This past January optometrists from around New York State gathered in Lake Placid for our first NYSOA Winterfest Annual Meeting. This year we combined our annual meeting with a CE program and a winter festival for our families.

The CE program, which was arranged

by SUNY Optometry, provided quality education and was well received by our attendees. During the free time on Friday and Saturday our members enjoyed all Susan Fisher, OD that this Olympic venue had to offer. We were skiing, skating, tobogganing, and of course power shopping at the many stores along Main Street.

The highlight of the weekend was the passing of the Olympic torch. All of the families gathered in a circle at the speed skating oval and passed the burning torch to each other. Finally the caldron was lit by our incoming President, Dr. Chris Colburn.

We then participated in our own NYSOA Olympics with skating, curling and hockey competitions. It was a fabulous evening of socializing and fun in the chilly night air of Lake Placid.

We would like to make this an annual event and encourage everyone to put this on their calendar for next winter. It is a wonderful opportunity to have a vacation with your family, attend excellent CE lectures, get an update on NYSOA business, and build camaraderie with your fellow optometrists.

It is a chance to speak with the leadership of the NYSOA and with Mr. Jim Lytle, our legal and legislative consultant. There will be an update on our legislative agenda and health care reform. You can have lunch with your NYSOA colleagues and ask questions of the leadership. This is schedules for Sunday, March 30 from 12-2. I hope to see you all there.

Optometrist of the Year, 2014 Denise Whittam, OD

For over twenty-five years, Dr. Denise Whittam has dedicated her life to professional activism, community service and optometric education.

Dr. Whittam came to the profession of optometry as a respected licensed optician and as a special education teacher who taught developmentally disabled children.

She is a 1991 graduate of the State University of New York, State College of Optometry, where she was a student leader within VOSH and AOSA. Dr. Whittam is the current President of the Children’s Vision Coalition (CVC) in New York State (a not-for-profit foundation composed of professional and lay persons). She has taken the knowledge and experience she acquired as a teacher of at-risk children and transformed it into action through the education of parents, care givers, teachers, school nurses, school administrators, and other professionals who work with school-age children about the importance of vision in the learning process.

As the current President of the SUNY College of Optometry’s Alumni Association, Dr. Whittam has had a significant impact on her peers by educating them about the importance of advocacy, engaging students at career development symposia, leading “Lunch and Learn” lectures at the College; and she is an active member of the SUNYCO Career Development Center where she serves on the Advisory Board.

A Past President of the NYSOA and a Past President of the Optometric Denise Whittam, OD Society of the City of New York (the largest Society in New York State with over 300 members), she is the only optometrist in New York State to have held all four Presidencies.

Dr. Whittam also serves as the Editor of Prospectus, The Journal of the NYSOA and she has written numerous articles that have appeared in Optometric Management, Contact Lens Spectrum, the Review of Optometry, Vision Monday, Eyecare Business and 20/20 Magazine.

She serves as a Professional Affairs Consultant for Vistakon and lectures to optometrists throughout the country.

Dr. Whittam is a charter member of InfantSee in New York, and she has expanded the role of the Children’s Vision Coalition to be a voice for vision/eye care in the community.

In 2013, for the first time, CVC was present and visible at the New York State Association for the Education of Young Children Conference, New York State Association of School Nurses Annual Conference, New York State PTA Annual Convention and the New York State Occupational Therapy Association Annual Conference. In 2014, under the guidance and leadership of Dr. Whittam, the CVC will expand its presence to include the above organizations as well as the New York State School Boards Association, Council of New York Special Education Administrators and the New York Reading Association.

Dr. Denise Whittam has been a tireless spokesperson for the profession of optometry on a local, state and national level and she has demonstrated her skills as a leader within organized optometry and the optometric education community. She has translated her knowledge, experience and understanding of the role that vision plays in learning into a commitment to educate the educators and those who are part of the school community. For these reasons we believe that Dr. Denise Whittam would make an outstanding recipient of the American Optometric Association’s Optometrist of the Year in 2014. ▪

5 Executive Director’s Report to the Membership ▪ April 2014 Why Should I Belong? Is it Time for A New Paradigm?

What is the key message for membership in the NYSOA in a rapidly changing environment? In the old days, it was considered to be an imperative to be a member of your professional association.

