THE PURPOSE BEHIND SERVING

Why the act of serving remains one of PDS ®’ cornerstones.

This past week, I was reflecting over the year, and it’s amazing how much we have accomplished in giving back. The most recognizable accomplishment was completing our 100 Wells for Ethiopia campaign, but we have done so much more: Smile Generation ® Serve Day, where the dental teams cared for thousands of patients for free; Braveheart with KaBOOM! in Colorado; international outreach trips to Fiji and Ethiopia (in Ethiopia, we are working very hard to help establish the second dental school in a country of 80 million people); and countless little known ”pro-bono“ dental services done by our bighearted providers. I’ve never been more proud of this organization.

I believe that companies that perform best over time build a social purpose into their operations that’s as important as their economic purpose. PDS' purpose is service, centered on community engagement for oral care, with the local community being the centerpiece. But, we’re also broadly defining community to encompass national and international efforts.

PDS commands enormous resources that can influence the world for better or for worse, and our strategies will shape the lives of employees, partners and patients. Business is an intrinsic part of society, and like family, government and religion, it has been one of society's pillars since the dawn

of the Industrial Age. Great companies work to make money, of course, but in our choice of how we do so, successful companies should be thinking about building an Enduring Institution as much as thinking about hitting their numbers. We need to have direct financial results while being keenly aware of the need to help people and society. People and society cannot be afterthoughts or inputs to be used and discarded – they are core to our purpose.

PDS has a purpose, and we touch many people.

We provide jobs that enhance people’s quality of life; our supported clinicians provide dental services that improve the lives of patients; we have a strong network of suppliers and business partners; and we ensure financial viability that provides resources for improvements and innovations for our future. Profits are essential, but we must view them as the way we build an institution that serves a larger purpose.

We should not be afraid to take actions that provide societal value regardless of whether or not they are profitable. One thing I have personally experienced is that when I am focused on serving others instead of being served, it’s contagious. In the workplace, I see people having an

easier time serving patients, serving their teammates, and serving their community. My prayer is that the attitude of service spills over to our personal lives, to our families and our spouses, thus improving all our relationships.

The old Ethiopia proverb says, “When spider webs unite, they can tie up a lion.” No lions were harmed on the last Ethiopia trip, but progress was made, and lives were changed.

From September 7-16, nine clinicians, seven dental assistants, eight helpers and two maintenance specialists served the wonderful people of Ethiopia. On the trip were: Dr. Jantzen Anderson of Colorado Springs 3; Dr. Fesha Gebrehiwot of Oro Valley 2; Dr. Jessica Hetcko of Westminster 1; Dr. Tanisha Jackson of Redlands; Dr. Christina Kraft of Oro Valley 1; Dr. Katie McCann of Aurora 2; and Dr. Minh Pham of Perris; hygienists Holly Simpson of Aurora 1 and Kate Whitehead of Ft. Collins 1.

Also on the trip were the recipients of the first-ever PDS Serve Foundation™ Dental Assistant Ethiopian Scholarships: Karina Garcia of City of Industry; Lindsey Gonzalez of Perris; Nicole Juarez of Redlands; Bich Nguyen of Laguna Niguel; and Rachel Warner of Murrieta. Lastly, the group included PDS National Support Center team members Judy Forsythe, Kyle Guerin, Mark Laramore, Mario Mariscal,

Rachael Parker, Barbie Vartanian, Don Webb, Brandon Webb and President Steve Thorne and his family.

PDS ® also continued to strengthen its relationship with two incredible in-country partners, who largely contributed to the trip’s success: The Relief Society of Tigray (REST), who provides all the in-country logistics, and Mekell University. REST is a strategic partner of charity: water and responsible for drilling and implementing all water projects. Mekelle University is one of only three dental schools in Ethiopia. PDS is committed to partnering with Mekelle University and providing them with equipment, supplies and training to improve oral healthcare in the region.

