Dear Neighbors and Friends of Valleywise Health,

For nearly 150 years, Valleywise Health has served residents of Maricopa County, regardless of circumstance. It is the only public teaching hospital and health care system in Arizona. It offers comprehensive, safe, high-quality integrated medical and behavioral health services through a seamless continuum of care that keeps our community healthy, self-sufficient, and effectively treats illness. Throughout the Valleywise Health system, the client base is often underserved, under-or uninsured, low-income, and ethnically diverse. As a safety net healthcare system, Valleywise Health understands that promoting community health goes beyond providing health care and is dedicated to addressing the social determinants of health - life conditions that impact individual, family, community health and well-being. Valleywise Health is committed to providing exceptional care, without exception, to every patient, every time.

The Grants Department supports Valleywise Health and its mission to promote health in the community through various programs and from the relationships and collaborations it continues to build. Since 2003, grant-funded projects have contributed over $150 million to the Valleywise Health system and its efforts to provide exceptional care and improve health outcomes. Annually, the Grants Department supports existing programs and initiatives, identifies, and brings new opportunities to expand service delivery throughout the Valleywise Health system to meet the needs of the community.

With this annual compendium, you will learn more about the work of the Grants Department through its stories and successes and hopes for continuing to build on the efforts of the team and its partners.

Dear Valleywise Health Community,

The Grants Department is a vibrant component of the Valleywise Health system. Our work helps to support the health care needs of our most vulnerable citizens. From writing grants and managing programs, to collaborating with physicians, healthcare allies, and patients, or to advancing nationally recognized best practices, the Grants team is focused on addressing the physical, emotional, educational and financial needs of our community.

Healthcare is much more than completing a medical visit and leaving with a prescription. Healthcare is investing in vital resources to enhance the quality of life of our most marginalized community members. Supporting the mission of Valleywise Health, our goals are plentiful. They include improving access to care, increasing efficiency and access to support services, broadening revenue sources, identifying innovative partnerships to support access to care and using evidence-based solutions to improve outcomes. Not only do we invest in patients, but grant programs also support training and technical assistance, learning collaboratives to enhance skills, analyzing and reporting on data, and sharing our achievements with the community.

This Compendium broadly highlights many of our vital grant funded programs. Whether patients are seeking life-saving behavioral healthcare substance abuse treatment through our Medication Assistance Treatment grant funded by the Substance Abuse and Mental Health Services Administration, stigma-free PrEP services available system-wide, Federal funding received as a direct result of our Federally Qualified Health Center designation, preventative screenings through the Arizona Well Woman HealthCheck program, or providing culturally responsive care through our refugee and immigrant clinic, it is clear that grants help to save lives.

The brief grant program descriptions in this document only tell part of the story. They don’t convey the skills, passion, dedication and enthusiasm of the Valleywise Health Grants team along with our internal and external partners that ensure these programs are successful. We take tremendous joy in helping our community lead healthier lives and are grateful that we can be a part of transforming care for those most in need.

Refugee Health Program

The Refugee Health Program at Valleywise Health has an exceptionally skilled and diverse team of providers, nurses, medical assistants and cultural health navigators. They have long-standing experience serving an average of 11,000 refugees annually across Women’s Health, Pediatrics and Primary Care. Multi-lingual Cultural Health Navigators (CHNs) assist in providing health education activities and targeted health outreach to increase health literacy among refugees with critical health care needs. Combined, they speak 18 languages fluently and facilitate an integrated, team-based approach to health care delivery. CHNs provide intensive care coordination and case management services that create trust and empower families in their ability to navigate the health care system.

Department of Economic Security Health Literacy

A grant received from the Department of Economic Security has empowered Valleywise Health to focus on increasing health literacy through education delivered in a manner that is culturally and linguistically sensitive patient-centered. When new refugees arrive for care at Valleywise Health, they are interviewed by Cultural Health Navigators (CHNs) to determine the educational and social needs of each person in their family. Based on their needs, refugees are given the opportunity to attend classes on health topics such as general and dental hygiene, vaccinations, preventative health practices, child well-being and safety, home safety, choosing a provider and making clinic appointments.

CHNs also screen for mental health needs using evidence-based protocols. Preventative health knowledge can be very limited, or sometimes unknown, within a refugee’s home country. Our goal is to provide health education topics that have been proven to successfully help refugees integrate into our health care system.

Over 400 refugees have attended health literacy classes in topics such as COVID-19 vaccine resources, urgent care and using the emergency room, the importance of breast and cervical cancer screening, and scheduling referrals and specialty appointments. Classes have been taught in languages such as Burmese, French, Kinyarwanda, Swahili, Arabic, Dari, Kiswahili, and Kiyemba.

A recent publication on the experiences of patients who receive care at the Refugee Women’s Healthcare Center (RWHC) stated that

Having trained medical interpreters who work as CHNs at the RWHC was a key driver of utilizing the clinic. The women said that because CHNs are women like themselves and belong to the same culture, nationality, and language, they are more open to communicate with them regarding their feelings, their health, and their other needs, which makes it easier for them to use prenatal care. (Banke-Thomas et al., 2017).

