Write a 2-3 sentence summary of incident, avoiding language that would indicate a decision on responsibility, always using “allegedly”. The above summary was formulated on the initial incident information provided by on site investigation and eyewitness statements who witnessed the incident and/or additional written statements, testimonies, or official memorandums. It is imperative for you to understand that the summary does not indicate a presumption of responsibility for any or all of the allegations. As indicated in the Student Rights in the Conduct System, all students are “considered innocent of the charges until proven responsible by a preponderance (51%) of evidence. In instances where the safety and welfare of a student has been determined as a concern, a representative from the Office of the Dean of Students may contact a parent or legal guardian, as per FERPA provisions.”
You have three (3) business days from the receipt of this notice to make an appointment to meet with me to begin the campus
disciplinary process. To do so, call your direct phone number or email first.last@liu.edu. Should you reach a voicemail system, it is imperative that you leave your name, reason for calling, and a phone number where you can be reached. Complete information regarding the disciplinary process is available in the LIU Student Handbook, found on the Campus Life section of the LIU Website.
It is your responsibility to contact me. Failure to make an appointment within three (3) business days of the receipt of this notice or failure to attend the meeting on the chosen date will result in a hearing being held in your absence. Should a hearing be held in your absence, a decision will be made solely on the incident report. It is important that you pay prompt attention
to this matter and take responsibility for your disciplinary future. All decision letters will be sent to the student’s MyLIU email address. It is the responsibility of the student to check their MyLIU email address, in a timely fashion, for updates regarding their status in the judicial process.
