THE DONOWAY CENTER NEW HOPE FOR ADVANCED PANCREATIC AND LIVER CANCER PATIENTS Patient’s once diagnosed with locally advanced Pancreatic and Liver Cancers were given little hope and often offered limited treatment options all with certain short life expectancies despite the treatment they were receiving. However, a new procedure called Irreversible Electroporation, also known as IRE or NANO-KNIFE for short, has changed all that. Patients are seeing a doubling and tripling of their life expectancies based on recently published reports. Though currently used in patients with known locally advanced disease it has now found its place in the operating room and used when patients are “unexpectedly” found to have more advanced disease than pre-operative imaging shows.

Dr. Robert Donoway, a surgical oncologist trained at SloanKettering, is the only surgeon performing these procedures. During the procedure, in the operating room, after it has been determined that the patient’s tumor is non-resectable, fine needle probes are placed through the tumor using ultrasound guidance to ensure there is no injury to critical structures such as the Bile Ducts and Portal Vein. High voltage current up to 3000 volts is delivered through the probes in 90 microsecond pulses synchronized with the patient’s heart rhythm to avoid any cardiac problems. The procedure lasts between 1 & 2 hours using imaging to follow the ablative process. After the procedure is completed the probes are removed and the patient is taken to ICU step down for a 24-hour observation period. Most patients are discharged in 2 or 3 days following surgery and because there is no major surgery to recover from they are able to start postoperative neo-adjuvant type chemotherapy and radiation sooner. After 3 to 6 months of combination treatment patients are re-evaluated and restaged. A number have been converted to Resection candidates, which is still deemed the standard of care. Preliminary results have been very encouraging and have more than doubled many patients' life expectancy. Clinical trials are ongoing to use new novel agents including monoclonal antibodies and vaccines as the next line of treatment in this highly selected & heavily pre-treated group. Yoga in the Park – The City of Aventura will be offering free yoga classes from 9:00am-10:00am at Founders Park on Fridays (February 2,9, 16, 23). Yoga mats will be provided if necessary, but students may bring their own mat, a small towel and water for convenience. Registration is not required, and classes are open to all. Residents are encouraged to obtain a park ID prior to attending class in order to expedite access to the park. Non-residents will be required to pay the park entrance fee ($5 per adult and $3 per child under the age of 18, if they are accompanied by an adult) upon arrival to Founders Park. unday morning, February 25th, all families, friends, and neighbors are invited to take a bike ride with the local police department and learn
about bicycle safety at 9:00am at Founders Park, 3105 NE 190 Street, during the “Ride with the Aventura Police” bike safety and ride event. Once again, the event will be hosted by the City of Aventura and the Aventura Police Department in an effort to promote safe cycling practices. Love, Friendship and Loyalty – The Perfect Dedication for NMB BY BEVERLY PERKINS
T
he City of North Miami Beach invited everyone to the unveiling ceremony of Stephane Bolongaro’s world renowned Totor sculpture back in September. However, Hurricane Irma made an appearance and delayed the anticipated event. Fortunately, Mother Nature settled down, things started to resume back to normal, and a second date was set. Many came out to see the large figure and the new Totor Art Plaza in front of City Hall on an incredible South Florida December evening. Totor was donated to the City of North Miami Beach in July 2016 by famed French artist and former North Miami Beach Gallery owner Stéphane Bolongaro. Designed and sculpted in honor of his mother Carla Pia Roux Bolongaro’s pet dog Totor, the sculpture has attracted an incredible amount of widereaching attention. Totor the Dog symbolizes Love, Friendship and Loyalty. Totor was Monaco’s featured piece in March 2017 during Mr. Bolongaro’s art exhibition, which hosted His Serene Highness Prince Albert II. The artist launched his second exhibition of Totor at the Promenade du Paillon in Nice, France, in July 2017. This dedication in North Miami Beach is the
third installation worldwide and the first in the United States of America. Prior to the unveiling ceremony, a VIP reception was held with delicious sliders and other treats sponsored by Morton’s The Steakhouse, La Parisienne, Croissan’Time and many others. VIP attendees received a Totor magnet, 3D printed mini Totor keychain and a white chocolate sculpture of the dog courtesy of Le Chocolatier. Following the ribbon cutting ceremony and unveiling of Totor, attendees enjoyed live music by Big City Band. During the ceremony, art contest winners of “What Would Totor Do” were awarded. Contestants, students in grades K-8, had to portray Totor in a unique setting. The winners received Barnes & Noble gift cards and certificates. The grand prize winner received the same plus a miniature sculpture of the prized pup.
The design of the Art Plaza was done by South Florida engineering, planning, and surveying firm, Keith and Schnars, and donated to the City at no cost. The big reveal showed the residents that the City not only wants to have a safe and beautiful residential community, but one which implements art in public spaces.

