he SuperPATH® approach is perhaps the most notable, recent advancement in total hip replacement surgery, a technique that works on those that are active as well as those who are weakened by their chronic hip arthritis. Whatever the condition of the patient, it makes for a quicker return to function. Dr. Peter Ameglio, owner of Ameglio Orthopedics had the pleasure of meeting Jill Dillon a retiree who wished to be out of pain and return to her active lifestyle as soon as possible. The 85-year-old is an avid hiker and says, “Over the course of the last several years I’ve just worn out my hips. I needed both replaced and as quickly as possible so I could get back to regular life. I like to travel and am very active.” When she heard it could take up to six months to heal from hip replacement surgery she says, “It really depressed me.” Then she found Dr. Ameglio and the surgery that is putting him on the map, SuperPATH. While SuperPATH is not a new technique (it was introduced sixteen

years ago), it’s an approach to hip replacement that is not commonly done. Although developed in 2003, Dr. Ameglio is the pioneer of the surgery in Southwest Florida. What makes this approach so different and so desirable to patients such as Dillon, is that the surgery is performed in a way that does not require the hip to be dislocated. All other total hip replacements require hip dislocation in which the entire leg is rotated until it comes out of joint. This results in injury to the joint capsule and small muscle attachments of the hip. Following surgery, the patient is instructed in movement precautions to reduce the risk of postoperative hip dislocation. SuperPATH does not carry any of these precautions since the hip is never dislocated. Dr Ameglio states, “The SuperPATH approach is much less traumatic. The surgery isn’t easier for the surgeon, but it’s much easier on the patient physically. They experience less pain and the recovery time is shortened significantly. It just makes sense. This isn’t just my opinion; clinical studies prove it.”

Traditional Values “I went to physical therapy immediately af ter my first surger y,” says Dillon. “The therapist said, ‘Oh if Dr. Ameglio did your hip you’ll be out of here in no time.’” That prediction came true and she quickly moved on to her second surgery. “Now less than two months after my second hip I’m ready to go on a hiking trip with my daughter. It’s really remarkable,” she says. Dr. Ameglio opened his solo orthopedic practice in July with the help of his wife. “Audrey has given me the confidence to pursue my own faith-based practice; from the culture to the décor, she designed a space that reflects my heart and desire to serve others.” Patients have also embraced the move and welcoming atmosphere, one even asked if she could move in. “I want to be able to bring comfort to patients through medicine of course, but on the other hand, it’s also nice to pray with them, if they are open to it, and bring comfort in that way as well.” says the surgeon. better alternative to another traditional hip surgery where he would now risk dislocating both hips while on the road, the pilot consulted with Dr. Ameglio who suggested SuperPATH.

That combination of caring and expertise has people in pain, seeking him out daily. People like Scott Hedge, a commercial airline pilot, marathoner, and cyclist. He had his first hip replaced at the age of 51. On a work trip six months later, he had a layover in Las Vegas and went to the Grand Canyon. He squatted down to take a picture and when he stood up, his hip dislocated. “I had just completed a 400 mile plus cycling trip and thought I was in fairly good shape, then this happens. I was shocked.” Seven years later, his right hip started hurting during his airline commutes and walks through airports. Searching for a

It’s not a miracle, you still have to put in the work, but my muscles came back much faster than last time.” He says the pain management was remarkable too and took prescription pain meds just one day. Seven weeks later, Hedge was back in the cockpit and had already logged another 300 miles on his bicycle. Besides marveling over the success and relative ease of their SuperPATH total hip replacements, both Dillon and Hedge rave about the treatment they received at the hands of Dr. Ameglio. “I was so impressed with his manor. He was thoughtful, a good listener and obviously loved the work he did,” says

S

tem cells are being used more and more by physicians for treatment of a variety of injuries and disease. Dr. Christopher Calcagni is one of the few podiatrists in Southwest Florida to use them when treating any number of issues affecting the foot and ankle. Founder of Coastal Foot & Ankle Center, Dr. Calcagni treats all aspects but specializes in minimally invasive foot and ankle surgery. A former football player at Oklahoma State University, he is also passionate about sports medicine and treating sports injuries, helping patients get back to their game as quickly as possible.

STEM CELL THERAPY HAS A VARIETY OF APPLICATIONS At Coastal Foot and Ankle Center, stem cell therapy is most often used to speed recovery from injuries and reduce inflammation and pain from chronic problems such as arthritis of the foot and ankle, Achilles tendonitis, plantar fasciitis and more. The stem cells Dr. Calcagni uses are embryonic and they have the potential to develop into many different types of cells within the body to speed the repair process. “If a patient has a small tear in a tendon, I can inject it with stem cells allowing the tendon to repair itself quicker.” Although Dr. Calcagni looks at surgery as a last resort, he says, “The injections also speed recovery following an operation.”

