From the Chair The past year has been one of growth and progress for the Department of Medicine with many notable achievements in education, clinical care, and research. As the largest and one of the most active Departments in the MUSC College of Medicine, the Department provides essential leadership to numerous programs across the University and South Carolina.

In the pages that follow, I invite you to learn more about the exciting programs, research initiatives, and treatment advances taking place within the Department. In particular, some noteworthy Departmental highlights from this past year include the following: • Our peers and our patients continue to rank Department of Medicine programs among the best in the nation. In 2017, MUSC Health was named by U.S. News & World Report as one of the country’s top 50 hospitals in the treatment of cancer.

MUSC was also recognized as high-performing in seven adult specialties, including gastroenterology and GI surgery, nephrology, pulmonology, and rheumatology.

• The Department’s clinical programs have continued to grow in both volume and quality over the past year. Our patient-centered medical initiatives as well as our faculty’s dedication to quality have resulted in increased patient satisfaction rates and financial success.

• In a highly competitive research environment, the Department saw total research funding increase by 9.6 percent this past year, with increases in both federal funding and industry sponsored funding.

• The Department’s core Internal Medicine Residency and Subspecialty Fellowship Programs are highly ranked and recognized nationally for their quality. Each year, applications to our residency program exceed the previous year, and the quality of the applicants has continued to rise along a similar trajectory.

Our Department—made up of nine divisions engaged in care at two hospital systems and multiple ambulatory practices—is guided by our and the Institution's vision to provide the highest quality of care, to educate the next generation of physicians, and to perform seminal discovery research. We are helping to fulfill this vision by demonstrating the five MUSC values of compassion, respect, innovation, collaboration, and integrity across our core mission areas of education, research, and patient care.

Palliative Care: Improving Quality of Life Ask Patrick Coyne, MSN, how long he’s been practicing and promoting palliative care, and he responds without hesitation: “I think I’ve been doing palliative care before they even had a name for it.”

Anecdotes aside, Coyne was first introduced to and intrigued by palliative care when he was in graduate school. He completed his thesis on cancer pain management. “I found early in my career that many individuals were suffering with serious illness, and I was challenged to improve the care that was delivered,” says Coyne, Director of Palliative Care at MUSC. In 2015, MUSC launched a new Palliative Care Program designed to enhance quality of life for patients suffering from serious illnesses. According to Coyne, the goal of the program is to promote comfort and improve quality of life for individuals with serious illnesses as well as their families. Palliative care embraces not only the clinical, but the spiritual and psychological needs of a patient and the family.

Facing an aging population of patients with multiple and serious illnesses, hospitals need strong palliative care programs, says Coyne. “We provide another layer of support for patients who have acute or chronic pain issues and for the primary medical team providing their treatment. This isn’t just a nice thing to do – it’s a necessary service.”

Under Coyne’s direction, the program has expanded rapidly over the last three years, growing from three team members to a 20-person interdisciplinary team. Coyne joined MUSC Health from Virginia Commonwealth University, where he ran one of the first palliative care programs in the country for 24 years.

MUSC Health’s program now offers palliative care services to both adults and children. Adult services are provided through both an inpatient consult service and an outpatient clinic at MUSC Hollings Cancer Center. “The number of patients we’re seeing has more than quadrupled since I began as director in July 2015,” says Coyne. In FY17, the team conducted over 300 pediatric consults and over 2,000 adult consults.

To help meet the rapidly growing demand for palliative care providers, a palliative care fellowship program has also been developed. Lauren Seidenschmidt, Palliative Care Program Coordinator, says that with Dr. Leigh Vaughan leading the way, the Palliative Care Program has entered its second year

having an ACGME approved Hospice and Palliative Medicine (HPM) Fellowship, and its first year participating in the National Residency Matching Program. “We proudly matched within our top five candidates and had a very successful year.

Our HPM Fellowship Program gives fellows the rare opportunity to work at MUSC, Roper St. Francis Hospital, and the Ralph H. Johnson Medical Center. We also give our fellows the opportunity to work with both adults and children.”

In addition, thanks to a $180,000 grant from the Duke Endowment, Coyne has been able to offer palliative care training to caregivers throughout rural South Carolina, leading two-day CME-eligible courses at South Carolina Area Health Education Consortium sites.

Studies show palliative care, especially when started early, can help patients live better and longer. While it doesn’t involve expensive state-of-the-art technology, palliative care does require those who deliver it to be adequately trained. As Director of Palliative Care for MUSC, Coyne has made this his mission.

Simply stated, “I want to see us as one of the national leaders in palliative care in the next few years,” Coyne says.

Department of Medicine Quality Improvement Project Providing high-quality, cost-efficient health care has become an increasingly important part of the national health care discussion.

In 2014, an interdisciplinary team within MUSC’s Department of Medicine embarked on a collaborative effort to reduce health care costs and improve the quality of care provided to some of the most vulnerable patients.

Using predictive-modeling technology, the Quality Improvement Project on Internal Medicine High-Utilizer Patients—led by Skip Schumann, M.D., Assistant Professor of Internal Medicine, and Marc Heincelman, M.D., Assistant Professor of Internal Medicine—worked to identify high-utilization patients by analyzing patients’ electronic health record data upon admission.

In 2017, Schumann and Heincelman, working in collaboration with the Health Services Research Team, successfully completed the first phase of the project.

electronic health record of all patients admitted to internal medicine services and identified those most at risk to utilize hospital resources,” says Schumann.

“Once identified, we began an intervention with the goal of reducing pharmaceutical costs, laboratory costs, and length of stay.”

Now in its third year, the project is taking steps to improve the ability of the model to accurately predict high-utilization admissions. In addition, work is underway to investigate other avenues to improve the value of a hospitalization, such as timely and effective discharge planning and disposition.

An interdisciplinary team with backgrounds in statistics and economics, skill sets in data analytics, extraction, and reporting, project management and quality improvement methodologies,

and many years of health care experience has been instrumental in advancing the goals of the project.

