General Information LOCATION

The 156th Annual Session of the Ohio Dental Association will take place at the Greater Columbus Convention Center (GCCC) complex, Columbus, Ohio at 400 N. High Street, 43215. The headquarters hotel is the Hilton Columbus Downtown Hotel, 402 N. High Street, Columbus, Ohio 43215, (614) 463-1234; the Hilton is connected to the GCCC. Most ODA Annual Session events and courses will be held in the GCCC or the Hilton. ODA staff will be onsite at the GCCC from Wednesday, September 14 through Sunday, September 18.

FREE CE COURSES

The ODA Annual Session registration fee includes the ability to earn 18 free CE credits, online. All free CE courses can be accessed online from September 19 – December 31, 2022. In addition to the online courses, attendees have the opportunity to earn CE credits while attending the Table Clinics in person on Friday in the Exhibit Hall. New this year, interactive sessions titled ODATalks will be offered in the Exhibit Hall. All of the online courses offered, as well as the Table Clinics, and some of the ODATalks, qualify toward the Ohio State Dental Board’s biennium licensure requirement.

If you have special needs and require accommodation to fully participate in the conference, please contact the ODA Annual Session department at (614) 4862700 prior to Annual Session, and the ODA Registration Desk located in the Greater Columbus Convention Center (GCCC) room A110 during the conference.

TAX DEDUCTION

Deductions are permitted for educational expenses - including registration fees, transportation, meals and lodging. Check with your financial advisor for details.

MOTHERS' ROOM

The GCCC has a designated nursing mother room located on the Central Atrium. Family restrooms are located in Upper A-Pod (behind room A-210) and another in the Center Lounge restrooms near the South Café & Marketplace. These single-use areas are equipped with a seat, electricity and a lockable door.

Mothers’ Rooms can also be used for personal medical needs.

MEDITATION ROOM

In an effort to support the holistic wellness of Annual Session attendees, the ODA and its Subcouncil on Diversity and Inclusion have created a space for meditation, reflection and/or prayer. The purpose of this space is to provide an accommodation for individual spiritual needs.

The room is located at the Hilton Columbus Downtown Hotel, Mary Frances Merrill Room.

TWO-HOUR LUNCH BREAKS

On Thursday, Friday and Saturday, most CE courses will break for two hours for lunch at various intervals between 11:30 a.m. and 2:30 p.m., giving attendees the opportunity to spend time in the Exhibit Hall with no need to travel off-site for lunch – concessions and a dining area are located in the Exhibit Hall and in the main corridor outside the Hall.

SEMINAR SEATING & COURTESY

Plan to arrive early: seating for all seminars is on a first-come, firstserved basis. Also, please set all electronic devices to “silent” during seminars. Seminar room locations will be on the Annual Session Mobile App.

ATTIRE

Business casual dress is recommended for the convention. Dress warmly, in layers, as meeting rooms tend to be cool.

COVID-19

Annual Session will follow all city, county, state and federal COVID-19 protocols and CDC policies in place during the 156th ODA Annual Session.

PHOTOGRAPHY RELEASE

Annual Session attendees grant permission to the Ohio Dental Association and their agents to utilize, in perpetuity, the attendee’s image or likeness in an effort to promote the Ohio Dental Association and its events or meetings. Attendees waive any right to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.

DISCRIMINATION/ HARRASMENT POLICY

The ODA has zero tolerance for any form of discrimination or harassment, including but not limited to sexual harassment by participants or our staff at any of our meetings or events – in person or virtual. The awards include the presentation of the ODA's highest honor, the Joseph P. Crowley Distinguished Dentist Award. Dr. Usman has a philosophy of proactive treatment versus "waiting and watching" which will minimize untoward sequelae such as infection and discoloration of the tooth as well as facilitate an optimum and conservative, long-term restorative solution. Starting with a thorough understanding of diagnosis and the role of frequent recall, she will demonstrate the management of various patients who have undergone trauma or have gross caries which threaten the premature loss of the permanent tooth. A review of diagnosis and the various spectrum of pupal therapeutic techniques from apexogenesis to indirect pulp capping, modified apexification, and regenerative treatments. The clinical uses of various materials such as MTA, Endosequence bioceramics, Biodentine calcium silicate cement and collagen containing materials which have been demonstrated to have applications, will be compared. Ceramic materials do not handle the same as metal-based restorations and improper selection or handling can lead to premature failure. This course will teach you how to select and handle different types of ceramic materials that are currently used in dentistry that will be much less confusing than typical marketing rhetoric. The ability to bond these dental ceramics allows the practitioner to perform more conservative tooth preparations. However, teeth, like the human body, are 3 dimensional and limitations with 2D radiology exist.

