One Million Dollar Cumulative Gift Patti & Warner Blow Advancing Research Since 2001, Patti and Warner Blow of Naples, Florida, have been generously giving back to their adopted Columbus community by supporting vision research at the William Havener Eye Institute at The Ohio State University. They have established the Patti Blow Research Fund to advance research in angiogenic diseases of the eye. the same spirit of kind, compassionate help to others.” The annual distribution of this fund shall be used to support the education and training of residents in the Department of Ophthalmology.

“Resident education is one of the strengths at OSU and continues to improve thanks to the generosity of donors like the Crocis,” stated Chief Resident Matthew Ohr, M.D. After Dr. Croci completed his residency internship and two years in the Air Force as a Flight Surgeon, the Crocis settled in Athens in 1971, where he practiced for more than 31 years. Dr. Croci and Suzanne relocated to Savannah, Georgia five years ago after his retirement. They enjoy the nature in Coastal Georgia as well as warmer winters. Dr. Croci continues to volunteer in ophthalmology, helping with the Lions Lighthouse Foundation at St. Mary Community Center. As a licensed volunteer in medicine in Georgia he examines and treats uninsured and underserved patients two full days a month.

Resident Education Endowment According to Dr. Henry Croci, “the educational experience as an ophthalmology resident shaped our lives in such a positive way, we wanted to be able to help future residents experience the same thing.” That motivated Dr. Croci and wife Suzanne to create the The Henry G. He graduated from The Ohio State University School of Medical and has remained loyal to his alma mater. He has been very supportive of the Ophthalmology Department for many years.

Magnuson Education Fund Elizabeth Tighe Magnuson

Elizabeth Magnuson of Upper Arlington passed away on January 11, 2007. She married Dr. Robert Hubbell Magnuson on August 2, 1943. He preceded her in death on April 17, 2001. They had four children, ten grandchildren, and four great grandchildren.

Dr. Magnuson began the Robert Hubbell Magnuson Ophthalmology Education Fund for the Havener Eye Institute. The Fund supports projects and activities in the Department that lead to significant improvement and excellence in its education and research programs.

The fund enabled the establishment of the Magnuson Conference Room which is used for resident and medical student education, research conferences, departmental activities.

Richard H. Keates, M.D.

In December 2007, Dr. Thomas Mauger and Chief Resident, Dr. Matt Ohr, traveled to Akin-Akrosa, a rural area one hour north of Accra, capitol of Ghana.

Dr. Keates, Emeritus Professor of Ophthalmology, returns to the OSU Havener Eye Institute specializing in cornea and laser refractive surgery (LASIK).

Dr. Keates is a graduate of Jefferson Medical College, Philadelphia. He completed his internship at Albert Einstein Medical Center, Philadelphia and his ophthalmology residency at the Manhattan Eye, Ear & Throat Hospital, New York. He was awarded a Heed Fellowship in Cornea. Originally, he was a Professor of Ophthalmology and Director of the Corneal Service and Eyebank at OSU. He left OSU in 1990 to become Chairman of the Department of Ophthalmology at the University of California, Irvine. Most recently, Dr. Keates was a Professor at New York Medical College at the New York Eye & Ear Infirmary.

Twenty years ago, there were only fourteen ophthalmologists in the entire country of Ghana providing eye care to a large population.

Since then, the number of ophthalmologists has increased from fourteen to fifty, but that is still one ophthalmologist for every one million people. Almost half of the country’s ophthalmologists practice in the capitol city thus leaving the ophthalmic nurses to do the bulk of the work in rural areas.

Dr. Mauger performed over 50 extracapsular cataract surgeries during his week-long visit using supplies donated from Alcon, Allergen and Pharma. He was assisted

by Nurse Marian from Holland and Nurse Diane from Ghana. Marian volunteers six months of her time each year in Ghana and screens patients for surgery. Doctors Mauger and Ohr were excited to have the opportunity to help serve those in need in under-privledged parts of our world.