Patient Brenda Cass had been suffering from foot pain for years. “I’m a little flat footed and one foot kind of turns in when I walk. It would sometimes burn when I put my foot down and it was really getting painful.” Cass wears orthotics, tried steroid injections and stretching, but the pain did not improve. “It was affecting my lifestyle. I do a lot of walking.” She had heard about the use of stem cells and remembers, “I was a little apprehensive about it but the tendon really hurt and no matter what I did, I just couldn’t work it out.” About the injection, “He sprayed the area to numb it and it was over, just like that.” Over the next week the pain in her foot gradually subsided and now she says, “I’m walking easily on it and it feels really good. I’m very glad I had it done.” Performing stem cell injections for nearly five years, Dr. Calcagni has even used the procedure to treat his parents. “If possible, I don’t want my parents, or patients, to undergo surgery so I do everything I can to avoid it.” Steroid shots may help with some injuries and conditions in the short term, where stem cell injections actually help to repair the problem. Most insurance does not cover stem cell injections. However, Dr. Calcagni explains, “If a patient wants to get better quicker, heal faster, and decrease trips to doctor, this is the best option. Operations put you out of commission, wearing a boot limits you, and time is money.”

His parents began vacationing in the area in the early 90’s. He laughs, “I always wanted to move here, but they thought no one lived here year around. Most of his family members practice internal and family medicine, the path he originally thought he’d travel. “But during my last semester at Oklahoma State one of my uncles asked if I was sure about that because medicine was changing.” With that in mind he began looking at other platforms and during a break spent time with one of his uncle’s colleagues who was a podiatrist. You can go back to work and resume your normal life very fast compared to conventional methods.” Besides in the dental chair, Dr. Eshra also uses PRF and iPRF in cosmetic and facial rejuvenation procedures such as microneedling. “We treat a patient for overall health, not only oral health. Our education and degrees qualify us to be familiar with facial aesthetics in general,” says Dr. Eshra. Microneedling uses a series of small needles to puncture the skin at different depths, introducing growth factors to accelerate healing and collagen production. The procedure helps to firm skin, minimize fine lines and wrinkles, improves microcirculation, and even the appearance of acne scars. Dr. Eshra is constantly learning, staying up to date with the latest research including the applications of PRF, often consulting with colleagues around the country. “The results when using PRF and iPRF are superior to any conventional procedure,” he says. Pelvic Pain IS IT IN YOUR HEAD OR IN

C

hronic pelvic pain is a common problem in women, but because of its private nature, many patients suffering from it don’t talk about it openly. Women with this kind of pain typically seek help from their OB/GYN specialists, as well as a range of other specialists, and sometimes they are unable to uncover the root of the problem. According to Dr. Joseph Magnant, founder of Vein Specialists, says women, as well as men with unexplainable pelvic pain could be suffering from a venous condition called Pelvic Congestion Syndrome (PCS). PCS is a condition characterized by the following signs or symptoms: large varicose veins from the groin, vulva, or gluteal fold; leg swelling, ulcers or stasis dermatitis; pain in the pelvis and abdomen with squatting or crouching; painful intercourse; painful menstruation; pain in buttocks, posterior thigh, or knee with prolonged sitting or standing. That was the case with Lindsay. Her problems began during her second pregnancy; she was only 28-years old. The young mom says, “I started noticing a lot of leg pain and swelling; my feet were huge.” In addition, “The back of my thigh was throbbing, aching with pain. I developed a varicose vein in the back of my leg and could pinpoint that spot as the source for a lot of my pain.” She had trouble staying on her feet during the day and suf fered from restless leg syndrome at night. Her doctors attributed the pain to her pregnancy and advised wearing compression stockings. That’s not an easy task for a pregnant woman in Southwest Florida who needs compression up high, around the groin. Lindsay felt relief from her venous pain within a week after her treatment as well. She says, “I feel young again, not like a 30 year-old in an 80 year-old body.” “Women rarely talk openly about their pelvic congestion symptoms. There are more patients with treatable causes of chronic pelvic pain than we may realize and rather than labeling them as having psychogenic pelvic pain, I think investigating for a treatable cause such as iliac vein compression is the best course of action,” says Dr. Magnant. PHYSICAL THERAPY CAN DO