According to Schumann, the overarching goal of the initiative is to improve the value of care delivered to patients.

“Value in health care is quality plus service over cost; thus, we are trying to improve both by decreasing potentially ineffective and unwanted costly medical treatments.”

In the United States, five percent of the patient population accounts for 50 percent of health care expenditures.

Historically, these “high-utilizer” inpatients have been defined as the top 10 percent most expensive patients admitted to internal medicine services.

“In phase one, our model independently analyzed multiple data elements in the

Biomarker Discovery in Kidney Diseases Renal glomerular diseases, characterized by faulty podocyte structure, are a leading cause of end-stage renal disease (ESRD) in the United States. Diagnosing and treating these life-threatening diseases is a challenging task and requires an invasive and costly renal biopsy procedure. However, researchers in the Department of Medicine’s Nephrology Division are working to develop a novel procedure that will revolutionize the way glomerular diseases are diagnosed, without the need of biopsies.

Deepak Nihalani, Ph.D., Associate Professor and SmartState Endowed Chair in Renal Disease Biomarkers, and his team are using new proteomic and informatics technologies to identify patterns of biomarker proteins in urine and plasma that can predict diagnosis and prognosis of patients with renal diseases.

Recently, the group identified a set of genes that were used to develop a diagnostic assay to identify patients with a common form of glomerular disease known as recurrent focal and segmental gloemrulosclerosis (rFSGS). Importantly, this assay will be able to distinguish rFSGS patients from other glomerular

diseases, including membranous nephropathy, lupus nephritis, and diabetic nephropathy. This discovery may lead to the development of similar tests that can replace the kidney biopsy as a gold standard of diagnosis for glomerular diseases. Using a diagnostic test instead of kidney biopsy will enable doctors to diagnose patients faster and earlier in their diseases without the need for a potentially risky and expensive procedure.

Researchers in the Nihalani lab are now working on using these new findings to create a novel human podocyte cell-based assay that can be tested clinically. They are also identifying markers that can diagnose acute kidney injury, predict whether patients with acute kidney injury will require dialysis, and predict which diabetic patients are at greatest risk of developing kidney disease. Future studies will test these markers. Identification and testing of protein biomarkers will lead to more accurate diagnoses, enable targeted treatments, and ultimately prolong and improve the lives of millions of patients with kidney disease.

Enhancing Patient Care with Health Information Technology (HIT) Cara Litvin, M.D., is an Associate Professor of Medicine in the Division of General Internal Medicine. As a practicing general internist and health services researcher, Litvin has led and participated in many research studies conducted within the Primary (Care) Practices Research Network (PPRNet) to explore how health information technology (HIT) can be used to measure and improve the quality of health care.

Founded in 1995 by Litvin’s longtime mentor, Dr. Steven Ornstein, PPRNet is a national primary care practice-based research network designed to improve health care in its member practices while conducting research important for the entire primary health care system. Over 400 practices with nearly 2,700 providers in most U.S. states have participated

in PPRNet research. These practices use electronic health records (EHRs) and pool data for quality improvement and research purposes. Member practices benefit from practice quality reports and educational programs. Many also participate in federally funded research projects, which help identify and disseminate strategies to provide evidence-based high-quality care.

Litvin began working with PPRNet in medical school at MUSC where she realized the critical role EHRs could play in improving the delivery of high-quality primary care. During her residency training at New York University School of Medicine, Litvin wrote a perspective published in the New England Journal of Medicine highlighting the need for widespread EHR adoption. In 2008, she returned to South Carolina and PPRNet, completing an academic generalist fellowship and obtaining a master of science degree in clinical research in 2010.

She then received a Mentored Clinical Scientist (K08) career development award from the Agency for Healthcare Research and Quality (AHRQ) to further develop her research skills. As part of this training, she studied clinical informatics (Oregon Health and Science University).

Litvin cites PPRNet’s core multidisciplinary team of investigators, including faculty from the Departments of Internal Medicine, Family Medicine, and Public Health Sciences, as well as the College of Nursing, as a unique feature and strength.

“I’ve been extremely fortunate to have had and continue to have close relationships with PPRNet investigators who have helped me develop and refine my research skills," says Litvin.

Most PPRNet studies involve site visits or webinars with participating practices, during which all practice team members from the physicians to the front office staff are encouraged to participate in quality improvement activities.

In the future, Litvin hopes to continue to develop better metrics to assess quality and value in primary care, improve our understanding of how to effectively utilize clinical quality measure reporting to manage populations, and explore how mobile health technologies can be better integrated with primary care. “It’s amazing to see the widespread adoption of EHRs since I was in medical school.

However, there is still much more to learn about how EHRs and other HIT tools can lead to improved patient-centered care," says Litvin.

“Our laboratories are our member practices. We work with an incredible group of practices across the country who teach us about how high-quality care can be most effectively delivered," says Litvin. “We know use of EHRs alone does not automatically result in improved quality. Through our research, we’ve been able to identify specific strategies to help practices use EHRs and other HIT tools to actually improve care. For example, empowering staff members to act on standing orders, use of registries for outreach, staff education, and continued feedback via clinical quality measures all have been shown to be effective strategies.”

Currently, Litvin is leading two group randomized controlled studies in which health information technology tools are being used in conjunction with a

practice-based quality improvement model to improve adherence to clinical practice guidelines in PPRNet practices. “In my current R01 project, we’ve actually started to explore whether HIT can help practices avoid overuse of unnecessary tests, which is a new twist on improving quality,” says Litvin. She also currently serves as a co-investigator on additional PPRNet projects, all related to improving primary care safety and quality.

In addition to these research activities, since 2015, Litvin has also served as the Clinical Informatics Director for the PPRNet Qualified Clinical Data Registry which has been approved by the Centers for Medicare and Medicaid Services (CMS) to enable PPRNet to submit Clinical Quality Measures (CQM) data to CMS to satisfy Physician

Department of Medicine LEAD Program: Cultivating Health Care Leaders innovation . leadership. resilience.

