For example, in the “Tell-Show-Do” technique, a pediatric dentist might name a dental instrument, demonstrate the instrument by using it to count your child’s fingers, and then apply the instrument in treatment. There are many other techniques applied by pediatric dentists to comfort a patient. They include modeling, coaching, distraction and parent participation. But by far the most preferred technique is praise. Every child does something right during a dental visit, and pediatric dentists let children know that.

Occasionally a child’s behavior during treatment requires

assertive management to protect him or her from possible injury. Voice control (speaking calmly but firmly) usually takes care of it. Mild sedation, such as nitrous oxide/oxygen or a sedative, may also benefit an anxious child. If a child is especially fearful, other sedation techniques or general anesthesia may be recommended.

Choosing a Daycare, Preschool Get the facts. Do the homework. Trust your judgment. Choosing a healthy, safe place for your child to thrive while you’re at work is a daunting decision. Daycare horror stories making national news may suddenly keep you awake at night. But take a deep breath. Safe, healthy options for childcare are out there. It’s important to gather information, do the legwork, and ultimately trust your gut.

Home DayCare or Center? Home daycare may be the best fit for your child’s “home away from home.” These providers tend to have more flexible hours (especially helpful if you work an atypical work schedule). They may have less rigid schedules and policies which set you more at ease. If it is important to you that your caregiver be open to making lots of individual accommodations (irregular naptimes, for example), a home provider might suit you and your child best. Some home providers are regulated, but many are not so it’s a personal choice dependent on whether you’re comfortable. Licensed daycare centers offer many advantages. Centers may hire certified early childhood educators with training in child development and have more clearly outlined policies so everyone knows what to expect. Also, having more than one caregiver on the premises provides peace of mind 6

Get the Facts: Narrow the Possibilities There are regulated daycare centers, regulated home daycares, unregulated home daycares, and preschools. Begin your search early. Ann Douglas, author of Choosing Childcare for Dummies, suggests embarking upon your quest between eight weeks and a year before your backto-work date. Contact your local child care referral agency by checking Child Care Aware, a national consumer education parent hotline (1-800-424-2246) and web delivery system (www.childcareaware.org). The National Child Care Information Center (1-800-616-2242 or www.nccic.org) is also a good start. Ask the referral agency about licensing requirements in your area and how to collect information about complaints or licensing violations. Ask about financial assistance programs for which you may qualify.

(especially if you’re concerned about TV watching or unfamiliar adults in a home setting). Centers offer reliability so you won’t have to scramble for care if one caregiver is ill or on vacation. The spaces in a center are typically kid friendly and designed with safety in mind. Because of government regulation, child to caregiver ratios, safety requirements, and criminal record checks are standard. Do the Homework: Visit and Assess Quality The American Academy of Pediatrics (AAP) recommends you consider the following 13 guidelines as you evaluate the quality of a provider: • Supervision. Are children supervised at all times, even when they are sleeping? Is discipline positive, clear, consistent, and fair? • Handwashing/Diapering. Hands should be scrubbed with soap and water for at least 10 seconds and then rinsed and dried. The faucet should be turned off with a paper towel.

Quality Preschools The National Association for the Education of Young Children (NAEYC) suggests the following 10 signs are indicative of a good preschool:

OPEN PLAY EVERYDAY • Open Weekends • All day admission • Ages 2yrs. – 12 yrs. • Reduced prices for 12mo. – 23 mo. • No charge for Mom and under 12 mo.

• Director Qualification. The director should have a college degree and have worked in childcare for at least two years. • Teacher Qualifications. The lead teacher should have a college degree in a child-related field and have worked in childcare for at least a year. • Child:Staff Ratio. One family home caregiver should care for only two babies. The fewer children each caregiver care for, the better. • Immunizations. Does the provider have records proving that other children are up to date? • Toxic Substances. Are cleaning supplies and pest killers kept far from children? • Emergency Plan. Are first aid kits and emergency plans in place? • Fire Drills. Are these practiced monthly? • Child Abuse. Can caregivers be seen by others at all times so a child is never alone with one caregiver? Is it kept out of reach and labeled properly? • Staff Training. Have all caregivers been trained in first aid and CPR? • Playground. Is it developmentally appropriate, clean, and inspected regularly for safety?

Trust Your Judgment: Notice your Gut One of the most helpful factors to consider when choosing a provider is the interaction between caregiver and children. Is there good communication? What is the caregiver’s interpersonal style? Notice how the interaction makes you feel. Is it a place you would look forward to coming each day? 5 Ways to Boost Your Preschooler’s Vocabulary Let’s Get Ready to Read!