That imperative started to shift at the end of the 20th Century and has transformed itself into a mindset that focuses on, “what do I get for my dues”, “what is the value proposition, what is the ROI,” etc.?

Today we are faced with a generation that can have it all for free—free CE, free information and free social and professional networking on their terms, whenever they want it. Why should new doctors of optometry pay dues and belong to the NYSOA/ AOA? What role do we play in their lives that would compel them to join and pay association dues?

When the NYSOA/AOA was created over one hundred years ago the profession was young, communication was primitive and fellowship was an important commodity. We banded together to learn from one another and promoted the profession legally and legislatively. We paid dues as part of our professional responsibility and participated as members in the affairs of the association. Collectively, we helped shape the profession of optometry as we knew it and as we wanted it to be. We gave, rather than took and for the most part we were successful.

Maybe the reason for belonging to an association and paying dues is an antiquated paradigm, replaced by a world of smart phones, computers and, virtual communities. We are at the beginning of a transformation of how we become educated as we see colleges and universities offering on-line credit courses as cost effective ways for students to gain an education. While the movement is still in its infancy and not without glitches, there is no doubt in my mind that the internet may become the preferred classroom of the future.

The way we communicate has also taken a dramatic shift and traditional news print publications are being replaced by on- line and electron formats. The days of association newsletters and professional journals may be numbered and professionals have almost unlimited access to information that once was the province of professional associations.

Maybe it is time for us to rethink the traditional models of associations and membership and what that implies and cast our message in terms used by insurance companies – premiums.

The concept of Mutual Insurance Companies is not new.

They have existed for hundreds of years and members who pay premiums are the shareholders of the company. Annual premiums are paid to a Mutual and if claims are low, premiums stay low. If claims rise, so do the premiums. The underlying

structure of an insurance company is risk and exposure and while an Optometric Mutual Insurance Company would not be insuring an actuarially determined risk, it would be insuring the shareholders’ right to practice the profession the way he or she chooses.

The insurance model is not a foreign concept to optometrist.

After all, doctors, young and old, male or female, employed or self-employed purchase malpractice insurance, life insurance, automobile insurance, home owners or renters insurance, disability insurance, etc. They pay premiums annually with the hope that they won’t have to use their insurance. It is there in case; and most doctors would never go uncovered. We don’t ask insurance companies,” What do we get for our premium dollars?”

We buy insurance as a way to protect ourselves from an adverse event that may expose us to financial harm.

Other less serious side effects were conjunctivitis, lid irritation or lens discomfort including dryness, mild burning or stinging. Contraindications: Contact lenses should not be worn if you have: eye infection or inflammation (redness and/or swelling); eye disease, injury or dryness that interferes with contact lens wear; systemic disease that may be affected by or impact lens wear; certain allergic conditions or using certain medications (ex. some eye medications). What about an insurance policy that pays dividends in good times and bad? What about an insurance company that protects us and pays a dividend to the insured through advocacy that ensures us that we are not exposed to financial or economic harm?

By recasting the relationship of a member to his or her professional association in terms of insurance premiums we may have a more compelling case as to why optometrists must belong to their Optometric Mutual Insurance Company (NYSOA/A0A).

Fabry Case Study

Albert M. Morier, M.A., O.D., Christopher Shiomos, O.D., Joshua Mali, M.D. Patient History

This 38 year old white female first presented 2 years earlier for an assessment after having poked her left eye with her own finger. During that examination the patient was noted to have corneal verticillata. She had been seen by prior eye doctors and was told to have these corneal findings since the age of 12 years old. The patient has also undergone successful LASIK refractive surgery to correct her nearsightness at the age of 32. Corneal verticillata were noted at her consult visit for refractive surgery.