Today’s successful health care leaders must demonstrate all three. That’s why Department of Medicine Chair Don C. Rockey, M.D., appointed a task force charged with creating a stronger culture of leadership.

In September 2016, the Department of Medicine’s LEAD (Leadership in Academic Medicine) Program welcomed its third cohort.

The program, led by Elisha Brownfield, M.D., Associate Professor of Medicine and administered by MUSC’s Department of Medicine, is designed to enhance individual skills associated with leadership, self-discovery, team-building, and driving change.

This year, the program celebrated the graduation of 23 leaders from the Department of Medicine and across the University.

Since its inception in 2014, 75 physicians, scientists, clinical, and administrative leaders have completed the program.

“With so much change now in all of the mission areas in academic medicine, strong and committed leadership is more important than ever before,” says Rockey.

The program features one- to two-day classroom sessions led by nationally recognized leadership experts who also oversee group-project work conducted in between sessions. Other program highlights include a 360 self-assessment, executive coaching, insight into MUSC’s culture and financial structure, and a capstone project to support learning objectives.

Eligible participants include faculty and staff from the Department of Medicine and individuals from other departments and colleges across the University. Candidates who display high leadership potential are nominated by their Division Director or supervisor.

In 2015, the Department of Medicine introduced another initiative, LEAD: Foundations designed for early- to mid-career future leaders. In January 2017, the program led by Drs. Abby Kazley and Jami DelliFraine from MUSC’s Department of Healthcare Leadership and Management, welcomed its second cohort. The cohort class was comprised of 33 faculty and staff from the Department of Medicine and across the University.

The program allowed me to examine my strengths and weaknesses and make meaningful change, thus giving me enhanced confidence as I assumed my new roles," says former graduate Pamela Morris, M.D., Associate Professor of Medicine.

Over the past decade, some of the best academic health institutions in the nation have increasingly focused on leadership development, says Kathy Church, Owner and Principal Consultant with Strategic Focus Associates. She has collaborated with Brownfield to design and refine the LEAD program.

“Academic medical communities like MUSC recognize that leadership skills and attitudes are essential to meeting our missions of excellent patient care, innovation, and education. Everyone in an academic medical community is a leader, and leadership skills can be learned and will improve with practice,” notes Brownfield.

This program has proven to be one of MUSC’s most dynamic programs in leadership development and supports the MUSC Imagine 2020 Leadership Initiative. Now, the LEAD program is becoming a template for leadership development across MUSC and work is underway to adopt a similar model enterprise-wide.

Faculty Development Program: Guiding Junior Faculty Along Paths to Success Mentoring is a defining feature of academic medicine. But as the field of academic health care has grown more complex over the past few decades, navigating the professional terrain is proving to be ever more challenging, especially for junior faculty.

Thus, the Department of Medicine has created a Faculty Development Program, led by Gerard Silvestri, M.D., Vice Chair for Faculty Development, which is designed specifically to address this need and to support junior faculty with guidance on career development.

Now in its fourth year, this highly effective program guides faculty through a stepby-step mentoring process to help them clarify their goals and develop a long-term career vision; thus, facilitating their career advancement and promotion, and, ultimately, success in their academic careers.

To provide transparency and accountability, each faculty member at the rank of instructor, assistant, and associate professor completes an Individual Development Plan with support from a mentor, in which specific achievable short- and long-term goals are defined.

The plan is reviewed and approved by the faculty member’s Division Director.

This innovative strategy ensures that the goals of the individual, their mentor, and Departmental leaders are aligned.

To further support the program, mentorship is emphasized. Intensive group and one-on-one mentoring sessions and workshops are offered throughout the academic year and cover a variety of topics and skills important for faculty success, including the promotion process, how to give a lecture, scientific writing, grantsmanship, and negotiation.

“We believe our faculty members are our greatest asset. Secondly, we believe that the pervasive ‘sink or swim’ model of mentoring on the tenure track is inefficient and ineffective. So, we’ve developed a new game plan for success,” says Silvestri.

This program has proven to be successful on many different levels. Studies show that faculty members are twice as likely to get promoted if they understand the promotion requirements, says Silvestri. “Since the program’s inception, we’ve seen an

increase in the percentage of faculty who understand the College of Medicine’s promotion criteria from about 60 percent to about 90 percent.”

The program has also helped to provide resources and training opportunities in grantsmanship for junior faculty, says Jim Oates, M.D., Vice Chair for Research.

“Dr. Silvestri and I meet with each junior faculty member to increase his or her chance of a successful grant application. We advise them on how to use South Carolina Clinical and Translational Research Institute (SCTR) and Advancement, Recruitment, and Retention of Women in Science (ARROWS) resources for scientific consults and external grant review, and to develop and engage strong mentoring teams. The success of career development applications in the Department speaks to the value of the program.”

“Ultimately, it is about faculty satisfaction and retention,” says Silvestri. “We want to create a positive career experience for our rising stars. They are the future of our institution. We need to show them the path to succeed.”

The Faculty Development and Mentoring Operations Team—including Gerard Silvestri, M.D., Julie Driggers, Joe Gough, and Kate Taylor—has continued to evaluate, enhance, and modify the program over the past four years based on the needs and feedback from the Department and program participants. Now, work is underway to establish a Mentorship Awards Program to honor faculty members for their contributions to the scientific, education, and patient care missions of the Department through exemplary mentorship.

The Division of Cardiology has experienced continued expansion across its clinical, research, and educational programs. As the only comprehensive academic health science center in South Carolina, the MUSC Heart and Vascular Care Center offers more heart disease specialists and treatment options than any other hospital in the state.

Division faculty members offer outstanding tertiary and quaternary patient care, conduct leading-edge research, and deliver specialized training for the next generation of cardiologists. Their agenda-setting initiatives have brought about several remarkable developments this year.

