Publisher prevention, and technology. You’ll also learn how prevalent venous disease is. It runs much deeper than visible varicose veins and is often the cause of medical problems that many physicians find hard to diagnose. In Southwest Florida, we are fortunate to have access to some of the finest healthcare professionals such as Dr. Andrew Belis, Partner and President of Or thopedic Center of Florida. This issue we feature Dr. Belis who is specifically known for minimally invasive foot and ankle surgery as well as sports medicine. Inside you will hear from one of his patients who returned quickly to their active lifestyle after surgery. And speaking of sports, those of you who know me, know I’m a Red Sox fan. Spring Training is a particularly exciting time of the year for me. Who knows, maybe I will see you at one of the games! Thank you for Living Local.

“There is nothing quite like having an integrated multispecialty orthopedic practice with specialists from every field of care to serve our patients,” says Dr. Andrew Belis, Partner and President of Orthopedic Center of Florida. “We work together to combine diagnostic studies and collaborate in providing the best treatments possible. We have numerous specialists treating conditions from head to toe. It’s important for comprehensive patient care and it’s where we shine.” Belis is a podiatrist and foot & ankle surgeon; however, there are times when foot and ankle problems stem from other issues. In a practice like Orthopedic Center of Florida, their specialists work with Dr. Belis to treat the patient as a whole.

“Not many podiatrists are integrated into a multi-specialty orthopedic group. I’ve been one of the trailblazers in this area,” he says.

My approach is minimal with a small incision, which provides a proven quicker recovery and speedier rehab.

Dr. Belis’ passion for fixing things started as a child. He laughs, “My dad never hired a handyman. I have always been good with my hands and understand how things work.” And although his father, a Greek immigrant, pushed him in the direction of medicine, it seemed a natural inclination. He remembers dissecting a cow’s lung and heart in 6th grade science class. “Everyone hated it except me,” he laughs. “I finished in no time then went around helping everyone else. I was fascinated with biology and anatomy.”

While Dr. Belis treats all aspects of the foot and ankle, he specializes in minimally invasive and arthroscopic surgery. Take for instance injuries to the Achilles tendon where he says, “My approach is minimal with a small incision, which provides a proven quicker recovery and speedier rehab. It’s rewarding for me because my patients do so well and heal much quicker than the traditional larger open approach.”

He is still fascinated by it today performing a range of techniques including ankle arthroscopy, “using a camera to look in the joint and treat minor cartilage injuries, to more complicated procedures, such as removal of painful bone spurs and ligament reconstruction from sprains and chronic ankle weakness,” says Dr. Belis. “I really enjoy performing reconstructive procedures on patients suffering from flat foot and other deformities,” he says. In some cases, he is even able to treat bunions in a minimally invasive procedure that includes smaller incisions. Sports medicine is another aspect of his expertise that lead the podiatrist to treat Gary Cerny. Cerny was on an indoor climbing wall when he fell backwards fracturing his ankle. He remembers, “The first physician I saw told me it was 50/50 whether I should have surgery.” He went the no surgery route.

It healed quickly and within a month I was better by leaps and bounds. – Gary Cerny | Patient

GARY CERNY, WITH HIS TWO SONS

Over the course of the next eight months, Cerny was constantly in pain and his ankle frequently swelled. “The turning point for me, was a trip with my two boys to Lego Land,” he says. “After ten minutes my foot looked like a balloon and I thought, ‘this is crazy. I can’t live like this.’” Cerny’s boys attend school with Dr. Belis’ children and the two occasionally saw each other socially. “He was uncomfortable for a long time and it was not normal,” says the physician. The next time the two met was in Dr. Belis’ office where he took X-rays, an MRI and performed a diagnostic, fluoroscopic guided injection to validate the location of Cerny’s pain. Fluoroscopy is a technology that uses an X-ray to produce a real-time image of the injection site on a monitor, enabling Dr. Belis to see the exact location of the needle. When the needle is in the exact spot, he injects dye and anesthetic to be certain, then evaluates the location of the damage which helps determine the next step.