Corneal photography was performed and patient was questioned about medication use as a possible cause. No further workup ensued. After her assessment of her eye injury and reaffirmed diagnosis of corneal verticillata by her optometrist at age 36, the patient was referred for genetic counseling. Initial visit with a genetic pediatrician found an otherwise health individual other than a longstanding history of irritable bowel syndrome and anxiety. The patient was recommended for enzymatic and DNA testing. After the patient was lost to follow-up for close to a year, she was finally diagnosed with Fabry Disease after she was found to have one copy of the R220X mutation. The patient does not have any children at this time.

About Fabry Disease

Fabry disease is a progressive, debilitating, and often lifethreatening X-linked inherited disorder.1 The incidence is traditionally estimated to be 1 in 40,000 live births.2 It is an inborn error of metabolism resulting in deficient enzyme levels of the lysosomal enzyme alpha-galactosidase A (a-GAL).3 Alpha-galactosidase A deficiency prevents the effective metabolism of the glycoprotein globotriaosylceramide (Gb3).4 Consequently, Gb3 accumulates in lysosomes of vascular endothelial cells resulting in an intrinsic inflammatory cascade in virtually every tissue in the body.

Initially, symptoms include angiokeratomas (telangiectatic cutaneous lesions), acroparesthesia (severe pain in hands and feet), hypohidrosis (inability to sweat), gastrointestinal dysmotility, and various ocular manifestations.5 As the disease progresses, vascular complications of major organ systems ensue, leading to kidney failure, cardiomyopathy, and stroke.5 While previously thought to exclusively involve males, it is now clear that women can also be affected depending on the variable level of X-inactivation during embryonic development.6-8 Therefore, both males and females can display a unique clinical presentation of signs and symptoms across a spectrum of disease manifestations.

Ocular Manifestations

Ocular manifestations of Fabry disease have been published by several investigators.9-20 The most commonly reported ocular finding is corneal verticillata, a bilateral, whorl-like

pattern of cream-colored lines usually found in the inferior cornea and radiate outward. These deposits can range from faint to pronounced and are located at the level of Bowman’s membrane.

They can be seen as early as 6 months of age and fully apparent by age 10.10,19 Corneal whorling from both Fabry and use of the cardiac anti-arrhythmic, Amiodarone occur at the same level within the cornea.21 It is important to note that a patient with corneal whorling and on Amiodarone may have hypertrophic cardiomyopathy as a result of Fabry disease. So it’s crucial that one does not assume it’s from the Amiodarone especially if it’s a young patient.

Conjunctival and retinal blood vessels may exhibit tortuosity and aneurysmal dilatations. The pathophysiology is primarily caused by Gb3 accumulation within the vessel walls, resulting in endothelial cell dysfunction, abnormal blood flow, and hypercoagulability.

While blood vessel tortuosity is often seen among all patients, aneurysmal dilatations are not and are a sign that the disease is at a more advanced level.

There are 2 types of lenticular changes found to be associated with Fabry disease. One is a granular anterior capsular or subcapsular deposit radiating out from the periphery in a pattern that has been described as a ‘‘propeller cataract.’’18.

The second is a whitish, faint, linear cataract at or near the posterior lens capsule. This second type of cataract was first described

by Spaeth and Frost in 1965 and is termed a Fabry cataract as it seems to be unique to this condition 20.

Current Status of Patient

Patient continues to be in fairly good health without any evidence of clinical symptoms associated with Fabry Disease. Patient reports improvement with her anxiety and irritable bowel syndrome.

She is followed on an annual basis by a geneticist, optometrist, neurologist, cardiologist and dermatologist. Treatment with Fabrazyme® (agalsidase beta) enzyme replacement therapy (ERT) has not been initiated at this time. Her family will be monitored for evidence of the condition.