HIGHLIGHTS & ACCOMPLISHMENTS CLINICAL Over the past year, the Division has undertaken an aggressive strategic clinical expansion, including the development of new invasive, interventional, and electrophysiology facilities and has greatly expanded its footprint and capabilities in the cardiovascular center at Ashley River Tower. The Division has also had remarkable clinical growth in non-invasive areas with particular emphasis in advanced heart failure and cardiac transplantation.

The Division’s Adult Congenital Heart Disease Program earned the Adult Congenital Heart Disease (ACHD) Accredited Comprehensive Care Center designation from the Adult Congenital Heart Association (ACHA). MUSC Health is one of only a dozen centers in the country with ACHA’s gold-standard, seal of approval.

RESEARCH The Division’s basic and clinical research programs have continued to flourish with important accomplishments, publications, and funding of new projects. In the past year, Division investigators successfully obtained major new grant funding from the National Institutes of Health (NIH), Department of Defense (DOD), Veterans Administration (VA Merit), and American Heart Association (AHA) helping

to make the Gazes Cardiac Research Institute one of the nation's leading centers for heart failure research. Members of the Division published 82 peer-reviewed publications in FY17.

The Division additionally established a Cardiovascular BioRepository to serve as a core resource to investigators in cardiology and across MUSC.

ENDOCRINOLOGY

Over the past year, the Division of Endocrinology has continued to build momentum in education, research, and clinical care, with the goal of excellence in each area. At the forefront are new leadership and recruitment, allowing for the continuing expansion of the Division’s efforts. In May 2017, Timothy Lyons, M.D., Professor of Medicine, was appointed as Director of the Division of Endocrinology. Originally from Ireland, Dr. Lyons received his M.D. from the School of Medicine at Queen’s University Belfast. After his internal medicine residency at Royal Victoria Hospital, Belfast, he undertook formal training in both clinical endocrinology and research at Queen’s University Belfast. In 2001, Dr. Lyons arrived at MUSC as a clinical research fellow in endocrinology.

During his fellowship, he worked with John Colwell, M.D., then the Division Director, to help launch a landmark trial of patients with Type 1 diabetes — the Diabetes Control and Complications Trial (DCCT).

As an MUSC faculty member in the 1990s, Dr. Lyons began to collect samples from trial participants for study in the laboratory. In the early 2000s, he left MUSC to take a position as Division Chief of Endocrinology at the University of Oklahoma, where he built a multi-specialty diabetes center, supported by millions in funding from governmental and philanthropic sources, and worked with local Native American tribes to improve their diabetes care.

Now back at MUSC, Dr. Lyons is the first to hold the John A. Colwell, M.D., Ph.D., Endowed Chair in Endocrinology and will continue the work that his mentor started, realizing the potential of DCCT long-term data to yield invaluable insights into the vascular complications of diabetes.

Dr. Lyons' top priority for the Division is to develop an integrated multidisciplinary diabetes (clinical and research) program, involving clinical, translational, and laboratory scientists. He brings a committed interdisciplinary leadership approach back to MUSC, which will be essential to address the challenges posed by diabetes and its complications. His vision is to improve the health of MUSC’s patients and to develop effective new treatments and preventive measures.

HIGHLIGHTS & ACCOMPLISHMENTS CLINICAL The Division has continued to expand its clinical programs over the past year, offering specialized outpatient and inpatient diagnostic and treatment services to patients throughout the Charleston area and the region. Subspecialty clinics utilize a multidisciplinary approach to treat all aspects of endocrinology, including diabetes, thyroid disease, thyroid cancer, osteoporosis, and pituitary disorders.

Certified diabetes educators, podiatrists, nutritionists, and nurse practitioners experienced in diabetes management and endocrine testing provide integral support and expertise in the Division’s clinics. Collectively, the team provided care for over 6,200 outpatients in FY17.

The Division's Diabetes Management Service cared for over 1,400 inpatients between its Ashley River Tower, Medical University Hospital, IOP, and Children's Hospital locations in FY17.

RESEARCH

In FY17, research efforts continued to be supported by government and private funding and included diverse areas such as Type 2 diabetes mellitus, diabetes complications and vascular disease, metabolic bone disease, lipid and intermediary metabolism, obesity, and the genetics of diabetes.

GASTROENTEROLOGY AND HEPATOLOGY

The past academic year has continued to define the Division of Gastroenterology and Hepatology as a progressive center of excellence dedicated to advances in laboratory and clinical science, state-of-the art clinical care, and the education of future leaders in digestive and liver diseases. The Division’s nationally recognized Digestive Disease Center brings together a multidisciplinary team of gastroenterologists, surgeons, hepatologists, and radiologists to provide the highest level of comprehensive care for patients with a wide variety of digestive diseases and disorders.

The Division specializes in areas such as advanced endoscopy, inflammatory bowel disease, and motility disorders, and its educational program trains 12 fellows per year. In addition to being leaders in the clinic, Division faculty are also leaders in their respective areas of research which span such varied interests as biliopancreatic disease to esophageal reflux and inflammatory bowel disease, colon cancer and polyps, liver disease and liver failure, hepatopulmonary syndrome, nutrition and obesity, liver fibrosis, and liver transplantation.

HIGHLIGHTS & ACCOMPLISHMENTS CLINICAL In addition to establishing several new clinical offerings in just a few short years, the Division’s physician-scientists and research faculty are tirelessly working to expand our understanding of the most challenging digestive diseases afflicting patients today.

In August 2017, MUSC Health was once again recognized as high-performing in gastroenterology and GI surgery by U.S. News & World Report, meaning it ranked among the top 10 percent of hospitals in the nation for complex-care in that specialty.

Established more than 20 years ago, the liver transplant program at MUSC has some of the nation’s highest liver transplant survival rates and some of the nation’s shortest lengths-of-stay. Under the direction of Ira Willner, M.D., the liver transplant program has continued to expand, with over 100 transplants performed in the last year.

