Serving Humanity to Honor God by providing exceptional and cost-effective health care, accessible to all. This is the mission and guiding philosophy of Methodist Healthcare. This Impact Report is intended to bring our mission to life, provide information on our award-winning programs and technological advances and illustrate what we are doing to partner with organizations in our communities to fulfill our mission. We hope the results presented in these pages give you an appreciation of our belief that together, we can harness our collective knowledge and resources to ensure everyone in our community has access to quality care. Our mission compels us to ensure a safe, compassionate, equitable care experience for anyone who seeks it. Every day, Methodist Healthcare staff go to work with our shared focus centered on the same thing: our patients. Our focus and effort positively impact the care experience at the bedside and beyond. As a learning health system, we also create tools that transform vast amounts of patient data into systems that improve patients care. With approximately 12,000 talented colleagues concentrating on our one shared purpose, we are giving our patients healthier tomorrows. Last year was no exception. When faced with a threat to public health and safety, like

that of a pandemic, Methodist Healthcare illustrated its commitment to doing what is right for our people, our patients and our communities. Our impact was even greater as we quickly pivoted and designed new ways to reach our community where they live, work and play. It was important to the community that we do all we could to maintain this flow of community support. As always, we will face the future with a plan that requires that we determine community need, and then extend our resources to meet that need – wherever it may occur. We will keep finding innovative and effective ways to serve the physical, mental and spiritual needs of the community. Methodist Healthcare System (MHS) has evolved to meet the growing needs of the greater San Antonio community, and our commitment since opening our first hospital in 1963 has never wavered: to continuously improve the health and wellness of our community by “Serving Humanity to Honor God.” Formed in 1995 by Methodist Hospital’s Board of Trustees, Methodist Healthcare created a 50-50 co-ownership between Methodist Healthcare Ministries of South Texas Inc., South Texas’ largest non-public funding source of community health care for uninsured patients: and HCA Healthcare, the nation’s leading provider of health care services. Responding to a Pandemic When faced with a threat to public health and safety, like that of a pandemic, Methodist Healthcare is committed to doing what is right for our people, our patients and our communities.

Since we confirmed our first patient case in February 2020 (the first case in Texas), we treated 8,968 COVID-19 inpatients through December 31, 2020. A response of this magnitude has been possible because of the unwavering commitment and tremendous sacrifice of our employees, who have been guided throughout our response to COVID-19 by these four core principles: 1. Be there for patients 2. Protect our employees and caregivers 3. Partner with others to improve care beyond our own organization 4. Be a resource for our communities and governments

BE THERE FOR PATIENTS • We doubled down on cleaning procedures, enhanced screening, and limited visitors. • We were the first health system in South Texas to implement a universal masking policy for all staff to help reduce the spread of the virus – even before the CDC recommended such a policy. • In 2020, we performed 127,181 COVID-19 tests for the San Antonio and Central West Texas Divisions. • 2nd Hospital in Texas and 1st in HCA to receive and perform Abbott COVID-19 Rapid Test. • 99.5% of all COVID-19 tests performed in the San Antonio Division were completed in less than 24 hours from collection time. • COVID-19 positive patients do not have to pay for their treatment in our hospitals. • Through HCA, we offered a free, hotline for patients who have lost jobs and/or health insurance to guide them through their coverage options. As of December 2020, 7,200 patients were assisted through the hotline, which addressed more than 20,300 calls.

PROTECT OUR EMPLOYEES AND CAREGIVERS • We distributed 374,072 N95 masks, 62,869,606 exam gloves, 2,697,154 Level 1 masks and 43,692 face shields and 2,433,138 isolation gowns. • We collaborated with major hotel chains to provide housing for providers who work directly with COVID-19 patients. • At a time when hundreds of hospitals and healthcare systems were laying off or furloughing employees, we introduced a pandemic pay program that helped provide paychecks to employees unable to work as government mandates halted many elective procedures. We also offered quarantine pay for patient care staff who meet criteria for exposure to COVID-19. • We provided multiple resources to our staff who worked endless hours and shifts such as: virtual mental wellness seminars, counseling, 24/7 advice lines and coping tools; financial assistance in the form of tool kits, webinars and advice lines; emergency child care options; on-site mini-marts and free “Doctor on Demand” telehealth visits.


PARTNER WITH OTHERS TO IMPROVE CARE BEYOND OUR OWN ORGANIZATION • We have shipped COVID-19 testing instruments and test kits to Kansas, Colorado, and Texas Gulf Coast areas. • We implemented five COVID-19 clinical trials at Methodist Healthcare including: ECMO research trial, Therapeutic Plasma Exchange, Remdesivir clinical trial, and Emergency Use Mesenchymal Cells and Convalescent Plasma. • We collaborated with STRAC (Southwest Texas Regional Advisory Council), the City of San Antonio and other community organizations and shared data and best practices to improve emergency operations plans throughout the region.