Lessons learned

The ocular manifestations of Fabry disease are signs that a fatal insidious disease is lurking within the patient’s genetic code slowly damaging their kidney’s, heart and brains. The nonfatal affects of the disease acroparaesthesia (pain in hands and feet), vascular skin lesions (angiokeratoma), hypohidrosis or anhidrosis (poor or non-existent sweating), and gastro-intestinal maladies (i.e. spontaneous diaarhea) makes for a very poor quality of life. Females carriers are affected as much as males (although this takes a longer time) which is contrary to what we were taught in our basic genetic courses where female carriers carry the affected gene but do not express it 22.

With careful observation, eye care professionals can hasten the diagnosis and have a major impact of the lives of the patient, their siblings and their progeny. ▪

Orientation Day Presentation An introduction of the NYSOA was given to incoming first year students during their orientation. The other student organizations at SUNY presented and described their organizations as well. The goal of this presentation for the NYSOA Student Society was to introduce the officers: Chelsea Ashlaw, President; Matthew Roe, Vice-President; Christie McKenzie, Technology Liaison; Dr. Joseph Stamm, Faculty Advisor; and briefly describe ways to get involved during the school year.

Eye CUP (Eye Community Update Poster) This is a monthly publication created by the NYSOA Student Society. Each month there is an update included from the Student Council at SUNY, an update about legislative information or events held by the NYSOA Student Society, a joke section, an AOSA update with events or information, a featured doctor of the month and a featured student club of the month. The posters are created on 11x17 paper and printed in color, as there are photos included in many sections. They are hung in the bathroom stalls on all student floors. There are new Eye CUPs made each month.

There has been a lot of positive feedback from the students and staff about these posters.

September 2013

AOA-PAC Congressional Advocacy Conference With donations from Dr. Chung, Dr. Thau, and Dr. Richter 5 students were awarded travel grants to allow them to attend the Conference in Washington DC. The students were selected based on those who had been most involved with Organized Optometry over the course of the 2012-2013 school year. The SUNY AOSA Trustee and myself reviewed the points and the students were selected. Many other students from the 2nd and 3rd year classes also attended the conference to help with the legislative efforts.

WOO! (Week of Organized Optometry) The NYSOA Student Society in collaboration with the AOSA Trustee and Trustee Elect, planned WOO. WOO week began with the AOSA sponsored vision show for students to meet representatives from a variety of corporations including WalMart, Allergan, Heine, Keeler, Welch-Allyn, Bausch and Lomb, and many others. There was food provided and raffles offered.

On Tuesday of WOO the NYSOA Student Society Officers and AOSA officers had an organized optometry information booth during lunchtime. A flyer was distributed from the NYSOA Student Society with up to date state and national legislative information presented in an easy to understand manner.

Thursday during WOO week was a mandatory Constitution Day lunch meeting for first years. At this meeting Dr. Joseph Stamm gave a presentation about how optometry has changed over the years because of organized optometry efforts and how important it is to be involved.

Lastly on Friday of WOO, there was an event entitled "Organized Optometry Celebration" for all students. At this event brief presentations were given from the leaders of the NYSOA Student Society and the AOSA about the planned events for the year and open leadership positions. The importance of organized optometry was also presented.

Informational Meeting The meeting was held the week following WOO to give information to students about our two new officer positions.

Information was given about the Secretary position and the Eye CUP Editor position. In order to run interested students had to attend this meeting and then write a brief paragraph about why they wanted the position. The paragraphs were anonymously posted on a polling website and the winners of each position were announced. Alison Schmidt (Class of 2015) was elected as Secretary and Kiranjeet Sran (Class of 2017) was elected Eye CUP Editor. The winners were told these were interim positions to be held until the official school elections occurred in the spring and they would then be able to run again.

NYSOA Leadership Retreat Alison Schmidt (NYSOA Student Society Secretary) and myself attended the NYSOA Leadership Retreat where we were able to participate in discussions and activities. This was of great benefit as we got to network with optometrists from across New York State and learn more about the NYSOA. The retreat gave us an opportunity to see the importance of being involved in organized optometry and gave us both motivation to stay involved as we complete our education.

Lunch Panel with Dr. Fisher and Dr. Stamm This was a lunch meeting open for all students during which Dr. Fisher and Dr. Stamm did a question and answer panel.

