President's Post Vicky Garrison Editorial

Greetings to everyone as we enter one of my favorite times of the year! For some, this season unites us in celebrating our faith in God. For others, spending time with loved ones and friends may be a highlight of this season. Holidays are not always a favorite for many people. Some may be painfully grieving the loss of someone close, and many others face undesired solitude during this month. If you know somebody struggling this month, please brighten their life with a call, visit, or mail.

December brings a wrap-up to the year, allowing a time to reflect on highs and lows, good and bad, and things that worked and those that didn't. I encourage you to give yourself and others grace as you remember the low times. Learn from mistakes and then let go of toxic emotions that can stand between you and your joy. The only person in charge of your joy is you!

Celebrate the challenges you rose to overcome, and take time to appreciate the small things that made you smile this year. If you can't think of any, consider keeping a Gratitude Journal next year. Tracking what we are thankful for helps us recognize that life is not always strife. Much of the time, the cumulative small things throughout the year, often forgotten, far outweigh the rough spots.

And finally, as we reach the end of 2023, I'd like to thank and honor our members, families, and caregivers. Your perseverance, commitment, and compassion are truly commendable.

For those who celebrate as I do, Merry Christmas! For everyone - have a safe and happy holiday season!

PVA Regional Office Updates

HOLIDAY WISHES FROM JASON

I just want to take a moment to wish everyone a wonderful Holiday season.

Reflecting on 2023, as many of you know, for me, it didn't start very well.

I want to thank the Chapter, especially Vicky, for the support that was shown to me during the unexpected loss of my 45-year-old brother back in January. The flower bouquet displayed at his funeral from the Chapter was beautiful and deeply appreciated by my family. I also want to take time to acknowledge those members we lost during this past year and their families. They will not be forgotten.

From a benefits side, during 2023, many of you were able to obtain benefits from new programs such as the PACT Act, Family Caregiver Program, Travel eligibility, Ancillary benefits, a second auto grant in the future, or, in some cases now, increase in established benefits, grants from long-standing appeals, and sustained independence from what the VA SCI Center had to offer. It was a pleasure and continues to be a pleasure being able to advocate on everyone's behalf. I am also very pleased to have Ellie, the Regional Office's new Administrative Assistant, on board. She is quickly becoming an invaluable asset.

2023 was very productive in my role as a Senior Benefit Advocate (SBA) here at the SCI Center. Looking back on this year's productivity gives me a reason to exhale a little bit and validate why I felt so busy with my role as an SBA and my collateral responsibilities as a Region Training Coordinator. As we close out the year, I will be coming up on directly assisting in some sort of capacity close to 300 clients, the majority being members of the Minnesota chapter. In the past, the average is around 225.

I am also expected to surpass over $2,000,000 in new monetary benefits, separate from what Tami's office obtains. I realize that does not compare to the regional office's productivity, but for a hospital-based NSO, it is a high dollar amount. I am also approaching over 900 various types of contacts, including interviews, phone calls, emails, conferences, and claims, whether monetary or hospital.

This past year, the SCI Center made a great selection in hiring Dr. Himmler, the new SCI Chief. It was also great to see various other positions get filled that greatly benefit the Chapter. I am excited to see the resumption of chapter activities, such as in-person board meetings, Pizza night, and other activities that benefit those admitted.

I am really looking forward to what 2024 brings as we know our work as Service Officers will never be finished, and I will continue to work very hard on everyone's behalf and further collaborate with the Chapter as it is not mistaken upon me as to you all depending on me. But as we close out 2023, I wanted to take a moment of reflection and wish everyone a wonderful holiday season.

Walking Again!

This past summer, after years of following the development of exoskeletons for use as a mobility device, I decided to inquire. I'd always felt that an exoskeleton would be too cumbersome and slow, but with an open mind, I took the plunge.

My logic was that even if it isn't functional, it could give me the ability to be upright AND mobile for enough time each week to provide health benefits.

