Afer 60 days’ employment, full-time and 130hour/month teammates become eligible to participate in our group health, vision, and dental insurance plans ofered through Nova Healthcare with the Cigna Medical Network, and Disclosed Rx. The health, vision and dental plans are efective on the first day of the month following the teammate’s 60th day. Teammates participate in a Section 125 pre-tax premium plan for their share of monthly health, vision, and dental premiums through payroll deductions.
Teammates covered under Paducah Bank’s group health plan also enjoy wellness benefits through our partnerships with HealthWorks Medical LLC, BluMine Health Clinic, MuuvWell, and Risk Strategies. Benefits include access to a clinical care coordinator, free annual physical exams, free preventative and urgent care clinic visits, free one-on-one wellness coaching, and onsite physical and mental health fairs and events. This benefit is available afer 30 days of employment.
Health Savings Accounts are also available for those teammates participating in our group health plan, allowing for pre-tax payroll contributions from the teammate (limits established by the IRS) and including employer contributions into the account ($166.67/month for the single coverage and $333.33/month for joint coverage).
Life, Accidental Death and Dismemberment, Long Term Disability Insurances
Group Life Insurance coverage is 2.5 times annual salary up to $400,000.
Group Accidental Death and Dismemberment coverage is 2.5 times annual salary up to $400,000.
Long Term Disability coverage is 60% of monthly salary up to $5,000.
Supplemental Insurance Benefits

Flexible Time Of
Flexible Time Of (“FTO”) is awarded afer 90 days of service, and is to be used at the full-time teammate’s discretion while coordinating with his/her co-workers and team leader, for illness, doctor’s visits, vacation, and personal leave.

Teammates regularly scheduled to work 30 hours or more per week are eligible for the benefits described in this Teammate Benefit Guide on the first day of the month following 60 full days of employment.

Before meeting the new hire waiting period of 60 days, you are eligible to complete the necessary paperwork to enroll yourself and your eligible dependents in our benefits. Once you enroll, you cannot make changes to
your elections until our annual open enrollment period. The benefits you elect during open enrollment will be efective for one calendar year.
Qualifying Life Event changes in status include marriage; divorce; legal separation; status change; birth or adoption of a child; change in child’s dependent status; death of status, child or other qualified dependent; change in residence due to an employment transfer for you and your spouse; or change in your spouse’s benefits or employment status.
You must notify HR or your Risk Strategies Care Coordinator, Ashley Bates, of the change in family status and apply for change in coverage within 30 days of the occurrence of the event.
Generally, you may enroll in the plan, or make changes to your benefits, when you are first eligible. However, you can make changes/enroll during the plan year if you experience a change in status. As with a new enrollee, you must submit your paperwork within 30 days of the change or you will be considered a late enrollee and you may not be eligible to enroll.
Examples of changes in status:
• Marriage, divorce, or legal separation
• Birth or adoption of a child
• You or your spouse takes an unpaid leave of absence
• Change in employment status for you or your spouse
• Death of spouse, child, or other qualified dependents
• You become eligible for or lose Medicaid coverage

Do we have a network?
Yes. Your network is Cigna/Center Care PPO.
Do we have new ID cards or numbers?
Yes and no. You do have new ID cards with the Cigna information/logo, but the ID numbers are the same.
Can I only go to a doctor or facility that is in-network?
If a doctor or facility is not in-network, they may still accept Cigna. However, please note that if you choose to use an out-of-network provider, then you are subject to out-ofnetwork benefits. What should I do if scheduling or billing doesn’t recognize my health plan?
Please tell your provider that your health plan is under the Cigna network and ask them if they are in-network with Cigna. They should be able to submit a claim to Nova Healthcare Administrators (Nova) using the information on your ID card, which will then be processed under Cigna's network.
How will I know what my health plan has paid?
Afer any medical service, Nova will send you an Explanation of Benefits (EOB) or you can access your EOB online. This is a breakdown of what medical treatments were billed and what benefits were paid, along with indicating what you, the patient, are responsible for paying.
Can balance bills still occur?
Yes. A balance bill can still occur in some cases with out-of-network providers.
What should I do if I receive a balance bill?
If you receive a bill from your provider, either a physician or medical facility, you need to compare it to the EOB that you received from Nova. Contact the provider and confirm why they are billing you for more than your EOB, then contact Risk Strategies for assistance.

