For example, in the “Tell-Show-Do” technique, a pediatric dentist might name a dental instrument, demonstrate the instrument by using it to count your child’s fingers, and then apply the instrument in treatment. There are many other techniques applied by pediatric dentists to comfort a patient. They include modeling, coaching, distraction and parent participation. But by far the most preferred technique is praise. Every child does something right during a dental visit, and pediatric dentists let children know that.

assertive management to protect him or her from possible injury. Voice control (speaking calmly but firmly) usually takes care of it. Parents Get Separation Anxiety Too
O
ne of the hardest things some new parents must learn to do is leave their new bundles of joy at a daycare or with some other caregiver. Many times the new parents’ minds are flooded with what ifs. What if he cries and won’t stop? What if he thinks I’m not coming back? What if …? What if …? During a recent conversation with a family member, I saw the dread come over her face when she talked about the fear of leaving her baby in coming months with a non-family caregiver. For the past six months she and my nephew have been tag-teaming their baby’s care. Work has allowed him to be with their son during the day while she is at work, and her to be at home with the baby during late afternoon and into the evening while he is at work. Not a bad deal, but in a few months that will all change. So what should parents look for when selecting an early childcare program for their little ones? First, arm yourself with information about the various types of programs available to you. Second, select a few programs and visit them. Next ask the right questions of daycare directors or school administrators. Selecting the Right
Early Childhood Learning Program
P
reschoolers – those energetic 3- and 4- year-olds enjoying their first school experiences – are an enthusiastic bunch, excited about learning, playing and making new discoveries. They’re capable of learning language, reading basics, even early math and science concepts. But what they glean most from preschool are crucial social, emotional and character lessons. Early childhood programs offer learning experiences that lay the foundation for later success in school. With so many different programs, how do you choose the right one for your child? Begin by considering what kind of setting best suits your child and your family’s needs. ■ Full-day programs (often called daycare) may be in an individual provider’s residence or in a commercial childcare center. ■ Lab schools are affiliated with colleges or high schools and use student teachers who are
supervised by professionals. Some also combine programs with research studies on effective early childhood education. ■ Montessori schools are operated on the philosophy that young children learn best through direct sensory experiences such as blocks or pegboards. This philosophy is reflected differently at each Montessori school. ■ Nursery or preschools are private or locally funded, secular or religious and situated in either private facilities or municipal buildings. When selecting a program, parents should ask about the size of the school, the number of days and hours offered, the availability of extended hours, how children are grouped, class size and program cost. After deciding which type of program best meets your criteria, you should visit more than one preschool or daycare site so you have a basis for comparison.
What to Look For in a Preschool or Childcare Program The National Association for the Education of Young Children (NAEYC) recommends that parents seek out: ■ Well-trained staff – The professional qualifications and training of teachers and staff are crucial to a quality program. Ask about the degrees and certificates held by the director and teachers, and find out what steps they take to provide staff with ongoing training. ■ Low child-to-teacher ratio – The number of adults and children is important because it helps determine how much individual attention your child will get. There should be at least one adult for every: – four infants – five younger toddlers (12 to 24 months) – six older toddlers (2 to 3 years) – ten 3- or 4-year-olds ■ Small group size – In addition to low teacher-to-child ratios, the overall size of the program is important. Look for a program with fewer than six to eight infants in a group, 10 to 12 toddlers, and no more than 20 preschoolers.

■ Low staff turnover – Teachers who have been in a program longer establish bonds with the children, and those relationships help children grow and learn. Low turnover is also usually a sign that the program values good staff and works to keep them. ■ A safe and healthy environment – Check that the program is licensed by the state. Optimally it will have NAEYC accreditation, which demonstrates that the program has undergone a rigorous evaluation. Make sure that the facility looks clean, and that all children are under adult supervision at all times. Staff should be able to describe clear health and safety procedures, as well as policies for handling emergencies. ■ Take the time to choose carefully. Choosing a preschool means building a relationship with people who will give your child a great start on learning. Adapted from the NAEYC brochure “Early Years Are Learning Years.”

encouraged? ■ Will the school’s administration refer you to par-

POTTY TRAINING AFTER DARK: The Road to Clean, Dry Nights By Malia Jacobson
W
hen my first daughter showed signs of potty readiness at age 2, I was more than ready to help her ditch the diapers. Before long, she was proudly sporting Elmo underwear and staying dry all day long. I was ecstatic. As a potty-training novice, I was certain that it would be a matter of weeks before she was dry at night, too. Of course, reality wasn’t nearly as neat and tidy. Although daytime training was quick and easy, nighttime dryness proved elusive. Months passed, and she pottied like a pro during the day but clung fiercely to diapers at night. After two long years (and many unsuccessful attempts) she finally started waking up dry, and we packed away the diapers for good. In retrospect, it was no big deal: it’s highly common for a 4-year-old to wear a diaper to bed. Eventually, most kids achieve nighttime dryness without intervention. But I could have avoided some anxiety if I’d known all of that going in. Our situation was far from unusual; experts say that many parents need to adjust their expectations about nighttime dryness. According to Tai Lockspeiser, M.D., assistant professor of pediatrics at The Children’s Hospital in Aurora, Colo., nighttime bladder control is a maturational process that can lag behind daytime bladder control by months or years. Twenty percent of kids still have nighttime accidents at 5 years of age, and doctors don’t define bedwetting until children are 6 years old. So it’s completely normal, even expected, for kids to take their time with nighttime potty training. But the delay leaves many parents like me stuck in a waiting game, wondering when daytime potty learning will 10