Ultimately, the use of 3D imaging / CBCT in endodontics is about avoiding surprises, assessing prognosis, and having the information to treatment plan cases most effectively for long-term restorative success. Studies have shown that endodontists change their treatment plan approximately 60% of the time when using 3D imaging vs. 2D imaging.

Case presentations of 2D images along with CBCT scans (identifying significant splits/branches on canals that can be misinterpreted on 2D, assessing the number, shape and location of canals) to review & understand endodontic findings on 3D imaging as well as sharing treatment plan modifications based on the 3D information so as to minimize incomplete endos (CDT3332) i.e., entering and finding a crack/perforation, will be shared. 3D imaging also provides another diagnostic modality for more challenging patients (gaggers, special needs, etc.) and is a vehicle that can provide better patient communication by reviewing the scan with the asymptomatic patient.

Our ability to treat peri-implantitis is hampered by a paucity of information on host-microbiome interactions that underlie the disease.

By characterizing molecular events at the mucosal-microbial interface in the peri-implant crevice, we discovered an active peri-implant microbiome surrounding healthy implants. Microbial genes regulate human genes encoding cell development, metabolism, morphogenesis, adhesion, gap junctions, cell-cell signaling and immunoinflammatory pathways, suggesting a role for commensals in protecting epithelial integrity. In disease, we discovered that microbial dysbiosis in the peri-implant sulcus promotes abandonment of hostbacterial transactions that dictate health and instead drives a move towards chronic programming of a non-healing wound.

Mistakes can be devastating to the practice. During the investigation as well as in the preparation for and during Dr. Shelbourne’s trial it became apparent that the practice’s billing and coding systems were faulty.

Unfortunately, more than two out of ten people suffer from at least one of six types of orofacial pain, and approximately 12 million adults in the United States have pain related to temporomandibular disorders that are inadequately managed by conventional dental therapeutic approaches. An individual may have more than one form of TMJ disorders and may also have overlapping conditions, such as migraines, fibromyalgia, and other forms of widespread pain. Orofacial pain has varying causes (injury, genetics, and environmental factors), and may affect various body systems. Orofacial pain has a substantial impact on the quality of life for those affected, and effective care often involves multiple disciplines within medicine and dentistry that requires specialty knowledge. Many, if not most, general practitioners and even other dental specialists are unaware of the valuable resources that an Orofacial

Pain Specialist can provide to them and their patients. The primary goal of this course is to provide an easy-tocomprehend overview of orofacial pain conditions that commonly present in the general dental office daily. Each of seven main areas within the field of orofacial pain will be reviewed and illustrated with real-life examples. Of course, temporomandibular disorders and an evidence-based approach to effective management will be discussed.

GLASS CEILING TRACK

While the term glass ceiling is a metaphor used to represent an invisible barrier that prevents a given demographic from rising beyond a certain level in a hierarchy, attendees of the 2022 ODA Annual Session will have a difficult time believing any of these highlighted speakers are in any way limited.

The goal of this track, featured over all three days of the conference, is to showcase compelling female speakers who are not only considered to be experts in their fields, but who navigate the roles of academics, entrepreneurship, practice management, and clinical dentistry. While this track features phenomenal women, attendance is certainly not limited to them.

