Pediatric dentists are trained in many methods to help chil-

dren feel comfortable with dental treatment. For example, in the “Tell-Show-Do” technique, a pediatric dentist might name a dental instrument, demonstrate the instrument by using it to count your child’s fingers, and then apply the instrument in treatment. There are many other techniques applied by pediatric dentists to comfort a patient. They include modeling, coaching, distraction and parent participation. But by far the most preferred technique is praise. Every child does something right during a dental visit, and pediatric dentists let children know that.

What if a child misbehaves during treatment?

A

Occasionally a child’s behavior during treatment requires

assertive management to protect him or her from possible injury. Voice control (speaking calmly but firmly) usually takes care of it. Mild sedation, such as nitrous oxide/oxygen or a sedative, may also benefit an anxious child. If a child is especially fearful, other sedation techniques or general anesthesia may be recommended.

When deciding on child care for their infants, many parents struggle with questions about quality. Childcare research has shown that the setting (in-home, family or center) matters less than what happens within that setting. Babies need to be able to form a trusting relationship with providers, which allows for an emotional bond to form. Low provider-to-child ratios are key. Equally important are questions on what staff salaries and benefits are like. Low pay is responsible for rampant turnover within the childcare industry, thus making it harder for your baby to build a comfortable bond with his or her caregiver(s). Here are some tips to keep in mind when investigating childcare options for your baby: • Visit the daycare site before enrolling your child. Visit at times when you can observe the interactions of children with each other and with adults. • Spend some time observing the infant room. Visit several times to judge how caregivers respond to tears or times of stress. • See if infants are allowed to have their naps when it feels right for them. Unlike older children, babies have a harder time being treated as part of a group and need to follow their own inner schedules. • Make sure providers are interacting with babies. According to a study by the

National Institute of Child Health and Human Development, “the amount of language that is directed at the child in child care is an important component of quality provider-child interaction.” • Ask how care providers handle babies’ special needs, such as food allergies or attachment to a pacifier. • Check the qualifications and credentials of the caregivers. Degrees in early childhood education and accreditation from the National Association for Family Child Care and the National Association for the Education of Young Children provide good starting points. • Get names of parents whose children have attended or are attending and call them to inquire about the childcare center or providers.

Choosing Child Care, Early Learning Programs for Toddlers and Preschoolers Early spring is the time many families begin their search for an early learning childcare program for fall placement. It also is a time when many early childhood education programs are holding open houses and welcoming prospective parents who may have a lot of questions about choosing the best program for their child, according to the National Association for the Education of Young Children (NAEYC). Consider the following checklist of tips from the NAEYC, the American Academy of Pediatrics and Child Care Aware when visiting programs for the first time: 1. Are there opportunities to visit the center? Can you visit and stay awhile to get a good sense of the environment? If your child is enrolled in the program, can you visit the center anytime it is open? Can you see all the areas that your child will use? Are visitors screened or is their identification checked, so that only approved adults can visit the center and pick up children? 2. What is teacher to student ratio? Ask about and observe group sizes and the number of teaching staff in each classroom. NAEYC recommends no more than 12 toddlers with at least two teaching staff. For preschoolers, group size should not exceed 20 children with at least two teaching staff always present. 3. Is play, including dramatic play and blocks and active play outdoors, integrated into classroom topics of study? Play not only supports children’s intellectual development; it is also very important for the physical development of children. 4. Does the programs promote the health and nutrition of children? 5. Are the children in the program engaged with one another and their teachers? Listen and watch for a happy buzz of activity – neither too quiet nor too loud. 6. Do teachers use positive speech? Are they loving and responsive to the children? 7. Is the program able to adapt to the needs of each individual child? A good program should be

able to adapt without ignoring the whole group. 8. Are there strategies in place to attract and maintain qualified, well-trained staff? 9. Does the program have a strong connection with the families of each child and the community? What type of additional training has the staff had during the past year? Do outside experts provide training? 10. Is the program licensed by the state? Are there any outstanding violations? Is the program accredited or in the process of becoming accredited? 11. What are the hours of operation? What if you are late in picking up your child? 12.Is there a qualified health professional, such as a doctor or nurse, for the program? The national standard recommends that center-based infant-toddler programs be visited by a health professional at least once a month, and all other childcare programs should be visited at least once every three months, according to a brochure produced by the American Academy of Pediatrics. 13. Do children need a medical exam before they can enroll? Have staff members been checked by a doctor to be sure they are healthy? What are the policies when chil-

dren are mildly ill? 14.What is the discipline policy? Do the children go on outings? If they travel by car, van, or bus are the proper child safety seats, booster seats, and seat belts used? Is there someone besides the driver supervising the children during transport? 15. Are the program’ policies on meals, discipline and issues such as toilet training the same or similar to yours? How long have staff members worked at the center? How much experience do they have with children of your child’s age? 16. What happens if your child is sick or the childcare program is closed? 17.What are the costs? How are payments made? Are there other services available in addition to child care? Do these cost extra? 18.Ask for references and contact information from parents who use the program as well as at least one parent whose child was in the program during the past year. 19. Can you talk to staff on a regular basis? If there was something sensitive you needed to bring up, would you feel comfortable talking to them?