BE A RESOURCE FOR OUR COMMUNITIES AND GOVERNMENTS • Through HCA COVID-19 funding, we were able to give a total of $100,000 to local community groups such as the San Antonio Food Bank, Communities In Schools, Enroll SA and a digital divide education project. • With the help of HCA, we are offering a free mental health program to area middle and high schools through EVERFI, a leading education technology provider. To date, the program is being offered to over 200 students at four schools. • We created a series of “Health Matters” videos for our community, focusing on: COVID-19 Safety, Mental Health, Healthy Eating and Hands-Only CPR. The videos received 27,500 views. • Methodist Healthcare partnered with Methodist Healthcare Ministries, HCA Health Trust and the Health Collaborative to distribute nearly 40,000 pounds of food to residents of Atascosa County. Over 1,200 families were given food boxes. • MHS worked hand-in-hand with the South Texas Blood and Tissue Center to hold muchneeded convalescent plasma donor drives to help in the care of COVID-19 patients.

2020: A Special Year of the Nurse 2020 was deemed “Year of the Nurse and the Midwife” by the World Health Organization.

These extraordinary individuals are vital front-line workers, displaying courage, care and commitment to their patients every day throughout our organization. Despite the added challenges of COVID-19, nurses have consistently shown their resiliency and willingness to go above and beyond for their patients and each other. Caring for patients like family requires a team of nurses who cultivate an environment of love and support for one another. Every day, we witness patients who are touched by their compassion from the moment they enter our facilities, until the moment they leave our care.

Some highlights of 2020: • In 2020 Methodist Healthcare reached an all-time high patient experience score of 81st percentile • Hired over 850 nurses and 57 nurse interns We will continue to invest in such technology as iMobile and other technology solutions to enable our nurses to be more efficient and collaborative in caring for patients. We’ve added beds, grown surgeries, added new services and become the leader in complex and cutting edge care for patients. It is a great time to be a Methodist Nurse and we are proud of the contributions they make to our patients and our community.


Clinical Excellence Program for RNs Methodist Healthcare Registered Nurses who provide direct patient care are eligible for a career ladder that recognizes their self-paced self-motivated professional growth. The program: • Fosters leadership growth • Rewards high-quality patient care, teaching and education • Promotes and acknowledges exceptional customer service RNs enroll in the program, apply their current qualifications and begin tracking qualifying events that accumulate points toward identified career levels. RNs enrolled in the Nursing Clinical Excellence Program can achieve RN Clinical Ladder and RN Clinical Expert levels, RNs who do not participate in the program can achieve GN/RN and RN Advanced levels.

Methodist Healthcare Nursing Residency Programs Methodist Healthcare Nursing Residency Programs consist of intensive structured specialty orientation designed to support transition of the graduate registered nurse to bedside nursing practice in a specific clinical setting. Our Residency Program is accredited nationally by American Nurses Credentialing Center with distinction and Department of Labor Industry Apprenticeship Program. Each Nursing Residency focuses on development and verification of proficiency in key entry level technical, interpersonal and critical thinking skills required for safe practice. Residents are introduced to the Methodist Healthcare mission, interact with members of the health care team and acquire knowledge of regulatory requirements related to clinical practice in an acute care setting. Methodist Healthcare System Education & Research Department supports all nurse Residency Programs coordinating learning activities, time and attendance, feedbacks and other personal related functions. Each Nurse Resident is designated as a full time, 40 hours/ week employee. Upon completion of the program, the Nurse Resident is reclassified to the appropriate hours/week schedule (ex. 36 hours/week) and transferred to the unit of hire.

Partnering With Our Physicians Throughout the pandemic in 2020, we worked closely with our physicians to create a safe and protected patient care environment.

To address medical practice or business disruption, HCA Healthcare granted rent deferral with owned medical office buildings (MOBs) This helped to maintain availability of medical care and related businesses and services for patients and the community (“COVID-19 purposes”) as permitted by federal waivers.

We connected physicians with experts to do live Q&A sessions during our physician confidence and resiliency webinar support series. Additionally, division-wide town halls were held with CEOs, CNOs, and CMOs for primary care, surgeons and rural facility partners to share information and understand needs related to the COVID-19 pandemic. Through HCA CARES, HCA Healthcare provided education to physicians regarding the CARES Act, including a website and hotline, for COVID-19 purposes as permitted by federal waivers.


Graduate Medical Education Methodist Healthcare Graduate Medical Education (GME) provides clinical experiences for physician trainees from residency and fellowship programs. We are proud of the training opportunities offered at our facilities for a lifetime of learning. GME includes medical, surgical, and other subspecialty residency programs and fellowships. The accredited program is managed by a Graduate Medical Education Committee (GMEC) that establishes and implements policies and procedures regarding the quality of education and the work environment for the residents in all programs. Program Directors and Faculty ensure all educational programs are conducted in full compliance with the guidelines of the Accreditation Council for Graduate Medical Education. In 2019, we offered clinical rotations for 42 programs in 36 various specialties. A total of 525 residents, fellows, medical students and physician assistant students participated in these clinical rotations.