Featured Speakers Ashleigh Briody, DDS, MS

Dr. Ashleigh Briody earned her DDS from LSU School of Dentistry in New Orleans and completed her residency in Oral & Maxillofacial Pathology at The Ohio State University where she earned a certificate and master’s degree. She is a Fellow of the American Academy of Oral and Maxillofacial Pathology and a Diplomate of the American Board of Oral and Maxillofacial Pathology. Gina Dorfman, DDS

For almost two decades, Dr. Gina Dorfman has continually proved herself to be an innovative leader in the field of dentistry. As a dentist, practice owner, and entrepreneur, Dr. Dorfman has had the opportunity to engage with dental professionals across the country with the goal of helping them grow and thrive.

Dr. Dorfman graduated from the University of Southern California in 1996, earning a degree in Biochemistry. She completed her dental training at the Herman Ostrow School of Dentistry in 2000 and shortly after started her practice near Los Angeles, California.

While starting a practice in a saturated market came with its own challenges, the practice grew tremendously over the years, expanding to several locations. Because of her experience running multiple practices and leading a big team, Dr. Dorfman developed a unique insight into how efficient practices organize and operate and how dental teams can work together to overcome obstacles and reach their goals.

Anita Gohel, BDS, PhD

Dr. Anita Gohel is a clinical professor and program director of Oral and Maxillofacial Radiology at the University of Florida College of Dentistry, and she is a board-certified diplomate of the American Board of Oral & Maxillofacial Radiology. Dr. Gohel received her BDS from Calcutta University in India, a PhD in Oral Biology, and a certificate in Oral and Maxillofacial Radiology from the University of Connecticut. She has served as a director and president of the American Board of Oral & Maxillofacial Radiology; as a commissioner for the ADA National Commission on Recognition of Dental Specialties and Certifying Boards and a member of the CODA Review Committee on Oral and Maxillofacial Radiology Education.

Betty Haberkamp, DDS

Dr. Betty Haberkamp is a 1984 Graduate of Case Western Reserve University and completed a General Practice Residency at Cleveland Metro General/Highlandview Hospital (currently Metro Health) in 1985.

She is currently a Diplomate in the American Board of Dental Sleep Medicine and now devotes almost half her practice to sleep dentistry. Since returning to Cleveland, she served as Program Director for the General Practice Residency at the Cleveland Clinic for seven years and continues to maintain her involvement as Associate Program Director.

Cassandra Kelly

Cassandra Kelly is a private coach to influential leaders, both corporate and political. Purnima Kumar, BDS, MS, PhD

Dr. Purnima Kumar currently serves as chair of the University of Michigan School of Dentistry’s Department of Periodontics and Oral Medicine. Formerly a tenured Professor of Periodontology and Chair of the Oral Biology Graduate Program at The Ohio State University College of Dentistry, she received her dental degree from Annamalai University in India, and her master’s degree in periodontology and Ph.D in molecular microbiology from Ohio State. Her research focuses on the oral microbiome and its interactions with the host to promote health or cause disease and the role of human behaviors in shaping this ecosystem. Dr. Kumar was the Principal Investigator of the Oral Microbial Ecology Laboratory at OSU that is funded through the National Institutes of Health (NIH), the National Cancer Institute and the oral healthcare industry. She also serves as the Chair of the Continuing Education Oversight Committee for the American Association of Periodontology (AAP) and is a member of the AAP’s Taskforce for Women in Periodontics and the Taskforce for Future Science. She is a member of the Council of Scientific Affairs of the American Dental Association and is the organization’s official spokesperson on e-cigarettes and vaping.

Judy McIntyre, DMD, MS

Dr. Judy McIntyre attended the Harvard School of Dental Medicine, and subsequently obtained a coveted endodontics residency position at the University of North Carolina at Chapel Hill. Alongside renowned researchers in the field of endodontics, traumatology and pediatric dentistry, her research regarding traumatic dental injuries has been published in numerous

professional journals. Dr. McIntyre opened Hopkinton Endodontics in 2016. She enjoys serving on the AAE Board of Directors currently and has enjoyed past participation in the ADA, as well as the Massachusetts Dental Society. She is passionate about sharing her love for dentistry, treatment of trauma, and endodontics.

