Resident registration begins Monday, March 7 at 8:30am online at palatineparks.org and in person at Community Center, Birchwood Recreation Center, and Falcon Park Recreation Center. Non-resident registration begins Tuesday, March 15 at 8:30am online at palatineparks.org and in person at Community Center, Birchwood Recreation Center, and Falcon Park Recreation Center.

HOW TO REGISTER Use the registration form provided in this catalog (page 84); pick one up at Community Center, Birchwood Recreation Center, or Falcon Park Recreation Center; or download a form from palatineparks.org. 1. List the program number of the chosen program or event in the “PROGRAM #” box. 2. Write the name of the program, participant’s full name, birthdate, gender, and fee. A participant must be age appropriate when the class starts. Write in the fee(s) of the activity. Please include appropriate signatures and emergency information. Incomplete forms will be returned, which will delay the processing of your registration. 3. In-person registration may be processed by cash, check, or credit card. If dropping off a registration form, provide all necessary information and include a check for payment. Forms cannot be processed unless all information is provided. 4. As soon as your registration has been processed, your check will be deposited or your credit card will be charged. Your receipt will indicate if you have been placed on the wait list. Please verify this information. Note: Each household must use their own registration form. Placement with a friend or relative cannot be guaranteed. CONFIRMATION/RECEIPT Walk-in registrants receive a receipt immediately. Online registrants may print a confirmation at the time of registration. Class Full: If the class is full, you will be placed on a wait list. Wait List: If you are placed on a wait list for a class, and an opening becomes available, you will be contacted by customer service staff. Within 24 hours of being contacted, payment must be made online or at Community Center, Birchwood Recreation Center, or Falcon Park Recreation Center. Transfer or Refund: If you wish to be transferred to an alternate class or receive a refund or household credit, fill out the appropriate form and drop it off at Birchwood Recreation Center, Community Center, or Falcon Park Recreation Center. See next page for the transfer/withdrawal and refund policy. WAIVER REQUIRED For insurance purposes, Park District programs and activities require a signed waiver. All adult (18+) participants must sign the waiver. Children under 18 must have waivers signed by a parent or legal guardian. Patrons WILL NOT be able to participate in classes or programs if the waiver is not signed.

Classes will be canceled starting Monday, March 28 if they do not reach the minimum number of participants. SPECIAL REGISTRATIONS Special registration forms and separate payment are needed for affiliate sports, C.A.R.E., the Preschool Program, Palatine Tiger Sharks Swim Team, and Turkey Trot. SPECIAL NEEDS AND INCLUSION SERVICES Palatine Park District does not discriminate against eligible residents for participation in programs on the basis of a disabling condition. Residents with special needs should notify staff when registering so that smooth inclusion may occur. In addition to the programs listed in the program catalog, the Park District provides specialized services for people with disabilities through NWSRA (see page 17 for more information). Note: When registering, the parent of the individual should note on the registration form if an NWSRA aide is needed in order for the individual to successfully enjoy the program. ALLERGIES/ILLNESS If your child has an allergy or illness, please note it on your registration form. Park District Staff will accomodate to the best of our ability. If you have questions or concerns, contact the Park District. CATALOG CHANGES/ERRORS DISCLAIMER Occasionally, there may be an error that appears in print or online regarding days, times, fees, or other information. When such errors occur, Palatine Park District will do everything possible to correct the situation promptly. For the most current information visit palatineparks. org. CANCELLATION POLICY If a program does not reach its minimum number of participants seven days prior to the starting date of class, it may be canceled. If a class is canceled, participants will be called and have the option of transferring to another class or receiving a refund.