RESEARCH The past year has been a productive and successful time in the Division for both clinical and basic science research. Division faculty continue to address important issues in biliary and pancreatic medicine. Joseph Elmunzer, M.D., and team received NIH funding to establish the best method to prevent pancreatitis

after ERCP procedures. In addition, Gregory Cote, M.D., was awarded an NIH/NIDDK U34 grant to evaluate the impact of minor endoscopic sphincterotomy on the natural history of recurrent acute pancreatitis in patients with pancreas divisum.

Growth and progress have been an ongoing theme this year for the Division of General Internal Medicine—led by William Moran, M.D.— with notable achievements in education, clinical care, and research.

The Division encompasses a highly talented and committed faculty who are recognized nationally in the areas of medical education, ethics, clinical care, and research in health care disparities and outcomes. The Division continues to be recognized for its significant teaching efforts and leadership in the MUSC College of Medicine as well as its strong community partnerships and outreach programs. Most recently, successful recruitment has allowed for the continuing expansion of the Division’s efforts, promising an exciting year.

GENERAL INTERNAL MEDICINE

HIGHLIGHTS & ACCOMPLISHMENTS CLINICAL In FY17, University Internal Medicine (UIM) successfully renewed its Level 3 Patient-Centered Medical Home (PCMH) recognition, the highest award given by the National Committee for Quality Assurance (NCQA) to facilities for their patient-centered processes that focus on highly coordinated care and long-term, participative relationships.

Under the direction of Kim Davis, M.D., the Division’s UIM primary care team provided care for 43,000 outpatients in FY17 at its Rutledge Tower and East Cooper practices.

The Division expanded its geriatric program and outpatient services in FY17 with the recruitments of Mark Newbrough, M.D. (Geriatrics Section Head), and Amanda Overstreet, D.O., and successfully launched a program for geriatricssurgical co-management. Geriatrics services will be further expanded in FY18 with the recruitment of Julianna Marwell, M.D., and an advanced practice provider.

With leadership from Patrick Coyne, MSN, the Division continued to expand its robust Palliative Care Program offering both an inpatient consult service and an outpatient clinic at MUSC Hollings Cancer Center. In FY17, the palliative care team conducted over 300 pediatric consults

and over 2,000 adult consults. Palliative care services will be further expanded in FY18 with the recruitment of additional advanced practice providers and faculty, including Elizabeth Higgins, M.D. RESEARCH In FY17, the Division’s Health Services Research and Policy section is now over 95% funded with a diverse group of grants and contracts, and a budget that now exceeds $1 million.

Under the direction of Carolyn Britten, M.D., the Division of Hematology/ Oncology has continued to enjoy national and international prominence for its impactful contributions to the clinical, research, and education missions in FY17. As the largest academic cancer center in South Carolina, the MUSC Hollings Cancer Center (HCC) is the state's only National Cancer Institute (NCI)-designated cancer center and one of only 69 NCI centers in the U.S. As an integral component of the Hollings Cancer Center, the Division builds upon a long history of excellence in hematology and hematological malignancies, as well as solid tumor oncology and bone marrow transplant.

Division faculty are leaders in the fields of cancer care and blood disorders.

HIGHLIGHTS & ACCOMPLISHMENTS CLINICAL In August 2017, MUSC Health was once again recognized

by U.S. News & World Report as one of the country’s top 50 hospitals in the treatment of cancer.

In FY17, the Blood and Marrow Transplant Program at Hollings underwent a successful three-year accreditation renewal by the Foundation for the Accreditation of Cellular Therapy (FACT).

This accreditation applies to all services and facilities at Hollings that were inspected by FACT, specifically allogeneic and autologous hematopoietic progenitor cell transplantation, marrow and peripheral blood product collection, and cellular therapy product processing.

The Division’s Bone Marrow Transplant Program achieved Blue Cross Blue Shield (BCBS) Distinction Plus status. Through the Blue Distinction® Specialty Care program, hospitals are recognized as Blue Distinction Centers for their proven expertise, high-quality care, and patient results.

RESEARCH In FY17, the Hollings Cancer Center and Department of Psychiatry and Behavioral Sciences received an $8 million

U54 grant from the National Institute of Minority Health and Health Disparities and the National Cancer Institute to develop the MUSC Transdisciplinary Collaborative Center (TCC) in Precision Medicine and Minority Men’s Health.

The first research project—led by Chanita Hughes-Halbert, Ph.D., Professor of Psychiatry and Behavioral Sciences and SmartState AT&T Distinguished Endowed Chair in Cancer Equity (principal investigator), and Michael Lilly, M.D., Professor of Developmental Cancer Therapeutics and Associate Director of Translational Research—is focused on a new vaccine for prostate cancer (PROSTVAC®) in men at high risk for cancer recurrence.

Sara Giordano, M.D., Michael Lilly M.D., and John Wrangle, M.D., were awarded funding from the Hollings Cancer Center (HCC) Clinical Scholar Program to pursue research endeavors. The HCC Clinical Scholar Program is specifically intended to provide outstanding assistant, associate, and full professors dedicated time to focus on cancer-related clinical investigations. Clinical Scholars receive 20% salary funding for up to two years and an additional $5,000 in discretionary funding for research-related purposes.

INFECTIOUS DISEASES

Under the direction of Cassandra Salgado, M.D., MS, the Division of Infectious Diseases has seen continued growth and expansion across its clinical programs, research, educational activities, and public health initiatives. The Division’s clinical programs include all aspects of infectious diseases, from common community-acquired infections to life-threatening diseases in those with compromised immune systems.

Division faculty provide leadership for a variety of MUSC hospital and public healthoriented services, including infection control and hospital epidemiology, antimicrobial stewardship, vascular access, and transplant infectious diseases. For more than 25 years, the Division has provided comprehensive care and services to HIV-infected adults, with key funding support from the HRSA Ryan White Program and SC DHEC. The research programs in the Division continue to build on the long-standing strengths in HIV/AIDS, tuberculosis, antimicrobial resistance, geriatric and nosocomial infectious diseases. In addition, the Division maintains its commitment to educating the next generation of physicians and scientists to meet the many evolving challenges involved in fighting infectious diseases. Most recently, the Division has been energized with recruitments designed to advance clinical programs and subspecialty training programs, clinical and laboratory research, and global health programs.