Pastoral Care Methodist Healthcare has a team of chaplains dedicated to helping patients and their families deal with a health crisis, including specially trained chaplains for children, oncology patients, sexual assault survivors and critically ill patients. The chaplaincy program is considered a national benchmark for hospitals. Our chaplains made over 230,000 patient visits last year.

Charity Care As part of our mission, Methodist Healthcare provides care to patients without financial means to pay for hospital services. Services will be provided to all patients who present themselves for care at any Methodist Healthcare facility ER without regard to race, creed, color or national origin and who are classified as financially or medically indigent. Committed to Our Community Our mission statement of ‘Serving Humanity to Honor God’ defines our ultimate purpose, and that goes far beyond just services and facilities.

Methodist Healthcare treats the entire community in a number of ways, including providing care at our eight hospitals and seven freestanding Emergency Centers to everyone regardless of ability to pay, and through the direct support of more than 30 non-profit health and human services agencies serving the community. As the city’s second largest private employer, Methodist Healthcare is able to make a significant impact in the community beyond its 35 health care facilities and caregivers. The birth of Methodist Healthcare System in 1995 provided the greatest mechanism to address the needs of the underserved in San Antonio and South Texas through Methodist Healthcare Ministries. As one of the two co-owners of the Methodist Healthcare System, Methodist Healthcare Ministries receives half of all profits generated by the hospital system.

Community Health Needs Assessment and Community Health Priorities As part of our commitment to the greater San Antonio community, Methodist Healthcare participates in a county-wide health needs assessment every three years. This assessment is conducted through the Health Collaborative, a non-profit convening organization that brings together hospital systems, other health care organizations, local government and non-profit organizations in Bexar County to identify and solve community health issues through collaboration. Methodist Healthcare is proud to be a founding member of the Health Collaborative. In order to work in coordination with other health care systems and organization, businesses and non-profits, Methodist Healthcare has adopted the five health priority areas based upon the Bexar County community health assessment, the Bexar County community health improvement plan and the SA2020 goals. Methodist Healthcare and its hospitals have created community improvement plans based on these five health priorities so we may better serve you and the greater San Antonio community.

Our Partners

As Methodist Healthcare, we are proud to offer support to a variety of worthy causes. For more than 50 years, Methodist Healthcare has given back to the community through charitable donations, sponsorships and volunteering. As the most preferred health care provider in San Antonio, we are advocates for the advancement of the community. Our Methodist Healthcare employees also provide support and funding to hundreds of non-profit organizations of their choice, through an online giving portal. The portal also allows staff to search for volunteer opportunities throughout our community.

Partnerships Methodist Healthcare supports a variety of worthy causes through annual agreements with like-minded organizations. Sharing an enduring purpose to improve the health of our community, our relationships are based on mutual values, goals, and an advocacy for the wellbeing of our area families.

Community Giving HCA Hope Fund Our employees’ first goal is to take care of patients. Methodist Healthcare is also a family of co-workers, and so we take care of each other. The HCA Hope Fund was formed more than a decade ago in the wake of devastating Florida hurricanes so employees around the country who are in need – and those who want to help – have a single, reliable source to give and receive assistance. The well-named HCA Hope Fund has offered just that – hope. Its support has helped employees who have suffered everything from loss due to hurricane, tornado or fire to those dealing with unexpected catastrophic illness or accident. The fund also steps up to aid employees who are dealing with domestic violence, death of a loved one or other special situations. Since its inception, it has helped more than 22,000 families through contributions of more than $32 million is assistance. Through the HCA Hope Fund, 194 San Antonio division employees were provided support in 2020.

Hope Fund Provides Relief Aiden Carmona, an EHR Support Analyst in the San Antonio division and HCA Healthcare colleague of 12 years, didn’t know what to expect when COVID-19 caused businesses to close and so many changes to life as we know it. But it wasn’t long until the effects hit home. His husband, Jacob Davis, built a career in the hospitality industry as an events director for a major hotel chain. Due to the pandemic, Jacob was furloughed and overnight their household was reduced to a single income. Jacob immediately tried to find alternate work. Opportunities were scarce and the few positions he found available were flooded with applicants. It took six months before he was able to secure a new position. “We were lucky enough to have some savings, but we didn’t plan for six months of no second income,” Aiden said. “It was tough to pay bills.” The couple worried how they would be able to afford to keep their mortgage current. Aiden said, “Our finances were getting stretched thin. The first round of assistance and unemployment were expiring. It was a huge unknown.” Luckily, Aiden applied for assistance from the HCA Healthcare Hope Fund and it wasn’t long before he received much needed help to keep his family whole.