Susie Chang, M.D. Dr. Chang joins the Havener Eye Institute as an Assistant Professor of Ophthalmology with the Retina Division in August 2007. Dr. Chang graduated with a Sc.B. from Brown University in Providence, RI and received her M.D. from The Ohio State University in Columbus, Ohio. Following her predoctoral research fellowship at the National Human Genome Research Institute, National Institutes of Health in Bethesda, Maryland, Dr. Chang completed her internship in internal medicine at Riverside Methodist Hospital in Columbus, Ohio. She continued her education with a residency in Ophthalmology at the Cole Eye Institute in Cleveland, Ohio and a fellowship in Vitreoretinal Surgery at Massachusetts Eye and Ear Infirmary in Boston, Massachusetts.

“It kind of gets you back to why you went to medical school in the first place,” said Dr. Mauger, in describing why he schedules time for annual trips to underdeveloped countries to help people who would not otherwise receive care for eye diseases and conditions.

In 2008, Dr. Mauger is preparing for his third mission trip to Akim-Akrosa, Ghana and his second mission trip to Managua, Nicaragua.

Promotion & Tenure

Congratulations to Dr. Roberts & Dr. Foster Cynthia Roberts, Ph.D., the Martha G. and Milton Staub Chair for Research in Ophthalmology, was recently promoted to Professor of Ophthalmology, Biomedical Engineering and Surgery. She currently serves on the Editorial Board of two widely read journals in the ophthalmic field, and was Guest Editor of a Special Issue of the Journal of Cataract and Refractive Surgery, which focused on Corneal Biomechanics. Dr. Roberts also serves as a panel member of the Ophthalmic Devices Panel of the FDA. She specializes in research in corneal topography and corneal biomechanics.

Jill Foster, M.D. was promoted to Clinical Associate Professor of Ophthalmology. Dr. Foster is the Medical Director of Plastic Surgery Ohio, a division of Ophthalmic Surgeons and Consultants, Inc. In addition to her recent promotion, Dr. Foster is also head of Aesthetic Services at The Eye Center in Columbus, Ohio. Dr. Foster is a highly sought after speaker, lecturing internationally. She currently serves as a consultant and national trainer (teaching other physicians injection techniques) for botulinum toxin and fillers. Her particular interests in the field of Oculofacial Surgery include Cosmetic Surgery and Pediatric Oculoplastic Surgery.

Proceeds from the 2007 Golf Classic were used to update an exam room in the resident clinic. The old, worn-out patient chair was replaced. A beautiful, brand-new Haag Streit slit lamp was mounted on a new wheelchair accessible stand. A Stimuli acuity system was installed. This measures the patient’s vision using randomly generated letters instead of the old projected chart system. This eliminates the possiblity that the patient could memorize the chart, by providing a virtually limitless number of letter combinations. A huge thank you to all of our 2007 BuckEYE Golf Classic sponsors and participants.

Robyn D. Howson Housestaff Humanism Award Andrea Knellinger, M.D.

Congratulations to Andrea Knellinger, M.D., a second year resident, recipient of the Robyn D. Howson Housestaff Humanism Award, Andrea Knellinger, M.D. The award is given by The Ohio State University Medical Center to recognize a member of the housestaff who is widely regarded by faculty members, housestaff, nurses, medical students, and other healthcare professionals as routinely exhibiting attributes of ethics and integrity, caring and compassion for patients and patients’ family members, effective communication with patients, physicians, and other healthcare professional, and excellent customer service.

The award was recently renamed in memory of Robyn D. Howson, M.D., an internal medicine/ pediatrics resident, who lost her battle with ovarian cancer. This is especially poinant to those who remember her when she worked in the Department of Ophthalmology as a student.

Resident Roundtable Acute Retinal Necrosis

A 23-year-old African American male was referred to the Havener Eye Institute at Ohio State University for sudden onset of blurred vision OS for 2 days duration. He denied any medical history. He reported a 2 day history of mild discomfort and photophobia, but noted that he woke in the morning with blurred vision OS. His visual acuity on presentation was 20/20 OD and 20/60 OS. His IOP was 10 OD and 45 OS. Slit lamp examination was significant for 2+ injection OS with 2+ microcystic edema. He was also noted to have 2+ mutton-fat keratic precipitates (KP) and 3+ cell and flare in the anterior chamber. His vitreous demonstrated 1-2+ cell. The macula appeared unremarkable, but the peripheral exam revealed the lesion pictured (Figure 1). Lab work drawn initially included ACE, RPR, HIV, HSV/ VZV and toxoplasmosis IgG and IgM. Based on the clinical presentation, the patient was diagnosed with acute retinal necrosis (ARN). He was admitted to the hospital and started on intravenous acyclovir, Vigamox OS QID, Pred Forte OS QID, Cosopt OS BID, and Aspirin 325 mg PO daily. He received a single intravitreal injection of 0.1 cc of ganciclovir (4 mg/0.1 cc).

