Dear PDS Teammates,

Throughout the year you have served your communities with free dental care, building a playground, and numerous office drives that collected toys, coats and food for those in need. You have reached out internationally with support for charity: water and dental mission trips to Fiji and Ethiopia.

To Serve is at the heart of PDS. I am truly inspired and proud of all that you have done to help those who are less fortunate.

As we near the end of 2012 and look toward 2013, I want to share plans for the upcoming year and introduce the PDS Serve Foundation, a 501(c) non-profit organization. At its core, PDS is a service organization, and the PDS Serve Foundation is an extension of this. PDS will seed the Foundation with its platform, people and passion.

The PDS Serve Foundation has two primary mandates. One is to serve communities with improved oral healthcare, and the other is to help PDS family members enjoy more successful relationships, marriages and families. A key part of the Foundation is to help people experience true service by providing opportunities and funding for those interested in participating in the act of service itself – locally, nationally and internationally.

The PDS Serve Foundation is committed to supporting the communities where

you live and work – encouraging, helping and providing resources to serve local and national needs. One of the first endeavors for the Foundation is the creation of the PDS Serve Bus, a motor coach with two complete operatories created with the sole purpose of helping serve the underserved. With PDS-affiliated dentists and staff volunteering their time and talents, the bus will serve

Ethiopia

On Oct. 21, 2012, 15 PDS ® team members and supported dentists boarded a plane in Los Angeles, ready to embark on an incredible adventure.

We would spend approximately 30 hours traveling, passing through Washington, D.C. and Addis Ababa, Ethiopia before finally landing at Makale Airport. We each had our own expectations of what the trip would entail; some of us had been to Ethiopia before, some of us had even been to this exact region before, and others had never stepped foot on African soil. None of us expected to have the type of experience that we did.

The dental care team that traveled to Ethiopia included three general practitioners, one oral surgeon and one pediatric dentist. The doctors on the trip were Dr. Kenneth Burson, Dr. Tanisha Jackson and her assistant Nichole Juarez, Dr. Walter Jefferson, Oral Surgeon Dr. Kouros Izadi, and Pediatric Dentist Dr. Eric Ellis. Also on the trip were Stephen Thorne, president, founder and CEO, and members of his family; Joe Feldsien, senior vice president of professional partnerships; Tom von Sydow, vice president of specialties; Matt Hall, director of marketing; Judy Forsythe, director of educational development; Mark Laramore, director of strategic business development/surgical services; Mario Mariscal, CAD/CAM manager; Keith Sloan, construction superintendent; and Barbara Vartanian, operations manager.

While in the region of Tigray, the dental care team collaborated with dentists and dental students at the Ayder Referral Hospital. Ayder Referral Hospital is a teaching hospital for the College of Health Services, Mekelle University. The dental team served both at the hospital and at a health clinic outside of the village of Hawzien. Together, the dentists were able to serve nearly 2,000 patients, providing treatments that included extractions, fillings and hygiene. The patients served included adults and their children, members of the military and border patrol, and also children from the local school for the blind.

Henry Schein Cares provided supplies and dental equipment, such as sterilizers and

Nomad portable X-ray units, to the university to be used for treatments and as part of the continuing education for the Mekelle University dental students. PDS will continue to partner with Mekelle University and Relief Society of Tigray (REST) to provide equipment, supplies and training to help improve oral healthcare in the region. Procter and Gamble Crest Oral B provided toothbrushes and toothpaste to give to the local patients.

While in Ethiopia, the team was also able to experience the impact and the mission of charity: water firsthand. We visited villages that had been blessed with the gift of clean, accessible drinking water, and we met women and children who have been able to excel in school and business because of the gift of water. We also had the opportunity to visit a village where women don’t have access to clean water, and they have to collect dirty water that will most likely make them sick. One of the representatives from this village shared with us the hardships of these women who, even while sick, have to travel long distances for dirty water. Men from outside villages won’t marry these women because they are sick. The water crisis that we’ve all heard about is real, and water changes everything.

With or without water, the joy that all of the people in the villages shared was infectious, and at every turn you could find the PDS team dancing, jumping or singing with the locals. The camaraderie of the team was second to none, and the bonds that were built there will last for a long time.

We all left Ethiopia changed, impacted and motivated to do more. When you go on a trip like this you receive much more of a blessing than you can ever give. The PDS team is committed to being a blessing to the people of Tigray, Ethiopia for years to come.