The goal of the lunch was to not be a lecture but to be more of an open discussion session. This was a success as students were very interested in the current legislation and had questions for Dr. Fisher and Dr. Stamm. (Dr. Landrio was supposed to come with Dr. Fisher to have a panel of three doctors, but had a family emergency right before the meeting.)

Vision Expo Planning Committee

The committee was opened to all students at SUNY and if they were interested they had to e-mail in why they wanted to be part of the committee and what they could contribute. A group of 10 students were selected from those interested to help plan the NYSOA Vision Expo Luncheon for students at Vision Expo East. The committee is composed of students from all class years.

Each Tuesday we meet with Francisco Lucio, the Director of Career Development and Minority Enrichment. The event is in the works of being planned out and our goal this year is to make the event shorter and more interactive. As a committee we decided we didn't want to have just an informative lecture but make the event more interesting.

The meetings will continue to be held every Tuesday until Vision Expo East to ensure the event is planned and everything is ready for the luncheon.

“Networking with the Doctors Social”

The NYSOA Student Society in collaboration with the Career Development Center and SOAPP (Student Optometric Association for Private Practice) planned this event. This was modeled after a speed-dating event where there were 16 optometrists from the New York City area and 50 SUNY students. The participating doctors were all from various modes of optometric practice. There were three or four students at each table with one doctor. Every four minutes the doctor at each table rotated to a new table. The students led the discussions and the doctors answered the questions.

Following the speed dating portion there was a reception with wine and hors d'oeuvres where the students were able to mingle with the doctors they had met and ask any other questions they may not have had time for at the tables.

This event received a high volume of positive feedback from both students and doctors. The doctors said they would have liked to have more time at each table, but our goal in the planning was for the students to be able to talk to each doctor and find out about all the modes of practice. This event is planned to occur again next year.

Following Your Dreams in Optometry

This event was held on a Friday afternoon when there was no clinic so more students would be able to attend. Dr. Chris Colburn came to SUNY to give a presentation on being able to follow your dreams as an optometrist. Dr. Colburn gave information on how to join a private practice and the best way to make a profit as a new optometrist. Following this

There was a lot of positive feedback about this event and how well spoken Dr. Colburn was. The students said they would like for him to come back for another event.

For the spring semester I would like to continue to have at least one NYSOA Student Society event each month. We will keep planning the Vision Expo event each week; the event is scheduled for March 29, 2014. There will continue to be monthly publications of the Eye CUP hung up around the student areas at SUNY. We will also contribute articles to Prospectus as needed for the Optometry Student Corner. I will also be obtaining information from the third and fourth year students to get their email that is not affiliated with SUNY and what states they plan to get licensed in. This information will allow the NYSOA and other state organizations to have contact with recent graduates.

In January, Dr. Stanberry (President of SUNYCO) and the Student Society will be coordinating an event to allow the students and faculty at SUNY to interact. We would potentially like to plan a round table event similar to the idea of the Networking Social.

Continued from page 14 With the Career Development Center there will be a team approach taken to plan a volunteering event on Martin Luther King Jr. Day for students to provide eye care to those impacted by Hurricane Sandy.

In February there will be a group of 8-10 students attending the NYSOA Annual Meeting in Lake Placid. During this trip students will be visiting a local optometrist's private practice office to see how the practice is run.

17 Certificates of Waiver for use of CLIA-waived tests Last year, Governor Andrew Cuomo signed into law legislation promoted and strongly supported by the New York State Optometric Association which authorizes optometrists to perform waived tests pursuant to a Limited Testing Registration issued by the State Department of Health (DOH). The proposal would also clarify that optometrists may conduct certain clinical laboratory tests (e.g., CLIA-waived tests) without a clinical laboratory permit, provided such tests are solely as an adjunct to the treatment of their patients. The purpose of this notification is to alert you that the Department of Health has begun processing applications for the appropriate certificate which will allow optometrists to perform these “waived” tests.