Acute Retinal Necrosis is an acute peripheral necrotizing retinitis that typically affects otherwise healthy individuals of all ages. It is reported to be caused by herpes simplex virus (HSV) or varicella-zoster virus (VZV). Typically the presentation includes severe uveitis, retinal vasculitis, and retinal necrosis. KP and vitritis may be present early with a retinitis in the periphery that becomes confluent rapidly over a week. This may be associated with an occlusive vasculitis and papillitis. Retinal detachment may occur. The differential diagnosis includes CMV retinitis, Behcet’s disease, endophthalmitis, toxoplasmosis, syphilis, sarcoid, progressive outer retinal necrosis (PORN), and lymphoma. The diagnosis is made primarily on the clinical features. However, PCR techniques can be performed on aqueous or vitreous aspirates, although high false negatives have been reported. If untreated, the retinitis will develop in the contralateral eye in over 30% of patients.

Treatment consists of intravenous acyclovir 1500 mg/m2 ever y 8 hours for 7 days. Intravitreal ganciclovir has also been advocated. Following the I.V. therapy, the patient is maintained on oral acyclovir. Topical steroids may be started, however, systemic steroids are typically not considered until the patient has received 24 hours of antiviral therapy. Antiplatelet therapy, such as Aspirin, may also limit complications from vascular occlusion. Visual recovery depends on the extent of retinal damage. Vascular occlusion, optic neuropathy, and retinal detachment may limit overall visual outcomes.

Retina Studies RIDE: A Phase II Study of the Efficacy and Safety of Ranibizumab Injection in Subjects with Clinically Significant Macular Edema with Center Involvement Secondary to Diabetes Mellitus Alan Letson, M.D. (PI), Frederick Davidorf, M.D., John Christoforidis M.D., Susie Chang, M.D.

Cornea Studies Rates and risk factors for fungal keratitis among contact lens wearers Thomas Mauger, M.D. (PI), Richard Lembach, M.D., and Reynell Harder-Smith, D.O.

This study evaluates the effectiveness of Ranibizumab in subjects with diabetic macular edema. Subjects are randomized into 3 groups: Ranibizumab in two different doses or sham. Subjects will receive drug or sham injections monthly for 36 months. Contact Jill Salerno with questions about this study at 614-652-2620.

Recent reports suggest that there has been an increase in fungal keratitis amongst contact lens wearer. The CDC is conducting an ongoing investigation into whether this increase is related to contact lens solutions which have been recently pulled from the market. This study will determine if there is a link between contact lens wear and eye infections. It will also identify behavioral and environmental risk factors with a retrospective review of past cases of fungal keratitis.

DRCR: Diabetic Retinopathy Clinical Research Network Intravitreal Ranibizumab or Triamcinolone Acetonide as Adjunctive Treatment to Panretinal Photocoagulation for Proliferative Diabetic Retinopathy – Protocol J John Christoforidis, M.D. (PI), Frederick Davidorf M.D., Susie Chang, M.D.

DYME: A double-masked, randomized study of the safety and effectiveness of DYME as an agent for selective staining of the anterior capsule during cataract surgery Thomas Mauger, M.D. (PI), Richard Lembach, M.D., Dave Castellano, M.D., and Reynell Harder-Smith, D.O.

This 56 week study evaluates the effectiveness of Ranibizumab vs.