carry over into nighttime dryness. While parents can’t speed up the developmental process, they can help encourage dry nights with these simple steps.
Age Matters Start with realistic expectations. While 88 percent of kids develop nighttime bladder control by age 6, the timeline varies widely. Boys typically train more slowly than girls, says Lockspeiser. Kids who are exceptionally deep sleepers and those with developmental delays may have more difficulty with wetting as well, she says.

Potty Practice The best way to encourage nighttime dryness is to practice good daytime habits, notes Steve Hodges, M.D., a pediatric urologist at Wake Forest University Baptist Medical Center. Children should use the toilet as soon as they feel the urge – holding can strain the bladder and worsen nighttime wetting, he says. And using the toilet before bedtime is a must.
Encourage Digestive Health “Constipation is probably the most underappreciated cause of bedwetting,” says Hodges. “It plays a role in 30 percent of the cases I see.” A full bowel puts pressure on the bladder, making nighttime accidents more likely.

Cut Caffeine Limit fluids two hours before bedtime, particularly caffeine-containing beverages. “Caffeine is a diuretic, so it promotes urination. Drinking it before bedtime will make it harder for kids to stay dry at night,” says Lockspeiser.
Ensure Nighttime Toilet Access Ensure that kids have access to the toilet at night. Nightlights in hallways and bathrooms can help kids find their way easily. If the trek to the toilet is too far or involves stairs that tots can’t navigate on their own, parents can place one of the small portable toilets commonly used for potty-training in their room at night.
Skills versus Pills Doctors may recommend treatment options for persistent wetting that doesn’t resolve by age 6. Bedwetting alarms, called enuresis alarms, help children learn to stay dry by waking them at the first sign of wetness. Medicines like desmopressin can help prevent accidents by slowing nighttime urine production. While pharmaceutical treatment can be useful for special situations – sleepovers or vacations, for example – it

Staying Clean: Encopresis What about kids who soil at night? Encopresis is the term for soiling in inappropriate places after age 4. While nighttime soiling in preschoolers and school-age children is much rarer than bedwetting, it’s also more emotionally distressing for the family and socially isolating for the child, says Collins.

See a doctor if a child who has been potty-trained and dry at night for months begins wetting at night. A urinary tract infection is a common culprit for sudden wetting. “We also consider stressors or social challenges, like a move, a new school, or divorce,” says Lockspeiser.

First, take a look at their daytime bowel habits. “Chances are, a child who is soiling at night is holding during the day,” says Collins. Dietary changes, increased fluid intake, and changes to the child’s daily routine can help encourage regular elimination during the day. If improved daytime habits don’t resolve the problem, families can progress to more advanced encopresis treatment, which may include supplements, suppositories, and behavioral therapy to treat severe constipation and holding habits. The good news: these treatments have a high success rate and kids benefit from increased confidence and self-esteem as the condition improves. Parents’ attitudes are highly important as kids develop nighttime control. “Treat it as a problem-solving exercise – a family science experiment,” says Collins. Above all, make sure that children know that nighttime wetting or soiling is not their fault. O
ne of the keys to raising a happy, well-adjusted child is nurturing his or her self-esteem. What follows is an interview with María del Carmen Mejía, a local clinical social worker with Clarity Child Guidance Center. In addition to her duties as a social worker, Mejía teaches a series of parenting classes annually at the center. One of the classes offered is “How to Nurture Your Child’s Self-Esteem.”

What is self-esteem? A. I don’t think there is any one definition of self-esteem. There are many aspects to self-esteem. It encompasses self-acceptance, self-respect, feeling loved, having worth, having confidence in ones self and viewing ones self as an equal to others.