Dr. Alan Mendelsohn Has Been a Pioneer in Cataract Surgery for 30 Years
W

hen I completed my fellowship training at Bascom Palmer Eye Institute in 1987 and began practicing ophthalmology in Hollywood July 1987, I was part of a new wave of ophthalmologists who brought the exciting technique of out-patient cataract surgery to Broward County. Our predecessors would admit patients to the hospital the night prior to surgery and keep them hospitalized for an additional night following surgery. Avoiding the hassles and expenses of hospitalization was attractive, but even more advantageous was avoiding the risks of hospital –induced infections. Out-patient cataract surgery became an almost instant success.
Outpatient Cateract Surgery For the next decade, those of us on the “cutting edge” of cataract surgery in Hollywood were doing cataract surgery by a procedure called Extracapsular Cataract Extraction whereby we removed the nucleus of the cataract in its entirety. This necessitated that we make a large incision in the eye. The cataract was removed, followed by insertion of an intraocular lens (IOL). At the completion of surgery, five to seven sutures were required to close and secure the surgical incision. The eye was pressure patched. For the next three weeks, we encouraged our post cataract surgery patients with the following refrain: “Do not bend, do not lift, do not exercise, do not exert.” Finally, after three weeks our patients could become “free agents” again and resumed their normal lives. We began suture removal in the office, and by the sixth post-operative week
the patient received a final new eyeglass prescription. The IOLs utilized in that era provided fairly good distance vision, but eyeglasses were needed to see near and intermediate distances, with fine tuning of distance vision.
“No Stich” Cateract Surgery While moving cataract surgery to an out-patient basis was considered revolutionary at the time, an even bigger milestone transpired with the transformation of cataract surgery in the mid to late 1990’s to a highly improved technology called Phacoemulcification (phaco). Instead of removing the cataract nucleus in its entirety, with a high-tech ultrasound we could break up the nucleus into tiny fragments and suck them out. This paved the way to tiny incisions in the eye and the advent of “no stitch” cataract surgery. Phaco brought quicker, safer, and better visual outcomes to the cataract surgery arena. With great pleasure, on the very first post-operative day, I was now able to encouragingly tell my patients, “You can now bend, lift, exercise and resume 100% of your activities!” Even more importantly, post-operative edema, infections, and inflammations plummeted with the new technology. Cataract surgeries were now routinely scheduled two weeks apart instead of two months apart. Concurrently, there have been remarkable enhancements in the instruments that we utilize pre-operatively in the office to ascertain the optimal custom fit intraocular lens for any given patient. The quality of these high tech ophthalmic devices and ultrasounds utilized pre-operatively for Iens determinations, as well as the expertise
of the staff actually making the measurements, are of paramount importance for excellent visual outcomes.
30th Anniversary From the onset of my career in 1987, through to the present time, we have been blessed with newer generations of intraocular lenses (IOLs) which provide superior vision. Another career milestone occurred within the past ten years with the arrival of the multifocal ReStor IOL. With the ReStor IOL, we can generally correct most patients’ cataracts and enable them to see near, far, and everywhere in between without the aid of glasses or contact lenses. The ability to read, drive, work on the computer, and see without glasses is an incredible freedom that was never possible with traditional monofocal lenses. My journey in the world of cataract surgery in Hollywood has now hit three decades as I had my 30-year anniversary of practicing ophthalmology on July 1st! Perhaps the most incredible landmark and noteworthy step on this journey has been the landmark transformation in cataract surgery to the new era of laser-assisted cataract surgery that has commenced in earnest in the past five years. Femtosecond laser cataract surgery is highly reproducible, far more accurate, and far safer than older techniques of cataract surgery with a scalpel. The laser offers excellent precision thereby producing the absolute best visual outcomes in current cataract surgery. Furthermore, patients with low or moderate levels of astigmatism can have this astigmatism eliminated or greatly reduced at the same time as the cataract procedure, resulting in a markedly enhanced quality of vision and overall outcome. In essence, this is customized laser cataract surgery utilizing high-tech 3D imaging and tailoring the ultra-precise treatment to the unique size and shape of your eye.
My Own Cataract Surgery On a personal note, even though I am a cataract surgeon, naturally I am not exempt from developing cataracts myself. In fact, in 2014, at age 56, I began to notice the litany of complaints that I have heard from my patients. First, I noticed that while driving at night, I was not as comfortable or confident as I was driving during the day. Second, oncoming headlights