The initial appointment consisted of measurements, vitals, and a standing frame trial to ensure I could tolerate this position for a prolonged time. After being cleared, I was ready to start! After researching the different exoskeletons available, I chose the Indego. If medically cleared for a device, the choice of device is very person-specific. There are a couple to choose from at the VA, but the most important thing to remember is that everybody is different. The training time in the clinic can vary from person to person.

PT told me to expect 20-30 one-hour sessions, which would include training for my "walking companion," in my case, my husband, Mark. At that point, I would be issued one to trial at home for three months.

In my first session, I successfully got up and walked. Although it was robotic-like and very unnatural at first, I must admit I was hooked! I felt exhilarated actually to be walking!!! By session 12, I was going home with a trial unit. One week into the trial and I’ve logged over 10000 steps. I’m excited to see this number rise!

The entire process of getting a trial proceeded without any hitches. I attribute much of that to the skilled team of PTs at the Minneapolis VA SCI/D. I remain very grateful for their dedication to improving the lives of the veterans they serve!

Volunteer Opportunities The Minnesota Chapter will host the 2024 National PVA Spring Convention in May 2024. We’re recruiting volunteers to help us in a variety of ways. Please contact us if you would like to be a part of this spectacular event as we welcome other PVA members from all other locations.

Hello, I am (RET) SFC Todd Fringer, a multifaceted individual who combines the discipline and experience of a military career with the creativity of an artist and author. With a distinguished service background, I have been a proud member of the Paralyzed Veterans of America (PVA) for 18 years, a journey that began immediately following my qualification.

My work is currently on display in the Day Room at the SCI-D. My artistic journey is complemented by my achievements as a published and award winning author. My writings offer insights drawn from my life's challenges and triumphs, resonating with a wide audience that finds inspiration in my narrative.

Balancing the rigor of military discipline with the expressive freedom of the arts, I stand as a testament to the diverse paths that life can take and the resilience of the human spirit. Inpatient at SCI-D since February 2021.

Title: Together In Arms: The Power of Connection Among Veterans by: Todd Fringer

As we step into the festive season, a time of joy and togetherness, it's essential to remember the bonds that hold us united as veterans. The camaraderie and brotherhood we experienced while in service do not end with our active duty. They transform into a lifelong commitment to support and uplift each other, especially during times when one of us might feel alone or forgotten.

The holidays, though filled with cheer, can also be a period of introspection and sometimes, loneliness. This is where the true spirit of our service comes to light - in the form of a simple, yet profoundly impactful act: the Buddy Check. It's an extension of the care and watchfulness we practiced in active duty, a check-in to ensure our fellow veterans are not just surviving but thriving.

A Buddy Check is more than a routine call. It's an opportunity to genuinely connect, to ask, "How are you really doing?" and then listen. Really listen. It's in these moments of quiet understanding and shared experiences that we often find the unspoken words, the hidden struggles, and the silent calls for help.

This holiday season let's remind our Brothers and Sisters in Arms that they are not alone.

That the thousands of us who share this unbreakable bond are here, just a phone call away. It's about letting them know that their well-being matters to us, that we care deeply, and that we are here to support them, just as we did during our time in service.

By showing this care and attention, we do more than just check on each other; we reinforce the support network that is vital for our collective well-being. We mirror the commitment and dedication that defined our time in active service, now channeling it into ensuring that none of us has to face challenging times alone.

So, this holiday season, let's pledge to reach out. A simple call can make a world of difference. It's about being there, being present, and showing that the spirit of service extends far beyond the field - it's embedded in every act of kindness, every listening ear, and every reassuring word we share among ourselves.

MNPVA wants to make it easier for members to attend activities in their communities and just for fun!

Reimbursement is available for activities like concerts, sporting events, and theater!

Reimbursement may include the cost of admission, lodging, transportation, meals/food, non-alcoholic beverages, and parking for MNPVA members. Reimbursements cannot be made until event date has passed and attendance confirmed by signature on form.

Itemized receipts required and must be accompanied by a signed reimbursement request form. Some restrictions apply.

