Panzyga (Immune Globulin Intravenous [Human] - ifas) is indicated for the treatment of PI in patients 2 years of age and older; this includes, but is not limited to, congenital agammaglobulinemia, common variable immunodeficiency, X-linked agammaglobulinemia, Wiskott-Aldrich syndrome, and severe combined immunodeficiencies; cITP in adults to raise platelet counts to control or prevent bleeding; and CIDP in adults to improve neuromuscular disability and impairment.

WARNING: THROMBOSIS, RENAL DYSFUNCTION, AND ACUTE RENAL FAILURE

• Thrombosis may occur with immune globulin intravenous (IGIV) products, including Panzyga. Risk factors may include: advanced age, prolonged immobilization, hypercoagulable conditions, history of venous or arterial thrombosis, use of estrogens, indwelling central vascular catheters, hyperviscosity, and cardiovascular risk factors. Thrombosis may occur in the absence of known risk factors.

• Renal dysfunction, acute renal failure, osmotic nephrosis, and death may occur in predisposed patients who receive IGIV products, including Panzyga. Patients predisposed to renal dysfunction include those with a degree of pre-existing renal insufficiency, diabetes mellitus, age greater than 65, volume depletion, sepsis, paraproteinemia, or patients receiving known nephrotoxic drugs. Renal dysfunction and acute renal failure occur more commonly in patients receiving IGIV products containing sucrose. Panzyga does not contain sucrose.

• For patients at risk of thrombosis, renal dysfunction, or acute renal failure, administer Panzyga at the minimum dose and infusion rate practicable. Ensure adequate hydration in patients before administration. Monitor for signs and symptoms of thrombosis and assess blood viscosity in patients at risk for hyperviscosity. EFFICACY AND SAFETY IN PATIENTS WITH PI AGED 2 YEARS AND OLDER

Panzyga® Helped Prevent the Occurrence of Serious Bacterial Infections (SBIs) in a Clinical Study1,2

A 1-year, prospective, open-label, noncontrolled, nonrandomized, multicenter, Phase 3 trial evaluated the efficacy and tolerability of Panzyga—including the number of SBIs as a primary endpoint, and the number of days of hospitalization and days missed of work/school as secondary endpoints—in 51 adult and pediatric patients with PI at a maximum infusion rate of 8 mg/kg/min.

PRIMARY ENDPOINT

The primary endpoint of the study was the rate of SBIs with treatment per patient-year.

The

overall annual rate was 0.08 SBIs per patient

• 4 infections over 50.2 patient-years

SECONDARY ENDPOINTS

Secondary endpoints included the occurrence of an infection of any kind or seriousness; time to resolution of infections; use of antibiotics; the number of days of work/school missed; the number of days of hospitalization; and incidence of fever. Additional assessments included the safety of Panzyga and the effects of treatment on quality of life (QoL) measures.

Contraindications

Panzyga is contraindicated in patients who have a history of severe systemic hypersensitivity reactions, such as anaphylaxis, to human immunoglobulin and in IgA-deficient patients with antibodies against IgA and history of hypersensitivity.

Low Incidence of Adverse Events With Panzyga Even When Administered at 14 mg/kg/min in the Extension Study2

A supportive extension study of the primary study evaluated the safety and tolerability of higher infusion rates (up to 14 mg/kg/min) of Panzyga for 3 months in adult and pediatric patients with PI previously treated with at least 3 infusions of Panzyga at an infusion rate of 8 mg/kg/min without premedication

The primary endpoint of the extension study was the safety and tolerability of Panzyga when administered at infusion rates from 8 mg/kg/min up to 14 mg/kg/min. The secondary endpoint was the effects of treatment with Panzyga on QoL measures.

• Patients who did not require routine premedication were included in this study

• In this study, 1 out of 21 patients experienced serious adverse events

• No adverse events leading to study withdrawal, or adverse events leading to death, occurred during the study

Warnings

and Precautions

Monitor renal function, including blood urea nitrogen and serum creatinine, and urine output in patients at risk of developing acute renal failure. Hyperproteinemia, increased serum osmolarity, and hyponatremia may occur in patients receiving Panzyga.