The goal of the Serve Ethiopia project is simple: to better equip the Ethiopian people to serve the dental needs of their community. This year we brought a much needed focus on education and training. PDS-supported dentists worked side-by-side with Ethiopian dentists and students to share in serving patients and educating them on proper techniques.

In four days, PDS-supported dentists and Ethiopian students were able to see 663 patients. Treatments included: root canals, extractions, restorations and hygiene. The

team worked in two locations: the Ayder Referral Hospital on the campus of Mekelle University and a health clinic in the town of Hawzien. The conditions of the health clinic resembled field dentistry, with the team using mobile dental units, benches, chairs, and anything else they could find to serve the people in need.

We can’t forget to mention that this trip couldn’t have been done without the incredible donations of Henry Schein ® Cares and KaVo Kerr Group, who provided all of the supplies, as well as four brand-new dental chairs for the hospital, which were a fantastic upgrade to what the hospital previously used.

One of the highlights of the trip was the opportunity to see all the efforts from charity: water and the generous donations from everyone being put to work up front and personal. We had the opportunity to visit a village that now has safe, clean drinking water and a well that was funded by one of our “Get CAD/CAM, Give Water” promotions. We also had the opportunity to visit a village that will soon have water thanks to our “100 wells for Ethiopia” campaign.

I’ve said it before and I’ll say it again, “Water changes everything!”

MY ETHIOPIA EXPERIENCE

I feel fortunate to have had the opportunity to travel to Ethiopia last September as part of the Pacific Dental Services®’ dental mission. Overall, the trip and the service that we provided was a humbling experience, and I am honored to have been part of a fantastic team that continually donates its time and energy to serve a community that needs our help the most. The trip not only signified a time I took part in a worthwhile cause, but on a personal level, it also allowed me to reflect upon my own upbringing, making it clearer to me the tremendous magnitude of the service being provided.

Being from neighboring country, Eritrea, my upbringing in a small village consisted of similar struggles to what we witnessed in the Ethiopian villages that we served. Specifically, most of the villages we visited lacked access to basic drinking water just like my own village.

As young kids, my siblings and I often travelled several miles in search of water due to the fact that our approximate 500 villagers depended solely on one well for drinking, cooking, washing, cleaning, etc. This essential source of water was often contaminated with animal dung and/or human contamination. Consequently, we often got sick as kids because there was little choice beyond the one well to which we had access. Even worse, this one well that hundreds of people and animals relied on for basic needs would run out during dry seasons. When this happened, we were forced to go further distances to neighboring villages in search for water. As a result, the search for water migration occasionally created tension and bloodshed with neighboring villages.

Reflecting on all this, I cannot help but feel overwhelmingly touched and proud in having the opportunity to witness and be a part of PDS ®' efforts in collaboration with charity: water to resolve these villages’ lack of access to clean drinking water. I can only imagine how these efforts are impacting the lives of the villagers in more ways than most of us can comprehend, and I am forever grateful to have had an opportunity to see such tremendous service in the works.

I am fortunate to have had an opportunity to serve others and give back while working in my profession. I am also fully aware that access to dental care in such remote locations is extremely lacking. As such, there’s no better feeling than knowing that our mission served to target some very needy and vulnerable communities. Furthermore, having had the good fortune to be part of such a worthwhile cause, I can definitely say that I gained more from the experience than I gave. I look forward to taking part in many more dental missions to come.

SERVE FIJI 2013

Never have I encountered such joy-filled people as I did on the islands of Fiji. I don’t know if it’s the sunshine and beautiful beaches or the fresh coconut water, but these people are happy. They don’t have much in terms of material possessions, but they take great care to utilize their natural resources and truly enjoy each other’s company.

This past July, I had the privilege of joining members of the PDS ® family to experience this culture first hand on the third PDS-supported trip to the Mission at Natuvu Creek on the island of Vanua Levu. The team consisted of PDS-supported clinicians Dr. Keith Johnson and his wife, Dr. Antonia Moa, Dr. Nicole Smith and Dr. Ariella Pessah. Joining the clinicians were Operations Manager Jen Hamilton, Operations Manager Brian Taylor, Vice President of Marketing Matt Hall and ”We Serve“ Manager Kyle Guerin. This group proved to be incredibly hard working, with a dedicated passion to serve. Over the course of the week, the team saw over 170 children and adults, performing critical and preventative oral care and providing dental health education to those who otherwise have no access to dental services.