He explains, “If it numbs the joint and if pain is relieved, then that is a good indication of how the patient might feel if we operate; it gives us a level of predictability. I pride myself in diagnosing something thoroughly before diving in.” After weighing the options this time, Cerny said yes to surgery. “It healed quickly and within a month I was better by leaps and bounds,” he says. Today he is back to playing basketball and tennis, riding bicycles with kids to school, but still says “no” to rock wall climbing. “The surgery changed his life,” says Dr. Belis about Cerny. The physician also changes patients’ lives with preventive care, especially in regard to diabetics. In the case of a patient who suffered for years from diabetic toe ulcers, Dr. Belis determined the recurring problems stemmed from an extremely tight tendon. “I performed a simple procedure that involved lengthening the tendon and in two weeks the ulcer healed.”

Dr. Katrine Farag

IS TOOTH WEAR

WEARING YOU DOWN

?

Aging is a part of living, and it doesn’t just happen to your skin but also your teeth. In fact tooth wear is one of the more common concerns from patients of Dr. Katrine Farag, owner of Advanced Dentistry of Fort Myers.

ABOUT

“People are practicing more preventive care enabling them to keep their teeth longer. Now those teeth are experiencing wear and tear for many more years.”

Dr. Farag was born in London but grew up in Charlotte, North Carolina—she still says ya’ll. After graduating high school, she moved to Nashville, TN to complete her undergraduate studies in Neuroscience at Vanderbilt University. In 2005, she met her future husband who opened a dental practice in Port Charlotte and received her DMD in 2008 from Nova Southeastern College of Dentistry in Davie, Fl. Dr. Farag purchased Advanced Dentistry of Fort Myers in 2016. She takes a personal interest in each and every patient who comes into the office, and patient comfort is her foremost concern.

She often hears complaints from patients that when they smile they don’t see their teeth the way they used to; the teeth have become shorter. She says there are three factors to consider when talking about tooth wear: erosion, abrasion, and attrition. A sign of erosion is discoloration. Dr. Farag explains, “The teeth are starting to get more yellow because the layer of enamel is disappearing.” This is typically caused by chemicals such as the acid in sodas or acidic reflux, that eventually wear away the top layer of enamel, exposing the underlying ‘dentin.’ The dentin has a yellowish tint.

Beautiful Smiles Begin Here! MARCH/APRIL 2020

| mylivinglocal.com

“Keeping up on your regular dental visits can be less painful and much less expensive in the long run.” - Dr. Katrine Farag Abrasion is most often caused by over brushing, using a hard toothbrush. This physical action abrades the enamel, and similar to erosion, leads to a yellowish tint on teeth.

have to treat the entire joint instead of just the teeth,” says Dr. Farag.

“Attrition comes from chewing forces over the years. It begins to wear away at the length of teeth leaving patients with an uneven tooth line and/or short teeth.”

There are some easy things you can do now to slow the progression of tooth wear. The first one is to keep up on your regular dental visits. Staying ahead of changes in the mouth can be less painful and less expensive in the long run.

The key to treating tooth wear begins with figuring out the cause. “Eliminating the cause is the first step in treatment, then we can decide how to treat the teeth cosmetically,” says Farag.

Dr. Farag also says, “Check your toothbrush. Get rid of the brushes with the hard bristles. Throw them away and look for brushes with the softest bristles.”

Tooth color bonding is often used to brighten smiles in the case of erosion and abrasion. Whitening agents won’t work for these conditions because they target enamel. Once the enamel is gone and the dentin exposed, teeth whiteners don’t have a mineral base to cling to, and will no longer work.

She also advises cutting down on or eliminating acidic drinks and seeking treatment if you think you might have acid reflux. If you just can’t say no to that morning coffee, afternoon soda, or nightly glass of wine wait for at least 20 minutes after consuming before brushing. “That seems to be the magic number for the acid to dissipate and the PH in your mouth to get back to normal making brushing safer for your teeth.”

If you are suffering from either erosion or abrasion, besides having yellow tint to your teeth, you may also be experiencing sensitivity. Crowns and veneers are often used in the case of attrition. When teeth get worn down it affects the bite and could lead to TMJ and joint pain. “Now we

Tooth wear is a normal part of aging that Dr. Farag says everyone will experience. “The key is to stay on top of it so that when it becomes excessive we can step in before any more damage is done.”

Varicose Veins are More Than Just a Cosmetic Concern.

It wasn’t painful at all. Dr. Magnant explained everything. It was such a positive experience. aricose veins run in Ann Spang’s family; both V her mother and father had them. Hers began developing in her 20’s. In 2001 she had her first

procedure to treat the bulging and unsightly veins in her right leg. She remembers, “I had to wear compression stockings and I was allergic to latex, so it was a miserable experience.” So miserable in fact that she waited nearly 20 years before seeking medical attention for the veins in her left leg. Not only were the veins visually unappealing, but Spang was having trouble going about her regular, daily activities. Her legs swelled so much one time during a cruise that she had to ice them down at night.