The funding that has been received through awards for submitted grant proposals has enabled Valleywise Health FQHC clinics to lean forward in efforts that otherwise would not have been possible. This has included capital purchases, expanding staff to trial new interventions, and enhance IT initiatives. Through these efforts we are establishing the infrastructuvre to increase patient access and improve quality care which leads to improved health of the population we serve.

Federally Qualified Health Centers FQHC

Federally Qualified Health Centers (FQHCs) were created in 1965 under Lyndon Johnson’s War on Poverty. These local clinics give patients, many of whom had never seen a doctor, access to primary care. FQHCs serve patients who are uninsured, underinsured or unable to pay for their healthcare needs. FQHCs also provide a medical home for patients who reside in Medically Underserved Areas, helping those who are least able to access care. All ten of the Valleywise Health Community Health Centers are officially designated as FQHCs. Additionally, four of the ambulatory clinics located in the Comprehensive Healthcare Center on the main campus of Valleywise Health have FQHC designation: Women’s Care, Pediatrics, Internal Medicine and Dental.

Every three years, Valleywise Health submits a comprehensive and competitive grant application to the federal Health Resources and Services Administration. A successful application enables Valleywise Health to continue serving the community with full FQHC status.

Valleywise Health tracks data regarding the performance of our health center in terms of patient outcomes which is compared to national FQHC health center data. The Valleywise Health Ambulatory Quality Initiative Workgroup reviews our performance measures monthly (such as cancer screenings, childhood immunization status and screening for depression) to discuss how we are meeting quality metrics or what interventions could be implemented that best serve our patients and providers to help meet standardized data.

Medication Assisted Treatment

(MAT)

In 2021, Arizona residents experienced 3,558 non-fatal opioid overdose events and 2,006 confirmed opioid deaths. Through our Going to the MAT to Fight Opioid Use Disorder (OUD) program, Valleywise Health is providing prevention, treatment, and recovery assistance for low-income, racially/ethnically diverse, and at-risk individuals who battle prescription drug and opioid addition in Maricopa County.

Awarded in October 2021, the Substance Abuse and Mental Health Services Administration Medication-Assisted Treatment – Prescription Drug and Opioid Addiction grant provides funding to address gaps in care by building capacity through an expanded workforce and adding long-term strategies such as providing access to essential education and training activities.

MAT Services Implementation

MAT services began implementation at our Valleywise Community Health Center - South Central site, with planned expansion to our Avondale and Mesa Community Health Centers, to provide coverage across the valley. MAT services include medication treatment in combination with psychosocial engagement, which has been found to be an effective intervention for individuals with OUD.

The Valleywise Health behavioral health staff, including licensed professionals and peer support specialists, work in collaboration to engage the individual and support them during enrollment and ongoing care. Peer support specialists are individuals who share their own lived experiences, offer valuable guidance, help to build recovery skills, assist in addressing social determinant of health needs and improve social connectedness.

Studies have found that peer support specialists are an effective way to improve relationships with providers and social supports, reduce rates of relapse, increase satisfaction with overall treatment and increase treatment retention.

SOUTH CENTRAL

HIV PrEP Navigation

PrEP (pre-exposure prophylaxis) is a medication for people who are HIV negative that helps prevent getting HIV through sexual transmission. Valleywise Health provides PrEP referral and prescription services throughout our health system which is part of our overall strategy to reduce the incidence of HIV among at-risk individuals.

Grant funds have been crucial to reducing the spread of HIV and identifying those at-risk. Opt-out HIV testing is completed at all Valleywise Health FQHCs, our emergency departments, available at our adolescent clinic for patients aged 15 and older, and through collaboration with partners. For example, our subcontractor RipplePhx provides HIV testing at outreach events such as community colleges, bars, and nightclubs. Outreach activities are a key component to access high-risk populations for both HIV testing and PrEP education and referral. Funds have also been utilized to help uninsured patients pay for their PrEP visits, reducing a huge significant barrier to care for many of our patients. Valleywise Health completed a new landing page on our website that allows members of our community (age 18+) to purchase an HIV home testing kit. Additionally, we are purchasing several hundred HIV home test kits to provide to local organizations who otherwise do not have access to provide HIV testing to their own clients. These organizations include case management organizations, homeless services, food banks, and other non-profits that regularly encounter people who may be at higher risk for HIV.

As part of our opt-out HIV testing program, the medical director of our main HIV clinic (Valleywise Community Health Center – McDowell) has provided training to all medical providers at the FQHCs regarding HIV testing, PrEP prescription, and HIV linkage to care referrals. In 2021, our health system served more than 140 patients with PrEP prescriptions.

PrEP funding is a benefit to the person at risk of living with HIV as it enables them to access care they may otherwise be barred from due to cost. It equally benefits our providers as it enables them to work with patients over the long term with less fear of care interruption or resource challenges.
