A+ % How do we measure change?
1. Trust of local people in clinic: 90% of patients report a good or excellent service in quarterly review of evaluations 2. Number of people seen by a doctor per year: records kept by the clinic (compare with baseline figure for average visits to see a doctor) 3. Incidence of health problems: records kept by the clinic. Identification and monitoring of most prevalent health issues conducted by doctor and medical staff on an ongoing basis with consequent targeted initiatives aimed at reducing these each quarter
What key information do we need to monitor to know whether we are achieving the changes we would like to achieve?
4. Amount spent on healthcare per family per year: aim for a reduction from baseline average (there may not be a reduction overall since the clinic will provide a service which wasn't previously available to many)
What are our targets for this programme
5. Amount of time spent travelling to access healthcare/doctor: reduction from baseline average 6. Health workshops and information sessions provided: records of the workshops and sessions provided by the clinic are recorded and kept by clinic, including number of participants attending each session, objective, results