I was incredibly blessed by this trip, the culture and the opportunity to serve. I believe we all learned an invaluable lesson about enjoying life from the Fijian people, who seem to epitomize the phrase, “No Worries” or in Fijian, “Sega Na Lega!”

BRAVEHEART 2013 DENVER:

ALL IN A DAY’S HARD WORK – BUILDING A PLAYGROUND AND PROVIDING $75K IN DONATED DENTISTRY

Your eye is on the clock, and your computer is fired up. You’re ready to take off at the shot so you don’t miss out. Is it those hot-selling JT/JZ concert tickets that are keeping you on edge? Is the Macy’s fire sale about to start – and you’re determined to buy that one pair of shoes left in your size? No on both counts… you’re waiting to sign up for Braveheart!

It’s a little crazy to think people would not only rush to sign-up, but get incredibly excited at the prospect of digging holes, moving mulch and mixing concrete. Let’s not forget about those good dentists providing donated dentistry and enduring several other forms of physical labor for an entire day. But, year after year, that’s exactly what happens when hundreds of Smile Generation ® volunteers gather for the annual playground build and dental clinic called Braveheart.

On July 20, 2013, 350 volunteers and residents of the Denver community joined forces to build a new, safe playground for the students at Redeemer Learning Center. Planning for the playground began at a Design Day event held in April, when Redeemer Learning Center students, parents and faculty met with organizers from KaBOOM! and The Smile Generation to design their dream playground. The new playground was completed in one day and will provide more than 1,000 children in the community with a safe place to play for years to come.

Smile Generation-trusted clinicians and dental care teams also set up an on-site dental clinic and, in a matter of hours, treated nearly 170 patients, providing more than $75,000 in donated dental services.

Treatments included cleanings, extractions, root canals and CEREC ® CAD/CAM same-day crown restorations. The PDS ® Serve Foundation mobile dental clinic was on-site for the clinicians to utilize in providing quality care and serve the Denver community members in need.

We are thrilled that we had a hand in helping to build a play area for the children. We are the ones blessed for serving the Denver area community members with the on-site dental clinic that provided donated dentistry to those who don’t otherwise have access to dental care. Seeing smiles on the faces of the kids and parents is what makes this day so gratifying. The willing hands and hearts of Smile Generation team members who continue to have such a passion for this project make it rewarding each and every time.

THE THIRD ANNUAL SMILE GENERATION SERVE DAY

On August 24, 240 PDS ® -supported offices and 2,700 volunteers served 1,600 patients with great love. This effort of love produced more than $2.4 million in donated dental services. Our supported clinicians served patients from community centers, churches, schools, abused women and children shelters, and other local organizations. A number of offices chose to serve existing patients who had fallen on tough times and who desperately needed a break.

Feelings about FILLINGS

It is more common for dentists to place composites than amalgams. This has been verified by procedure tracking. While I would prefer NO amalgams being placed, there are still many HMO plans that require us to offer this as the least costly alternative. Currently, the main "go to" restoration choice is composite, but what if we had a better choice, a more consistent reproducible choice, with reasonable chair time and ease of use?

There are several resources reporting that composites are not the ideal long-term restoration we had always thought. One resource includes The Journal of American Dental Association, which had a recent article stating the failure rate of composites and amalgams at the two-year mark is 6.2%. These results were based on more than 200 providers of differing experience levels placing over 6,000 fillings. When I see a study like this, I want to know where we stand, where we are headed, and what focus areas can positively impact our results. Yes, an appropriately placed and sized composite can be a fine restoration and has superior cosmetics compared to an amalgam, but if costs were equal to patient and provider, which of us would choose the ceramic inlay over the composite? What if chairside efficiency to place the inlay was comparable to the composite?

