Q. Do I need to bring my student’s immunization records with me on enrollment day? A. Generally, parents/guardians do not need to bring any immunization records to school on enrollment day. Families who are new to Wisconsin may bring these records to school to be copied, but in most cases the MMSD is able to obtain these records electronically from the State of Wisconsin’s immunization database. Q. When will I learn who will be my child’s teacher? A. Some schools share who your child’s teacher will be at enrollment. Others may wait until all new students are enrolled. Your school will share with you the process on enrollment day. Q. I am taking care of my niece and her parent(s) or legal guardian(s) do not reside in the MMSD attendance area. How do I enroll her into school? A. You will need what is known as Educational Guardianship. To complete this, go to the Enrollment Office at the Doyle Administration Building, 545 W. Dayton Street, Room 100, Madison, WI between 8 am—4 pm Monday through Friday or by appointment. You will need to provide proof of residency (utility bill, lease, or mortgage) as well as a photo I.D. It is also helpful to have a letter from the parent or legal guardian providing permission for Educational Guardianship, along with a birth certificate or medical record to provide proof of age for the child. Q. I am currently living (not a temporary housing situation) with someone and cannot provide proof of residency. What should I do? A. You will need a Co-Residency Form. To complete this form, a parent will need to visit the office of their child's school, along with the person with whom they live. The person(s) with whom you live will need to provide proof of residency (gas & electric bill, lease, or mortgage with their name on it). Both you and the person(s) with whom you live will need to provide a photo I.D. Q. I am taking care of my niece and her parent(s) do reside in the MMSD attendance area. How do I enroll her in school? A. A parent will need to go to the school and complete a Rights to Records form. This form will provide you access to the student’s records. The parent(s) will need to enroll their child in the school that serves their attendance area. Q. I currently do not have a permanent residence. How do I enroll my child? A. Your child may be eligible for additional educational services through the Federal McKinney-Vento Homeless Assistance Act. To find out if your child qualifies, complete the Student/Family Residence Questionnaire and take it with you to school on enrollment day.