— Ann Spang, Patient

“My legs were hurting and felt very heavy. My feet would hurt and swell. I started noticing some skin discoloration and the varicose veins were getting worse. They were moving up from my calf to my thigh. That scared me,” she remembers. After researching physicians specializing in vein treatment, Spang called Dr. Joseph Magnant, founder of Vein Specialists. “That’s all they do in his office—veins,” she says. “It made me feel very comfortable knowing that this is the only thing they focus on at Vein Specialists, and he is a Board-Certified Vascular Surgeon performing all procedures.”

Following her firs t appointment she remembers, “I always thought my veins were just a cosmetic problem. I never really knew about vein disease.” Dr. Magnant explains, “Venous disease is a medical condition like hypertension and high cholesterol and deserves investigation and potential treatment before complications occur.” Venous disease affects about 20% of the U.S. population, or roughly 40-50 million adults. Venous insufficiency occurs when valves in the leg veins fail to close, making it difficult for blood to return to the heart. When the blood can’t return, it pools in the legs resulting in increased venous pressure which causes varicose veins, swelling, inflammation, and skin conditions such as stasis dermatitis and venous ulcers. A majority of people who are suffering from venous disease don’t even realize they have it; not all signs or symptoms are as obvious as bulging varicose veins. Some patients may experience nighttime leg cramps, Charlie horses, restless legs or frequent urination, what Dr. Magnant refers to as the “nighttime triad.” Other patients may have skin pigmentation and thickening around the ankle for which they have sought the attention of a dermatologist. While the most advanced stage, venous ulceration might land other patients in a wound care center for many months of wraps and even hyperbaric oxygen tank treatments. What all these patients may share is the underlying condition of venous insufficiency. Venous insufficiency is an easily diagnosed and treatable condition in the hands of a Board-

Certified Vascular Surgeon focused on vein disease management. Spang had an ultrasound of both legs by a Registered Vascular Sonographers, which revealed leaking veins or venous insufficiency. Most of the time the superficial veins are the problem veins which is good news for patients since after treating the superficial veins, blood can still return to the heart through the normally functioning deep veins. The abnormal veins can be sealed by either heat or medical adhesive techniques. These are procedures that Dr. Magnant has performed on more than 28,000 patients in Southwest Florida. He performs this procedure in the office, under local anesthesia and in a sterile environment, and it takes approximately 30 minutes to complete. Most patients can return to normal activity within 24-48 hours. Two weeks after Spang’s first treatment, the same procedure was done on the leaky veins in her right leg. She says, “It was not painful at all. Dr. Magnant explained everything thoroughly and talked to me through the entire procedure. We actually had fun. It was such a positive experience.” After her first vein procedure nearly 20 years ago, Spang’s veins did not completely disappear. Today, thanks to Dr. Magnant, they are completely gone. “It’s a miracle,” she says. Adding, “And when I played golf, I was struggling by the 16th hole, but now I can walk all 18 and it’s nothing.” Spang advises anyone with painful varicose veins not to wait any longer and call Vein Specialists immediately for a consultation.

Barbara Bukousky | Patient

When to Replace

To be able to get up and walk & not be in a wheelchair is amazing AND I’m not in pain.

B

arbara Bukousk y was born with hip dysplasia, a congenital hip disorder affecting 1 out of every 1,000 babies born. This deformity occurs when the hip fails to form normally. Barbara was unaware that she had this disorder until her early 30’s. That’s when she took a fall and shortly thereafter started to experi-

ence severe groin pain.“In 1988 I began visiting every doctor I could find in Southwest Florida and around the state, and no one could tell me what was wrong.” Some physicians even dismissed her, suggesting the pain was in her head. In 1991, she traveled to Boston to find a solution and found herself under the care of Dr. Stephen Murphy. Barbara underwent extensive reconstruction surgery to preserve her hip joint, the result of the hip dysplasia. Since total hip replacements typically last from 10-30+ years, this surgery was designed to delay the need for a total hip replacement - which Dr. Murphy informed Barbarba she eventually would need. When that time came little did Barbara know Dr. Murphy, co-inventor of SuperPATH® total hip replacement, would be the surgeon who personally introduced Dr. Ameglio to this cutting-edge technique. Although developed in 2003, Dr. Ameglio is the pioneer of this procedure in SW Florida and is the most experienced SuperPATH surgeon in our area. With a traditional hip replacement, even after the hip heals the potential exists for it to dislocate during normal activities. Therefore, patients are sent home with a list of movement precautions that they must adhere to for 2 months. SuperPATH carries none of these restrictions. This is the only hip replacement where both the hip is never dislocated, and no muscles or tendons are cut. According to Dr Ameglio, “The SuperPATH