Your benefits provide coverage for healthcare services that are medically necessary. To determine medical necessity, coverage for certain services, procedures, inpatient admissions and surgeries must be approved by Nova Healthcare Administrators, Inc. in advance. This is called precertification.
The plan requires precertification for certain covered charges. The request for precertification must be made at least 72 hours in advance of services being rendered, or within 48 hours of the first business day following emergency services and/or admission. Your provider will be required to send information to Nova for the precertification process.
If you have questions about which services require precertification or before you receive care which requires precertification, contact Nova toll-free at (855) 206-1040 with the following information:
• The name of the patient and relationship to the covered employee
• The name, identification number, and address of the covered employee
• The name of the employer
• The name and telephone number of the treating physician/provider
• The name of the medical care facility, proposed date of admission and proposed length of stay
• The diagnosis and/or type of surgery
Precertification is not a guarantee of benefits. Participant eligibility, plan requirements, including provisions and exclusions, will determine available benefits. Your Summary Plan Description should be reviewed for limitations relating to a specific type of service.
Precertification for the following non-emergency services must be obtained before medical and/or surgical services are provided (subject to the limitations set forth in the Schedule of Benefits):

• Cigna/Center Care is the Preferred Provider Organization (PPO) in which you will locate in-network providers and facilities.
• If an in-network provider is seen, you will not be balance billed.
• If you do not seek care from an in-network provider, out-of-network benefits now apply, and you can be balance billed.
• Nova Healthcare is your Third-Party Administrator (TPA) and will process claims. Any precertification and prior authorizations must still be completed through Nova.
Since you have a High Deductible Health Plan, all services go to the deductible and once this is met coinsurance kicks in, until you meet the max out-of-pocket. These start over every calendar year.
Individual deductibles are embedded, which means that each member can only be out their deductible/max out-of-pocket. The family deductible/out-of-pocket is the total deductible and max out-of-pocket that can be met with all members of your family.
This process can take up to 14 business days, depending on the information that is needed and provided by your healthcare provider.
Failing to have a service precertified results in the member being responsible for the full amount of the bill.

To be a part of the plan’s network, these doctors and facilities must meet certain credential requirements and agree to accept a discounted rate for covered services under the health plan. These health care professionals are considered in-network.
If a doctor or facility has no contract with Cigna, they are considered out-of-network and can charge you full price. It’s usually much higher than the in-network discounted rate.
You’re probably being charged full price. We don’t have a contracted relationship with out-of-network doctors and facilities. So we can’t control what they charge for their services. And their rates may be higher than the discounted in-network rate.
You may be billed for the diference between the doctor’s bill and what your plan will pay. Many health plans list an amount that is the most they’ll pay for a certain service received out-of-network. If the doctor or facility charges more than your plan pays, you will be billed the diference.
Your share of costs is diferent—and usually higher. When you use a doctor or facility that is out-of-network, your deductible and other out-of-pocket costs may be much higher than the in-network cost. Review your plan materials for details on your specific medical plan.

DisclosedRx is contracted with several specialty pharmacies. If any members have questions about which specialty pharmacies are available for their medications, they can reach out to DisclosedRx with the specific medications or questions.
Generic medications are a chemical copy of the original brand, with the same active ingredients. Generic medications must meet the same quality standards for approval by the Food and Drug Administration (FDA) as brand-name medications. Generic medications must prove to work the same way, providing the same benefits and efects. Generic medications help provide cost savings for members, and DisclosedRx is commited to helping identify these opportunities when available.
The 2025 Premier Formulary is the list of drugs included in your prescription plan, which can be found on the Portal site. Inclusion on the list does not guarantee coverage. *Brand-name drugs may move to non-formulary status if a generic version becomes available during the year. Not all the drugs listed are
covered by all prescription plans; check your benefit materials for the specific drugs covered and the copayments for your prescription plan. For specific questions about your coverage, please call the phone number printed on your member ID card.
DisclosedRx’s 4-Tier formulary structure is as follows:
TIER 1 – Formulary/preferred generic drugs, select Over the Counter (OTC) drugs, and select brand-name drugs