When charity: water spoke at XP, it was nothing less than inspiring. The video they showed about the difference PDS ® is making by providing clean water to Ethiopian villages was moving; so moving, in fact, that the part of the Southwest region under Carol Garner decided they wanted to do more.

Collaboratively, the operations mangers and Owner Doctors decided they wanted to raise enough money to build their own well. They set aside 90 days to raise $5,000. This money would be enough to give an entire village clean water. “I love how motivated my team is to giving and making a difference in the lives of people,” Garner said. She also agreed that an extra $1 for every CEREC® CAD/ CAM could be donated specifically to this campaign. They got their teams on board and went after it.

They had their own campaign page on the charity: water website where the total could be tracked. Staff reached into their own pockets and gave. They shared their goal with their patients and asked them to contribute as well. Each week they were able to watch the total donations get closer and closer to the $5,000 they needed.

In the end, Carol Garner’s 14 offices raised a total of $5,961. With the $1 contributions of CAD/CAM, they raised a total of $10,922! The team raised enough to build not one, but TWO clean water wells in Ethiopia! “I’m just so proud,” Garner said. “While we love that our name will be on two separate wells in Ethiopia, it means more to us that we have been a part of changing the lives of these people.”

I couldn’t agree more.

On Nov. 10, 2012, PDS® volunteers gathered together to build a brand-new playground and host an on-site dental clinic for the families of Casa De Colores Shelter in Phoenix. Neither was an easy feat, but with some incredible teamwork, willing hearts and remarkable spirit, we made it happen.

Early Saturday morning, 180 PDS team members set off from 13 cities and five states to make their way to the Phoenix shelter. Our clinical support team got straight to work transforming a patio, an apartment and a small meeting room into

a full-service dental clinic. Throughout the day, the group of 65 clinicians and clinical support staff provided everything from cleanings to extractions to root canals for the shelter residents.

In just seven hours, the clinic served 91 patients and provided nearly $67,000 in free dentistry. A woman approached me in the afternoon with tears in her eyes and said, “They gave me a crown. I had no idea they were going to do that much.

I’m so happy.” Seeing what this incredible team could do in this ordinary space was amazing.

While the clinical team moved diligently on one half of the property, the rest of the crew was hard at work building a new playground. I imagine most people don’t even think about what goes into building a playground, but let me tell you, it isn’t easy. Alongside shelter residents and staff, PDS volunteers started from the ground up, mixing 12,600 pounds of concrete and moving 145 cubic yards of mulch by hand. The build teams put together the four main sections of the playground equipment and constructed a shade structure, four deck benches, four

raised planted beds, five planter benches and six trash containers. Additional teams hand painted two six-foot peace poles, 30 plywood murals and 50 paver stone murals. Finishing touches were added with tons of planting and additional landscaping.

By the end of the day, everyone was exhausted, but no one could debate that the remarkable effort put forth by everyone had accomplished great things. The shelter staff and residents were so grateful for the donated dentistry and the new playground. “This will be a way for the children to focus on something fun and take their minds off the pain they’ve experienced,” said the shelter director. “It will be a joy for generations to come.”

It’s wonderful to see all that can be done in one day, and we’re delighted that the shelter was so thankful and appreciative. However, for many PDS volunteers, the true blessing is the opportunity to give back and see the smiling faces of the families of Casa De Colores.

Fiji race for the Cure

Halfway across the world on the island of Vanua Levu, Fiji’s second largest island, lies a culture of people with hearts more beautiful than their surroundings. The Fijian people are some of the most generous, loving and serving people you will ever meet. This past June at a four-chair dental clinic situated at the base of an extinct volcano, eight people from PDS® provided their best in the service of these wonderful people.

At the Mission at Natuvu Creek, Dr. Dean Lambridis; Dr. Tanisha Jackson; Dr. Keith Johnson and his wife Dr. Anonia Moa; Hygienist Connie Austin; and Professional Development Coaches Valerie Kendrick and Clarissa Castillo got to experience the Bula Spirit firsthand. Their Fijian patients received extensive treatment, including restorations, extractions and hygiene. The Fijians said they were grateful for all of the services they received, but it was those of us on the trip who received the real blessings.