or Not Replace Your Hip approach is less traumatic. It’s easier on the patient physically, they experience less pain, and the recovery time is shortened significantly. It just makes sense and anatomical studies confirm it.” It made sense to Bukousky as well. Over the summer she had her right hip replaced by Dr. Ameglio via SuperPATH and was driving two weeks after the surgery. “I experienced little to no pain,” she marvels. Her hip was back in working order however, about 4 months later Bukousky began experiencing back and groin pain on the opposite side. “I would be up for two weeks and in bed for three, and thought it was time for a hip replacement on the left side,” she recalls. Dr. Ameglio began to investigate the cause of her pain. Although Barbara’s X-ray clearly indicated arthritis in her hip, this time her symptoms were slightly different from the right side. This raised the suspicion that Barbara may be suffering from sacroiliac joint dysfunction. “The symptoms of hip arthritis and SI dysfunction many times overlap making it challenging to diagnose the source of a patient’s pain.” The sacroiliac joint functions as a shock absorber, supporting the entire spine. It dissipates the shock to the sacrum and pelvis; however, with age, it’s less able to absorb shocks and becomes less mobile. SI dysfunction can also present itself after an accident, fall, childbirth, or simply picking up a suitcase.

PeterAmeglio.com @PeterAmeglioMD

Problems with the SI joint can result in pain in the buttocks, thigh, groin, and can radiate down the leg. SI dysfunction is not identifiable on X-rays or any imaging study, adding to the complexity of the diagnosis. “This is why understanding the difference is critical in avoiding an unnecessary hip replacement or revision hip replacement surgery”, adds Dr. Ameglio. Dr Ameglio is the only Orthopedic Joint Surgeon in SW Florida treating both the hip and the SI Joint. This gives him a unique perspective in understanding the source of patients’ pain. “15-30% of people with low back pain have sacroiliitis and in patients who have a lumbar spine fusion, up to 45% have sacroiliitis,” says the surgeon. “It’s a sizable number and it is critical to consider this diagnosis.” Dr. Ameglio performed an SI fusion on Barbara’s left side in November. Barbara is now living pain free for the first time in decades. “30 years is a long time to live the way I lived,” says Bukousky. “To be able to get up and walk and not be in a wheelchair is amazing AND I’m not in pain.” Convinced by the success of her surgeries performed by Dr. Ameglio, her husband has scheduled SuperPATH hip replacement with the surgeon. “There’s not too many doctors I trust anymore because of what I’ve gone through,” says Bukousky. “I love Dr. Ameglio, I believe in him, and most importantly, I trust him.” Dr. Ameglio opened his solo orthopedic practice in July with the help of his wife. Patients have embraced his move and the welcoming atmosphere. The surgeon is known for treating the whole person, for being a good listener, and a physician with empathy. “I love caring for others, I believe it’s what God created me to do,” expresses Dr. Ameglio. He and Audrey enjoy the outdoors and just celebrated their one-year wedding anniversary.

What Happens if My Will is Lost or Misplaced?

A

lthough most of our important documents are now digital, wills remain surprisingly “analog.” What we mean by this is that after a person dies, a Florida court typically needs to see an original, signed copy of a will that has been witnessed by at least two other people. A digital or photocopy is generally not acceptable. “Establishing” a Missing Will Bu t what happens if your will is misplaced or lost after you die? Florida law does anticipate such situations. Under Section 733.207 of the Florida Statutes, an “interested person” may attempt to re-establish your will in the absence of the signed original document . This is of ten done by presenting the court with a photocopy of the will.