Bone as Regulator of Energy Balance and Male Fertility after SCI: A Pilot Study University of Minnesota researchers are seeking participants with or without a spinal cord injury for a clinical trial. We are interested in how bone health affects insulin sensitivity and sperm health in adult males with spinal cord injuries. Participation in the research will take approximately 5 hours over 3-4 visits. Compensation of $200 will be provided for participation.

Researchers at the University of Minnesota (UMN) are conducting an anonymous online survey to gather information about individuals living with spinal cord injury (SCI) who have used psychedelic drugs after their SCI. The Brain Body Mind Lab is recruiting participants with complete or incomplete spinal cord injury to compare the effect of Remotely-delivered Cognitive Multisensory Rehabilitation versus Remotely-delivered Exercise to improve sensation and movement after a spinal cord injury.

ELIGIBILITY CRITERIA

•Be 18-75 years of age •Be able to hear and comprehend the English instructions given in the study •Have a complete or incomplete spinal cord injury (>1year) •Be able to self-transfer with some assistance and push a call button •Have an available care-partner to assist with the interventions •Not be pregnant •Be medically stable and no MRI contraindicationsParticipants will: •Either receive 12 weeks of physical therapy with Remotely-delivered Cognitive Multisensory Rehabilitation OR Remotely-delivered exercise, 3x/week, 45min/session for 12 weeks •Have 2 MRI scans of the brain + 3 clinical assessments •Fill in questionnaires related to function, quality of life, and body awareness

VA Research Study seeking veterans with spinal cord injuries who use wheelchairs and who use a lift to transfer to their wheelchair.

The purpose of this research study is to assess and refine our CMAP system (Comprehensive Mobile Assessment of Pressure), a seated pressure mapping system. Participation will last about 4 weeks.

Participants must have a smart phone and a home wifi connection.

Participants will use a pressure mapping device in their wheelchairs at home – this device sends seating information to a research therapist who will monitor activity and make recommendations for pressure injury prevention.

Participants will complete surveys and participate in interviews.

The study provides up to $100 compensation to participants.

JUSTICE FOR ALS VETERANS ACT MARKED UP On November 29, the House Veterans’ Affairs Subcommittee on Disability Assistance and Memorial Affairs marked up several pieces of legislation, including the Justice for ALS Veterans Act (H.R. 3790). This legislation would provide eligible survivors of veterans who died of service-connected ALS the additional monthly Dependency and Indemnity Compensation (DIC) benefit typically known as the DIC kicker. The next step is for the full committee to consider the legislation.

HVAC CHAIRMAN HONORED WITH MANSFIELD AWARD On November 15, PVA National President Robert Thomas and Chief Executive Officer Carl Blake awarded the 2023 Gordon H. Mansfield Congressional Leadership Award to House Veterans’ Affairs Committee Chairman Mike Bost (R-IL). PVA established this award to recognize strong leadership and support of our legislative priorities in the areas of veterans’ health care, benefits, and disability civil rights. Chairman Bost’s support was instrumental in the passage of several pieces of legislation important to PVA members, like the Veterans Auto and Education Improvement Act of 2022, which provides additional automobile allowances to eligible veterans. He also played a key role in ending the requirement for disabled veterans to unnecessarily

reapply for VA clothing allowances year over year, thereby demonstrating his commitment to ensuring veterans get the benefits they earned.

HISA BILL INTRODUCED IN THE SENATE Senators Catherine Cortez Masto (D-NV) and John Boozman (R-AR) recently introduced legislation in the Senate that would help disabled veterans and their families make accessibility and safety improvements to their homes. S. 3290, the Autonomy for Disabled Veterans Act, seeks to establish a single grant rate of $9,000 for VA’s Home Improvements and Structural Alterations (HISA) grant program, and then tie that rate to the same index the department’s other home modification programs use to calculate annual inflationrelated increases. The new rate would be provided to future grant applicants, and be payable regardless of the disability status (service connected or not).

