Project Management Survey Form This survey form gives you the chance to help us describe how our projects and programmes work, so that we can 'tell the story' of what Moving Mountains does. You have the expert local knowledge and years of experience running our projects and programmes: we need you to help us describe how our projects and programmes work so that we can improve the way we measure and report on our success.
The Logical Flow: Every project and programme we run should show a logical flow as outlined below:

What are our targets for this programme
This section is not complete: Through observation – are people in the community generally more healthy? Number of visits to the clinic? (if it is more accessible then more people will visit the clinic) Reports from the doctors / nurses on both the type of patients that are coming and their medical problems Attendance in school of the children in the community

What is the wider goal? ◦ What more general development impact are we trying to contribute to through this project/programme? ◦ How does this project/programme fit with our wider development strategy for the region and that of the local authorities or government?
This clinic helps the government in providing healthcare as from this community government facilities are a long distance away which therefore discourages people from visiting. This clinic helps to cover a region that the government does not reach. In relation to the UN MDG this project helps in achieving: 1. Reduce child mortality 2. Improved maternal health 3. Combat HIV/AID, Malaria and other diseases This project fits into wider long-term development by improving health. Ensuring that a community is health increases the ability of a child to get an education and an adult to participate in employment. Good health enables people to be active agents within their families, communities and further afield so that they can engage with and be drivers of social, political, health, environmental, cultural and economic change.
