Chronic Kidney Disease Management

At our chronic kidney disease program, we are dedicated to providing comprehensive care and support for individuals living with kidney disease. Our team of experienced healthcare professionals is committed to helping our patients manage their condition and improve their quality of life. We offer a range of services and resources to meet the unique needs of each patient.

Our approach:

• Multidisciplinary Care- We believe in a collaborative approach to care, involving a team of specialists including nephrologists, dietitians, social workers, and nurses. This ensures that all aspects of our patients' health and well-being are addressed.

Subacute Rehab

At Lincolnwood, we are dedicated to proactively addressing your rehabilitation needs in a modern and tranquil setting. Reflective of our approach, we start planning your discharge immediately upon admission. Individualized care plans are crafted to ensure optimal healing –fully preparing you for your return home.

Geared towards individuals recovering from orthopedic surgery, stroke, cardiac or pulmonary condition – our goal is to restore independence and maximum function. To get you back to yourself and home as soon as possible, we deliver cutting-edge therapies up to 7 days a week. Our expert rehab professionals craft custom rehabilitation regimens utilizing the latest rehab technology and techniques to accelerate your recovery.

Our unique brand of Alzheimer’s care is deeply rooted in a person-centered approach designed to help your loved one feel a sense of belonging and purpose while still preserving their sense of self.

We’ve created a secure Alzheimer’s neighborhood that truly feels like home to both our residents and their family members who are always welcome to visit. Our unique setting is defined by our combination of qualified caregivers, comprehensive innovative treatments, and specialized attention.

The safety of our residents is our foremost priority. Our high ratio of caregivers to residents allows for more one-on-one interaction, increased supervision, and greater peace of mind for family and loved ones. The Urgent SNF Service at Marquis Health Consulting Services supports a global health care expectation for the right provision of care in the right place at the right time.Therefore, our solution driven service aims to provide an alternative option to individuals not meeting acute admission criteria, however requiring and/or benefiting from supervisory skilled rehabilitation services, skilled nursing oversight or stabilization.

URGENT SNF PAYMENT OPTIONS:

Medicare A: (qualified 3 night) stay within 30 days

HMO: Willingness to accept without prior authorization with select HMO/ MCO

Private Pay: Reduced Private Pay All Inclusive Rate promoting the stabilization often necessary to avoid repeat and unnecessary ER/ED visits. All-inclusive services include room/board, meals, therapy and nursing services.

In partnership with our patients and or responsible party, every member of the team works collaboratively to develop an effective Care Navigation plan. Our belief in providing a better patient experience through quality and satisfaction will ensure successful navigation through the health care continuum.

Roadmap to successful navigation:

• Patient-centered needs/preferences identified on admission with assessments and continues throughout the stay

• Choosing the appropriate placement in the Marquis continuum

• Provides financial resources

Patient experience and outcomes:

• Provides patients and families with verbal and written information about available community resources

• Prepares the patient and caregiver for a successful discharge
