SERVICE REQUESTS When we click on the second option i.e service requests, we encounter 2 sub divisions: Activation Requests Common Layer Requests 1.1 ACTIVATION REQUESTS:

Under activation request form: Service type: ➢ A Customer can choose their service type according to their requirement. The list of service types that are available are: ▪ Bootstrap Activation ▪ Bootstrap top up ▪ Commercial plan ▪ Reactivation ▪ Renewal ▪ Top up ➢ Once a customer chooses their service type according to their requirement, he’ll need to select a plan for his service. Every service type will offer a plan. The plans are listed below: SERVICE TYPE PLANS Bootstrap activation BSNL Bootstrap top up 2 Months Commercial plan o 1 Year Plan


o 2 Year Plan o 13 Months Reactivation 1 Year Plan Renewal 1 Year Plan Top up o 1 Month o 2 Month ➢ Now the customer must choose the validity for the activation of the SIM The validity is listed below respective to their plans: SERVICE TYPE VALIDITY Bootstrap activation 1 Month Bootstrap top up 2 Months Commercial plan o 12 Months o 24 Months o 13 Months Reactivation 12 Months Renewal 12 Months Top up o 1 Month o 2 Months ➢ Upload KYC file ➢ Submit the Activation Request Form Under Latest Uploaded Requests: Viewers can view all the latest requests that is enteredbeen.
