RN – Health and Wellness Program Coordinator

Summary: The FSST Health clinic is seeking an RN-Health and Wellness Program Coordinator. This posi@on is FT (40 hours per week), Mon-Fri, and 8-5pm. This posi@on provides comprehensive nursing services to pa@ents who are referred for these services. Coordinates pa@ent health care programs and services as it relates to diabetes, preven@on and overall wellness. Maintains the public’s health by providing health services and educa@on. Supports clinical leadership in both safety programs and quality assurance.

Educa@on and/or experience: Bachelor’s degree in Nursing or Associates degree in Nursing with prior and related experience. Current RN license. 2-5 years’ experience in ambulatory care preferred, previous program coordina@on and/or case management experience.