Who can forget their child’s first word? It was likely your name, but maybe it was “bawww” (ball) which lit up your faces with a smile. It feels natural to celebrate, praise, and applaud our toddlers as they acquire new language skills daily. In turn, such enthusiasm reinforces a child to speak additional words. However, University of Iowa psychology professor Bob McMurray, Ph.D. says parents stop emphasizing language as their kids leave toddlerhood. Since a preschool child’s vocabulary is a critical predictor of school preparedness and reading comprehension, it is important parents do what they can to boost it. McMurray’s research indicates a vocabulary explosion (or “word spurt”) is dependent upon a child learning a mix of words, both easy and not easy and all at once. Interestingly, vocabulary explosions specifically require “more difficult words than easy words.” The professor and other language professionals make the following suggestions to boost your child’s verbal skills.

Mealtime Magic (Hold the Thesaurus) A child’s vocabulary can be enhanced by our talk at the dinner table. Use moments at mealtimes to introduce new words, especially challenging ones since you’ll have their attention in a pleasant setting. What to discuss? In a mealtime study, Dr. Diane Beals

and her colleagues of the University of Tulsa discovered that 3- and 4-year-olds who were exposed to uncommon words such as “boxer,” “wriggling,” or “tackle,” scored higher on later standardized tests at age 5. Beals says forget about serving up a thesaurus at the table and instead discuss “your day or something cool you saw at the store.” Bringing in new words helps them form connections between words and real-life events.

Guessing for Success Consider a little restraint to challenge your child. McMurray suggests instead of automatically doling out definitions for your preschooler when they stumped on a word’s meaning, you should give clues and allow her to figure it out on her own. If, for example, the word in question is “equestrian,” give her hints such as saddle, mane or stable. It can also be helpful to “tell them what it isn’t” So you could say “not cows, but…”

Stage Your Own ‘Show & Tell’ Who says “show and tell” is just for school? You can easily use this format to describe and discuss an object to expand their vocabulary outside of school. You might demonstrate the hand chopper you use to dice vegetables, explaining how the appliance functions and saves you time. You could show your child the checks in your checkbook, discussing how the small sheets of paper may be used for payments. The important thing is to have your child’s attention and provide something to touch and to see to anchor vocabulary within their memories.

including menu, hostess, variety, or beverage. If she wants to land on the moon, vocabulary words such as lunar, satellite, or gravity might be incorporated. Outside of pretend play, parents can help their children better remember the meanings of words by acting them out. Even simply explaining that shrugging your shoulders means “I don’t know” is helpful. Is all the acting and drama really necessary? Consider a new University of Chicago study which is the first to connect gesture, vocabulary and school preparedness. Conducted by Susan Goldin-Meadow, the results indicate children who use more gestures at 14 months have larger vocabularies at 54 months and are better prepared for school. Goldin-Meadow indicates “Child gesturing could play an indirect role in word learning by eliciting timely speech from parents” since a child pointing to an object like a cup might elicit a response from the parent such as “Yes, that’s a cup!” Try all five of these strategies to boost your child’s verbal repertoire today and help her become a better reader tomorrow.

Good Parenting Triumphs Over Prenatal Stress

A mother’s nurture may provide powerful protection against risks that her baby faces in the womb, according to an article recently published online in the journal Biological Psychiatry. The research shows that fetuses exposed to high levels of stress hormone – shown to be a precursor of babies’ poor cognitive development – can escape this fate if their mothers provide them sensitive care during infancy and toddlerhood. The new study represents the first direct human evidence that fetuses exposed to elevated levels of the stress hormone cortisol may have trouble paying attention or solving problems later on. But what may be more intriguing is the study’s second finding: That this negative link disappears almost entirely if the mother forges a secure connection with her baby. “Our results shape the argument that fetal exposure to cortisol – which may in part be controlled by the mother’s stress level – and early caregiving experience combine to influence a child’s neurodevelopment,” says study author Thomas O’Connor, Ph.D., professor of Psychiatry and Psychology at the University of Rochester Medical Center, and director of the Wynne Center for Family Research. For the study, researchers recruited 125 women at an

amniocentesis clinic in an urban maternity hospital, taking a sample of their amniotic fluid so that stress hormones in it could be measured. The mothers were at 17 weeks gestation on average; only mothers with normal, healthy pregnancies and subsequent deliveries were followed. When their children reached 17 months of age, researchers administered a Bayley infant developmental scale test, which relies on puzzles, pretend play, and baby “memory” challenges to gauge youngsters’ cognitive development. They also observed the baby and mother using the Ainsworth “Strange Situation” test, which judges childrearing quality, categorizing mom-baby pairs as either showing secure or insecure attachment to each other. With cortisol levels, relationship quality results, and cognition scores in hand, researchers analyzed how the first two measures might influence the third. Indeed, for children showing “insecure attachment” to their mothers, a high prenatal cortisol level was linked with shorter attention spans and weaker language and problemsolving skills. But interestingly, for kids who enjoyed secure relationships with their moms, any negative link between high prenatal cortisol exposure and kids’ cognitive development was eliminated. “This is such refreshing news for mothers,” O’Connor says. “Pregnancy is an emotional experience for many women, and there is already so much for mothers to be careful of and concerned about. It’s a relief to learn that, by being good parents, they might ‘buffer’ their babies against potential setbacks.” – Lisa Carpenter