We should not allow our concerns about the patients' ability to pay influence or dictate the treatment recommendations we provide. Our goal should always be to provide the best long-term clinical outcomes for our patients. What if we could charge the exact same amount for a composite and a CEREC ® inlay? I believe most dentists would choose the CEREC. In fact, I get consistent feedback from practitioners that if the cost of inlays was comparable to the cost of composites, they would choose ceramic-based inlays most of the time as the preferred treatment choice for their patients.

Pacific Dental Services® -supported dentists have placed over 30,000 inlays since the inception of CEREC into our treatment choices for patients. To get the best comparison year by year, we chose to look at the two-year remake rate for each year that they had been placed. Of course, to measure a two-year remake rate, the restorations need to be at least two years old. The data points we will be looking at are from the four-year period of 2008 to 2011. The remake percentage includes any remake done for whatever reason deemed necessary by the treating clinicians. This includes remakes due to wear, cracking, fracture, loss, and even the occasional remake solely due to patient cosmetic concerns, as all are tracked with the same code in our software.

Highlights:

• The remake rate of traditional filling materials is relatively flat.

• The initial remake rate of CEREC started off lower than the other filling materials.

• The CEREC remake rate continues to diminish.

• Keep in mind that the stats for 2011 are roughly half of the prior years, as this is a two-year remake percentage, and 2013 is just half over at the time of analysis.

As indicated in the Figure 2 graph, the remake rate of composite and amalgam restorations being placed by PDS ® -supported dentists over the course of four years has basically stayed level, while the twoyear remake percentage tied to CEREC inlays started at a lower point and is continuing downward.

Proposed reasons for the continued decrease in inlay remakes are:

• Increased volume placement of inlays has allowed practitioners to improve their skill level with this procedure.

• Digital technologies allow greatly magnified viewing of prep design, encouraging self-improvement of prep skills.

• Digital technologies provide the chance to dissect prep design and proposed restorations through means of depth and cross-sectional analysis.

• The use of digital technologies provides the capability for more accurate fitting of final restorations.

• The choices of CAD/CAM milling materials continue to increase, allowing practitioners to better match material choices to patient need.

All of us have self-selectively volunteered and been tasked with offering and placing the best restoration treatments for our patients. Sadly, we too often try to give the patient the "least costly" alternative over the "best choice" alternative. PDS-supported dentists have a lower remake rate with CEREC inlays compared to traditional restorations. In fact, it is close to one-half, and it continues to decrease.

With Patient Loyalty COMES EXTRA REWARDS

The “treatment seeds” we plant, over the years, can lead to bountiful harvests of production and even greater patient loyalty.

One of the nicest qualities of a stable long-term practice is being able to retain great long-time patients, bonded to your practice by trust. The “treatment seeds” we plant, over the years, can lead to bountiful harvests of production and even greater patient loyalty. Often, this production can turn an average week or month into something greater. Here is such a case:

Carolyn has been my patient for more than a decade and was presented with a fracture to her #11 porcelain jacket crown. We offered a few treatment options to her, but after presenting them to her, she became increasingly excited; soon her six-unit case eventually turned into a 22-unit case – and without too much convincing on our part, too! Carolyn’s main wish was for a smile that was whiter, brighter and more youthful looking. As a woman in her 70s, she had some medical issues, which required a sequenced, but aggressive treatment schedule to complete her case during three non-consecutive days.

Excepting her lower right bridge, we were able to incur no lab expenses, and this allowed my staff and me to simply enjoy spending three days together with our patient. We even bought her lunch each day and relaxed while the units were in the oven. While I was in the lab, Isaac and my staff made sure Carolyn was covered with a blanket and entertained. My entire staff was fantastic in handling her needs just so that I could focus on her dentistry. They were indeed as crucial to completing the case as I was.

For every case, we as clinicians try to reflect back and take away lessons learned in both patient management and technique. I must thank my staff as I don’t think they left anything to chance; everything went so smoothly because of their attentiveness to the patient.

