THE BIGGER PICTURE:

ROBOTIC PROSTATECTOMY Kim Bakker and his wife Karen are avid motorcycle enthusiasts. Each year, they say goodbye to Michigan winters and head to Arizona, where the warm, dry climate is ideal for riding, and the clear night skies are picture-perfect for Kim’s astrophotography hobby – or addiction, as his wife teases. Toss in Kim’s part-time maintenance work at Castle Farms in Charlevoix, and it’s easy to see that retirement hasn’t slowed down this former Michigan Department of Corrections officer one bit.

So, when Kim went in for a routine physical earlier this year, he wasn’t expecting a diagnosis of prostate cancer. The healthy 69-year-old had no history of prostate cancer in his family. He felt great. But his screening blood test showed that he had an elevated prostate specific antigen, or PSA, a marker for prostate cancer.

“Often times there are no symptoms with prostate cancer, so one of the best ways for us to know is through a PSA blood test,” explains Matthew Rohloff, D.O., urologist at McLaren Northern Michigan.

When a biopsy confirmed the diagnosis, Kim was determined not to let cancer affect his active life.

“There are a number of ways to treat prostate cancer, based on the patient’s unique circumstances,” states Dr. Rohloff.

A new offering at McLaren Northern Michigan, robotic prostatectomy using the da Vinci Surgical System, is one of the options.

BETTER OUTCOMES, FASTER RECOVERY Specifically built for prostatectomies, the surgeoncontrolled robotic technology utilizes four separate “arms” that allow for maximum dexterity and precision in surgery. Equipped with a 3D, high-definition camera, the robot also allows surgeons to see the “bigger picture” during surgery.

“The camera allows for a highly magnified anatomic picture so detailed that we can visualize all the individual nerves and vascular structures that were previously too small to appreciate,” explains Dr. Rohloff.

More detailed visualization can help to spare nerves, in turn helping to minimize post-surgery side effects such as incontinence and erectile dysfunction.

“The fact that we are able to offer robotic prostatectomy at a community hospital is incredible.”

- Dr. Matthew Rohloff Other benefits of robotic prostatectomy include minimized blood loss, fewer infections, shorter hospitalizations, and a quicker return to normal activities.

“Three quarters of patients go home the next day,” says Dr. Rohloff. “In the old days, it was routine to need a transfusion and to stay in the hospital four or five days.”

Kim’s total time in the hospital was 36 hours. And three weeks post-surgery, Kim recovered to where he felt physically normal, with no pain from the surgery.

A UNIQUE SPECIALTY Robotic prostatectomy technology has been around since the mid-2000s, but few urologists specialize in robotic surgery.

“The fact that we are able to offer it here, at a community hospital, is incredible,” notes Dr. Rohloff.

“Cancer sucks,” says Kim. “But I feel extremely fortunate to have the medical attention I got from the urology department at McLaren. I don’t believe there’s a hospital in the country that could have provided better care than what I received from Dr. Bryan Shumaker and Dr. Matt Rohloff.”

This year, due to the surgery, motorcycle riding around the Arizona desert was off the table, but Kim and Karen exchanged their biking for hiking, even completing a couple 5k desert treks. And Kim’s lenses are still seeking out wonders across the night skies.

ENJOY OUR WATER WONDERLAND, SAFELY If you’ve experienced a Northern Michigan summer, chances are you’ve waded, splashed, swam or floated in one of the many bodies of water “up north” has to offer.

Northern Michigan has some of the best options for summer fun, from Lake Michigan, Lake Charlevoix, and Walloon Lake, to the many smaller lakes dotted throughout our beautiful region. Throw in the Inland Waterway, Michigan’s longest water trail, connecting nearly 40 miles of lakes and rivers from Petoskey to Cheboygan, and Northern Michigan has “jump into a lake” covered.

But whether you’re paddling down the Inland Waterway, boating on your favorite lake, river, or pond, or floating around your backyard pool, Joseph Weistroffer, M.D., orthopaedic surgeon at McLaren Northern Michigan Neurosciences, wants to ensure you do it safely.

“There are 26,000 neck injuries a year in the U.S. from diving into water that’s too shallow,” says Dr. Weistroffer. “And of those neck injuries, 800 – mostly teenagers and young men – end up in paralysis, a devastating injury that changes your life forever.”