How can parents nurture their child’s self-esteem? A. There are a lot of things parents can do to nurture their child’s self-esteem. I group these in three categories: 1) Make sure the child feels loved and accepted, 2) Allow kids to develop unique goals, ideas and decisions on what they want in life, and 3) Give them freedom to make responsible choices to achieve success. Category 1: Make Sure the Child Feels Loved and Accepted ■ Know your child for who she is, ■ Know that your child is an unique individual with her own ideas, goals and dreams, ■ Differentiate your child’s needs and feeling from your own, ■ Acknowledge a child’s uniqueness as an individual and then enable her to learn what she can offer the world. Stated another way: What is special about her? 14

■ Make sure she feels heard, understood
and empathized with even if you disagree with her, ■ Acknowledge that her ideas are worthy of attention, ■ Listen to and respect her goals, decisions and dreams, ■ Make sure she feels successful based on her own definition of successful. Category 2: Allow Kids to Develop Unique Goals, Ideas and Decisions on What They Want in Life ■ Praise effort and give plenty of encouragement. Believe in your child, ■ Acknowledge that her ideas are worthy of attention, ■ Help create situations in which your child can experience success. Provide lessons or opportunities where she can develop a sense of accomplishment, ■ Acknowledge and recognize your child’s successes and the things that are important to her. For example, if you like tennis, but your child doesn’t and prefers reading, recognize and celebrate her achievements in reading. Category 3: Give Kids the Freedom to Make Responsible Choices to Achieve Success ■ Assign meaningful and appropriate tasks to do. This helps children feel capable and develop a sense of accomplishment, ■ Have reasonable expectations for your child and help her set reasonable goals, ■ Encourage your child to deal with choices responsibly. For example, when it comes to discipline, make sure your child knows she made the choice to disobey or misbehave, and in doing so she must accept the consequences that go along with the decision to misbehave.

What are some of the characteristics that may be exhibited by a child with little, no, or poor self-esteem? A. One characteristic of a child with low self-esteem is reluctance to take risks. Some other examples could be: ■ Shying away from peers, ■ Being easily frustrated, ■ Demeaning his or her own talents, ■ Continually putting himself or herself down, ■ Putting on a façade of high-self-esteem possibly by bullying others, ■ Becoming defensive because he cannot accept feedback, ■ Over-compensating by constantly placing self on a pedestal and putting others down, ■ Being easily influenced by peers and wants approval of the group, and ■ Giving in to peer pressure.

What are some of the characteristics exhibited by a child with a high-level of self-esteem? A. Some characteristics that may be exhibited by a child with a high-level of self-esteem could include: ■ Willingness to take risks; not afraid of failure because he understands that he or she learns from his or her mistakes, ■ Knowing he or she cannot be perfect, ■ The ability to laugh at himself or herself when he or she blows it, ■ Complimenting others easily, ■ Recognizing his limitations, but having an expectation to grow and improve, and ■ The ability to resist peer pressure.

Why is self-esteem important? A. Self-esteem is important because the way people relate to themselves impacts the way they relate to the world.
Q
All caregivers, not just parents should be concerned about a child’s self-esteem. How can parents be sure that a caregiver is nurturing their child’s self-esteem? What characteristics should they look for in a caregiver or teacher who will nurture their child? A. Look at how the caregiver talks to your child and how they relate to him. Does the caregiver provide children with opportunities to learn, grow and develop new skills? Does the caregiver teach and model? It’s very important to look at how they talk and relate to kids. ■

Advice for Parents of YOUNG CHILDREN in the Digital Age F
ast-paced changes in today’s digital world have parents grappling with an unprecedented challenge – how best to integrate media and technology into their children’s lives. It is a time to seize opportunities, according to the Fred Rogers Center for Early Learning and Children’s Media at Saint Vincent College. “Healthy use of media and technology by the youngest children is possible – and more important – than ever,” says Michael Robb, Ph.D., Director of Education and Research at the Rogers Center. Robb manages the Rogers Center’s newest program, the Fred Rogers Center Early Learning Environment or ELE. The site provides parents free digital resources and guidance and empowers them as teachers of early language literacy. “We are seeing an explosion in the number of digital resources that can have an impact on the intellectual, social, emotional and physical development of children birth through age 5,” says Robb. “The challenge for parents is to select the videos, games and devices that have a real, positive developmental impact – and use them in ways that promote growth.” Robb offers the following advice for parents navigating the digital age: Keep it interactive. The way a digital tool is used is as important as the tool itself; adult-child interaction should be emphasized. Rather than putting on a television show or handing over your smartphone and walking away, create a dialogue. Sit and discuss what you are seeing, ask questions, encourage imaginative ways to explore similar subjects in the outside world. Match use with age. Children’s needs change as they develop. Any media use with infants and toddlers should be an interactive experience with adults, such as reading an e-book together; older pre-school
children may enjoy exploring a touch screen or using video to record and view their play. Have fun, stay engaged. Children’s media and technology are best when they support active, handson, creative and authentic engagement with the people and world around them. Look for games, websites and apps that encourage outdoor activity, healthy eating, critical thinking and other real-world skills. Promote digital literacy. By modeling appropriate use of digital media, adults can help children learn to use the wealth of tools at their disposal in smart, healthy ways that complement their growth and give them greater access to the opportunities of the digital age.