Triamcinolone vs. sham as an adjunctive therapy with panretinal photocoagulation for patients who have proliferative or severe non-proliferative diabetic retinopathy who may develop diabetic macular edema. Patients will be randomized into three groups, and all will receive PRP laser. Subjects randomized into the Ranibizumab group will receive 2 injections, one at the beginning of the study and one at 4 weeks. Subjects in the Triamcinolone Acetonide group will receive an injection at the beginning of the study, and a sham injection at 4 weeks, and subjects in the sham group will receive 2 sham injections- one at the beginning of the study and one at 4 weeks.

Studies have shown that using a dye during cataract surgery makes the surgery easier and the outcome better especially if the patient has a certain kind of cataract. This study analyzes whether the dye, DYME, will aid the surgeon when performing cataract surgery. Neuro-Ophthalmology Studies

CATT: Comparison of Age-related Macular Degeneration Treatments Trials Frederick Davidorf, M.D. (PI), Alan Letson, M.D., John Christoforidis, M.D., Susie Chang, M.D.

IIH: Idiopathic Intracranial Hypertension Clinical Study Steven K. Katz, M.D. (PI) Idiopathic Intracranial Hypertension (IIH), also known as Pseudotumor cerebri, is a potentially blinding disorder affecting 1 in 20,000 people. The etiology is unknown; however, it is more common among obese women of childbearing age. Even though the incidence and morbidity of IIH are increasing, the treatments remain limited. The Department has partnered with the General Clinical Research Center at The OSU Medical Center to conduct a research study to identify factors (hormones/proteins) involved in IIH. Elucidating the mechanisms causing IIH will allow the development of more specific therapies for this disabling disease. The OSU Medical Center is looking for patients with suspected IIH who have NOT undergone diagnostic lumbar puncture (LP) and have NOT begun any treatment for the disease. The OSU Medical Center will coordinate and pay for the initial overnight hospital stay before the radiologically-guided LP. Patients enrolled in the study will provide samples of blood and excess cerebrospinal fluid obtained from the LP. Eligible patients will be seen on the day of contact or within several days depending on the degree of papilledema and risk of visual loss.

This two year study compares Lucentis and Avastin as treatment for subfoveal neovascular AMD. Subjects will be randomized into 4 groups: (1) Lucentis on a fixed schedule of every 4 weeks for 1 year; at 1 year, re-randomization to Lucentis every 4 weeks or to variable dosing. (2) Avastin on a fixed schedule of every 4 weeks for 1 year; at 1 year, re-randomization to Avastin every 4 weeks or to variable dosing (3) Lucentis on a variable dosing schedule for 2 years; i.e., after initial treatment, monthly evaluation for treatment based on signs of lesion activity. (4) Avastin on a variable dosing schedule for 2 years; i.e., after initial treatment monthly evaluation for treatment based on signs of lesion activity. AREDS 2: Age-Related EYE Disease Study 2 Alan Letson, M.D. (PI), Frederick Davidorf, M.D., John Christoforidis, M.D., L. Carol Laxson, M.D., Susie Chang, M.D. This study will determine if lutein, zeaxanthin and fish oil can slow or prevent the development of advanced age-related macular degeneration. Qualifying patients must have large drusen in both eyes or large drusen in one eye and advanced AMD in the fellow eye. The Eye & Systemic Disease

51st Annual Ophthalmology Postgraduate Symposium On February 29 - March 1, 2008, the Havener Eye Institute sponsored the 51st Annual Ophthalmology Postgraduate Symposium at the Hilton Easton in Columbus, Ohio.

This year’s conference focused on systemic diseases., an update for ophthalmologists on a wide variety of topics given by physicians that are experts in their specialties. These lectures provided practical review and new knowledge to both the general and specialty ophthalmologist.

Along with a new inductee, Dr. Don Lewis; Dr. Holton Letson and Dr. Henry & Suzanne Croci were recognized. Both achieved new heights of giving and have advanced to the Havener Society’s Fellow Circle.

Jim Tressel 2008 Schedule of Events Plan to attend our 3rd Annual Havener Eye Institute BuckEYE Golf Classic featuring Jim Tressel. The event will be held at the Golf Club of Dublin. Tickets to play will be $500 per player or $1,000 for a foursome.

The scramble format outing will begin at 10:00am with an open range and registration, followed by a photograph opportunity with Jim Tressel at 11:00am, and a shotgun start at 12:30 pm.