DOSING AND INFUSION RATES IN PI

Dosing and Rate of Administration for Patients Aged 2 Years or Older With PI

300 to 600 mg/kg body weight (3 to 6 mL/kg) administered every 3 to 4 weeks

1 mg/kg/min (0.01 mL/kg/min)

14 mg/kg/min (0.14 mL/kg/min)

• The initial infusion rate should be maintained for 30 minutes. Following the initial infusion, and if tolerated, the infusion rate may be gradually increased every 15-30 minutes, as tolerated, to the maximum infusion rate of 14 mg/kg/min (0.14 mL/kg/min)1*

• Determine the proper dose and frequency by monitoring the clinical response

• Adjust dose over time to achieve the desired trough levels of IgG and clinical responses

Overdosage

• With intravenous administration, overdose may lead to fluid overload and hyperviscosity. Patients at risk for complications of fluid overload and hyperviscosity include elderly patients and those with cardiac or renal impairment Dose

Warnings and Precautions

Aseptic meningitis syndrome may occur in patients receiving Panzyga, especially with high doses or rapid infusion. Hemolysis that is either intravascular or due to enhanced red blood cell sequestration can develop subsequent to Panzyga treatments. Risk factors for hemolysis include high doses and non-O-blood group. Closely monitor patients for hemolysis and hemolytic anemia.

• The example shown above illustrates a ramp-up schedule for a 70-kg patient with PI following a ramp up of 1, 4, 8, and 14 mg/kg/min

• For patients at risk of renal dysfunction or thromboembolic events, administer Panzyga at the minimum infusion rate practicable. Do not exceed 3.3 mg/kg/min (0.033 mL/kg/min). Discontinue if renal function deteriorates1

Warnings and Precautions

Monitor patients for pulmonary adverse reactions (transfusion-related acute lung injury [TRALI]).

Monitor blood pressure prior to, during, and following Panzyga infusion.

Carefully consider the relative risks and benefits before prescribing the high dose regimen (for cITP) in patients at increased risk of volume overload.

Panzyga is made from human plasma and may contain infectious agents, e.g. viruses and theoretically, the Creutzfeldt-Jakob disease agent.

EFFICACY AND SAFETY IN ADULT PATIENTS WITH cITP

Patients in a Clinical Study Receiving Panzyga® Demonstrated a Rise in Platelet Count and Decrease in Hemorrhages When Compared With Baseline1

Patients treated with Panzyga achieved a rise in platelet count

• 81% (n=29 [CI: 64%-92%]) experienced a rise in platelet count (≥50 x 109/L) within 7 days post infusion

Patients treated with Panzyga demonstrated a response in platelet count within 7 days, with a median duration of 14 days

• Median time to response was 2 days (range: 1-4 days), with a median duration of 14 days (range: 1-20 days)

Patients treated with Panzyga experienced a decrease in hemorrhages

• Among 36 patients, 64% (n=23) had bleeding at baseline compared to 14% (n=5) at Day 7

A prospective, open-label, noncontrolled, multicenter, Phase 3 study evaluated the safety and tolerability of Panzyga in patients with ITP aged 18 to 72 years at a maximum infusion rate of 8 mg/kg/min.

Adverse Reactions

PI—The most common adverse reactions (≥5% study patients) were headache, nausea, fever, fatigue, and abdominal pain.

cITP in adults—The most common adverse reactions (≥5% study patients) were headache, fever, nausea, vomiting, dizziness, and anemia.

Panzyga May Be Administered at 8 mg/kg/min, As Tolerated1

*Any infusional and study medication-related adverse events.

• Premedication to alleviate potential adverse drug reactions was not allowed in the study

• All patients except 1 received at least 1 infusion at the maximum rate of 8 mg/kg/min

• Three patients out of 40 did not receive a second infusion of Panzyga due to infusion reactions, including chills, headache, fever, and nausea

Adverse Reactions

Overdosage