Non-Formulary medications are drugs that have been removed from coverage on the Disclosed Rx formulary due to the availability of more efective or lower cost alternatives. To obtain a medication that is nonformulary, your physician must submit a prior authorization request for coverage. If the request is approved, that medication will be covered at your Tier 3 copayment/coinsurance level.
This program consists of state-of-the-art systems that help you get the high-cost specialty and brand medicines you need, at the lowest possible cost to you. This is done through Prior Authorization, Step Therapy, and/or Quantity Limits, etc. High-cost medications like specialty medications, injectables, and most name brand medications can now be accessed through the Agile Channel Management program, usually at zero cost to you.
DisclosedRx partners with GoodRx to find the best prices on your prescriptions. The GoodRx discount program is automatically included in your benefits with DisclosedRx.
1. Present your plan card at the pharmacy. The pharmacy will submit a claim.
2. A price comparison will be made between your insurance price and the price using GoodRx.
3. You pay whichever price is lower: the price using your member plan or the price using GoodRx.

You have many options for mail-order pharmacy, as DisclosedRx does not require the use of Postal Prescription Services. If you currently have mail order scripts with Amazon, Pill Pack, Kroger, etc., you may continue to fill through those pharmacies by simply updating your prescription benefit information (found on your ID card) with your mail-order pharmacy.

A: Well, the answer to this is simple! A 90-day supply would decrease the number of times you’d have to go pick up your prescription. Not only that, but your copay on a 90-day supply would be cheaper than geting three 30-day supplies. Note the following:

You’ll see that when you are geting a non-preferred brand medication, you would pay $100 for a 90-day supply (three 30-day supplies would be subject to a $50 copay each time, adding up to $150 over the course of 90 days). That means for every 90-day fill, you’re saving $50. For generic medication, you would save $10.
A 30-DAY SUPPLY?
A: Simply go into your favorite pharmacy and request a 90-day fill instead of a 30-day fill. The pharmacy will adjust the copay for you accordingly. You may also utilize a mail-order pharmacy, such as Postal Prescription Services or Prescription Mart to obtain a 90-day supply of your medication. Pays you an income when of work due to covered illnesses and/or of the job accidents. Benefits are payable directly to the teammate in addition to any other employee benefits and/or insurance benefits you may be eligible for. Benefits are eligible to begin as soon as seven days following the illness or immediately following the injury and continue income for three months or until your doctor says you can return to work, whichever comes first.
Provides benefits to help with your out-of-pocket expenses when faced with medical bills related to covered accidents. This coverage is great for kids in sports or adults with active lifestyles and/or occupations.
This plan ofers protection for your financial security and quality of life if you experience the batle of cancer. Provides benefit for expenses not covered by most major medical plans. Experimental treatment, stem cell transplant, transportation expenses, hotel expenses and out-of-network costs are a few of those not covered by most major medical plans. The plan provides an annual wellness benefit that helps ofset the cost. The $100 annual screening benefit is available for things like mammograms, pap smears, colon checks, or prostate checks. This benefit is payable for each member of the family who does one of the qualifying wellness checkups.
Pays a lump-sum payment of up to $1,000 to help with deductibles or other out-of-pocket expenses when hospitalized, or up to $1,500 when having an outpatient surgery. In addition, a $100 per day benefit (up to 15 days) is being added for rehabilitation care along with a $50 wellness screening benefit.
Available for you or your family. Prices vary by age, tobacco use, and level of coverage, and can be continued beyond retirement.

1. If you choose not to participate in any coverage, you cannot apply for that coverage until the next open enrollment period. It is to your advantage to make sure you understand your options.
2. If you choose coverage, you must stay enrolled for one year, unless you have a Qualifying Life Event (see page 10).
If you do not wish to make any changes, your current elections will roll over to the next year.
PLEASE NOTE: If you do not make any changes, you will not be able to make any changes throughout the year without a qualified life event. The next open enrollment will be in the 4th quarter of next year.