INFECTIOUS DISEASES

RESEARCH

With the arrival of Scott Curry, M.D., Assistant Professor of Medicine, the Division has continued to expand its nationally recognized Infection Control and Hospital Epidemiology Program. Under the directorship of Cassandra Salgado, M.D., Curry serves as the Associate Hospital Epidemiologist. His research is supported by an NIH K23 award to study the role of asymptomatic carriers in the transmission of C. difficile within hospitals. Clostridium difficile is now a leading cause of hospital infections, often associated with debilitating consequences, and high costs of care. Curry’s work will lead to better prevention efforts, sparing the most vulnerable patients under our care of this disease.

The Division continues to provide key leadership for the MUSC Center for Global Health (CGH), a University-wide initiative which provides pilot grants and support for a variety of international projects related to infectious diseases.

Most recently, under the direction of Eric Meissner, M.D., Ph.D., the Division partnered with the MUSC Biomedical Informatics Center to develop an HIV clinical data repository that will greatly enhance the ability to study the epidemiology of the disease and, more importantly, lead to improvements in the local treatment cascade of diagnosis, linkage, and retention in care. The database also offers trainees and junior faculty the opportunity to conduct patient-centered outcomes research with appropriate mentorship.

The Division’s global health activities will be further strengthened and expanded in FY18 with the recruitment of Susan Dorman, M.D., Professor of Medicine, who will lead a new joint global health initiative between the Division and MUSC’s Center for Global Health (CGH). Through this program, Dr. Dorman will work to expand the infectious diseases research portfolio related to global health, with initial emphasis on tuberculosis as well as Divisional strengths of HIV, infection control, and antimicrobial resistance.

The program will also expand international research and clinical opportunities for MUSC trainees. Additionally, as the recipient of an NIH K24 mid-career investigator award, Dr. Dorman will provide mentorship for trainees who are interested in pursuing related global health training.

NEPHROLOGY

The Division of Nephrology at MUSC has a long and notable history in the study of and potential treatments for kidney disease. Under the leadership of Joshua Lipschutz, M.D., the Division enjoyed another productive year marked with accomplishments in education, research, and patient care. With a team of nationally renowned faculty, the Division of Nephrology is consistently among the highest-ranked programs in the country.

Clinical faculty provide a complete range of consultative, diagnostic, and treatment services for patients with kidney diseases. The scope of care encompasses all stages of kidney disease, from earliest detectable changes in kidney function through endstage renal disease. In addition, faculty members are actively involved in innovative basic science, outcomes, and clinical research aimed at better treatment options as well as prevention of kidney-related health issues.

Active collaborations between clinicians and basic scientists are ongoing, and collaborations with faculty from other divisions and departments are common. The Division hosts a flourishing fellowship program that supports 10 fellows. The publications, honors, and grant funding received by Division faculty this year highlight the quality and ingenuity of work being done in the Division to advance cures for kidney disease through scientific discoveries, cutting-edge treatments, and renal transplants.

Over the past year, Division faculty cared for approximately 600 chronic dialysis patients in eight DCI outpatient hemodialysis units, the DCI home dialysis program, and the Ralph H. Johnson VA Medical Center (VAMC).

The Division remains one of the most active inpatient consult services, with five services at MUSC and the Ralph H. Johnson VAMC. Under the direction of Milos Budisavljevic, M.D., the Division also runs the MUSC plasmapheresis unit.

Zipporah Krishnasami, M.D., and David Ploth, M.D., continue to provide nephrology care, education, and research in Tanzania, Africa.

Established more than 30 years ago, the kidney transplant program at MUSC has continued to expand. Under the direction of transplant surgeon Derek DuBay, M.D., and Transplant Nephrology Section Chief, Vinaya Rao, M.D., the renal transplant program performs nearly 300 transplants per year, rating it in the top 10 most active transplant programs nationally.

RESEARCH The Division of Nephrology is proud of its diverse bench, translational, and patient-oriented investigations in autosomal polycystic kidney disease (ADPKD), acute kidney injury, glomerular disease, renal physiology, human and animal genetics, and the role of the renin-angiotensin system in renal disease.

Major new funding awarded to Division investigators in FY17, includes the following: Joshua Lipschutz, M.D., was the recipient of a three-year Collaborative Sciences Award in conjunction with Chip Norris, Ph.D., (MUSC), and Simon Body, M.D., (Harvard) from the American Heart Association to study the role of cilia in bicuspid and calcific aortic valve disease; and Seok-Hyung Kim, Ph.D., received an NIH COBRE project award.

Under the leadership of Lynn Schnapp, M.D., the Division of Pulmonary, Critical Care, Allergy, and Sleep Medicine continues to be recognized nationally for excellence in clinical care, cutting-edge research, and innovative educational programs. With a team of nationally renowned faculty, the Division offers a wide range of clinical services, including general and specialized ambulatory clinics, inpatient pulmonary consultation, interventional bronchoscopy, and critical care services. The Division supports a broad-based research enterprise from basic to clinical and translational science, working collaboratively to develop novel therapeutic approaches for the treatment of pulmonary diseases.

The MUSC Sarcoidosis Clinic became the first clinic in the state to be recognized as a Foundation for Sarcoidosis Research (FSR) and World Association of Sarcoidosis and Other Granulomatous Disorders (WASOG) Sarcoidosis Center of Excellence.

Division faculty and fellows remain highly productive scholars, with 69 manuscripts published in the past year. Studies led by Lynn Schnapp, M.D., were featured in the American Journal of Respiratory Cell and Molecular Biology (AJRCMB) “Red Alerts” highlighted articles, and chosen for “APSselect” highlight.

