As we compiled follow-up outcomes from local nonprofits in preparation for this report, I was impressed by the amount of work that has been accomplished in just a few years. Read more about the grants distributed by Live Well Foundation of South Lake on page 8 to 17.
Live Well Foundation grants are ensuring families have access to food, shelter, mental health, and educational opportunities now and in the future. But this is just the beginning. As I write this letter, we are closing the application process on another grant cycle.
The Live Well Foundation of South Lake County was created with a vision of better quality of life through improved health and wellness. Live Well Foundation Foundation of South Lake is created through an agreement between Orlando Health and the South Lake Memorial Hospital which resulted in South Lake Memorial Hospital relinquishing its seats on the board of South Lake Hospital. Orlando Health generously provided $128 million to the new foundation. The contribution coincided with the South Lake Hospital becoming a controlled subsidiary of Orlando Health.

“All funds generated by the Foundation will remain in South Lake County.” Even at this early stage of our existence, Live Well Foundation of South Lake intends to be a long-term partner in bringing more healthcare choices and healthier lifestyle services to the people of South Lake County.
“With the October 2019 dissolution of the South Lake County Hospital District, we felt it necessary to create an ongoing funding source to address healthcare needs in our community,” said Paul Rountree immediate past chairman of the SLMH Board of Directors.
Letter from the Executive Director Gratitude is what I feel when reviewing the amount of time and energy our board members have invested to create Live Well Foundation of South Lake. Since its inception in September of 2018, the directors have volunteered over 500 hours. These hours resulted in Live Well Foundation allocating $69 million in grant funding (see page 5) which supports Lake County’s Community Health Improvement Plan and focuses on social determinants of health as created by the World Health Organization. For our foundation, the numbers are a means to an end. What is most important are the residents whose lives will be impacted by the services provided.
I’m honored to serve South Lake County as the Executive Director of the Live Well Foundation of South Lake. The South Lake story is rich with history. What was envisioned 20 years ago in a portable unit on a construction site (you can read more about that on page 16) is being carried out and is unfolding one grant at a time.

The creation of Live Well Foundation of South Lake could not have happened without committed and passionate community support. For example, this year marks the 75th birthday of Orlando Health South Lake Hospital. The local hospital was birthed in 1938 by a group of young women from the Clermont area who saw a great need for a hospital to meet emergency healthcare needs. Twenty years ago, a group of community leaders came together with a vision for the south Lake community wellness campus. This vision has improved access to healthcare, and thus our region’s overall health and wellness. Members of our Board of Directors were among those community leaders who have steered these visions over the years. Three-quarters of a century later, what was once a small 9-bed hospital in a rural community is now a 150-acre health, education, and wellness campus promoting healthy lifestyles among the residents of South Lake County.

Our mission is to inspire, through grant-making, sustainable initiatives that enhance the health and wellness of the South Lake community. Our vision is for the community of South Lake to experience a better quality of life through improved health and wellness.
The Central Florida Hope Center addresses the physical and mental needs of individuals in South Lake County. Teaming with other nonprofits the Hope Center provides: • Food distribution • Counseling services Building Blocks Ministries 2021 Expansion and Enhancement Project Building Blocks Ministries is an adult day training program for adults with developmental disabilities. Find Feed & Restore provides permanent housing solutions, homeless avoidance, and nutritious meals to the community through their Housing First and Homelessness Avoidance programs. With the help of their dedicated volunteers, and a $295,470 grant from the Live Well Foundation of South Lake, Find Feed and Restore was able to convert an RV into a wellness coach as part of its Homelessness Avoidance program. Mobile wellness vehicles are effective tools for providing health care directly within the community and the Care Coach’s goal is to provide residents in the community the opportunity to discuss their health, housing status, and mental health to receive assistance and register in the HMIS system in a dignified manner.
Find Feed & Restore - Care Coach 2021 Capacity & Empowerment Grant