M

ost people are sent to physical therapy following an injury or surgery to help regain strength, improve range of motion, and healing in general. However, at Kriz Physical Therapy, patients like Peter Reines can also enroll in the Athletic Per formance program for help in improving their sporting activities. “I had minimally invasive back surgery and could’ve gone anywhere for physical therapy,” says Peter. “But I had heard such good things about Kriz and they were all true.” His surgeon prescribed 12 weeks of post-surgery physical t her apy to w hich Peter says, “They did a remarkable job bringing me back and getting me stronger.” Owners Tif fany and Mike Kriz specialize in a method called Functional Manual Therapy. It’s a hands on approach used to restore and improve strength and mobility. Mike explains, “After surgery, injury, or with many chronic conditions, movement p a t terns are al tered because people are trying to avoid or reduce pain. Many people must relearn movement patterns and it is best done with hands on, guiding body parts and regions through space efficiently.” That gentle, hands on approach worked for Peter who says, “Their

techniques work, no questions. But you still have to put the work in. It’s up to you, as a person being healed, to contribute to the process.” After 12 weeks of contributing to the process, Peter was feeling so much better he decided to enroll in Kriz’s Athletic Performance program for help with his golf game. Tiffany explains, “The program is designed to help athletes playing golf, tennis, pickleball, etc., play with better mobility, power, and balance.” The process begins with a head to toe evaluation of mobility, balance, and strength testing that is specific to the sport the patient wants to improve. “The tests translate to portions of a swing or stroke that may be limited, weak, or inhibited,” says Tiffany. “Then we take participants through a specifically designed exercise program and continually reassess as we go through the plan.” You don’t have to be a PT patient of Kriz to participate in the athletic program. Packages include nine sessions including evaluations. “They looked at my posture, how my knees work, my ability to turn my trunk, my shoulders, arms, and really all the parts of my body, to see where I was having issues with motion,” says Peter.

Everyone is different, but my game has really improved.” The Krizs met while attending school at Widener Universit y in Chester, Pennsylvania. Southwest Florida lawyer Scott Kuhn had never vacationed in Florida, he didn’t have relatives in the sunshine state, but 17 years ago he managed to end up here anyway.

The attorney says playing them, “is a release, you really have to focus.” Scott and his staff invest time getting to know their clients, even giving them personal assistance with doctor’s visits and needs other than legal. I don’t want to see these kids standing in food lines when they are their parents age,” says Beverly Pierret about the music program at Café of Life in Bonita Springs. A longtime volunteer at the café Pierret, founded the music program two years ago and says it’s changing the lives of the under privileged children who are involved in it. “Music is good for the heart and soul and well--for everything,” says Pierret. She began the program with one piano teacher and three students. Today there six instructors, 18 students, guitar has been added to the offerings, and there is a waiting list. There are also requests to add more instruments along with vocals and dance. The Café of Life was founded 20 years ago by a couple who wanted to offer homeless in the area, many of whom were veterans, a cup of coffee and support. The organization has bounced between several locations while growing and changing with the community.

In 2012 the Bonita Springs Women’s Club donated their building on Childers Street to the Café, giving the organization space for offices, storage, and events, including the music classes. Lessons are held three days a week as Public Relations and Transportation Director Nina Hart explains, “We buy keyboards, or they are donated, and give them to the children so they can practice at home.” The students sign a waiver that allows them to keep the instruments as long as they continue to take lessons. Hart says the program also benefits parents. “It holds them accountable. If they want a better future for their child, they must bring them here and participate too. Parents want more for their kids, but they don’t always know how to go about it. They see the value in this, and we see their joy and pride when watching their kids preform at our recitals.” Iris is leading the pack when it comes to the program’s success stories. Pierret remembers, “Her mother asked us if we could help. She was sitting on the couch

crying one afternoon because she had no talent. They were planning a talent show at school.” Iris had all kinds of talent bubbling underneath the surface, just waiting for someone to coax it out. At nine years old she began piano lessons and one year later was given a coveted scholarship to Learning from Elementary to Advance Piano or LEAP, a program by the Music Foundation of Greater Naples designed to help students from low-income families with private piano lessons. The Café of Life is privately funded and multi-faceted. Every weekday hot meals are prepared by different community groups and served under a gazebo at Leitner Park. The café actually built Leitner with help from the City of Bonita Springs. It also includes a small kitchen, playground, and soccer field. There is a food pantry and tables of donated clothing and goods; special attention is given to mothers and children on Tuesday. Last year, with the help of some 200 volunteers, 18,000 meals were served.

For those who can’t make it to the park, volunteers go into local migrant communities, delivering bags of grocery basics. There is also a transportation arm that helps get the underprivileged, the elderly, and veterans to the park for meals, to physicians’ offices, and other important appointments. These services are the tangible part of what the organization offers. What can’t be quantified, is the personal touch found here; the feeling of comfort and safety, the friendships that have been formed, and the kindness extended to those experiencing a wide range of need. And music plays a vital role. “Music is giving these students in our neighborhood confidence and it’s helping them in school too,” says Pierret. Hart adds, “I’ve seen a light develop in the eyes of these children. They have a different perspective now. They have the opportunity to dream bigger than the neighborhoods they are from.”