Amrita Patel, DDS

Dr. Amrita Patel graduated from New York University College of Dentistry and completed her residency in general dentistry at the Nassau University Medical Center, a State University of New York at Stony Brookaffiliated hospital. Dr. Mindy Streem is a Diplomate of the American Board of Orthodontics, a member of the College of Diplomates of the American Board of Orthodontics, a member of the American Association of Orthodontists and The American Dental Association.

Dr. Streem attended Harvard

School of Dental Medicine (HSDM), which consisted of two years of training at Harvard Medical School and two years at the dental school. During this time, Mindy served as Vice President of Harvard School of Dental Medicine and as President of the school’s chapter of Alpha Omega Dental Fraternity. Mindy earned her Doctor of Dental Medicine (DMD) degree from HSDM in 2005. After graduating from Harvard, Mindy earned her Master of Science degree and certificate in Orthodontics at The University of Michigan in 2008.

Najia Usman, DDS

Dr. Najia Usman graduated from SUNY Buffalo in 1995 with her DDS. After completing a one-year general practice residency at the Cleveland Clinic she furthered her interest in endodontics by completing a two-year endodontic residency at Oregon Health and Sciences University, Portland, Oregon in 2002. She was awarded her Diplomate Status by the American Board of Endodontics in 2009.

Dr. Usman has been in private practice since 2002. Though she practices full-scope endodontics, she has cultivated an interest in the endodontic management of trauma, pediatrics and resorptive lesions. Her study The Influence of Instrument Size on Debridement was published in the Journal of Endodontics (Feb 2004, vol 30, 110112) has been cited over 260 times.

The number of continuing education (CE) credits for each seminar is equal to the number of course hours for that class. However, not all course topics meet the mandatory requirement for renewal as mandated by the Ohio State Dental Board (OSDB) or other licensing agencies. It is the responsibility of each licensee to understand the specific CE rules of their respective licensure board.

The Ohio State Dental Board (OSDB) defines acceptable continuing education for all Ohio dental licensees as educational and scientific courses given by board-approved sponsors consisting of activities designed to: review existing concepts and techniques; to convey information beyond the basic dental education; and, to update knowledge on advances in scientific, non-clinical and clinical practice-related subject matter, including ethics, regulatory compliance, risk management, nutrition and evidence-based dentistry. The objective of continuing education is to improve the knowledge, skills and ability of the individual to provide the highest quality of service to the public and the profession. Ohio licensed dentists must complete 40 hours of acceptable continuing

Judy Bendit, RDH, BS

As the daughter of a dentist, Judy Bendit was born and raised in a dental office. She has over 45 years of experience in dentistry as a clinician, author and speaker. She serves on the faculty of the University of Pennsylvania School of Dental Medicine, where she is active with the alumni board. She also serves on the advisory board for the dental hygiene program at Palm Beach State College.

Judy is a member of the CAMBRA coalition, American Academy of Cariology, and American Academy of Pediatrics Section on Oral Health. She is a longstanding member of the American Dental Hygienists’ Association. Judy has been listed in Dentistry Today as a top speaker for over a decade.

Before the pandemic, she enjoyed volunteering as a clinician both at home and abroad.

Ashleigh Briody, DDS, MS

Dr. Ashleigh Briody is from Baton Rouge, Louisiana. Daniel Coleman, DMD, MS

Dr. Dan Coleman is a graduate of the Tufts University School of Dental Medicine in Boston, Massachusetts where he received his Doctorate of Dental Medicine, Post-doctoral Master of Science, specialty training and Board Certification in Periodontics. During his residency, Dr. Coleman was privileged to serve as Chief Resident. Dr. Coleman is a faculty member at the Harvard University School of Dental Medicine where he serves as a clinical instructor and lecturer in the Department of Periodontology. Recently, Harvard and the American Academy of Periodontology awarded Dr. Coleman with recognition as its Outstanding Educator of 2019. This award was given in part because Dr. Coleman serves as a speaker nationally and internationally to teach on topics of periodontal health and disease; additionally, he has co-authored a dental textbook entitled, “NonSurgical Control of Periodontal Diseases” published in 2015 which is used in several dental programs to train new dentists. Among Dr. Coleman’s achievements, he was named one of Incisal Edge Magazine’s “Top 40 under 40” dentists nationwide. Aside from his

numerous career achievements, Dr. Coleman dedicates himself to ensuring that his patients have an amazing experience when working with him through his core values of quality, compassion, and integrity.

