keep colon cells healthy and reduce inflammation. More work is needed to better understand all these potential roles. The fiber added to fortified foods (like certain breads, cereals, yogurts and more) tends to be inulin or gums. These fibers provide different health benefits than the dietary fibers from whole grains, vegetables, fruits, beans, nuts and seeds. Laboratory studies can isolate individual types of fiber to study the ways they may reduce risk of cancer, heart disease and diabetes. But trying to interpret results from studies with people can be complicated. That's because foods contain a mixture of different types of dietary fiber, and these foods also tend to

This recipe for Steak and Asparagus StirFry takes less than 20 minutes to prepare. It's from Cooking Light magazine.

Combine first 6 ingredients in a small bowl, stirring well with a whisk. Heat a large skillet over high heat. Add 1 tablespoon oil to pan; swirl to coat. Add beef, stir-fry until browned but not cooked through (about 1 1/2 minutes). Place beef on a plate; discard liquid in pan. Return pan to high heat. Add remaining 2 teaspoons oil; swirl to coat. Add asparagus and bell pepper; stir-fry 2 minutes. Add crushed red pepper and green onions; stir-fry 30 seconds. Reduce heat to medium-high. Add stock mixture; cook 3 minutes or until sauce is slightly thickened. Return beef and any juices to pan, and cook for 1 minute. Serve with precooked jasmine rice, available in pouches, if desired. Serves 4 (serving size: 1 cup) Per serving: 228 calories, 21 g protein, 9 g carbohydrate, 12.2 g fat, 55 mg cholesterol, 3 g fiber, 444 mg sodium.

ith its million-year-old granite monoliths and thousandyear-old trees, Yosemite and Sequoia are two of the crown jewels in America's National Park Service. Located on the western slope of California's Sierra Nevada Mountains, combining a visit to these two parks can provide an unforgettable family oriented vacation. The parks are distinctly different and stunningly beautiful. Yosemite Valley is a wonder of geology with its steep granite peaks and cascading waterfalls. Sequoia is home to the largest trees in the world. As National Park Ranger Dominic Papia pointed out, "It's all about time. When you visit these parks you see things that are so old and so magnificent. It's a humbling experience." It was President Woodrow Wilson who signed the act creating the National

Park Service on Aug. 25, 1916. Its mandate was "to conserve the scenery and the natural and historic objects and the wildlife therein and provide for the enjoyment of the same in a manner and by such means as will leave them unimpaired for the enjoyment of future generations." Our understanding of what "unimpaired" means has matured significantly. We no longer allow burning embers to be shoveled off the crest of Glacier Point to create a "fire fall." Cabins that used to festoon the most famous stands of giant sequoias have been removed. Recycling has been introduced, along with attempts to control the number of cars entering the parks. At the same time, the park service and the Delaware North Corp. (which manages the hotels and concessions in both parks) strive to provide visitors with an experience that combines comfort, safety and ecological sustainability. Sequoia and Yosemite National parks are both easily accessible by way of California's Central Valley. And visitors coming from distant places can arrange to fly directly into Fresno -- which is the perfect jumping-off point to visit the parks. Unlike the Eastern Sierra, which rises as a vast escarpment out of the Owens Valley, the western slope ascends

gradually through farmlands filled with almond and olive groves and gently rising foothills that were once populated by gold-hungry Forty- Niners. From Fresno the nearest park is Sequoia on state Route 180. The entrance is at Ash Mountain, past the scenic town of Three Rivers. That's when the climb begins. From the park's entrance the twolane road snakes its way up 130 curves and switchbacks. The payoff for making the ascent is spectacular as you suddenly find yourself surrounded by magnificent stands of giant sequoias rising hundreds of feet into the air. The park's most recent innovation is the Sequoia Shuttle. It leaves several times a day, seven days a week, beginning May 21 from multiple motel locations in the town of Visalia (44 miles north of Fresno). For a fee of $15 these shuttles will ferry visitors the 36 miles into the park with stops at key locations including Lodgepole Village (and campground), Crescent Meadow, the Giant Forest Museum, the General Sherman Tree and the park's premier accommodation, the Wuksachi Lodge. If you chose to stay overnight, simply reserve the time you wish to return to Visalia. Your entrance fee to the park is included. There are, however, no refunds. After visiting Sequoia and descending back toward Fresno, you can connect

with state Route 41 north. This is the road that will lead you to the southern entrance of Yosemite National Park. But there is a wonderful resort location just outside the park in the town of Fishcamp that is well worth a one- or two-day stopover -- the Tenaya Lodge. Family and pet-friendly, the Tenaya Lodge offers everything you could desire in a mountain resort of 297 guestrooms, suites and cottages. There are pools to swim in, trails to hike or mountain bike on, and a variety of restaurants in which to dine. There's the upscale Embers dining room (renowned for its flaming confections) and the Timberloft Pizzeria (with its outside deck) along with the congenial Jackalope Bar and Grill. After a tough day on the trail you can also relax and savor one of the soothing treatments offered at the lodge's Ascent Spa. In addition to couples and families, Tenaya Lodge is equipped to handle large parties for retreats and weddings. Shortly after you enter Yosemite National Park you'll come to the historic Wawona Hotel, the oldest hotel in the park. Its Victorian-era architecture and rustic charm (including shared bathrooms) harks back to 1876, the year the hotel opened for business. before descending through the Wawona Tunnel. More than a mile in length, the tunnel was hewn through solid rock and completed in 1933. The visual impact of entering Yosemite this way -- especially for the first-time visitor -- offers one of the most spectacular vistas imaginable as the valley floor, Half Dome, El Capitan and Bridal Veil Falls appear in all their splendor. It was President Abraham Lincoln who first made the preservation of Yosemite Valley an American priority in 1864. A decade later Yosemite became part of America's fledgling National Park System. Stretching over 747,956 acres, Yosemite National Park is visited by approximately 3.7 million visitors a year. Providing accommodation for that many visitors is, to say the least, a challenge. It is also a good reason to try to visit the park during the off-season: October through April. Where you choose to stay is a question of budget and availability. One of the most popular (and affordable) places to stay in the valley is Curry Village, with its rabbit warren of tent cabins. Curry Village is also home to the Yosemite Climbing School and store, where you find out everything you need to know about the numerous trails and climbing opportunities Yosemite offers. More luxurious is the Yosemite Lodge, with its unbeatable location a

quarter of a mile from the base of Yosemite Falls. The lodge features 241 recently redesigned rooms, including family rooms with bunk beds. The Mountain Room with its view of Yosemite Falls is one of the most popular dining rooms in the park. And at night during the summer there are ranger talks in the lodge amphitheater. And then there is Yosemite's landmark hotel, the Ahwahnee. Designed by Gilbert Stanley Underwood, the hotel opened its grand doors to visitors in 1927. With its towering timbered dining room, grand lounge and guestrooms with unparalleled views, a stay at the Ahwahnee is an unforgettable experience. As the high season for visiting Sequoia and Yosemite National parks approaches, room rates and availability will become more and more of an issue.
