NJNG teamed up with MPCF in 2017 to offer an additional funding opportunity to local non-profit organizations that collaborated on a program or project, building on each agency’s strengths, to increase service capacity in an innovative way. In 2019, the following three programs were each awarded $5,000 through the Accomplish More Together Grant initiative: 1. HEALTHIER AT HOME
CentraState Medical Center and Caregiver Volunteers of Central Jersey
Frail adults receive three months of follow-up support by trained Caregiver volunteers, including weekly home visits, reassuring calls, and grocery shopping upon their discharge from CentraState Medical Center. Volunteers receive specialized training by the hospital staff.
