2021 OPEN ENROLLMENT OCTOBER 26 — NOVEMBER 13, 2020

Open Enrollment is your one time a year opportunity to add, change, or remove benefit enrollment elections that will take effect on January 1, 2021. Outside the open enrollment period, employees must make changes to their benefits elections within 31 days of a qualifying status/life event change.

Core and HRA Plan enrollees WILL be automatically re-enrolled in your current Health Plan. Premium Plan enrollees will be automatically enrolled in the Core Health Plan if you do NOT make any health insurance changes. Aetna Vision coverage will continue as well if you do not make any changes. If ADDING dependents, copies of applicable documentations are required to validate the enrollment. Required documentations must be turned in by December 31, 2020, to enroll new dependents in your plan. You must ACT to participate in a 2021 Flexible Spending (FSA) health care and/or dependent care reimbursement account. All medical plan changes and FSA enrollments/ re-enrollments must be submitted through Employee Self-Service (ESS) by Friday, November 13, 2020.

2021 PLAN CHANGES & HIGHLIGHTS • Premium Plan option is removed, and enrollees will default to the Core plan unless HRA plan is elected through ESS. • Employees will pay a slight premium for individual coverage. • HRA Plan subsidy remains $1,260. • Teladoc with $0 copay includes behavioral health and dermatology. • Dental Calendar Year Max and Lifetime Orthodontia Max increase from $1,500 to $2,000.

PREMIUM PLAN DISCONTINUED In an effort to streamline costs and simplify our benefit offerings, we are eliminating the Premium plan choice from our plan options for 2021. We will continue to offer the $1,000 deductible Core plan and the $3,000 deductible HRA plan with the Health Reimbursement Arrangement. There are no changes to the deductibles, co-pays or benefit levels of these plans. We encourage current enrollees of the Premium Plan to consider contributing additional pre-tax dollars to a Flexible Spending Account. This can help offset the increase in the deductible amount you will experience as a result of moving to the Core or HRA Plan in 2021.