became bothersome, and eventually so did the rising or setting sunlight in my face while driving. Third, I had always been an avid reader, able to read small print journals or novels for hours at a time prior to the inception of my cataracts. But in 2013 and 2014, I noted that reading tiny print became more of a chore and challenge, curtailing my reading duration. Finally, and most importantly by far, I needed to be positive that my vision was excellent to examine my patients and perform surgical procedures. I decided that it was time to proceed. In July 2014, I was the personal beneficiary of the wonderful transformations of modern ophthalmology that I described. I had advanced laser cataract surgery with the insertion of an ReStor lens in my right eye, with the same procedures to my left eye the following week. My vision is now phenomenal! I wore eyeglasses starting at six years of age and then began with contact lenses around age 13. Until age 56, I needed glasses or contact lenses for everything! Now, for the past three years, I wear no glasses, no contact lenses, nothing. Of course, I practice what I preach and always wear sunglasses with UV400 blocker and polarization outdoors during daylight hours for ocular protection. My quality of vision and depth perception are the absolute best of my entire adult life. I am delighted! Now at age 59 and feeling like the bionic man, I look forward to my next decade serving the community as an ophthalmologist on this thoroughly enlightening journey!

The Victory Center Nears Completion BY BEVERLY PERKINS

he Victory Center, which educates individuals with autism and related developmental disabilities, is nearing completion of its brand new 8,500 square foot school. The construction of the new facility began in 2016 and will allow the center to expand its capacity and serve many more children and their families in South Florida. The Center offers evidencebased intervention using the principles of Applied Behavioral Analysis (ABA) in a unique community setting. Every student has an individualized plan with custom goals and interventions to meet their educational, social and behavior needs. The school proudly hosted a hard-hat tour for invited guests to see the progress of the state-of-theart campus, which will include space for students to obtain behavior therapy and to acquire everyday living skills to be functional members of the community. Each classroom will be equipped with its own bathroom and kitchenette. The school will also include a life skills room with showers, a washer and dryer, murphy bed, a microwave, stove top and dishwasher. This space is geared to help kids learn how to develop basic cooking skills, how to do their laundry, 22 FEBRUARY 2018 | OURCITYAVENTURA.COM
as well as teach the importance of basic hygiene. Other elements of the school will also include a multimedia center with flat screen TVs, game consoles, a play center, individual therapy rooms, in addition to sharing a theater for graduation ceremonies, music room, dance studio and art studio with the MAR-JCC. “Our new building will have more opportunities to expand and improve programs for children with developmental disabilities,” said Victory Center CEO Raquel Prancer. “We are excited for the final touches and we can’t wait for the students to begin enjoying their new space.” The nonprofit school was founded in 2000 by Barry and Judi Nelson, the parents of a child with Autism who found themselves frustrated by the lack of appropriate educational opportunities. United We Stand – A Salute to Military Heroes BY BEVERLY PERKINS
T
he Aventura Marketing Council /Chamber of Commerce (AMC) expressed their gratitude to those in the military with their annual event at Turnberry Isle Miami. Hundreds of community and business leaders came out to show their support and give thanks at this year’s “Salute to the Military” breakfast hosted by Memorial Healthcare System. The occasion featured a tribute to all members of the military and a special address by Colonel Eric LeFevre, Strategic Plans Officer, Florida National Guard. Service members, veterans, and their families were welcomed by AMC Chair Gary Pyott, Memorial Regional South CEO Doug Zaren, and Memorial Healthcare System President and CEO Aurelio Fernandez. The moving program began with recognition of the Dr. Michael M. Krop Sr. High NJ-ROTC which was followed by ROTC cadets of North Miami Beach Sr. High and Charger Battalion presenting the colors. Wesley W. Wray, AMC’s talented star from the Aventura Young Stars Showcase, passionately sang “God Bless America”. Florida Army National Guard and Employer Support of the Guard and Reserve (ESGR) leaders, including Peter Caspari, Rebecca Heinl, Col. Cindy Harkrider and Sgt. Major James Root, Janette Chandler, Family Readiness Support, and 841st Engineer Battalion received special appreciation. Also, on site was Kristy Nevils, Federal Business Coach for veterans, to share information about programs that give active military and veterans the opportunity to do business with the federal government. AMC members proceeded to salute the illustrious 32-year military career of Colonel LeFevre, a leader in air defense artillery and regiment commander,
training soldiers on military, occupational and leadership skills. The decorated colonel synchronized the Guard’s response to the terrorism events of 9-11, has been a counter-drug coordinator, and participated in almost every major disaster response in Florida since 1992. “It never ceases to amaze me the things I see our soldiers accomplish with their energy, ingenuity and drive to succeed. They have hundreds of professional skills that directly translate to the private sector. They also have the skills of leadership and teamwork, are mission-focused, reliable and loyal,” stated Colonel LeFevre. “The American workforce is certainly strengthened by the hard work of our veterans,” In a closing salute to the magnitude of military service, LeFevre shared the words of General Douglas MacArthur, “However horrible the incidents of war may be, the soldier who is called upon to offer and give his life for his country is the noblest development of mankind.”