Interestingly, after Isaac and I “broke down the game film”, we saw a few things we wanted to change. We even pointed the flaws out to the patient, offering to perfect the result, but Carolyn chose to love what we gave her. That’s patient retention and trust right there! We resolve to apply these lessons on all our future cases and keep practicing better.

Power of TECHNOLOGY

Why Power Brushes Have Become a Hygienist’s Best Friend

What an exciting time to be in the profession of dental hygiene! Medicine and the dental profession are learning more every day about the detrimental effect that a localized infection like gum (periodontal) disease has on an individual’s overall health.

The Centers for Disease Control (CDC) and Prevention released data in September 2012 that shows a higher prevalence of periodontal disease than was originally thought. The CDC estimates that 47.2% of American adults have mild, moderate or severe periodontal disease – an infection in the gums that has been positively linked to cardiovascular disease and diabetes. More than ever, the dental hygienist plays a key role in helping the patients he/she serves in managing their oral health, overall health and ultimately their quality of life.

In today’s modern dental practice, technology is playing a big role in how we manage the long-term health of our patients. From digital radiography to enhanced oral cancer screening, from lasers to ultrasonic scalers, the technology exists to individualize patient care and deliver Clinical Excellence. Perhaps the dental hygienist’s most important job as a preventive health care professional is to educate and motivate patients toward effective home care. With the advent of power toothbrushes, effective and efficient plaque biofilm removal has never been easier.

Modern-day power toothbrushes were first introduced in the early 1960s and used a rotating-oscillating head to remove plaque. The rotating-oscillating technology is still popular today, but has advanced slightly to include a pulsating motion. The newer sonic technology provides bristles that move with high amplitude and high frequency creating sound waves that cause fluid motion, disrupting plaque biofilm both in contact with

the bristles and beyond the physical contact of the bristle. It is important to note that while power toothbrushes are effective at removing plaque biofilm on smooth and interproximal surfaces, thorough interproximal cleaning with floss or other interproximal devices is still indicated.

This past July, PDS ® -supported offices were proud to introduce the Philips Sonicare FlexCare+ power brush to their patients who can benefit from power brush technology. This brush represents the latest in sonic technology and ease of use. The FlexCare+ has five modes of action, a Smartimer and a Quadpacer designed to accommodate your patient’s individual needs. The Smartimer motivates patients to reach the recommended brushing time of two minutes while the Quadpacer divides the two minutes into 30 seconds per quadrant. A 2007 clinical study in the Compendium of Continuing Education in Dentistry showed that patients with gingivitis using Sonicare technology as opposed to a manual toothbrush saw a 71% reduction in bleeding points over a four-week period.

More and more, technology is assisting in the management of periodontal disease and disease prevention. Today, patients are seeking out dental practices and healthcare providers who utilize cutting-edge technology, from digital radiography to lasers to the latest in home care devices. Gone are the days for the one-size-fits-all management of periodontal disease. Periodontal disease is a complex and multifactorial infection with systemic ramifications that requires an individualized approach for each patient, both professionally in the dental office and at home. However, while the disease may be complex, simplicity is key for home care compliance. Offering a power brush that is proven to be more effective than a manual toothbrush, along with floss, may be just what the dentist has ordered. Or in this case, it may be just what the dental hygienist has recommended.

Diet. For many of us, this word is always in the back of our minds. We worry about our health, how we look, whether we’re getting enough of the right foods or too much of the wrong ones and how all of this will affect our bodies as we get older. And we have good reason to think that way: Your eating habits and activity level affect your health more than anything besides smoking1

With our busy lives, nutrition is often the first thing we let slip. Sometimes, getting a burger from the drive-thru just seems easier. But the foods we get away from home usually have less fruit, vegetables, whole grains and dairy 2 – all of which are important for a healthy diet.