Currently, the lifetime HISA grant benefit is worth up to $6,800 for veterans who need a housing modification due to a service-connected condition. Veterans who rate 50 percent service-connected may receive the same amount even if a modification is needed due to a nonservice-connected disability. Veterans who are not service-connected but are enrolled in the VA health care system can receive up to $2,000. These rates have not changed since Congress last adjusted them in 2010, even though the cost of home modifications has risen nearly 50 percent since that time. Years of inattention

x have diminished the effectiveness of this program, and it is long past time to update grant rates to realistic levels.

We appreciate the efforts of Senators Cortez Masto and Boozman to correct that by increasing grant rates and tying them to a formula, so they remain current for years to come.

VA RELEASES 2023 NATIONAL VETERAN SUICIDE PREVENTION ANNUAL REPORT The 2023 National Veteran Suicide Prevention Annual Report has been released by the VA. The report provides suicide data among veterans and non-veteran U.S. adults. This year, the report includes the first full year of data that was impacted by the COVID-19 pandemic.

In 2021, 6,392 veterans died by suicide, which is an increase of 114 veterans above 2020. However, the report does highlight several areas of improvement.

Suicide rates for men over the age of 75 fell by 8.1 percent and rates among men aged 18-34 fell by 1.9 percent. As with previous annual reports, veterans with recent engagement with the Veterans Health Administration and the Veterans Benefits Administration saw lower numbers of suicide.

There were several impacted groups in 2021 that the report calls out including women veterans, American Indian and Alaska Native veterans, homeless veterans and justice involved veterans. These populations all saw an increase of suicide rates in 2021.

The report also points out the need for a “whole-ofnation public health approach to veteran suicide prevention.” During 2021, suicide increased among the civilian population as well, likely due to the stress of the COVID-19 pandemic.

In response to the report, House Veterans’ Affairs Committee Chairman Mike Bost (R-IL) put out a press release where he said, “As a veteran myself, along with my fellow veterans on our Committee, I know that it is a top priority of ours to continue breaking down the stigma that surrounds mental health, and expand access to assistance that works for every single veteran. We need our veteran community to know that help is out there, and under my leadership we will continue to

explore new methods and initiatives to end this crisis and make a difference in the sad statistics we continue to see year after year. Whether you’re dealing with financial hardship, substance abuse, PTSD, or anything else, the resources are there to get back on track and there is absolutely no shame in asking for a helping hand to get there.”

LEGISLATION INTRODUCED TO PROMOTE ACCESS TO MODERN ASSISTIVE DEVICES Advancements in technology can provide life-changing options for veterans with spinal cord injuries and disorders (SCI/D) and a small group of Representatives want to make sure VA properly evaluates those who are clinically eligible for modern assistive devices that promote independence and mobility. H.R. 6373, the Veterans Spinal Trauma Access to New Devices Act (Veterans STAND Act), which was introduced by Representative Jack Bergman (R-MI) and three other members, codifies the requirement that VA offer SCI/D veterans an annual assessment focused on their spinal health. It then takes steps to improve this program by requiring the department to actively conduct outreach to SCI/D veterans on the benefits of having such an annual evaluation and ensuring that consideration of assistive technologies, like personal exoskeleton devices, are part of these assessments. The Veterans STAND Act also directs VA to consult with assistance device experts when developing or changing relevant guidance, and requires biennial reports to Congress on the extent to which veterans are being prescribed and are using personal exoskeletons issued by the VA. The bill does not alter the Veterans Health Administration’s current guidelines on how veterans are assessed for assistive devices, but it helps ensure SCI/D veterans are given the opportunity to be legitimately evaluated for these lifechanging technologies.

HVAC SUBCOMMITTEE HOLDS HEARING ON APPEALS On November 29, the House Veterans’ Affairs (HVAC), Subcommittee on Disability Assistance and Memorial Affairs held a hearing titled, “Appeals: Ensuring High Quality Decision-Making for Veterans’ Claims on

x Appeal.” This oversight hearing focused on the appeals process at the Board of Veterans’ Appeals (BVA), with attention on areas such as quality assurance, improved metrics and outcomes, as well as decreasing the time an appeal takes to be appropriately adjudicated.