Such preparation has a positive influence in every way. Sure, it’s important to have all the notebooks and markers from the back-to-school shopping list stowed inside backpacks for the start of the term, but inspiring a positive attitude about learning can define how your children see their time at school and their promise within. The task of inspiring enthusiasm about learning in your child - from toddler to teen - can prove to be very difficult. It calls for active participation from both parent and child, as well as applying strategies that will further help a child embrace talent, interests and the many opportunities to be discovered at school. Several methods can encourage and motivate an interested mind: ▸▸ Starting conversations and debates at home that make kids think – These discussions can focus on almost anything - current events, trivia, media - from emotional to fact-based topics. Such dialogue allows the student to develop skills while instilling confidence and interest in participating in these kinds of discussions at school. ▸▸ Small and simple activities that are enchanting for your child’s mind – Supporting daily reading, playing video or board games, or sparking imagination through creative activities all nurture a positive view of learning and education.
While these methods seem small, the results can be tremendous and influence the choices children make at school and for the rest of their lives as they grow. It is important to remember that students spend more of their day at school than they do at home. Choosing a school that inspires your child to discover passions and maximize potential is the most important part of the preparation process. n 1982, 12-year-old Andre Agassi and his doubles partner, Roddy Parks, won the National Indoor Boys 14’s Doubles Championship in Chicago, IL. Less than a year later, he was shipped off to Nick Bolletieri’s Tennis Academy in Bradenton. He would go on become one of the greatest tennis players in history. Agassi famously trained exclusively for tennis from a very young age and reached greatness. Most of our children are not prodigies like Agassi. Many however, face pressure to specialize in a sport at increasingly younger ages. The emergence of club teams and travel sports combined with a prioritization of excellence over participation and the year-round warm weather of South Florida makes this trend even more pronounced here than in other areas of the country.
Different Strokes for Different Sports…
Does Specialization Make Someone a Better Performer? Author Malcom Gladwell famously coined the “10,000-hour rule.” This “rule” is an adaptation of an old theory about chess masters. It suggests that becoming a master requires around 10,000 hours of dedicated practice and training. Gladwell applied this theory to life more broadly: to become a true expert takes a lot of hard work! Proponents of sports specialization have adapted this theory: if a child shows an aptitude for a sport, having them focus on this sport early sets them on a path to attaining those 10,000 hours, becoming great and potentially gaining a college scholarship.
Sports Specialization: Should Kids Focus on What They are Best At? A 2013 study by the National Institute of Health suggests, however, that timing may be what matters most. The study’s conclusion states “Some degree of sports specialization is necessary to develop elite-level
skill development. However, for most sports, such intense training in a single sport to the exclusion of others should be delayed until late adolescence.”
Health Risks There are essentially two types of sports-related injuries: traumatic injuries and overuse injuries. Overuse injuries are typically caused when a person does the same physical motion over and over, causing wear and tear. Playing a single year-round sport significantly increases the risk of these types of injuries. Dr. James R. Andrews, founder of the Andrews Institute for Orthopedics & Sports Medicine has become an outspoken critic of specialization. “I have seen my patient population and surgical cases get increasingly younger. Children, parents and coaches need to realize that kids need to take a break from

The benefits and timing gained from specializing vary significantly by sport. Sports where athletes tend to be elite at a young age favor earlier specialization. Parents of star gymnasts or tennis players often contemplate home-school their children to maximize training time. According to a 2016 NCAA survey, almost nine out of ten college gymnasts said they had specialized by the age of 12. Other sports had far less pronounced benefits and trends toward specialization. Football players, for instance, had the lowest levels of high school specialization. In fact, every touchdown scored in this year’s NCAA National Championship Game was scored by an athlete who played multiple sports throughout high school. Interestingly, none of them play any other sport while in college. In fact, multi-sport collegiate athletes are extremely rare.
Should You Make the #1 Sport the Only Sport? Specializing in a single sport at a young age is a bad idea. A study that tracked young athletes between 1990 and 2011 concluded, “For most sports, there is no evidence that intense training and specialization before puberty are necessary to achieve elite status. Risks of early sports specialization include higher rates of injury, increased psychological stress, and quitting sports at a young age.” For High School kids, it’s a bit of a different question. Sports participation like theater or art or music are as much about a teen’s self-identification and pursuit of happiness as they are about pursuing excellence. Being good at something is important to selfesteem and makes that activity more fun. So, if your 14-year-old daughter wants to drop Basketball to focus on Volley Ball, tell her to go for it!