Growing Pains

Growing Pains – They’ve Got Nothing to Do With Growth! by CHRISTINA ELSTON

They’re plenty confusing to parents – these mysterious pains that plague kids’ legs at night and reliably vanish by morning. Medical tests show that there’s nothing wrong, and your child’s doctor has no tips on preventing or treating the pain. “Growing pains” are a common childhood ailment. And yet, they’re not even caused by a child’s growth. “It is true that muscles, tendons, ligaments and bones grow continuously, but the growing process alone seldom causes pain,” explains Francis Y. Lee, M.D., an orthopedic surgeon at Children’s Hospital of New York and Columbia University. In fact, growing pains are most common from ages 3 to 12, when kids aren’t having rapid growth spurts. So why call them “growing pains”? Because all children eventually outgrow them, says William Oppenheim, M.D., a pediatric orthopedic surgeon with the Luskin Children’s Clinic of Los Angeles Orthopaedic Hospital and the UCLA Department of Orthopedics.

• in both legs, and • in the thighs, calves and behind the knee. In the morning, kids who had been suffering from growing pains get around just fine; you’d never know they had a problem. “During the day they’re totally normal,” says Meghan Imrie, M.D., associate clinical professor of orthopedic surgery at Stanford University. Any lab tests that your pediatrician performs will also come back normal if growing pains are the problem.

What They Aren’t What They Are Growing pains are the third most common type of pain in children ages 3-12 (headache and stomachache rank first and second), impacting between 15 percent and 30 percent of this age group. The pains tend to occur: • at night, • for a few nights at a time, every one to three months,

The most important thing to know about growing pains is that this is a “diagnosis of exclusion,” Imrie says. That means growing pains are diagnosed after doctors rule out everything else. Here are some symptoms that do not indicate growing pains and should be reported to your child’s doctor: • pain that is focused in one spot or one leg only (localized); • pain focused in the joints, making a child feel


“stiff” in the mornings; • pain that’s persistent, rather than coming and going in spates; • pain accompanied by fever, swelling or tenderness; • pain with injury or bruising; • pain that causes a child to limp during the day, or curtail his physical activities. Similar to growing pains is a condition called transient synovitis. Its cause is also unknown, but this condition brings day and nighttime joint pain in the knee or hip. Kids with transient synovitis don’t have fever or other signs of infection, but the pain may become so severe they walk with a limp. The condition usually clears after three or four days, with no long-term effects. Painful joints with swelling and fever, on the other hand, are a big red flag, and you should have your child seen by a doctor immediately. This can be a sign of joint infection, which Oppenheim warns can cause permanent damage within a day or two. A hopeful sign, believe it or not, is if your child’s pain stays the same. “More worrisome things like a tumor or arthritis become worse over time,” says Imrie. Those are two of the serious conditions connected with nighttime leg pain in kids. Others include leukemia (cancer of the white blood cells) and lymphoma (cancer of the lymphatic system). Less-serious causes of leg pain include injury or overuse of muscles (strains), ligaments (sprains), bones (stress fracture) and joints (cartilage bruise). Long-distance runners can develop pain in the thigh, which may indicate stress fracture. Basketball players can have pain over the front of the knee, a condition called Osgood-Schlatter’s disease (apophysitis). But these things usually get better when physical activity is reduced, explains Lee.

Causes No one knows what causes growing pains, “and that makes it a bit of a problem,” says Oppenheim. Especially frustrating since the condition was first described in 1823! He points to a Journal of Foot and Ankle Research article from 2008 that shows that research on the topic has been plentiful, but “lacking in scientific rigor.” Still, there have been plenty of theories about the cause of growing pains:

• Muscle fatigue – This theory hasn’t been tested, but parents of kids with growing pains often report that they’re linked to increased physical activity. • Bone fatigue – Studies have shown that the bones of kids with growing pains are less dense (and so weaker) than those of other children, leading some to suspect the pain comes from bone fatigue. • Genetics – Experts suspect that low pain thresholds run in families, and some studies show that kids with growing pains have lower pain thresholds than their peers without. • Joint hypermobility – Another untested theory is that children with growing pains are “double jointed,” a trait also common to children with painful conditions such as fibromyalgia. Oppenheim’s personal theory is that because children in this age range are so physically active, they make the periosteum (a membrane containing blood vessels and nerves that covers the bones) sore. It’s much thicker in kids than in adults, and could be less flexible. But Oppenheim says he doesn’t have research to back this up. “Nothing has really been pinned down,” he says.