Harold L. Crossley, DDS, MS, PhD

Dr. Hal Crossley is Professor Emeritus at the University of Maryland Dental School. A native of Rhode Island, Dr. Crossley received a Bachelor of Science degree in Pharmacy from the University of Rhode Island in 1964. He was later awarded Master of Science (1970) and Doctorate degrees (1972) in Pharmacology.

The University of Maryland Dental School in Baltimore awarded Dr. Crossley his DDS degree in 1980. The liaison between the classroom and his part-time dental practice produced a practical approach to understanding the pharmacology of drugs used in the dental office. Other areas of expertise include the pharmacology of street drugs and chemical dependency.

He serves on the Maryland State Dental Association’s Well-Being Committee, is an active member of Phi Kappa Phi, Omicron Kappa Upsilon Honorary Dental Society, the American College of Dentists, International College of Dentists, and an honorary member of the Thomas B.

Hinman Dental Society. He was the recipient of the 2008 Gordon Christensen Lecturer Recognition award presented by the Chicago Dental Society and the recipient of the 2012 Award of Distinction presented by the Academy of Dentistry International for his efforts in Continuing Dental Education.

He has been a consultant for the United States Drug Enforcement Administration and other law enforcement agencies since 1974. Drawing on this unique background, Dr. Crossley has become nationally and internationally recognized as an expert on street drugs and chemical dependency as well as the clinical pharmacology of dental drugs.

Todd C. Davis, DDS

Dr. Todd Davis earned his DDS degree from The Ohio State University College of Dentistry in 1989 after which he completed a two-year general practice residency. He maintains a comprehensive restorative practice in Atlanta, Georgia.

In addition to practicing, Dr. Davis enjoys teaching. He is on the visiting faculties of the L.D. Pankey Institute and Spear Education. He has lectured nationally and internationally on various topics including predictability in restorative care, functional esthetics, occlusion, TMD, practice management, case acceptance, and philosophy. He also facilitates small group learning in his office

and throughout the country as well as doing one-on-one consulting in offices. Dr. Davis helps dentists with clinical, behavioral, philosophical, and business principles.

As a dentist, practice owner, and entrepreneur, Dr. Dorfman has had the opportunity to engage with dental professionals across the country with the goal of helping them grow and thrive.

Dr. Dorfman graduated from the University of Southern California in 1996, earning a degree in Biochemistry. She completed her dental training at the Herman Ostrow School of Dentistry in 2000 and shortly after started her practice near Los Angeles, California.

While starting a practice in a saturated market came with its own challenges, the practice grew tremendously over the years, expanding to several locations. Because of her experience running multiple practices and leading a big team, Dr. Dorfman developed a unique insight into how efficient practices organize and operate and how dental teams can work together to overcome obstacles and reach their goals.

A recognized speaker and author, Dr. Dorfman is regularly invited to teach other dentists

across the country. She has presented at multiple local and national dental meetings, such as the Townie Meeting and Practice on Fire Live. Recently, she’s had the honor to join the faculty at the Dental Success Network, a unique community dedicated to advanced learning and collaboration.

Dr. Dorfman is also a frequent contributor to several industry blogs and is the host of Behind the Smiles Podcast which features the most disruptive and influential members of the dental community who are moving dentistry forward. Dr. Dorfman spends numerous hours in continuing education each year to further her dental expertise and is always on the cutting edge of technology. Her passion for creating systems and leveraging technology to streamline practice operations is what eventually led her to cofound YAPI, a paperless dental software company, where she is currently serving as the COO. When she is not working, Dr. Dorfman enjoys hiking, reading, and spending time with her husband Ken, their two children, Mila and Lenny, and their German Shephard named Axel.