any individuals suffer from chronic and acute conditions such as headaches, neck/ back pain, fibromyalgia, facial pain, and sciatica to just name a few. Unfortunately, for quite some time there has been a lack of understanding in regard to chronic pain which has led to poor treatments and even worse outcomes. Neck pain is an extremely common area in which many experience discomfort. There are three general types of neck pain; ▸▸ Acute – Pain which lasts less than 4 weeks. ▸▸ Subacute – Pain which lasts 4-12 weeks. ▸▸ Chronic – Pain which last 3 or more months. Most who suffer from acute neck pain don’t even know what caused the issue, and oftentimes don’t even seek treatment. However, when the neck pain becomes subacute or chronic, then it’s likely that some form of medical guidance and/ or treatment is required. Depending on the cause and conditions, there may be specific ways for effective treatments. These recommended tips can help prevent neck pain, and may also help alleviate any symptoms occurring:

Stay Hydrated The discs between the vertebrae in the cervical spine (neck) are comprised of approximately 80 percent water at birth. However, as one ages the ratio of water decreases, which can cause the discs to lose height, the spine to come out of alignment, and the pressure on the spine to increase. Staying hydrated throughout the day can help prevent further degeneration of the cervical discs, therefore eliminating neck pain. ▸▸ Set a daily water goal. Women typically need 2.2 liters, while men need 3 liters. ▸▸ Keep a reusable water bottle nearby at all times. ▸▸ Create new daily habits such as drinking a full glass of water when waking up each day.

Limit Hand-Held Phone Use A common habit many don’t even think about, but do all day long is cradle a phone in their neck while talking. This results in extra strain on the cervical spine. alentine’s Day is one of the largest days of the year where most people decide to give flowers, chocolates or something a little more unusual as the perfect symbol of affection towards each other. Thus, we are suggesting that you must be wary during this holiday, as giddy burglars have their hearts racing at the thought of many unattended residences because, so many people have plans for romantic dates and getaways. Additionally, most people become so focused on the act of giving or receiving extra affection that they become less alert to the tricks of online retailers and online cybercriminals looking to prey on the vulnerabilities and inattention of others. Indeed, it is worth noting that online cybercriminals try to take advantage of the increased activity of Valentine’s Day preparations to cause as much damage as possible through various tricks of the trade. Invitations, for example, to download apps “designed” for Valentine’s Day can be a trap, particularly if you accept them from free marketplaces. Spam and e-greeting cards also continue to be popular in the lead for fraudulent activities up to Valentine’s Day simply because they are effective at tricking people into clicking on and opening malicious links. Criminals often add to the end of legitimate Universal Resource Locator (URL) addresses taking shoppers to corrupt sites. Also, there is Quick Response (QR) Code displaying Valentine’s Day advertisements. The danger lies not in the codes themselves, but in what can lie at the other end of the advertisements. For example, when a person uses their smartphone to scan a QR Code, they are taken directly to the site without its URL being displayed or the person being asked first if they want to go the site. With so many teens and adults now browsing and shopping using small screened smartphones and tablets, there are risks to online activity because they cannot always see the full website address. Here are some safety tips that can be used when ordering online: ▸▸ Whether using a home computer or mobile device and before entering any credit card information to order anything online, ensure that you either recognize the URL or that it begins with HTTPS to confirm that the site is secure. ▸▸ When you have finished buying your significant other a sweet treat, consider cleaning the cookies on your computer so you do not leave a footprint for the crooks. ▸▸ While there are daily reports of “harmless” social media pranks, Valentine’s Day provides additional avenues for bullies. Always log out and make sure you are using a good strong password so someone else cannot access your account pages. ▸▸ Do not break someone’s heart or bank account by unintentionally sharing a malicious link with friends as they can so easily go viral on Facebook and Twitter. ▸▸ In addition to taking extra precautions when ordering online, the following safety tips can help ensure that your Valentine’s Day is not spoiled: ▸▸ Do not announce your plans on social media. ▸▸ Wait to post pictures Whatever your plans, we hope that you have a safe Valentine’s Day!