SHALLOW STATISTICS “About 20 percent of neck injuries occur in people who are doing a trick dive or showing off to someone,” Dr. Weistroffer says. He also notes that many neck injuries occur when diving into shallow water, or water less than four feet deep.

Wind, water currents and watercraft create continuous shifts within a lake, pond or river bottom, resulting in shallow areas that can extend out away from shore into areas that previously had depth. Additionally, debris such as logs, sunken fixtures and boulders can move into areas, creating new obstacles.

“Bottom line, if you don’t know how deep the water is, don’t dive,” cautions Dr. Weistroffer.

DON’T DRINK AND DIVE Outdoor summer activities can also lend themselves to the use of alcohol, which can affect coordination, balance and judgment.

“Things can really change when you mix in alcohol with swimming, diving or any water sports,” says Dr. Weistroffer, who suggests waiting to imbibe until after you’ve had your fun in the sun.

“Drinking before or during water activities could lead to consequences you may never be able to forget.”

PLAY IT SAFE Whether you’re having fun on a boat, a beach, or a board, it’s important to respect our beautiful lakes, rivers and ponds. These tips will help keep you – and your family – safe while on, and in, the water: Swimming and Boating • Learn to swim! Enroll yourself and your kids in swimming classes • Learn CPR – your skills could help save a life • Wear life jackets that meet U.S. Coast Guard requirements regardless of the size of your boat or the swimming ability of the boaters • Don’t use air-filled or foam toys such as water wings or noodles in place of life jackets • Check your local weather conditions before heading to the water Beaching It • Supervise kids closely while enjoying beach days • Always swim with a buddy, preferably with a lifeguard on duty • If caught in a rip current, swim parallel to the beach to get out of the current, then swim toward shore • Beware of cold water; it can quickly sap your strength And remember, water intensifies the sun’s rays, so don’t forget the sunscreen!

DIVING INJURIES: MINIMIZE YOUR RISK • Know the water’s depth before you dive • Don’t dive in a lake or pond if you can’t see the bottom. Sand bars, logs or boulders may be present • Never dive into an above-ground pool – most are only four feet deep On a diving board: • Make sure the water is at least twice as deep as the height you’re diving from • Only one person on the board at a time • Don’t run on the diving board • Dive off the end of the board, not the sides • Bounce only once • After diving, move away from the landing area

NEW BEGINNING We all know the feeling of putting on those few extra pounds – the squeeze of our favorite jeans that seem to have somehow gotten “smaller;” the numbers on the scale staring back at us with their dismal verdict. When that happens, we know it’s time to curb our indulgences, dust off our exercise routines, and get back into those jeans again.

But for many people with morbid obesity – those with a body mass index (BMI) of 35 or higher, and who are more than 100 pounds overweight – losing weight is a battle that’s rarely won alone.

Alayna Wesener was one of those people.

“At 30, I felt my entire life was ruined. I needed a seatbelt extender on an airplane. I couldn’t cross my legs and I could barely tie my shoes. I also had unexplained infertility,” recalls Wesener, who at 332 8 | IN GOOD HEALTH

Over the last 30 years, obesity has become the most prevalent disease in the world. More people suffer from the health effects of obesity than any other disease.

Why? There are many factors, ranging from lifestyle choices such as overeating and not exercising, to other more complex reasons such as genetics, metabolic factors and socioeconomic factors.

THE OBESITY EPIDEMIC Obesity is diagnosed by BMI, waist circumference and other symptoms. A BMI over 25 is considered overweight, and over 30 obese. Here in the U.S., one out of every three adults age 20 and older is obese, according to the Harvard School of Public Health1.

And the disease has reached epidemic proportions globally, according to the World Health Organization (WHO) with four million people dying each year from the effects of obesity2.

MORE THAN AESTHETICS Affecting far more than physical appearance, obesity is a medical problem that increases the risk for other health problems such as heart disease, diabetes, high blood pressure and certain cancers.

“Morbid obesity is a complex, chronic condition,” explains Andrew Hollenbeck, M.D., bariatric surgeon at McLaren Northern Michigan. “It’s a result of neurochemical changes that occur in the brain after a person gains a substantial amount of weight. As a result of these chemical changes, diets are often unsuccessful, and surgical intervention has been shown to be an effective treatment option.”