The mission of the Community Foundation is to serve the community through the connecting of resources and intentional philanthropic resources. Working closely with the Food Providers is an example of that connection. For the past two years the Community Foundation has served as oversight for the $545,000 grant provided by the Live Well Foundation to address food insecurity. Five of our local Food Providers have been working to combat hunger through collaborative efforts, providing food resources for the growing need in the South Lake community. Their efforts work to eliminate the duplication of services, enhancing their services for those in need all while exhibiting unity within the Thiscommunity.project is designed to address the growing issues of obesity, high cholesterol, diabetes and food insecurity, as statistically shown in the Lake County Community Health Needs Assessment. By increasing the healthy foods provided to our underserved families, they will have a greater ability to positively impact their health.
To date: • 603 cases have been managed and these individuals have been connected to needed services and housing.
The Lake-Sumter State College Foundation enables and enhances the lives of Lake-Sumter State College students, faculty, and staff through scholarships, program enhancements, special events and facilities support. The Foundation cultivates and assists private donors who wish to support its mission of improving higher education in our local community. The Foundation applied for, and received funding, for two grants totaling $2,170,500 to expand educational opportunities for nursing students as well as the development of a Medical Laboratory Technology Lake-Sumterprogram.

• Expand its Associate of Nursing (ASN) Education Program to create more opportunities for enrollment, degree completion and job placement.
• Utilize a Dedicated Educational Units (DEU’s) model. This will embed the program into the Orlando Health South Lake Hospital campus environment and increase the number of program graduates to 100 annually who will be prepared for immediate job placement through their apprenticeship experience of the DEU.
• Create a simulation center on the LSSC Clermont campus that will be used for training current and future professionals in health-related fields. The addition of the Medical Laboratory Technology program will prepare students to work as medical/ clinical laboratory technicians and technologists. The development of an Associate in Science in Medical Laboratory Technology will meet the needs of many local employers and help fill the 22 job openings per year in Lake and Sumter Counties. LSSC will be the only MLT program offered by a Florida College System institution in the Career Source Central Florida region. The program content will include but is not limited to:
At the time of publication, the courses for this program have been approved by the Lake-Sumter State College Curriculum and Instruction Committee and have been moved forward to the regional accreditation body, the Southern Association of Colleges and Schools Commission on Colleges for approval. The goal is for the program to begin in fall 2022.
The Neighborhood Center of South Lake (TNC) was founded in 1972 and has made it its mission to serve as a pathway to develop, empower and educate members of the South Lake community with food insecurities. Originally founded by a faith-based organization, TNC provides services to local South Lake County residents regardless of their own faith experience. They have long since outgrown their current facility and began a search for a property that could house a new food bank building, plus establish a community garden. TNC formally asked for and received a land gift (9acre parcel) from the City of Groveland to help make this project a reality. The Live Well Foundation of South Lake provided an $800,000 grant to assist with their capital improvements and it also allowed TNC to provide fresh quality produce options for clients and they offer ongoing case management services and referrals to partner agencies.

Community Health Centers, Inc.’s mission is to provide quality and compassionate primary healthcare services to Central Florida’s diverse communities. The $167,140 grant they received from the Live Well Foundation of South Lake allowed them to hire a fulltime pediatrician and support team to provide pediatric medical services at their Four Corners location. Four Corners has seen increased growth and according to the 2019 Community Health Needs Assessment it is estimated that there will be more youth residents than middle-aged residents in the area causing an increased need for medical services.

The Live Well Foundation of South Lake was able to provide FUMC with a $50,000 grant to expand their current Meals on Wheels program. By the end of 2021, FUMC provided: 12,351 meals to Meals on Wheels 3,411 meals for Building Blocks Ministries 3,314 meals for Wesley Christian Academy
First United Methodist Church’s Meals on Wheels program currently serves residents of the South Lake community that live within a seven-mile radius of their church. These ready-to-eat meals are delivered every weekday to residents who are shut-in, recovering from an illness or surgery, are in need of assistance with food shopping and meal preparation and/or show a financial need. Seeing a need outside of its current delivery radius, FUMC looked to expand its services, including a larger delivery area as well as partnering with Building Blocks Ministries to provide healthy lunches for clients attending their day program.

The mission of the City of Groveland is to enhance sustainable development and preserve Groveland’s natural charm through a progressive Community Development Code that encompasses new urbanist design principles and improves health, wellness, and education outcomes for all residents. The United Groveland Health and Wellness project aims to improve the health and wellness of its residents through collaborative efforts. To date, grant funds have been spent for the HOPE Movement Center, utilization of the Elese Tomlin Empowerment Center and extension efforts for the South Lake Trail connection. The projects increase access to fitness as well as recreational and educational resources while providing improved physical health and wellness for the South Lake 12community.