This year's President's Club was once again held at the beautiful Palmilla Resort in Los Cabos, Mexico. Read all about the trip from the doctors themselves, and check out the photos from their amazing trip!

This year, The Smile Generation enthusiastically sponsored eight Komen Race for the Cure events across the country. Hundreds of PDS® employees, affiliates and patients reached out to show their support and commitment to their communities. In total, The Smile Generation successfully formed teams in 13 major cities. Funds raised through Race for the Cure go directly to the regions in which they’re held, with 75% allocated to local services and 25% to national research. According to the Komen website, there are 2.5 million breast cancer survivors in the United States. The Race for the Cure events were created to quickly make the vision of “A World without Breast Cancer” possible. Witnessing the energy and passion for the cause itself and the events is incredible. Our teams were eager to participate, armed with the knowledge that any proceeds earned would provide treatment, research and prevention for thousands. The Smile Generation is proud of our partnership with Susan G. Komen.

Smile Generation Serve Day

The second annual Smile Generation Serve Day took place on Aug. 10, 2012. Knowing that together we can make the greatest impact, we set aside this day for all PDS®-affiliated practices across the country to open their doors to in-need individuals. This is an opportunity for each office to give back and support the community that supports them every day. Though we work together toward this common goal, Serve Day is unique to each office as the team decides who they are going to serve, what they want to provide, and how they will run the day. This year, over 200 PDS-affiliated offices took part serving shelters, group homes, churches and everyone in between. Almost 2,000 PDS team members volunteered their Saturday to reach people who would otherwise be without basic dental care. More than 1,400 patients were served, and nearly $1.9 million dollars in free dentistry was provided. PDS has a passion to help people in need, and Serve Day 2012 was an exceptional example of putting this passion into action.

PDS® Makes an Impact at the ADA

This year’s American Dental Association’s Annual Session took place in San Francisco and boasted over 32,000 people in attendance. Dentist attendance increased slightly from last year to over 10,000, and PDS was present among the 7,185 exhibitors. This year, we differentiated ourselves by having Owner Dentists host the session and serve as spokespeople at the booth, reception and during various meetings. The PRIVATE PRACTICE +TM identity was well represented as these Owner Dentists engaged with potential candidates, key opinion leaders, media and the leading vendors/manufacturers in the industry.

Dr. Charles Rodgers and team continued the planning for the PDS Institute, the clinician’s forum for higher learning and Clinical Excellence. When speaking of how vendors and manufacturers received the idea of the PDS Institute, Dr. Rodgers said, “We secured specific content, personnel, and financial commitments from several vendors in support of the PDS Institute coursework. Ivoclar, 3M, Velscope, Tusla and Nobel Biocare reaffirmed commitment, as well. We are continuing to work with other potential vendors considering support: Dentsply, GC America and Vitadent.”

Our presence at the ADA this year is best summarized by Dr. Sonny Chokka:

“What stood out in the way PDS represented itself this year was the continuing of the corporate veil being lifted, revealing the wonderful clinicians that are at the heart of delivering dentistry to our patients. The intentional effort for PDS to put clinicians out front as the face of this organization has created a buzz in the dental community. What you hear is, ‘You guys are different.’ With the passing of time, we will turn the stereotype of corporate dentistry on its head.” It is evident that, in all respects, PDS is emerging as the Greatest Dental Company in America.

Our 20’ x 20’ booth piqued interest for those passing by, and the new look and feel of the brand collateral helped to provide relevant, yet simple information about opportunities to join PDS. Owner Dentists now blanket our collateral as the face and image of the organization like never before. This approach was not only well received, but encouraged PDS-supported Dentists to share genuine stories with those taking the collateral with them. Dr. Carolyn Ghazal described, “I spoke with a professor from UCSF and was able to engage her easily on the premise of clinical leadership.”

At the second annual PDS reception, Steve Thorne and PDS-supported Owner Dentists hosted the who’s who of academia, media, vendors/manufacturers, organized dentistry and more. The focal points of the reception were the re-launch of PacificDentalServices.com and the recent presence of our Owner Dentists in the media.

“This year the ADA proved to be a HUGE success for PDS as we forge forward making our clinical presence known and recognized,” said Dr. Todd Cockrell. “Amongst many of the industry's key opinion leaders, we have and are breaking through the stigma of being stamped ‘corporate’ dentistry. We now have momentum in the industry as well as political circles as a group of clinicians who truly strive for excellence in who we are, what we do, and what we offer our patients in our individual environments.”