20.Does the childcare program have records proving that the other children in care are up-to-date on all their required immunizations? 21. Does the childcare program have an emergency plan if a child is injured, sick, or lost? Does the childcare program have first aid kits? Does the childcare program have information about who to contact in an emergency? Parents should also think about what they see based on what they know about their child. Are the teachers treating children in ways that will be comfortable to their child? Are the materials and activities likely to be of interest and to be challenging but achievable to their child? Is the program interested in what is important to the parent and work with the family to give a child the high quality experiences he or she deserves? Overall parents should feel comfortable in an early education setting. Chances are if a parent does not feel comfortable, neither will the child. It is also important for a parents to understand that programs will not only meet a child where he or she is, but also present challenges to improve that child’s cognitive, social, emotional and physical development.

These guidelines from the American Academy of Pediatrics (AAP) are based on the idea that infants may be suffocating after turning their faces into soft bedding or nearby soft objects. And they’ve been shown to work. From 1992 to 2003, the rate of SIDS deaths actually dropped by more than half (from 1.2 deaths per 1,000 live births to 0.53 deaths per 1,000).

New parents are obsessed with keeping their babies safe. Many will tell you that they check on their sleeping infants repeatedly just to be sure the babies are still breathing. The fact that, each year, 2,500 infants in the United States are victims of sudden infant death syndrome (SIDS) is a big reason why. SIDS, the sudden, unexplainable death of a baby under 1 year old, is the leading cause of death for infants from 1 month to 1 year of age. Typically, parents will check on a sleeping infant and find her dead – a tragedy that haunts them for the rest of their lives, according to the American Sudden Infant Death Syndrome Institute. And yet, despite 16 years of public health messages urging parents to follow a set of guidelines proven to help prevent SIDS, a recent study published in the journal Pediatrics found that 26 percent of mothers of 3-month-old babies don’t adhere to recommendations to: • always put an infant to sleep on his back, • keep the crib free of soft objects like stuffed toys, pillows and quilts, and • not have the baby sleep in the same bed with his parents. The decline was attributed to health recommendations, which began in 1992, to put babies to sleep on their backs instead of their stomachs. New SIDS Prevention Tips Now, new research points to the use of fans and the training of childcare providers in SIDS prevention as other effective ways to reduce the incidence of SIDS. A recent study by Kaiser Permanente’s research division found that using a fan in a baby’s sleeping room cut an infant’s risk of SIDS by 72 percent, compared to

rooms with no fan. Kaiser Permanente researchers also found that infants using a pacifier while sleeping were at reduced SIDS risk, perhaps because the pacifier prevented an infant from pressing completely up against soft bedding or a mattress. Meantime, the AAP is reporting that training childcare providers in SIDS prevention also helps reduce risk. An estimated 20 percent of SIDS deaths occur while a baby is in the care of someone other than the parent, the AAP reports. In a recent study, 1,212 providers in 264 childcare programs received AAP-approved SIDS prevention training that emphasized placing infants to sleep on their backs. Following the training, the providers reported an increased understanding (from 59.7 percent to 87.8 percent) of the need to do this. And the percentage of providers who actually did place infants on their backs increased from 51 percent to 62.1 percent. The study underscores the need for continued education of parents, expanded training of parents and caregivers and even statewide mandates regarding SIDS prevention in childcare settings, the AAP reports. The American SIDS Institute recommends the following additional tips to help reduce SIDS risk: • Keep the baby’s crib in the parents’ room until the infant is at least 6 months old (studies show that infants are safest when their beds are close to their mothers). • Have your sleeping baby dressed in just enough clothes to keep her warm, without having to use a blanket. And keep the room temperature at a comfortable setting. Overheating may increase the risk of SIDS. • Don’t expose your infant to tobacco smoke. The greater the exposure, studies have found, the greater the risk of SIDS. • Breastfeed your baby, if possible, since studies have shown that breastfed babies have a lower incidence of SIDS than formula-fed babies. • Don’t expose your baby to people with respiratory infections; SIDS has often been associated with minor colds or gastrointestinal infections. • Talk to your pediatrician immediately if your baby experiences periods of not breathing, going limp or turning blue, or if your baby stops breathing or gags excessively after spitting up.

But the economic downturn means they might also feel more pressure to stay at work, if they’re lucky enough to have work. As of early February, the federal Labor Department reported that 3.6 million jobs had been lost since December 2007. Half of those losses

came during November and December of 2008 and January of 2009; experts and regular folks alike are still feeling for the bottom of the long, steep slide. This means that people with jobs are doing what it takes to keep them. So how can men balance their jobs as “providers” with their new roles as “hands-on dads”?