First United Methodist Church - Meals on Wheels2021 Capacity & Empowerment Grant
The expansion of cardiac services is an answer to the 2019 Community Health Needs Assessment that showed the leading cause of death in Lake County is heart disease. The improvement of access to cardiac care with this project along with the quality of care through the expertise and best practices of Orlando Health Heart and Vascular Institute physicians and leaders as well as other cardiovascular leaders. The expansion will include coronary artery bypass surgery (CABG), heart and valve repair and replacement, surgery of the aorta and surgery for holes in the heart. The hospital will also obtain the AHCA Level II Cardiovascular Licensure for CV surgery services and their state-of-the-art operating room will be outfitted with the appropriate medical and surgical equipment. Additionally, a second electrophysiology lab (EP) will be built to support the increase in complex cardiac patients seeking treatment. The development of this expansion program will coincide with the construction of a new bed tower to be completed in 2023. As part of the program, eight additional ICU beds will support the program’s need for higher levels of post-procedure care. The improvement of access to cardiac care with this project along with the quality of care through the expertise and best practices of Orlando Health Heart and Vascular Institute physicians and leaders as well as other cardiovascular leaders.
The expansion of these services will allow for a greater capacity for inpatient services, including additional intensive care beds to accommodate Level II trauma services. In addition, the opening of a dedicated women and pediatric services ambulatory platform to include a Level II Neonatal Intensive Care Unit (NICU) will allow for parents to keep their newly-born children close to home and allow for continued collaboration with Orlando Health Winnie Palmer Hospital for improved outcomes. The soon-to-be-constructed 5-story tower will include ambulatory space for women and pediatric services on the first floor while the second story will include the expanded intensive care for trauma services. The third floor is slated to be the home of the level II NICU while the fourth and fifth floors will be expanded inpatient medical-surgical rooms. The new five-story tower will ultimately alleviate the need for patients and their families to travel to Orlando or other urban markets for needed services.
Orlando Health South Lake Hospital strives to improve the health and quality of life of the individuals and communities in South Lake County. The hospital identified a need to expand women, pediatrics and trauma services as well as their cardiovascular surgery services. They applied for and received funding for two grants totaling $55 million to aid in their expansion efforts.

LifeStream Behavioral Center creates hope, supports recovery and promotes health. LifeStream noticed that due to the pandemic, the youth in South Lake County began to experience a significant deterioration in their mental health and an increase in substance misuse. To proactively address this issue, they proposed the Youth Recovery Care Center (YRCC) which will provide integrated treatment for co-occurring disorders, which includes short-term LifeStream Behavioral Center 2021 Capacity & Empowerment Grant
Habitat residential and outpatient treatment for youth aged 1217 and their families. The Live Well Foundation of South Lake granted LifeStream a $600,000 grant to create the YRCC in South Lake County. To date, eight youths were enrolled in the center and two are projected to discharge successfully at the time of this publication. Once they are discharged from the program, these youths will enter the job corps program and continue to work with outpatient services. 14
for Humanity of Lake-SumterSouth Lake Preservation and Repair 2021 Transformation Grant
Habitat for Humanity of Lake-Sumter exists to provide affordable housing solutions in Lake and Sumter Counties. In addition to its mission of “bringing people together to build homes, communities and hope,” Habitat for Humanity of Lake-Sumter (HFHLS) has been a leader in innovation to build a more stable and sustainable future for the individual and community. In 2015, HFHLS established its Preservation and Repair program to provide safety and accessibility to residents who owned an affordable home but who, due to finance, ability, and life circumstance, were living in unsafe and unhealthy housing conditions. As of publication Habitat for Humanity of Lake-Sumter has been working diligently to line up subcontractors, key volunteers and materials for critical home repairs.
- Mission United Veteran Transportation Program 2021 Capacity & Empowerment Grant United Way of Lake and Sumter Counties seeks to create long-lasting change and sustainability in the community by addressing the greatest needs in the areas of income, health and education, by providing opportunities to improve the quality of life for at least 40% of the county’s residents. Mission United is a Veteran specific program, designed to exclusively serve those who have served.
New Wave Christian Church began its Expressive Arts Therapy groups in May 2019 out of the need for alternative mental health treatment groups to meet the needs of children and teens who were not responding well to traditional forms of therapy. Even in the midst of the COVID-19 pandemic, the Expressive Arts Therapy group was able to reach 240 children/ teens per year. Through a $223,060 grant provided by the Live Well Foundation of South Lake, New Wave Christian Church has been able to reach 360 teens/children, far surpassing their goal of 240. Group members continue to enhance and increase their individualized as well as collaborative pieces through art, music, or journaling. Specific members have learned to focus on specific instruments or art modalities that specifically speak to their inner selves, self-confidence, and mental health including depression or anxiety.
The United Way of Lake and Sumter Counties found that many veterans within the South Lake communities were forgoing specialty appointments at the Orlando VA Medical Center due to transportation needs. Though the Orlando VA does provide free transportation, it is not equipped to handle the volume of requests from the area. Through a $122,856 grant from the Live Well Foundation of South Lake, the Mission United team has made more than 100 trips from the Clermont VA Clinic to the Orlando VA Medical Center in Lake Nona. These services have made a truly significant impact on Veterans’ abilities to access their specialty medical service needs, including radiology, cardiology, urology and pulmonary care.