RESEARCH Division investigators were successful in obtaining significant extramural funding in FY17. Specifically, Patrick Flume, M.D., garnered a number of new grants to augment his cystic fibrosis research program; Timothy Whelan, M.D., received industry support to study rituximab therapy in patients with idiopathic pulmonary fibrosis; Charlie Strange, M.D., and Tatsiana Beiko, M.D., were each awarded grants from the Alpha-1 Foundation; Dee Ford, M.D., MSc, FCCP, received a new NIH RO1 to study an “informed assent” approach toward in-hospital CPR in chronically ill patients with reduced life expectancy; Lynn Schnapp, M.D., received a grant from Ironwood Pharmaceuticals to study lung pericytes as targets for anti-fibrotic therapeutics; and Andrew Goodwin, M.D., MSCR, was awarded an NIH/ NHLBI K23 and a Center for Genomic Science pilot award to examine the role that extracellular micro RNA play in the

The Pulmonary Division was named Division of the Year for the third year in a row, and Nick Fox, M.D., was named Fellow of the Year at the 2017 Internal Medicine Resident Award Day. Autoimmunity has been a longstanding strength in the Department of Medicine. Under the leadership of Richard Silver, M.D., the Division of Rheumatology and Immunology has continued its strong tradition of excellence in patient care, innovative research, and outstanding training focused on the pathogenesis of autoimmune diseases.

The Division is internationally recognized for its care and research relating to scleroderma and lupus. HIGHLIGHTS & ACCOMPLISHMENTS CLINICAL MUSC Health was once again rated as high-performing in rheumatology by U.S. News & World Report in August 2017, meaning that it ranked among the top 10 percent of hospitals in the nation for complex-care in that specialty.

RESEARCH The Division maintains a robust research program and had another successful year securing support from a variety of external sources, including NIH, NHLBI, DOD, and the Lupus Research Alliance. In the past year, Division faculty published important research articles in leading medical and scientific journals such as the European Journal of Immunology, Nature Genetics, Annals of the Rheumatic Diseases, and Arthritis & Rheumatism.

Recent work by Betty Tsao, Ph.D., Professor and Richard M. Silver, M.D., Endowed Chair for Inflammation Research, identified an association between causal genetic variant in the NCF1 gene and multiple autoimmune diseases, and was published in the January 2017 issue of Nature Genetics. Titled

“A missense variant in NCF1 is associated with susceptibility to multiple autoimmune diseases,” the manuscript’s authors include Jian Zhao, Ph.D., Assistant Professor; Yun Deng, M.D., Assistant Professor; Diane Kamen, M.D., Associate Professor; and Gary Gilkeson, M.D., Professor.

New federal and major foundation grants awarded in FY17 highlight the spectrum of the Division’s research activities.

These include: Carol Feghali-Bostwick, Ph.D., garnered a new R01 to study the mechanisms of activity of a novel anti-fibrotic peptide; Gary Gilkeson, M.D., received a new NIH T32 training award to study factors related to ethnic disparity in systemic lupus erythematosus and scleroderma in African Americans; Diane Kamen, M.D., received a Midcareer Investigator Award to augment her lupus research and mentoring program; Tamara Nowling, Ph.D., received a DOD grant to study glycosphingolipids as therapeutic targets and biomarkers of lupus nephritis; and James Oates, M.D., received VA funding to determine mechanisms through which lupus serum leads to endothelial dysfunction and target those pathways.

Medical Education The Department of Medicine is deeply committed to the education and training of skilled clinicians, researchers, and educators bound to become the health care leaders of tomorrow. Our Internal Medicine Residency Training Program supports a diverse group of nearly 100 residents. The excellence and high quality of our residents are the hallmarks of our residency program. Residents work closely with our dedicated faculty, each other, and our students to create a thriving learning environment. Philanthropy The MUSC Department of Medicine believes that investing in the promotion of our faculty’s professional growth and the enhancement of learning for students is vital to the future of medicine. In order to maintain this high level of scholarship, we must compete for the most outstanding professors and physician researchers. The Department of Medicine currently has four endowed chair campaigns underway which pay tribute to several key faculty members whose contributions to MUSC have been exemplary.

Once established, these endowments will enable the Department to recruit the next generation of leaders. They will confer academia’s highest honor upon our most accomplished medical scientists and educators, allowing these dedicated individuals to pursue research that improves human health and cultivate the bright young minds who will lead future innovations in medicine and health care.

and physicians by both private insurers and governments. Although MUSC is a state-assisted medical center, safety net hospital, and medical university, the state of South Carolina provides less than 5% of MUSC's operating budget.

All of which means that gifts from donors have never been more critical to enabling MUSC to achieve its mission of providing extraordinary care and engaging in ground-breaking research that benefits the Charleston community and beyond.

We made a gift to MUSC not despite our loss, but because of it. Our daughter, Helen, passed away in November of 2015.

In our grief, my husband, David, and I were comforted by the knowledge that everything that could have been done for Helen was done at MUSC. But in the end, she was not able to overcome the many complications of Myhre Syndrome, a very rare connective tissue disorder.

We wanted to find a way to express our deep appreciation to the physicians and staff in the Department of Medicine for the extraordinary and compassionate care provided to Helen. We knew our gift would help others at MUSC. But a most unexpected side effect is the tremendous sense of comfort and healing the gift has provided—to us.

And the healing continues. Knowing on a very personal level how rewarding charitable giving can be led me to volunteer at MUSC. I have learned much over the past year about MUSC’s groundbreaking research—research that could one day lead to better therapies and even cures for many life-threatening diseases. Maybe even—one day—the complications of Helen's disease. The future of medical research has never been brighter.

I have also learned that the environment for funding that research has never been more challenging. Almost daily, there are news reports of decreased reimbursements to hospitals

Because of gifts from donors, MUSC can attract the highest quality physicians, faculty, and scientists who are doing vital research that is brimming with promise. Just as throwing a pebble in the water creates a ripple effect extending far beyond the original entry point, gifts to MUSC have an enduring impact benefiting us and our loved ones—and continuing well past our own lifetimes.

We chose to use our gift to MUSC to establish a lectureship in Helen's name. The lectureship fund will enable the Department of Medicine to invite a renowned leader in the field of rare pulmonary diseases to deliver an annual lecture and spend time with faculty, fellows, residents, and students.