K

MARCH/APRIL 2020

But of course, if you could just present a photocopy “no questions asked,” then there would be no point to requiring the original document. So things are not quite that simple. The person offering the copy must prove the will is valid–i.e., that it reflected the intentions of the person who purportedly made it. One way to prove the will is to present the testimony of at least one “disinterested” witness. And if there was not even a copy of the will available, there must be at least two disinterested witnesses. Beyond the witnesses, the person trying to establish the will must also prove to the probate court’s satisfaction that the decedent–the person who died–did not actually intend to destroy or revoke the purported will. Indeed, according to Florida law, when an original will is

“missing,” it is presumed the decedent intentionally destroyed it. So if you present a photocopy of a will, the judge will ask, “How do you know the decedent did not intentionally destroy the original because she wanted to revoke the will?” In some cases, this may be proved by presenting evidence the original will was known to exist after the decedent died. It is not uncommon for papers to be lost or misplaced in the process of cleaning out a deceased person’s home. If that is the case, a judge may be more inclined to admit a photocopy. care to keep your will in a secure location that only your designated personal representative can access. Another option is to give the original will to your estate planning attorney for safekeeping. (You should generally not put a will in your safe deposit box, as it may be impossible for your personal representative to access it immediately after your death.) Keeping your original will safe is important if you want to avoid any unnecessary delays in the probate of your future estate. Pets are more than just animals; they are family members for many people. But what happens to them when the family breaks up? When there is divorce or sickness? A financial set back, job loss or relocation? Many of them end up at the Gulf Coast Humane Society (GCHS,) a not-for-profit, no-kill shelter. In 20 1 9 t he shel ter rescued a whopping 2532 animals and adopted 2309 of them. “Many people are surprised to know we’ve been around since 1947 and at our current location off of MLK in Fort Myers since the early 70’s,” says Executive Director, Jennifer Galloway. Formerly known as Lee Count y H umane S ociet y, t he name was changed in the late 90’s; people often confused the shelter with the county’s animal services department. In addition, the organization’s reach goes far beyond Lee County. The shelter partners with smaller towns all over Southwest Florida that have limited resources. Animals are often euthanized in smaller communi-

ties because there is no medical care or even worse, no space for them. Galloway says, “Florida is one of top states that still euthanizes animals. By helping rural communities we help reduce those numbers.” When it comes to adoptions, “One of the biggest myths is that because an animal is in a shelter, they must have done something bad when that is just not the case.” Galloway says most animals end up there because owners can no longer take care of them. Once in the shelter, it is her mission to ease the fears of the animals, make sure they feel loved and leave with new skills. She likens a shelter stay to summer camp saying, “We do enrichment. We have certified trainers. We have play dates so they can go in the yard with other dogs. We want them to keep their social skills.” The same holds true for cats. GCHS has a free roaming cat room complete with a lanai for felines to lounge in the sun. Galloway explains, “It makes an easier transition into a home if the cats aren’t stuck in cages while they are here.”

“For A Greater Cause” is a monthly initiative of livinglocal, spotlighting a local nonprofit that is making a difference in our community. livinglocal also makes a monetary donation to the nonprofit on behalf of its advertising partners. Not all cats are adoptable; there is a large feral (free roaming cats) population in Southwest Florida. GCHS partners with Fortunate Ferals, another local non-prof it organization that teaches people how to colonize feral cats with an emphasis on spaying and neutering. “It’s our goal to reduce the cat population problem in the state,” says Galloway. The influence of GCHS is indeed much greater than the walls of the shelter. “We go into the community with animals to ease tensions and stress,” says Galloway, adding, “Even 911 call centers because it’s such a stressful job. The animals bring smiles to their faces.” The same holds true for children who are experiencing trauma. Shelter dogs make regular cameos at places such the Children’s Advocacy Center of Southwest Florida and Valerie’s House. Animals are used for humane education programs in local schools and at summer camps, and volunteers take them to nursing homes and adult day care programs throughout Southwest Florida.

GCHS was founded before the National Humane Society, and while a member of that organization, receives no funding from it. GCHS is privately funded, relying on donations and volunteers. Volunteers walk dogs, take them on car rides and even home for observation. If any behavior issues are noticed, trainers will work with the animals. Dog-training classes are offered to the public as well. “The goal is to get them in a home and keep them in a home,” says Galloway, “We do all we can to give our animals good skills.” Perhaps one of the biggest needs, besides funding, is that for foster parents. This is especially true during kitten season when babies whose eyes aren’t even open, are abandoned and have to be bottled fed. Ditto for puppies and injured animals. “There is only so much we can do in the rehabilitation center,” says Galloway. “It’s just like with people. We get better faster in a home, with fresh air, love and sunlight. I’m a true believer that heals us all.”