Omnicam

The new CEREC Omnicam acquisition unit was debuted this past August at the CEREC 27.5 convention.

Several PDS® members had an early "hands-on" experience the night before the grand presentation. It was immediately apparent that the new Omnicam had numerous advantages:

• NO powder

• Color

• Smaller imaging head

• Improved stitching

One of the many standing ovations at the grand presentation was the announcement of "No More Powder!" This was easily understandable as the application of powder can be very technique sensitive. Powder removal will slightly impact direct costs, but will also increase ease of use while reducing acquisition time. Obviously, the removal of powder from the process will also greatly improve the clinician’s emotional status – you will just feel better when you do not have to mess with powder.

The second improvement is the use of color for the proposal. If you prefer, you can still view the model in yellow, but I cannot imagine passing up viewing the enamel, tissue and prior restorations – all in color. With the simple chance to clearly see where the prep ends and the tissue begins, consider how much easier it will be to draw the digital margins! No second guessing or returning to the operatory for a quick peek at where the real margin was. If the doctor chooses to design the restoration in front of the patient, the patient can see the adjacent teeth in color, including the self-evident problems the adjacent teeth have: broken fillings, decay, worn crowns, etc.

While the BlueCam offered only one view along the vertical insertion axis line, the new Omnicam takes continuous 3D images and stitches them together as you slowly move the camera about. There is no need to kick a foot pedal; the camera automatically starts imaging once it is within the range of what it identifies as tooth structure. It is not critical to be in the exact spot and take the

top/down only view. If the imager falls out of the appropriate range, the camera emits a different sound as a warning and stops imaging, at which point the user can reposition and continue with the imaging process. Not only is the top/down view still part of the model, but so are all the side views, interproximal views and any other angles the doctor may deem critical for a more beneficial patient outcome.

A preliminary concern was voiced regarding how huge the file size would be with all of the multiple images on one model, but it turns out this won’t be a problem. Once the user moves forward with proposals, all of the extraneous data is trimmed. The end result is a file size similar to the current ones.

A smaller imaging head is also a key part of the new camera. The redesign includes a change in the shape of the whole handheld imager: a more ergonomic shape that fits better and easier in the user’s hands. The end portion is designed to mimic the shape and feel of a hand piece. This new design also makes it easier to access the tightest areas of the oral cavity, allowing for easier imaging of second molars and other troublesome spots.

The greatest appreciation for most convention visitors was regarding the removal of powder from the process, however, there was one more aspect that greatly benefits PDS-affiliated doctors and team members. Considering the multiple states we practice in and the various regulations regarding assistant responsibilities, anything that addresses usage across multiple states is a plus. With the new Omnicam, virtually any team member can image the quadrant to be prepped, the opposing and the bite. Then all the doctor needs to do is remove the portion of the image where the tooth to be prepped resides and prep the tooth. The prepped tooth can be imaged in mere seconds and is quickly stitched into the complete model.

With all these improvements, the savings will not just be in powder. There will be many chair-side efficiencies: quicker imaging, quicker margin identification, less time required for doctors to image their portion, and easier access/handling of the camera. And don't forget about the potential income related to a large color view of the adjacent teeth that exhibit problems as well. Yes, we all want one.

When I was a boy, I remember watching a black-and-white TV.

That TV was fine then, but could give up my 50” LED now? No way! For almost two years we have enjoyed the use of a GALILEOS 3D CT in Laguna Beach, and nowadays there’s no way I could give that up, either!

The day of the install, my team watched in eager anticipation as the technicians completed their work. Then our Sirona rep asked for a volunteer to be the first scan. We gave the honor to a team member and watched anxiously as the computer digested the information. It presented us with 3D images of her skull, and then I saw it… Hold on… That looks like a cyst!

Here are two other cases from September:

1) Idiopathic Dentigerous Cyst

The patient was a 16-year-old female who was asymptomatic and in good health. The cyst traveled from distal #32-#27 and was very expansive. It is currently being treated by decompression/marsupialization following extraction of tooth #32. Upon opening the lesion, the surgeon remarked that the mandibular nerve was just hanging in hollow space.