In addition to making a current gift to MUSC, we decided to make a planned gift of life insurance proceeds, with the goal of increasing the value of the original gift and establishing a permanent legacy for Helen.

Our hope is that the lectureship will bring greatly needed awareness and knowledge about rare diseases to the MUSC community and will perpetuate Helen's memory and her joy for life despite her shortened lifespan.

And as David and I have already learned, the ripple effect of a gift can extend very positively back to the giver.

Donald O. Castell, M.D., Endowed Chair in Gastroenterology The Department of Medicine's strong tradition of inquiry and discovery would not be possible without the generosity of our many loyal donors who believe in the mission of academic health care.

Gifts to support the Department help advance medical science, develop new treatments and cures, train new generations of physician-scientists, and improve the health of those in Charleston, South Carolina, and beyond. We extend our deepest thanks to those individuals, foundations and corporations who have made a significant impact through their profound generosity.

Donald O. Castell, M.D., Endowed Chair in Gastroenterology Donald Castell, M.D., Professor of Medicine and Director of MUSC’s Esophageal Disorders Program, has been a beloved and highly-respected clinician, educator, mentor, and researcher since his arrival at MUSC nearly 20 years ago. As a tribute to his exceptional legacy and to ensure future support for research and teaching in his field, the Department of Medicine has initiated a campaign to establish the Donald O. Castell, M.D., Endowed Chair in Gastroenterology.

The endowed chair will enable us to recruit a leader in gastrointestinal disorders and assist in the training of GI fellows at MUSC as well as visiting fellows. Funded through philanthropic gifts, endowed chairs are an important tool for recruiting and retaining world-class, nationally recognized scholars. They provide a dedicated source of funds in perpetuity to support research, teaching, and service activities of the chair holder.

“I can think of few other people in gastroenterology, or medicine for that matter, who have contributed so much. Dr. Castell’s mentorship and guidance, especially in scholarly endeavors, have helped countless residents, fellows, and junior faculty over the years,” says Department of Medicine Chair Don C. Rockey, M.D. “I can think of no better way to recognize his contributions to the field, and, more importantly, to all of those he has influenced, than by the development of the Don Castell Endowed Chair in Gastroenterology.”

Throughout his illustrious 45-year career, Castell has received numerous national, regional, and local honors, including the Bethesda Naval Hospital House Officer Teaching Award in 1963 (during his residency), the Basic Science Educator Award from Wake Forest Medical School in 1979, and the Civilian 40

Educator of the Year Award from Uniformed Services University of Health Sciences also in 1979. In 2001, he was named Clinical Educator of the Year by the American Gastroenterological Association (AGA).

“Don is what the best of teachers and mentors strive to be. He perfected the art of educating and building the next generation of physicians and clinical researchers.” —Amine Hila, M.D. His service to and leadership of the AGA, Dysphagia Research Society (DRS), American College of Gastroenterology (ACG), American Neurogastroenterology and Motility Society (ANMS), and other international organizations are well known and acknowledged. In 2013, he received the AGA’s highest award, the Friedenwald Medal recognizing a lifetime of achievement in gastroenterology.

Castell is internationally recognized as a leading authority on diseases of the esophagus and esophageal function, and has authored or co-authored more than 500 scientific publications.

According to long-time GI colleague, Reza Shaker, M.D., “Don’s pioneering studies in esophageal physiology and pathophysiology shaped our thinking in many aspects of this

field. His dedication to training the next generation of esophageal investigators has set the standard.”

While Castell is celebrated for his extraordinary patient care and his landmark research, perhaps his greatest legacy is the mentorship he provided to hundreds of trainees across multiple institutions throughout his career. Many of these physicians continue to seek his advice long after their training is complete.

“I consider the fact that I worked for 7 years directly under Dr. Castell's supervision, leadership, and mentorship as one of the greatest blessings that I have ever had,” says Amine Hila, M.D., who trained under Castell as a resident at Graduate Hospital and then as a GI fellow at MUSC. “Don is what the best of teachers and mentors strive to be. He perfected the art of educating and building the next generation of physicians and clinical researchers." Originally from Tunisia, Dr. Hila currently practices at Binghamton Gastroenterology Associates in New York. He and his wife made a significant early gift to the endowed chair fund. “By donating to this chair in Don Castell’s honor, we hope to support other faculty members who will make similar valuable contributions to clinical care, education, and research at MUSC.” Thanks to a generous contribution from Dr. Castell and his wife, Linda, as well as contributions from his former trainees and colleagues, the Castell Endowed Chair is well on its way to becoming a reality. “This endowment is not just a personal honor, but also a gift back to the Division of Gastroenterology and Hepatology,” says Castell.

“Investments in education are fundamental to fostering leadership and long-term progress. I am very pleased to help fund this endowed chair so that it can support an MUSC faculty member to be a leader in the field of gastroenterology, and simultaneously enhance his or her own education, while shepherding the education and research of future physicians.”

On September 23, 2017, Don Rockey, M.D., Department of Medicine Chair, announced the initiation of a campaign to establish the Donald O. Castell, M.D., Endowed Chair in Gastroenterology at a special Tribute Dinner in honor of Dr. Castell's legacy.

THE MEDICAL UNIVERSITY OF SOUTH CAROLINA Founded in 1824 in Charleston, The Medical University of South Carolina is the oldest medical school in the South. Today, MUSC continues the tradition of excellence in education, research, and patient care. MUSC educates and trains more than 3,000 students and 700 residents, and has nearly 13,000 employees, including approximately 1,500 faculty members. As the largest non-federal employer in Charleston, the university and its affiliates have collective annual budgets in excess of $2.4 billion. MUSC operates a 700-bed medical center, which includes a nationally recognized Children’s Hospital, the Ashley River Tower (cardiovascular, digestive disease, and surgical oncology), Hollings Cancer Center (one of 68 National Cancer Institute designated centers), Level I Trauma Center and Institute of Psychiatry.