Many of the witnesses stressed the need to recognize the distinction between the legacy program and appeals under the Appeals Modernization Act (AMA). The AMA was passed in 2017, effectively changing the appeals process and debate exists if it has been implemented as Congress intended. Several concerns raised by Congress and the witnesses were issues that PVA has previously addressed with HVAC, such as unnecessary exams being ordered, too many remands, and a lack of transparency for veterans awaiting the appeals process.

The Government Accountability Office (GAO) testified and reiterated several recommendations to improve the appeals process within the BVA. GAO stressed the BVA should establish policies and procedures to gauge its accuracy rate, monitor how veteran law judges incorporate feedback received in the review process, develop and implement evidence-based decision-making processes that address the underlying causes for the most common errors found, and that the Chairman of the BVA should study how to evaluate veteran law judges’ consistency in adjudicating decisions.

HVAC HOLDS HEARING ON VA’S EHRM On November 15, the House Veterans’ Affairs Subcommittee on Technology Modernization Oversight held a hearing titled, “Electronic Health Record Modernization [EHRM] Deep Dive: System Uptime.” The hearing was a “deep dive” into ongoing challenges for users of VA’s new electronic health record (EHR) and how they are being addressed. Subcommittee Chairman Matt Rosendale (R-MT) expressed disappointment in VA’s EHR performance, including outages and usability.

Ranking Member Sheila Cherfilus-McCormick (D-FL) said she was disappointed Oracle Health did not have a representative at the hearing, after the witness scheduled to appear had to cancel.

Lawmakers turned to Kurt DelBene, Assistant VA Secretary for Information and Technology, and Laura Prietula, the VA’s Deputy Chief Information Officer overseeing the EHRM integration office, for answers.

DelBene said the VA is working to hold Oracle Health more accountable for system reliability. The VA increased financial consequences under contract renegotiations in May 2023 related to technical performance and the end-user experience. DelBene said the VA has seen improvements as a result but noted that there are still areas of significant concern: incidentfree time, not hitting standards, and end user responsiveness. With the activities and improvements that are now underway, VA leaders are optimistic about the eventual success of the current program reset, the deployment at the Lovell Federal Health Care Center in March 2024, and the future full implementation of the EHR throughout VA. PVA MEETS WITH DOJ TO DISCUSS ADA ISSUES PVA, along with other disability advocates, recently met with the Department of Justice (DOJ), Civil Rights Division to discuss pressing issues for the disability community. The Civil Rights Division investigates, enforces, and issues regulations for the Americans with Disabilities Act (ADA). Numerous topics were addressed during the meeting. PVA urged DOJ to issue standards for medical equipment and moveable furniture, including hotel beds. PVA also highlighted the need for transparency and recommended DOJ release public reports with the number of ADA complaints filed, the number of complaints DOJ investigated, and how the complaints were resolved.

NEWS OF NOTE FTA Requests Public Input on Research Goals The Federal Transit Administration (FTA) is gathering public input for research goals and innovation for the next five years through the Exploring the Future of Public Transportation Research: A National Online Dialogue. The virtual conversation aims to focus policies

x and practices to make public transit safer, cleaner, and greener and more resilient, inclusive, and equitable, including for people with disabilities. Members of the public can submit ideas, comment, and vote on ideas submitted by others. Online input will be accepted through January 5, 2024.

NIH Recognizes People with Disabilities as a Health Disparity Population On September 26, the National Institutes of Health designated people with disabilities as a health disparity population. A health disparity designation will help encourage research specific to the health issues and unmet health needs of people with disabilities.

DOJ Sues Colorado for ADA Violations in Nursing Facilities The U.S. Department of Justice (DOJ) recently sued the state of Colorado for unnecessarily segregating adults with physical disabilities in nursing facilities. A DOJ March 2022 letter of findings outlined violations under the Americans with Disabilities Act (ADA) and recommended reasonable modifications to the state’s long-term care system. The complaint alleges Colorado violates the ADA by discriminating against people with physical disabilities through unnecessary segregation and failing to provide services in the most integrated setting.