He also invented OraViz, a cloud-computing 3D dental imaging technology using equipment already in most dental clinics. Dr. Purnima Kumar is the William and Mary K. Najjar Endowed professor of Dentistry and Chair of the Department of Periodontology and Oral Medicine at the University of Michigan. She received her dental degree from Annamalai University in India, and her Masters in Periodontology and PhD in Molecular Microbiology from The Ohio State University. She is a Diplomate of the American Board of Periodontology and a Fellow of the Executive Leadership in Academic Medicine (ELAM) program. Dr. Kumar is the Principal Investigator of Oral Microbial Ecology Laboratory at the University of Michigan that is funded through the NIH, NCI, and oral healthcare industry. Nathaniel Lawson, DMD, PhD

Dr. Nate Lawson is the Director of the Division of Biomaterials at the University of Alabama at Birmingham School of Dentistry and the program director of the Biomaterials residency program.

He graduated from UAB School of Dentistry in 2011 and obtained his PhD in Biomedical Engineering in 2012. He has served as an investigator on over 75 clinical and laboratory research grants, and published over 200 peer reviewed articles, book chapters, and research abstracts. His research interests are the mechanical, optical, and biologic properties of dental materials and clinical evaluation of new dental materials. He was the 2016 recipient of the Stanford New Investigator Award and the 2017 Innovative Research Fellowship both from the American Dental Association. He serves on the American Dental Association Council of Scientific Affairs and is on the editorial board of The Journal of Adhesive Dentistry and Compendium.

He has lectured nationally and internationally about dental materials. He also works as a general dentist in the UAB Faculty Practice.

Paul Levi Jr., DMD

Dr. Paul Levi graduated from St. Lawrence University in Canton, New York in May 1962 and entered Tufts University School of Dental Medicine in September of the same year. After graduation from Tufts in 1966 he accepted a General Practice Residency (internship) in Burlington, VT. The internship furthered his interest in medicine and specifically in periodontology.

He was allocated to the US Navy and served as an officer at Great Lakes Naval Training Center in North Chicago, Illinois for two years, where, because of his Tufts training, Dr. Levi was assigned to be the periodontist for the recruits.

While in Chicago, he met and married Patty J. Nichol in 1968, and following two-years in the Navy, they moved to the Boston, Massachusetts area where he completed postdoctoral periodontal training at Tufts under the mentorship of Dr. Irving Glickman. At the completion of his specialty training in 1971, Dr. Levi practiced periodontology in Burlington, Vermont for 49 years. He began teaching dental school at Harvard School of Dental Medicine in 1994 and at Tufts University School of Dental Medicine in 2003. He retired from private practice in September of 2019, and presently teaches, lectures, and does research at Harvard and Tufts. To Dr. Levi, dentistry and specifically periodontics

Judy McIntyre, DMD, MS

Dr. Judy McIntyre attended the Harvard School of Dental Medicine, and subsequently obtained a coveted endodontics residency position at the University of North Carolina at Chapel Hill. Dr. Jay Resnick is a 1972 graduate of The Ohio State University College of Dentistry.

Immediately after graduation, he completed a GPR at the Mount Sinai Hospital in Cleveland. After 47 years in private practice, he retired in 2020, accepting a faculty position at the School of Dental Medicine of Case Western Reserve University. He now serves as associate professor of comprehensive care and is the pre-doctoral implant training coordinator. Mindy grew up in Toledo, Ohio. She graduated from The University of Michigan Summa Cum Laude with both a degree in Voice Performance from the School of Music and a degree in Biology from the School of Literature, Sciences, and the Arts. Through her vocal training, she specialized in performing classical music and operatic arias in more than five different languages. Mindy performed the National Anthem at The University of Michigan’s commencement ceremony in

2001 on the 50-yard line at Michigan’s football stadium.

Mindy attended Harvard School of Dental Medicine (HSDM), which consisted of two years of training at Harvard Medical School and two years at the dental school.