20 Years in the Making - Bettering the South Lake Community One Square Mile at a Time
The Live Well Foundation of South Lake began in 2018, but the vision for community health and wellness began decades before. In the late 1990s, health care institutions worked to promote a health and wellness component to their services. Fitness centers across the country started to focus more on wellness activities and not just workouts, which led to several mergers with local hospitals. ORMC already saw the benefit of this merger on their downtown campus and began to explore options in the South Lake area. Around the same time, Lake-Sumter State College saw a need for a satellite campus in South Lake and an idea was Dr.born.Michael Ray, a local orthopedic surgeon, suggested that the area needed something for wellness and athlete training. Dr. Jeff Duke and Shannon Elswick joined the conversation, bringing years of teaching, coaching, and healthcare knowledge to the table. The vision was to build a campus where there could be various entities of businesses that would participate in the scope of sports, health, wellness and education. This vision is now the square mile between Citrus Tower Blvd. and Hancock Rd and includes Orlando Health South Lake Hospital, the National Training Center, Lake-Sumter State College, Legends Softball Complex, Florida Special Olympics, Florida Sports Injury and Orthopedic Center and Brandy Johnson’s ShannonGymnasium.Elswick was instrumental in the development of this campus, especially the location of Orlando Health South Lake Hospital. Elswick negotiated the land deal for the complex and also secured funding for the National Training Center from ORMC. The original concept for the National Training Center was to have a wellness community that offered enough sports facilities that any athlete would see it as a top-rate facility. This included a 70-meter pool, soccer fields, track and outpatient

South Lake, we continue to see this vision expand as we move forward. Orlando Health South Lake Hospital’s Womens, Pediatrics and Trauma expansion increases the capacity for inpatient services, including additional intensive care beds and a Level II Neonatal Intensive Care Unit. LSSC added three new programs including Medical Lab Technician, Respiratory Therapy and Physical Therapy Assistant. Many of these students will become embedded into the Orlando South Lake Hospital campus environment through the development of their Dedicated Education Units. It is truly amazing to see the progress this vision has made over the past 20 years. We cannot wait to see what the future holds.

• Florida Vision Quest received a $25,000 grant to provide eye exams for indigent children in South Lake.
• A local summer feeding program received $21,600 to feed South Lake children duing the summer months.
The Live Well Foundation of South Lake recognized the need for funding grants that fell underneath $50,000. We partnered with the Community Foundation of South Lake by provided a $1 million grant to support projects that fell outside of our scope. The Community Foundation of South Lake has the ability to award $50,000 in grants every year until the funds are exhausted. This non endowed fund has already made an impact within the community.
rehab facility within the National Training Center. Since opening, the National Training Center has hosted several elite athletes and teams. With a hospital being constructed and the National Training Center in its development stages, Dr. Jeff Duke worked to bring in the education component. The vision was to train the next generation of leadership in health and wellness. Dr. Duke worked with Lake-Sumter State College to write a degree in Sports and Fitness. LSSC and Dr. Duke quickly realized they would need a partner to obtain a fouryear degree and through a grant from the State of Florida, they found that partner in the University of Central Florida.