Kids Junction Resale Modern Dads More dads are going along on OB-GYN appointments, attending ultrasounds and singing to the baby in-utero, says Greg Bishop, founder of the nationally known mentoring and orientation program, Boot Camp for New Dads. “You have a lot more men becoming a lot more involved a lot earlier,” Bishop says. And after a bit of time off for the birth and more bonding, many men don’t feel right leaving that new baby all day. “By the time they go back to work, they know what they’re missing,” he says. Unfortunately, while dads want to be as involved as possible, society hasn’t quite caught up yet. Dads are still supposed to be the primary providers, and for the most part, they still fulfill that role. “For all the hoopla given to stay-athome dads,” Bishop says, “the fact is that we’re still a fairly traditional society.” An American Time Use Survey released in 2008 by the Labor Department backs this assertion. It found that on an average weekday in households with kids under age 6, mothers spent more than four times as much time caring for those children as did dads. On the weekend, dads did a bit better, but women still put in twice as much time. Bishop, whose organization provides education and support to fathers-to-be at more than 260 sites across the United States and internationally, says moms are the biggest factor in how involved the father of a child will be and how early he’ll get involved. Bishop advises women to encourage their partners to jump in, and then to step back and give men space to care for the baby on their own. His book, Crash Course for New Dads (Dads Adventure Inc., 2008), offers even more tips.

Working It Out Still, with long work days and ever-longer commutes, where’s a dad to find the time? Bishop recalls that during his first child’s first 6 months of life, his commute meant he didn’t make it home on weeknights until after the baby was asleep. One way for dads to carve out some time is to try for a more flexible work schedule by working

WORKING DADS at home part of the time, even just one day a week, and fitting in some baby time between emails and other tasks. Dads could also try to negotiate a later start time at their companies. Babies tend to be early risers, so taking the morning shift with the baby could give mom an extra half-hour of sleep and dad an extra 30 minutes of one-on-one time. Dads could also get to the office extra early and then make it home in time for some bonding in the evening. Not all bosses, however, will be baby-flexible. “If your employer doesn’t have any kids, find another employer,” jokes Bishop. But he points out that it’s unreasonable to expect an employer to cut a man some slack just because he has kids. New dads need to present their plans to bosses as win-win situations, making the case for how flexibility can make them more efficient and productive.

Bishop understands how tough it can be to manage a job and family, especially in today’s financial world. “It’s mind boggling the responsibilities these young men today have as they become fathers,” he says. The pressures can really mount once you add all the new-baby spending into the mix. Diapers, childcare and baby gear can get pricey. And it all comes at a time when mom – often one of the family wage earners – will be away from work, at least temporarily. Among companies who offer paid maternity leave, some don’t pay a full salary during the leave. To help keep things from getting out of hand, Bishop suggests putting together a budget and discussing your financial and work situations together. Try to approach this realistically, so that you can avoid spending more than you’re bringing in. And remember – new fathers trying to hold down jobs and still make time for a baby are blazing a fairly new trail. Both parents need to realize that no one gets it right all the time. “We tend to forget,” says Bishop, “that it was just a generation or so ago that [dads] weren’t supposed to do these things.” Christina Elston is a senior editor with Dominion Parenting Media.

Six Tips for Dads Greg Bishop offers these suggestions to help dads stay connected to their babies: 1. Take a photo. Keep your baby’s picture at the office where you can see it often. 2. Keep in touch. Call or text home at lunchtime for a baby update. Most camera phones will even let mom email a photo on the spot to keep you in touch. 3. Change diapers! “Changing diapers is just part of the territory,” Bishop says. “When you go trout fishing, you clean the trout.” 4. Go solo. Be prepared for alone time with the baby, which Bishop says is essential – especially for dads with little time to spare. 5. Be creative. Find your own way of doing things. Don’t worry too much about something going wrong while mom is away, and realize that her way isn’t necessarily the only way. 6. Play. Even when mom is around, make time to play with your baby every day. This has benefits for both baby and mom. – Christina Elston

Emergency Prep from a Pro Is your family ready for a crisis? What do your kids need to know and to whom should they turn in an emergency situation when they can’t reach you? Whether it’s a natural or man-made disaster, Jeffrey Upperman, M.D., director of the Disaster Preparedness Project at Children’s Hospital Los Angeles, is an expert with plenty of experience. He offers these four readiness tips for all of us: 1. Create Crisis Contacts. Contacts too close to home might be caught up in the disaster themselves, so have someone out of town in mind. Have your child chat on the phone with these folks monthly, so that they’re comfortable calling them if they cannot reach you in an emergency. Meet your neighbors and get to know other parents at your child’s school. Enlist them as alternative contacts for your family. “Those people are going to be your local contacts,” explains Upperman. “If there’s an issue, it’s nice to have two or three numbers that you could call.” Jot all these contacts down on a “business card” for your child to carry in a wallet or backpack.

2. Get Set at School. Talk to other parents and your kids’ teachers, and arrange for each of your children to have a “disaster buddy” whom they should stay close to in an emergency. Create a list of “helpers” – teachers, doctors, nurses, firefighters and police – to alert your children about whom they can approach and count on in an emergency. For young children, include photos or drawings of these kinds of “helpers.” 3. Have Supplies Ready at Home. Review your disaster supply checklist – the items you need on hand in emergency situations that could last several days. Don’t have a list? Head online to the American Red Cross Web site, www.redcross.org, or the Department of Homeland Security Web site, www.ready.gov. 4. Role Play and Review. Act out what might happen if a disaster occurred while your child was at school or at home.