What You Can Do There aren’t any proven treatments for growing pains, in part because it’s difficult to prove whether a given treatment worked or whether the child just outgrew the condition. Still, some things do seem to help: • Reassurance, • Heat and massage, if that feels good to the child, and • Over-the-counter pain medications such as Tylenol and Advil. “It’s basically just rubbing where it hurts, kissing the boo-boo,” says Imrie. Oppenheim puts it another way. “You can be sympathetic and carry them through it,” he says. “And you can realize it’s not the end of the world, and they will outgrow it.” Christina Elston is a senior editor and health writer for Dominion Parenting Media. A national survey released by Underwriters Laboratories, an independent product safety organization, reveals that children’s initial reactions might actually put them in danger during an emergency. While more than 90 percent of children say they would know exactly what to do if there was an emergency like a fire, only 47 percent chose the safest option: get out of the building immediately. UL urges families to consider the following safety tips in preparing for the unexpected: Children should know their full name, parents’ full names, address (including city and state), home phone number (including area code) and parents’ work phone numbers or cell phones before leaving the home. Designate an out-of-town relative or friend to be your family’s emergency contact and keep their information with you at all times. Prepare an emergency kit, including: five days worth of non-perishable food and water, a can opener, flashlight, portable emergency radio (handcrank, solar-powered or battery-operated), batteries, any prescription medication needed by family members, a first-aid kit, list of phone numbers for relatives, neighbors and utility companies, and pictures and descriptions of your family. If you have pets, include five days worth of canned pet food and water, sturdy leashes, harnesses or carriers, current photos and descriptions and a litter box. Develop and practice several disaster preparedness plans. Make sure your child knows the first thing he should do in the event of a storm or other disaster, regardless of his location. plan and mapping out each family member’s route of escape making sure each room has two exit options. Designate a meeting place where your family will reunite if separated. Consider posting the fire escape route on refrigerators and in each family member’s bedroom. Make sure your children know how to respond to an emergency in the environments they frequent, including schools, friends’ houses and public buildings like grocery stores. Every child is unique, with different temperaments and different interests. Every day they are learning and growing. Support them as they struggle to master more difficult skills and celebrate their unique successes with them. Be a part of playtime. Play is how young children learn about the world and practice new cognitive, emotional and social skills. A parent can learn a lot about their child’s developing skills and interests by participating in their play. Your questions, ideas, and attention support your child’s curiosity and creativity. Play interactions with you can also help them to develop skills to use when they are playing alone or with peers. Talk about feelings. Understanding and managing feelings is one of the hardest skills children under age 5 have to tackle. So respect what they are feeling by recognizing their emotions, listening to their thoughts and helping them work through their emotions. Be an example. Through your voice, your expressions, your gestures and your actions, your child learns from

you how to interact with other people and with the world. You are in a wonderful position to set a positive example for everything from solving problems to showing compassion to buckling seatbelts. Create routines and stick to them. Children respond to the expected. If you create a bedtime routine that has three steps and you do the same things every night, your child finds comfort in knowing what comes next and will respond in positive ways as a result of sticking to the plan. Make mealtime special. Whenever possible, make mealtime a time to be together and enjoy a healthy meal. This not only develops a good pattern for eating, but it creates an opportunity to connect as a family. Make books a part of your child’s life. Make reading fun. Cuddle up. Let your child choose the books. By reading to your children, you are helping them increase their vocabulary, inspiring their imagination, and setting the groundwork for a love of reading. Boys and Reading: The New Gender Gap

We’re so accustomed to hearing about girls lagging behind boys in math. Now comes the news that boys’ poor reading skills have led to a much bigger gender gap in our schools. A new study from the public education advocate Center on Education Policy (CEP) finds that boys are significantly behind girls in reading at the elementary, middle and high school levels. Researchers looked at scores from standardized tests in all 50 states between 2002 and 2008, and found that boys are significantly behind girls in reading. In some states, the percentage of boys scoring “proficient” in reading is more than 10 percentage points below that of girls. For their part, girls appear to be doing much better in math. The study found that at the proficient level, the number of states where girls outperform boys is about equal to the number of states where boys outperform girls. At the “advanced” score level, boys in the fourth grade still outperform girls in most states. In a statement released with the report, CEP President Jack Jennings urged “much greater attention” to boys’ reading skills beginning in the early grades. Overall, many states have narrowed the gap between male and female students in both math and reading, the CEP study found. But while gaps in elementary school reading have narrowed in 24 states, they’ve widened in 14. And while boys and girls have improved on fourth-grade reading exams since 2002, more girls than boys reached all three achievement levels – basic, proficient, and advanced – in 2008.
