KENNELRESORTS PET CONTRACT OWNER HEALTH CERTIFICATION & LIABILITY WAIVER

For Boarding, Day Camp, Dog Park Use, Grooming or Training at KennelResorts
I,____________________________, hereby certify that my dog(s)and/or cat(s): ________________________________________________________________________ ____________________________(plus any additional dog(s)and/or cat(s) that I may present in the future) will be in good health and will not be ill with any communicable condition when they come to KennelResorts for boarding, day camp, grooming, training or to use the dog parks. I have read and understand the following: OWNER ACKNOWLEDGES AND UNDERSTANDS THAT THE EMPLOYEES OF KENNELRESORTS ARE NOT VETERINARIANS AND ARE NOT EXPECTED TO DIAGNOSE OR DETECT ILLNESSES IN THE GUESTS THAT ARE STAYING AT KENNELRESORTS. IN ADDITION, OWNER UNDERSTANDS AND IS AWARE THAT VACCINES DO NOT PROTECT AGAINST ALL COMMUNICABLE ILLNESSES THAT MAY AFFECT A GUEST. OWNER UNDERSTANDS AND AGREES THAT THEY ARE ASSUMING (1) ALL RISK OF ILLNESS, DISEASE, HARM OR OTHERWISE TO THEIR PET BY ALLOWING THEIR PET TO STAY AT KENNELRESORTS AND (2) ALL RISK OF DAMAGES BY THEIR PET TO OTHER PETS, TO ANY KENNELRESORTS EMPLOYEE OR AGENT OR TO ANY KENNELRESORTS ASSET AND THAT KENNELRESORTS SHALL HAVE NO LIABILITY FOR ANY HARM TO SUCH PET.

I understand that I am solely responsible for any harm caused by my dog(s) and/or cat(s) while attending KennelResorts.

I further understand and agree that in admitting my pet(s) to KennelResorts that KennelResorts has relied on my representation that my pet(s) is/are in good health free of any communicable illness for the last 30 days, and has/have not harmed or shown “serious” aggressive or threatening behavior toward any person or another pet.

I further understand that KennelResorts and staff will not be liable for any problems that develop, provided that reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my pet(s) boarding, exercising, grooming, or training at KennelResorts.

I further understand and agree that if my pet requires medical attention for any reason, as determined at the discretion of KennelResorts’ staff, I will assume full financial responsibility for that care including transportation costs. I hold KennelResorts harmless in the event of an injury or accident during such transportation.

If applicable, I further understand that KennelResorts reserves the right to terminate and revoke my dog(s)’ annual KennelResorts membership, if for any reason or at any time it is observed that my dog is demonstrating unacceptable behavior. If that occurs, KennelResorts will reimburse me for the pro-rated cost of the annual membership. I understand that when dogs play socially nicks, cuts, scratches or bites may occur. I understand that KennelResorts reserves the right to delay notifying me about minor injuries until I pick up my dog.

If applicable, I further understand that during the hours the Dog Park is available to members, KennelResorts and staff are not responsible for monitoring my dog(s) within the Dog Park, and that I, the owner, am solely responsible for their care and their actions toward other dogs and people.
7.
I received a copy of the document SUMMARY OF POTENTIAL ISSUES at KennelResorts. It is my responsibility to read and understand the content of that document.
