Name: Barney Pooh IV Sex: Male Gender: Dino-Bear Age: 38 years Height: 7ft Weight: 225 lbs Medical History: ​He is allergic to pollen. He has not had any surgeries. He once ate honey but was rushed to the emergency room. Lifestyle Behaviors: ​He walks often to work but no other exercise. He sleeps about seven hours a night. He doesn’t eat vegetables often, but will often have meat, and will sometimes have the occasional small bags of candy. Medical Concerns/Family History:​ His family on both his mother’s and father’s side is prone to obesity and diabetes. Purpose of Consultation:​ The patient is most concerned for diabetes because his weight isn't causing him any immediate problems. This is why I’m going to focus on the diabetes issue, since that is what his family is prone to. Familial Risk: ​After you have the family tree, I considered the reason for the consult. Of the 13 people in this family tree, 6 of them were diagnosed with diabetes. This means that there is a 6/13 chance that he will get diabetes based on his family background. Percentage wise this is a 46.15% chance that he will contract diabetes. Risk Assessment: ​I wanted to find out the risk that my patient would get diabetes so I ran an online test on him in order to have more specific information on him. This online test basically took the lifestyle of my patient and turned it into a more readable results on a sheet.


Goals and Timeline​: One of the goals of my patient is to lose weight that is caused by fat and turn that weight into muscle by the end of 2018. Another goal that my patient has is to start a diet in the next couple of weeks and stick to it for three months. Medications:​ At this stage in my patient's life there is no need for any preventative medicine but if the diabetes starts to develop then he might have to take a future medication. One of the medications that my patient would be taking to prevent diabetes is pioglitazone which will possibly prevent type two diabetes. Pioglitazone makes the body more sensitive to insulin and lowers blood glucose levels. It is associated with weight gain and edema, or swelling due to fluid retention. Another medication that my patient could take in the future is Avandia. Avandia is an oral diabetes medicine that helps control blood sugar levels. Basically this drug is meant to be used instead of insulin. In fact it it were to be used at the same time it would change the effect. Other side effects include shortness of breath (even with mild exertion), swelling, rapid weight gain; changes in your vision; chest pain or pressure, pain spreading to your jaw or shoulder, nausea, sweating, etc. Nutrition Plan:​ A diabetes diet is a healthy-eating plan that's naturally rich in nutrients and low in fat and calories. Key elements are fruits, vegetables and whole grains. In fact, a diabetes diet is the best eating plan for almost everyone. Examples of this diet include apples, broccoli, and certain cereals or oatmeal.


Lifestyle Recommendations:​ I recommend that my patient gets regular exercise in order to help his future goals, as well as to help prevent diabetes. I also believe that he should keep track of his calorie intake and make sure he is getting the right nutrition. He also shouldn’t be making any decisions that would lead him into a state where he is inactive. Alternative Therapies: ​ Like I said previously he should be having daily exercise in order to keep his body active and able to heal itself. It would also help if he would make sure that he just avoided eating too much all fatty foods like most meats that would slow him down.