2) Massive Bone Loss and Sinus Floor Perforation resultant from Failing RCT #15

This patient was also asymptomatic and received a routine pre-surgical scan on the day of an anterior implant treatment with our periodontist. Even more intriguing, two years prior, this 80-year-old male had a sudden onset of severe pneumonia through the lungs to the plural space, requiring external drainage and six weeks of intravenous antibiotics. Medical doctors were not able to discover the source of the illness. On extraction, tooth #2 was very green, suggesting pseudomonas. The patient is now healing and reports he is “feeling great.”

Our team feels that we saved the first patient from certain future mandibular fracture, a problematic and unsatisfactory outcome, and likely a lifetime of unilateral mandibular and lingual numbness. We also suspect that we added years to the second patient’s life.

Since that first day, we have steadily discovered many infections and pathologies that we would have missed without our Cone Beam. Additionally, it makes all our surgical procedures safer and more predictable. Our GALILEOS is as crucial to our office culture as our CEREC® unit, because we all know that it allows us to provide better care for our patients.

Mother Nature's Lesson on Teamwork

Every fall, geese take to the sky and fly south. The flock eases into formation, with one taking lead, and the others trailing behind in two lines making the shape of a “V.” When the bird at the front tires, a different bird takes a turn, and then another when that one has reached his limit. In this way, they move together toward their goal – toward food and warmer weather.

These tactics help ensure the survival and ultimate success of the entire family of geese. In the same way, it seems our PDS family works to support each other: co-workers encourage and look out for each other, circling back to make sure everyone is heading in the same direction, knowing and willing to take a turn at the lead when others tire. Below are a few facts about geese from motivational speaker Katherine Eitel*:

• As each goose flaps its wings, it creates “uplift" for the other birds to follow. By flying in a "V" formation, the whole flock adds 71% greater flying range than if each bird flew alone.

• When a goose falls out of formation, it suddenly feels the drag and resistance of flying alone. It quickly moves back into formation to take advantage of the lifting power of the bird immediately in front of it.

As a benefits-eligible, full-time or part-time employee and affiliate, you have access to the highly regarded Back-Up Care Advantage program from the nationally recognized Bright Horizons Family Solutions.

Adoption Assistance

PDS is happy to support employees and affiliates who have chosen to start or grow their family through adoption. Expenses resulting from legal adoption can be reimbursed up to $5,000. If you have been full-time for one year with PDS or a PDS affiliate, you may qualify for the program.

Did you know that PDS also has several other resources to help with adoption as part of our benefits package?

• Employee Assistance Program (EAP) – Our online EAP has resources like a list of books on adoption, legal consulting service suggestions and guides on talking about adoption with family and friends. You can also call the EAP and speak with a trained adoption consultant. No enrollment in EAP is required.

• When the lead goose tires, it rotates back into the formation and another goose flies to the front position.

• The geese flying in formation honk to encourage those up front to keep up their speed.

• When a goose is sick, wounded or shot down, two geese drop out of formation and follow it down to help and protect it. They stay with it until it dies or is able to fly again. They then launch out to catch up with the flock or rejoin another formation.

Time helps refine and perfect processes, and just as geese have developed a system that furthers the success of the whole flock, we too must look after each other if we are to reach our goals. Ultimately, teamwork and leadership are the keys to success for each office focused on the common goal of delivering high-quality care while delivering the Perfect Patient Experience®

Back-up care is temporary care provided when a lapse or breakdown in normal care arrangements prevents you from getting to work. Your children or the adults and elders that depend on you for care are qualified to use the resources available in the program, including child care centers for kids up to age 12 from in-home professional caregivers.

Call the 24-hour hotline to access a team of Back-Up Care Consultants, who will find and schedule care on your behalf so you can get to work without worry and be sure your loved ones are well cared for. Reservations can be made up to 30 days in advance or even at the last minute during emergencies. Depending on the service requested, co-pays are just $15 per child for the use of a child care center, with a $25 per use maximum fee that accounts for all the children in a family. In-home care is just $6 per hour for up to three persons, regardless of age. Back-up care can be used up to 10 individual times per year.

• MetLife Legal Plan with Hyatt Legal has adoption coverage. It covers all legal services and court work in a state or federal court for an adoption made by a plan member. Enrollment in the pre-paid legal plan is required.

Future Moms

Participants in the health-care plan who are planning a pregnancy should enroll in the Future Moms program. You get a welcome kit with the well-recognized book “Your Pregnancy Week by Week” as a free gift.