FOOD FOR THOUGHT

It takes some extra time, but planning and preparing healthy meals and snacks can add years to your life1. A little extra effort now could mean great benefits to your health in the future. But with different diets becoming popular all the time, it’s hard to know what we should really be eating. Is it healthiest to cut out all breads and rice and just eat steak and eggs? Is it better not to eat any meat? A healthy diet isn’t as simple as not eating entire food groups. All types of food add different nutrients that our bodies need. The key is to get the right balance of all of the food groups.

Follow these tips to get the right balance in your diet 3 :

• CARBS : Don’t go without them, but pick the right ones. They give your body the fuel it needs for physical activity and help your organs work properly. But some kinds of carbs are better for you than others. Whole grains, vegetables, fruit and beans are good choices. They’ll give you vitamins, minerals, fiber and more nutrients your body needs. Bad carbs come from refined grains, which make them easier to digest. Skip things like white bread, white rice, pastries, soda and highly processed foods like packaged snacks.

• PROTEIN : A lot of different foods have protein. But some of those foods are healthier for you than others. A steak has a lot of protein, but it usually has a lot of fat, too. Choose fish, poultry, lentils and nuts instead. If you do want to eat red meat, pick the leanest cut, watch your portion size and only eat it once in awhile.

• FATS : You actually need fat in your diet. Healthy fats, that is. Plant oils, nuts and fish are healthy sources of fat. Try to limit the amount of foods you eat that have saturated fats, like cheeses and meats, and avoid trans fats. Read the labels on the foods you buy to make sure they don’t have any trans fat.

• FIBER : A diet with a lot of whole grains, beans, vegetables and fruits will help you get the fiber your body needs. Fiber can help protect you against certain health problems.

• VEGETABLES AND FRUITS : These foods can also help protect you against certain health problems. Don’t know what to pick? Go for color and variety: dark green, yellow, orange and red. Just remember, potatoes don’t count as a vegetable.

• CALCIUM : Calcium is important for healthy bones. Milk and other dairy products are good sources for calcium, but they’re not the only ones. Nondairy sources of calcium include collards, bok choy, fortified soy milk and baked beans.

• WATER : Skip the sugary drinks and have water instead. It has no calories, and you can find it anywhere. Sugary drinks give you just that: sugar.

• SALT: A diet that’s high in salt can cause health problems. Choose more fresh foods and cut down on processed foods, which are usually high in salt levels. And read the labels of the foods you buy to check how much salt (listed as sodium) is in them.

• ALCOHOL : Moderate drinking can be healthy, but not for everyone. Moderate drinking for women is up to one drink a day; for men, it’s up to two drinks a day. • MULTIVITAMIN : Taking a multivitamin, especially one with vitamin D, every day can help your health. There is such a thing as having too much of a good thing though. So don’t overdo it with the vitamins.

TOO MUCH OF A GOOD THING

Even if you try to eat a balanced diet, portion sizes can work against you. Find out what a serving of a food looks like on a plate so you’ll know if you’re eating too much or too little. You can also try using smaller plates for your meals. And serve plates at the stove instead of at the table, so you think twice before having seconds 4

• Order water or unsweetened tea instead of drinks with added sugar.

• Start your meal with a salad (with dressing on the side). It’ll help you control hunger and make you feel fuller sooner.

• Choose dishes that include vegetables, even if it’s only pasta with tomato sauce.

• Order steamed, grilled or broiled foods instead of ones that are fried or sautéed.

• When you’ll be in the car for a long time, pack fresh fruits or vegetables, low-fat string cheese or a handful of unsalted nuts to snack on.

• Skip the buffets. Order small or half-sized portions when you can. Or, ask for half of your food to be packaged to go when you order.

• Order fruit for dessert.

CLN197 – Specialty Diagnosis and Treatment Planning for the GP

This course offers the opportunity to work in a group setting reviewing cases and utilizing key diagnostic skills. You will clinically evaluate several specialty patient cases and brainstorm the best approach to patients, esthetics, sensitivity, and clinical efficiencies pertaining to each case, as well as learn how to recognize appropriate referral sequencing.