THE BRAIN AND BODY CONNECTION But weight loss surgery isn’t a simple, quick-fix solution to losing weight. In fact, it’s only the beginning of a lifelong commitment to a new, healthier life.

Before considering bariatric surgery, candidates typically spend six to eight months in education, weight counseling, nutrition, behavioral modification and psychological counseling in order to prepare mentally and physically for the changes and challenges that come with weight loss surgery.

“By changing the biochemistry of how the brain thinks about food, we are giving the patient a start-over; an opportunity for the brain and the body to reset to before the disease began,” says Dr. Hollenbeck.

“It’s not a guarantee, but it can be a new beginning.”

PRIMARY CARE HELPING YOU LIVE YOUR BEST LIFE We go to the doctor when we’re sick, right? Of course. But even when we’re feeling great, scheduling a regular, yearly physical or wellness exam can help ensure our health is heading in the right direction and is a great check-in for overall peace of mind.

A primary care provider (PCP) – family physician, internist or pediatrician – is trained to recognize and manage a wide range of short term, acute health problems and reoccurring chronic ones as well as provide preventive care and education. If tests or screening results warrant, she will refer to a specialist for further evaluation.

Before your appointment, ask if you will be having any medical tests that will require you not to eat for several hours before your visit. And be sure to bring a list of any medications you’re taking.

LIGHTING THE WAY TO

NEW HOPE Surrounded by an outdoor lovers’ paradise, Leland Houthoofd and his wife Kimberly were ready to enjoy retirement life on Michigan’s Drummond Island, just off the eastern tip of Michigan’s Upper Peninsula in northern Lake Huron.

Life was simple and predictable in this remote part of Michigan, the perfect haven for fishing and boating enthusiasts like the Houthoofds.

But in July of 2021, the unpredictable happened: Leland was diagnosed with glioblastoma – an aggressive form of brain cancer.

Traditionally, neurosurgeons would use either an operating room microscope, or look through loupes to provide magnification, in order to closely visualize and resect the tumor.

“But glioblastoma is an infiltrative tumor – it extends into parts of the brain that cannot be visualized using traditional methods,” explains Dr. Thomas.

SHINING NEW LIGHT Fortunately, a new technology at McLaren is helping light the way to new hope, and more time, for glioblastoma patients.

“Unfortunately, glioblastomas are not curable, but they are a treatable form of brain cancer,” explains Dr. Justin Thomas, a neurosurgeon at McLaren Northern Michigan.

Treatment involves surgically reducing the size of the tumor as much as safely possible to decrease tumor burden and optimize overall survival.

Leland underwent surgery shortly after his diagnosis in 2021, but a subsequent MRI earlier this year confirmed the presence of another tumor.

Through the use of an optical imaging agent, along with a new, high-powered operating room microscope, McLaren Northern Michigan neurosurgeons are able to better visualize, and more safely resect, brain tumors. Using special blue light filters on the surgical microscope, the imaging agent lights up the tumor tissue in a bright, fluorescent pink.

“This allows us to better distinguish the tumor from normal brain tissue and remove more of the harmful tumor cells,” states Dr. Thomas.

“The normal brain tissue acts as our ‘canvas’ in the background, allowing us to safely remove the brighter, abnormal areas.”

THE BEST CARE, WITH RESPECT AND DIGNITY After seeking a second opinion from a larger metro Detroit hospital, Leland made the decision to return to McLaren Northern Michigan for his most recent surgery.

“When Dr. Thomas said he could do surgery on the other tumors, we were elated,” remembers Kimberly.

“He said he felt very confident doing so. We believe God led us to him.

“Our experience with McLaren Northern has been great,” she adds. “People have treated us with nothing but respect and dignity. And because the hospital is affiliated with Karmanos Cancer Institute, we know we’re getting the best care and treatment.”

Lee owned a floor covering business before retiring two years ago. Once retired, he set about making their Drummond Island cabin into their permanent home, where they relocated full-time in late spring 2021.

A mild, gentle routine combining several exercises.

CIRCUIT

Tuesdays & Thursdays, 9 a.m. – 10 a.m.

This is a moderate level class with a mix of resistance training as well as a cardio burst for building endurance.

ON YOUR OWN

Mondays – Thursdays, 10 a.m. – 11 a.m.

Move at your own pace in this non-instructional hour.

Pool, hot tub and all equipment is available for your use.
