The Advance Care Directives portal was produced by the Legal Services Commission Web Services to allow South Australians generate their own binding future health care wishes. The ACD information and online form site was launched in July 2014 and to date has generated over 23,000 completed ACD online documents. A portal for the SA public and health-professionals. To date the site has generated over 23,000 Completed Advance Care Directives. A joint partnership project with SA Health, launched in July 2014.
